익명의 알코올 중독자의 영적 안녕, 죽음에 대한 태도와 삶의 질의 관련성 (Associations of Spiritual Well-being, Attitude toward Death and Quality of Life among Alcoholics Anonymous)

  • Purpose: The aim of the study was to investigate associations of spiritual well-being, attitude toward death, and quality of life among Alcoholics Anonymous (AA). Methods: This study was cross-sectional and descriptive in design. The data was collected from August to September 2018 with 133 AA members drawn from two provinces of South Korea. The data were analyzed using descriptive statistics, independent t-tests, one-way ANOVA with Turkey tests, Pearson's correlation coefficients, and hierarchical multiple linear regression analyses using SPSS/WIN 20.0 program. Results: The existential spiritual well-being (β=.52, p<.001), attitude toward death (β=.24, p<.001), dual diagnosis (β=-.17, p=.003), occupation (β=.12, p=.035) of the participants were significant factors, which explained 63.7% of the variance of quality of life. Conclusion: The study findings highlight the need to develop psychological nursing strategies to enhance the spiritual well-being and improve a positive attitude toward death based on the job and dual diagnosis among AA members to improve their better quality of life.

지역 정신보건센터에 등록된 조현병 환자의 삶의 질 및 관련 정신사회적 요인 (The Quality of Life and Related Psychosocial Factors of Schizophrenia Patients Registered in Community Mental Health Center)

  • Objectives This study aimed to identify the relationship between quality of life (QoL) and psychosocial factors of schizophrenia patients registered in a community mental health center. Methods The ninety patients with schizophrenia, diagnosed by Diagnostic and Statistical Manual of Mental Disorders-Fourth Edition-Text Revision, registered in a community mental health center were studied cross-sectionally. Four trained personnels interviewed each subject individually for the assessment of QoL, psychopathology, severity of depressed mood, insight of illness, attitude toward drugs, social function, degree of social supports and conflicts. Results QoL was significantly related to the psychopathology, depressed mood, social conflict, social support; and other aspects of QoL were slightly differently related to depressed mood, social conflict, social support and attitude toward drug by scales. In multiple regression analysis, depressed mood (20.0%) and social conflict (10.3%) were contributed to QoL assessed by Korean version of 4th revision of Schizophrenia Quality of Life. Social support (21.4%), social conflict (20.9%) and depressed mood (3.1%) were contributed to QoL assessed by Korean modification of the Scale to measure Subjective Well-being under Neuroleptic treatment. Conclusions These findings demonstrated that QoL of schizophrenia patients in a community is affected by depressed mood, social support and conflict rather than psychotic symptoms, insight and attitude toward drugs. And this result suggests the necessity of approaching not only to the psychotic symptoms but also to the psychosocial characteristics in caring schizophrenia patients in the community.

정신장애인 자원봉사 프로그램의 임파워먼트 효과 (Empowerment Effects of the Chronic Mental Ill's Volunteer Program)

  • 정신장애인의 임파워먼트를 위해서는 개인 내적인 변화와 더불어 사회적 인식의 변화가 필요하다. 자원봉사활동은 바로 자기실현과 자존감 증대와 같은 내적 변화와 더불어 정상화 및 가치 있는 사회적 역할 강화를 통해 참여자에 대한 사회적 인식 변화를 가져올 수 있는 기제로 알려져 있다. 이에 본 연구에서는 정신장애인이 자원봉사 서비스의 제공자로서 지역사회에 참여하는 프로그램을 구조화하고, 이의 실천을 통해 임파워먼트 효과성을 살펴보고자 하였다. 이를 위하여 본 연구에서는 첫째, 정신장애인 뿐 아니라 봉사활동의 장인 지역사회의 봉사 준비과정을 체계화하고, 둘째, 정신장애인들과 봉사활동을 함께 하는 일반인 협력자를 활용하는 것을 특징으로 하는 정신장애인 자원봉사프로그램을 개발하였다. 그리고 이 자원봉사 프로그램에는 시설거주 정신장애인 13명과 이들의 협력자인 사회복지전공 대학생 10명이 참석하였다. 프로그램 시행 결과 자원봉사활동에 참여한 실험집단의 정신장애인은 통제집단에 비해 자아이미지 측면에서 임파워먼트의 효과가 있었던 것으로 나타났으며, 일반시민으로서 프로그램에 같이 참여한 협력자들은 사전에 비해 사후에 정신장애인에 대한 태도 즉, 권위주의, 자비심, 사회생활제한, 그리고 지역정신보건 측면에서 모두 긍정적인 변화가 있었던 것으로 입증되었다. 이러한 결과는 그동안 봉사 대상자로 머물러온 정신장애인도 자신의 능력을 활용하여 봉사 제공자가 될 수 있다는 것을 보여주는 것일 뿐 아니라, 이러한 활동을 통해 자기 자신에 대한 인식과 일반인의 태도에도 긍정적 변화를 가져올 수 있음을 나타내는 것이다. 본 연구를 통해 제시된 정신장애인 자원봉사 프로그램이 실천현장의 상황에 맞게 활용되어 정신장애인의 임파워먼트와 지역사회 통합에 기여할 수 있기를 기대해 본다.

  • The consulting internist will encounter patients with psychiatric symptoms and those who take psychotropic medications in many settings. The internist must loam to feel comfortable interacting with patients who display psychiatric symptoms. while maintaining an open and unprejudiced attitude toward their evaluation. The proper delineation of psychiatric disorders from normal emotional reactions resets on a careful history, a mental status evaluation. and a knowledge of psychiatric syndromes. Many physicians tend erroneously to view behavioral changes only in a psychological framework Abrupt changes in behavior, personality, mood. or ability to function should be evaluated for possible organic causes. Then, the internists should take their consultation to psychiatrists and freely discuss psychologic problems of the patients.

비정신과 간호사가 경험한 정신질환자 간호의 어려움 (Difficulties in Caring for Psychiatric Patient as Experienced by Non-Psychiatric Nurses)

  • Purpose: The purpose of this study was to identify non-psychiatric nurses' difficulties in caring for patients with mental illness. Methods: Data were collected from eighteen general medical-surgical nurses working at a university hospital in Seoul, Korea. This study involved two focus group discussions and three in-depth individual interviews. All interviews were recorded and transcribed as they were spoken, and data were analyzed using qualitative content analysis. Results: General medical-surgical nurses experienced difficulties in 3 categories, 9 subcategories, 27 codes. The three categories were 'nurse' related factors, 'patient' related factors, 'resource' related factors. The nine categories were 'unpreparedness', 'nursing barriers due to stigma', 'undervaluing and avoidance of psychiatric nursing', 'eroding into the trap of a vicious cycle', 'facing unapproachable patients', 'dealing with unhelpful family members', 'burdening already overburdened staff', 'obstructive environment', and 'isolation of staff with heavy responsibilities'. Conclusion: The results of this study indicate the need to develop psychiatric mental health education programs for non-psychiatric nurses. Education about psychiatric mental health and support from institutions for non-psychiatric nurses can reduce their negative attitude toward psychiatric patients and difficulties in caring for psychiatric patients.
