운동실조형 뇌성마비 아동의 이중과제훈련에 의한 균형 및 일상생활활동의 변화 : 사례보고 (Changes in the Balance and Activities of Daily Living on Children with Ataxic Cerebral Palsy from Dual Task Training : Case Study)

  • Purpose : The purpose of this study has been performed to find the changes in the balance and activities of daily living on children with ataxic cerebral palsy from dual task training. Methods : This study selects 3 subjects for 7.6 years old who were diagnosed with children with ataxic cerebral palsy. The dual task training applied three times a week for six weeks to ataxic cerebral palsy. To assess changes in balance, we performed the length and surface area ellipse of central of pressure, Pediatric Balance Scale, We also assessed activities of daily living using Pediatric Evaluation of Disability Inventory. Result : There were statistically significant differences in the changes during training period for the static(length and surface area ellipse of central of pressure) and functional balance(PBS). There were statistically significant differences in the changes during training period for the Pediatric Evaluation of Disability Inventory including self-care and mobility domains, except for social function domain. Conclusion : The results from this case study suggest that dual task training may have a beneficial changes on balance and activities of daily living for children with ataxic cerebral palsy.

마비성 조음장애의 임상적 양상에 관한 고찰 (Some Clinical Aspects of Dysarthria)

  • Dysarthrias are a sort of neuromotor disorders because of the weakness of neuromotor controls. They are classified in six types on the basis of Mayo clinic research: flaccid, spastic, ataxic, hypokinetic, hypekinetic and mixed types. Five dysarthria types are investigated in this study. MRI, EMG, neuropathological tests are essential diagnostic processing. Visi-Pitch and Spectrgraphy, CSL are used for assessing dysarthria speech. Maximum phonation time, diadochokinetic rate, Voice Onset Time and substitution rate are the speech evaluation parameters. Maximum phonation time and diadochokinetic rates are the lowest in case of spastic and ataxic dysarthrias. Spastic dysarthria shows the substituted glottalized consonants. However, flaccid, ataxic and hypokinetic dysarthrias show the substituted aspirated consonants. VOT is the longest for hypokinetic dysarthria and the shortest for ataxic dysarthria. Jitter shows higher percentage in comparison with control group. Speech evaluation using experimental phonetic instruments help create on international standardization of speech evaluation for speech disorders.

향부자팔물탕으로 호전된 알코올성 다발신경병증 치험 1례 (A Case Study of Alcoholic Polyneuropathy Improved with Hyangbujapalmul-tang)

  • Objectives This study was aimed to report improvement of Soeumin patient diagnosed with Alcoholic Polyneuropathy using Hyangbujapalmul-tang. Method The 41-year-old man had severe foot pain, sensory slow down and ataxic gait. The patient was diagnosed Soeumin and medicated with Hyangbujapalmul-tang three times a day. Clinical improvement was evaluated with blood test, ataxic gait evaluation, deep tendon reflex and visual analogue scale (VAS). Results The symptoms of foot pain, sensory slow down and ataxic gait were dramatically improved after treatment. Conclusions Constitutional treatment for foot pain, sensory slow down and ataxic gait diagnosed with Alcoholic Polyneuropathy are potentially effective.

비유창성 화자의 후두 교호운동 특성 (Characteristics of Laryngeal-Diadochokinesis (L-DDK) in Nonfluent Speakers)

  • Laryngeal DDK involve with the rate, pattern, and regularity (periodicity) in opening and closing of vocal fold. This study was aimed at investigating the characteristics of laryngeal DDK between nonfluent and fluent speakers. One with an ataxic dysarthria (with cerebellar lesion) and the other with stuttering, and 13 normal speakers were evaluated. L-DDK were analyzed with MSP (motor speech profile, CSL 4400). Measures of DDK included: DDKavr, DDKcvp, DDKjit, DDKavp. An ataxic dysarthric speaker and a stutterer showed more reduced rate and aperiodic L-DDK (both adductory and abductory movement) than normal speakers. But the average L-DDK period (ms) in adductory movement in a speaker with stuttering showed more decreased than the other. Results from this study are preliminary. Nonetheless, results of L-DDK produced by nonfluent speakers suggested the possibility to have relation with slow rate of phonatory initiation and connected speech. In the future, perceptual studies are needed in conjuction with acoustic and speech production.

소뇌 경색 환자의 임상양상에 따른 보행의 변화 (Change of gait pattern of a patient with ataxic gait by cerebellar infarction)

  • ■ Objectives The goal of this pilot study is to observe the change of gait pattern according to the clinical status in a patient with ataxic gait by cerebellar infarction. ■ Methods We measured the spatiotemporal gait parameters of a patient with ataxic gait four times, in which, first, the patient was not able to walk independently, second, able to walk independently, third, unable to walk independently by general tremor, and, last, able to walk independently after disappearing of general tremor. ■ Results When a patient with cerebellar ataxic gait was able to walk independently and the cerebellar tremor was disappeared, the change of gait pattern was that step and stride length decreased and total double support and stance phase increased. ■ Conclusion When different clinical characteristics were improved, the change of gait showed same pattern.

  • Among the various dysarthric subtypes, diagnosis of ataxic dysarthria is rendered when the speech characteristics include imprecise and irregular articulatory breakdowns, marked degree of speech rate impairment, overall monopitch and monoloudness, and respiratory-articulatory incoordination. Traditionally, speech pathologists have relied only upon their ‘ears’ to describe and evaluate the dysarthric speech. A statement of percentage of correct words identified by a listener do not provide so much more than an index of severity. Within the same perceptual dimension, a carefully constructed speech intelligibility test can specify patterns of errors. The patterns can contain a diagnostic value as well as Provide strategies for remediation. The phonetically transcribed texts on single words and a standard passage, 'kail' produced by an ataxic dysarthria are presented in this report, with an emphasis of the articulatory error analysis. Furthermore,, acoustic tools [e.g., spectrography to measure formant transitions, segment durations, consonant spectra, etc.] are utilized to serve as basic measures that objectively document patients' speech intelligibility, Finally, the treatment methods [e.g., spectrography as a visual feedback, gestural reorganization using pacing method, DAF (Delayed Auditory Feedback)] to modify the dysarthric behaviors are presented.

알코올성 다발성 신경병증의 한방치료 효과 (Clinical Study of Oriental Medical Treatment on a Patient with Alcoholic Polyneuropathy)

  • Alcoholic polyneuropathy is a disorder of the peripheral nervous system that interferes with sensory, motor, and autonomic nerve function. This study was to report the effect of Oriental medical treatment on a patient with alcoholic polyneuropathy. Conservative Oriental medical treatment was done to a man of 84 years who was diagnosed as a alcoholic polyneuropathy and suffered from both distal dominant leg weakness & symmetrical paresthesia and ataxic gait. The changes of symptoms, reflexes and sensory test were checked by Toronto Clinical Neuropathy Scoring System(TCNSS). Reflexes, motor and sensory impairments were improved after the Oriental medical treatment. I reported good effects of Oriental medical treatment on alcoholic polyneuropathy. Oriental medical treatment can be helpful to improve the symptoms of alcoholic polyneuropathy.

Imaging Diagnosis of Sacrocaudal Dysgenesis in a Shih-tzu Dog

  • A 10-month-old, intact female Shih-tzu dog with a pelvic limb ataxic gait, and urinary and fecal incontinence since birth, was examined by computed tomography and magnetic resonance imaging. The butterfly hemivertebra of the fourth lumbar vertebra, agenesis of the third sacral and coccygeal vertebrae, and spina bifida of the remaining sacral vertebra were observed on the computed tomography images. Magnetic resonance imaging revealed traction of the conus medullaris. The dog was diagnosed with sacrocaudal dysgenesis with presumptive tethered cord syndrome, and concurrent infectious cystitis was found by ultrasonography and urinary analysis. The Shih-tzu recovered from the cystitis, but still has the ataxic gait and urinary and fecal incontinence. This report summarizes the case of a dog with sacrocaudal dysgenesis that was identified by imaging diagnosis.

소뇌경색 환자의 운동기능 회복에 따른 보행 척도의 변화 관찰 연구 : 피질척수로 손상환자와의 비교를 중심으로 (Change of Gait Pattern of Patients with Ataxic Gait by Cerebellar Infarction in Comparison to Corticospinal Tract Stroke Patients)

  • Objectives: This study aimed to compare the gait patterns of cerebellar infarction patients with those of corticospinal tract stroke patients through a follow-up of patients with ataxic gait due to cerebellar infarction and corticospinal tract stroke. Methods: We investigated two cases of patients with cerebellar infarction and two cases each of acute or chronic corticospinal tract stroke who were hospitalized at Wonkwang University Gawangju Medical Center from September 1, 2017 to February 15, 2020 based on medical records and gait analyses. The spatiotemporal gait parameters of each patient were measured three times at 2-week intervals except those of the chronic corticospinal tract stroke patients, which were measured twice at a 1-month interval. Results: Spatiotemporal gait parameters, which include velocity, cadence, step length, stride length, and single support, were consistently increased in the cerebellar infarction patients in comparison to the corticospinal tract stroke patients. The stance phase was decreased in all the patients. Conclusions: The cerebellar infarction patients' gait spatiotemporal parameters were found to consistently improve. Moreover, gait analysis can be used to effectively measure improvement of ataxic gait.
