Trends in Clinical Research on Herbal Medicine Treatment in Pediatric Asthma - Focusing on Randomized Controlled Clinical Trials in Traditional Chinese Medicine (소아 천식의 한약 치료에 대한 임상연구 동향 - 중의학 무작위 대조군 임상시험을 중심으로)

  • Objective This study analyzed randomized clinical trials (RCTs) on pediatric asthma using traditional Chinese medicine (TCM) to determine its efficacy and safety. Methods We searched electronic databases in English, Chinese, Japanese, and Korean and evaluated 35 articles published up to December 28, 2022. Based on data from the literature, we analyzed treatments, results, composition of herbal medicine, frequency of medicinal herbs, and decoction. Results TCM for pediatric asthma was compared to supportive therapy, inhaled corticosteroids, and standardized treatments. Of the 35 studies, 32 showed that TCM treatments were effective in alleviating asthma symptoms. Furthermore, TCM treatment was considered safe compared to common asthma treatments. Among the TCM treatments for pediatric asthma, the most frequent decoctions were Gamisaganmahwangtang (加味射干麻黃湯) and Saganmahwangtang (射干麻黃湯). Ephedra Herba (麻黄), Armeniacae Semen (杏仁), Asiasari Radix et Rhizoma (細辛), Belamcandae Rhizoma (射干) were herbs that were frequently used for asthma in TCM decoctions. Conclusion This review showed that TCM is effective in treating pediatric asthma. More clinical RCTs are needed to confirm the efficacy and safety of TCM treatment.

CpG DNA for Treatment of Allergic Diseases (알레르기 질환의 치료로서의 CpG DNA)

  • Atopy is a highly prevalent and serious health problem. The prevalence and severity of asthma and allergic diseases have increased over recent decades, particularly in industrialized nations. Early life infections may protect against the development of atopy and allergic diseases like asthma. The inverse relationship between the incidence of atopy and childhood infections has led to the 'hygiene hypothesis', which suggests that diminished exposure to childhood infections in modern society has led to decreased Th1-type responses. Th1 and Th2 responses are counter-regulatory. Reduced Th1 may lead to enhanced Th2-type inflammation, which is important in promoting asthma and allergic disease via up-regulation of IL-4, IL-5, and IL-13. It is now widely accepted that altered regulation of Th2 responses(and possibly the balance between Th1 and Th2 responses) is an important factor in the development of atopy. CpG DNA represent a novel class of drugs with substantial immunomodulatory properties. CpG DNA contain unmethylated motifs centered on the CpG dinucleotides, like bacterial DNA. These CpG DNA promote Th1 and regulatory type immune responses and suppress Th2 responses. In murine studies, CpG DNA are effective in prevention and treatment of asthma and allergic diseases. CpG DNA are just beginning to be tested in human asthma. While its precise mechanisms continue to be fully studied, CpG DNA offers considerable promise as a novel treatment for atopic inflammation. It may prove to be an important disease modifying therapy, or even curative therapeutic agent for asthma and allergic diseases.

Effect of Asthma Management Educational Program on The Disease Related Knowledge, Stress, and Self-efficacy of Asthmatics Allergic to House Dust Mite (천식관리 교육 프로그램이 집먼지 진드기 알레르기성 천식환자의 질병관련 지식, 스트레스 및 자기효능감에 미치는 효과)

  • Purpose: The purpose of this study was to investigate the effect of the asthma management educational program given to the allergic asthmatics receiving immunotherapy due to house dust mite on the disease related to knowledge, stress, and self-efficacy. Method: The subjects of this study were the patients received immunotherapy to house dust mite at a week interval after being diagnosed for house dust mite allergic asthma at the respiratory center of a hospital affiliated to the university. They were divided into the experimental group of 29 patients who received asthma management education, the control group of 32 patients, and 61 patients in total. Experimental treatment, as an asthma management educational program, was the group education of one time and the reinforcement education of three times with environmental therapy and immunotherapy to house dust mite. Results: The results revealed that the improvement in disease related knowledge, the improvement in self-efficacy and the decrease in stress, were significantly higher in the experimental group than the control group. Conclusions: The asthma management educational program had an effect on improving the disease related knowledge and self-efficacy, and decreasing the stress of the patients asthmatics allergic to house dust mite.

Hominis Placental Pharmacoacupuncture, a Promising Asthma Therapy (자하거(紫河車) 약침의 기관지 천식 치료 가능성에 대한 소고(小考))

  • Objectives : The aim of this study was to find out some rationales for using Hominis placental extract(HPE) pharmaco-acupuncture on asthmatic patients, and to suggest an appropriate clinical research of HPE pharmaco-acupuncture on asthma. Methods : Articles regarding immunologic experiments about HPE and clinical reports about HPE-contained pharmaco-acupuncture on asthmatic patients were reviewed by on-line and off-line searching. Results and Conclusions : HPE is suggested to have several pharmacological effects on allergic diseases including asthma as well as autoimmune diseases. There are some observational studies of HPE-contained pharmaco-acupuncture on asthmatic patients. However, the designs of these studies are not sufficient to investigate the exact effect of HPE pharmaco-acupuncture on asthma. To investigate HPE pharmaco- acupuncture as a novel asthma therapy, it will be needed to perform further experiments about HPE's pharmacological mechanism on asthma and to try further advanced clinical researches such as well-designed randomized controlled trial of HPE pharmaco-acupuncture.

Asthma-COPD Overlap Syndrome: What We Know and What We Don't

  • Approximately one in four patients with chronic obstructive pulmonary disease (COPD) have asthmatic features consisting of wheezing, airway hyper-responsiveness or atopy. The Global initiative for Asthma/Globalinitiative for chronic Obstructive Lung Disease committee recently labelled these patients as having asthma-COPD overlap syndrome or ACOS. ACOS also encompasses patients with asthma, ${\geq}40$ years of age, who have been cigarette smokers (more than 5-10 pack years) or have had significant biomass exposure, and demonstrate persistent airflow limitation defined as a post-bronchodilator forced expiratory volume in 1 second ($FEV_1$)/forced vital capacity of <70%. Data over the past 30 years indicate that patients with ACOS have greater burden of symptoms including dyspnea and cough and show higher risk of COPD exacerbations and hospitalizations than those with pure COPD or pure asthma. Patients with ACOS also have increased risk of rapid $FEV_1$ decline and COPD mortality. Paradoxically, experimental evidence to support therapeutic decisions in ACOS patients is lacking because traditionally, patients with ACOS have been systematically excluded from therapeutic COPD and asthma trials to maintain homogeneity of the study population. In this study, we summarize the current understanding of ACOS, focusing on definitions, epidemiology and patient prognosis.

The Effects of Sochongryong-tang on Serum IL-4, IL-5, and $IFN-{\gamma}$ in Asthmatic Patients (소청용탕이 기관지천식 환자의 혈청 IL-4, IL-5, $IFN-{\gamma}$변화에 미치는 영향)

  • Background : Asthma is considered to be an inflammatory disease characterized by airway hyperresponsiveness and pulmonary eosinophilia. Production of cytokines by bronchial epithelial cells may contribute to the local accumulation of inflammatory cells in patients with bronchial asthma. In many recent studies molecular biological methods have been used to investigate the role of cytokines in pathogenesis and new therapeutic targets of asthma. Objective : We aimed to identify the effects of Sochongryong-tang on Serum IL-4, IL-5, $IFN-{\gamma}$ in asthmatic patients. Material and Methods: The subjects consisted of 15 patients with asthma who had been treated with Sochongryong-tang for two weeks from February 2001 through June 2001. Sochongryong-tang is an herbal decoction which has traditionally been used as a therapeutic agent for asthma. Results : The serum IL-4 in asthmatic patients was increased significantly compared to the serum IL-4 in the normal control group. However, the serum IL-5, $IFN-{\gamma}$ in asthmatic patients showed no significant difference from the serum IL-5, $IFN-{\gamma}$ in the normal control group. The patients were treated with Sochongryong-tang for two weeks with no significant difference in the serum IL-4, IL-5, and $IFN-{\gamma}$. Conclusion : This study shows that the serum IL-4 may be a new therapeutic target of asthma. Further long-term studies must be made in a larger number of asthmatic patients.

Influence of age at complementary food introduction on the development of asthma and atopic dermatitis in Korean children aged 1-3 years

  • Background: Complementary food in infancy is necessary for human growth, neurodevelopment, and health. However, the role of allergen consumption in early infancy and its effects on the development of food allergy or tolerance remain unclear. Purpose: To investigate the influence of age at the time of complementary food introduction on the development of asthma and atopic dermatitis in Korean children aged 1-3 years. Methods: We combined data from the Korea National Health and Nutrition Examination Survey collected from 2010 to 2014 and analyzed 1619 children aged 1-3 years who were included in the survey. Multivariate regression analysis was used to identify associations among type of feeding, age at the time of complementary food introduction, and doctor-diagnosed atopic dermatitis and asthma. Results: Age at the time of complementary food introduction was not significantly associated with doctor-diagnosed atopic dermatitis and asthma in children aged 1-3 years. In the univariate analysis, children with asthma showed higher water and sodium intake levels than nonasthmatic children. However, this relationship was not significant in the multivariate regression analysis. Conclusion: The present study revealed no statistically significant relationship between age at the time of complementary food introduction and the risk of atopic dermatitis and asthma in young Korean children. A national prospective study is needed to clarify the influence of age at the time of complementary food introduction on the development of allergic diseases.

Ant-Inflammatory Effect of Prunus serrulata var. spontanea Extract in OVA-Induced Asthma Animal Model (벚나무 추출물의 OVA 유도 천식동물모델에서 항염증 효능)

  • The objective of this study was to determine the efficacy of a natural product of cherry tree (Prunus serrulata var. spontanea: PS) as a test substance for improving cytokine and ovalbumin-specific IgE using an ovalbumin-induced asthma disease model of 5-week-old male BALB/c mice. Lung tissue pathology was analyzed to confirm anti-inflammatory and asthmatic effects. As a result of examining the effect on changes in inflammatory cells in bronchoalveolar lavage fluid in an ovalbumin-induced asthma disease model by administering the PS sample, total cells, eosinophil, neutrophil, lymphocyte, and monocytes were significantly decreased. Concentrations of cytokine-based TNF-alpha and IL-4 and immunoglobulin E in serum were significantly increased in the asthma-inducing negative control group than in the normal group. However, high concentrations of PS decreased them. In histopathological examination of the lung tissue, it was confirmed that inflammatory cells infiltrated around the alveoli and bronchioles were increased in ovalbumin-induced asthma disease model. After administration of cherry tree extract, bronchiolar morphological changes such as mucosal thickening were slightly improved. From the above results, it was confirmed that extract of cherry tree significantly reduced inflammation expression and tissue damage in alveolar tissues. It was also confirmed that the cherry tree extract had an excellent efficacy in improving asthma inflammation.

Trends in research on the efficacy of herbal medicines for particular matter-induced asthma focused on Pubmed (미세먼지 유도 천식의 한약유효성 평가연구 동향 Pubmed를 중심으로)

  • Objectives: This study aimed to review the experimental research trends in asthma caused by particulate matter to conduct further researches, especially in clinical trials. Methods: We searched for the meaningful literature using medicinal herb for asthma through the Pubmed databases. Results: Finally, six studies were finally selected. These studies showed recent trends, from 2018 to 2021 and conducted mostly in South Korea. The type of fine dust and the method of inducing asthma are different for each paper, but the effective mechanism is relatively common. It was commonly confirmed that the ratio and number of eosinophils, th2 cells and related cytokines are decreased in BALF and lung tissue by administration of medicinal herb. Conclusions: Although the method of inducing asthma using fine dust has not yet been standardized, it is thought that more meaningful evaluation results can be derived if a standardized animal model is established in the future. Nevertheless, based on the results that herbal medicine is effective for particular matter induced asthma, it is expected that it will be the basis for expanding into future clinical studies.

Effects of Mahwangyoonpye-tang on Asthma Induced by Ovalbumin in Mouse (마황윤폐탕이 ovalbumin으로 유발된 mouse의 천식에 미치는 영향)

  • In order to study the effect of oral administration of Mawhangyounpye-tang against to asthma, astham was induced to allergy-sensitive Balb/c mouse with ovalbumin using method of Hatfield et al (1997). The changes of diameter lumen of upper portion of the trachea, lung weight, gross appearance of lung, histological profiles of lung and trachea, numbers of cellular compartments in the bronchoalveolar lavage fluid (BALF), numbers and morphology of the mast cells in the trachea, numbers of mucus-secretory cell in the broncus, morphology of the bronchus, ultramicroscopical appearance of surface of trachea and number of cilia and mucous-secretory cells by scanning electron microscope. Obtained results were as follows. 1. The diameters of trachea lumen were significantly decreased in asthma induced control groups and these decreasing were result from hypertrophy of mucous membrane. However, these phenomena were dramatically recovered in the Mawhangyounpye-tang dosing groups. 2. Lung weights and black spots, which were result from infiltration of inflammatory cells, were significantly increased in asthma induced control groups but these phenomena were dramatically recovered in the Mawhangyounpye-tang dosing groups. 3. Hypertrophy of mucous membrane of trachea and bronchus and !bronchioles in the lung, peritracheal, peribronchus and peribronchiolar inflammatory cell infiltration, and mucoid exudate deposit in the lumen were observed in asthma induced control groups but these phenomena were dramatically recovered in the Mawhangyounpye-tang dosing groups. 4. Cellular compartments including neutrophil and eosinophil were dramatically increased in the BALF of asthma induced control groups but these phenomena were dramatically recovered in the Mawhangyounpye-tang dosing groups. 5. Mast cell degranulation and decreasing of the numbers of mast cells were detected in the trachea of asthma induced control groups. However, these phenomena were dramatically recovered in the Mawhangyounpye-tang dosing groups. 6. Shed, decreasing of cilia cell and increasing of mucous-secretory cells in the surface of the trachea of asthma induced control groups but these phenomena were dramatically recovered in the Mawhangyounpye-tang dosing groups. In conclusion, it Is considered that Mawhangyounpye-tang has somewhat favorable effect on the asthma because the asthma specific series of abnormalities in respiratory system were decreased after oral administratin of Mawhangyounpye-tang in this study. In future, it is needed that the toxicological and dosagespecific study of Mawhangyounpye-tang to use against asthma with safe.
