  • Fine needle aspiration cytology (FNAC) of thyroid gland lesions has become a routine diagnostic method. And fine needle aspiration cytology is considered a safe, reliable and cost-effective means of selecting thyroid nodules with risk for malignancy. However, fine needle aspiration cytology of the thyroid may cause hemorrhage, infection, or trauma to adjacent structures. Hemorrhage sufficient to cause tracheal compression has not been reported. So we present a case of anterior neck hematoma causing tracheal compression after FNAC of the thyroid nodule.

Study on In Vitro Bovine Embryo Production with Follicular Oocytes Obtained via Ultrasound-guided Ovum Pick-up (OPU) and Slaughterhouse-derived(SHD) Ovary Aspiration in Korean Native Cows (한우에서 초음파 유래와 도축장 유래 난포란을 이용한 체외 수정란 생산에 관한 연구)

  • The present study was carried out for the comparative study on the collection of bovine follicular oocytes by ultrasound-guided ovum pick-up(OPU) and slaughterhouse-derived (SHD) ovary aspiration and in vitro production of bovine embryos with the follicular oncytes in Korean native cows. Bovine follicular nocytes were observed with a 6.5 MHz convex-array ultrasound transducer designed for intravaginal use and the oocytes were collected with the aspiration equipment attached to the ultrasonograph. Bovine ovaries were collected and transported in phosphate buffered saline from the local slaughterhouse, the follicular oocytes were collected by the aspiration method. The collected follicular oocytes in good quality were matured, fertilized and cultured in the media. The total number of the visible follicles and the recovery rate of follicular oocytes were increased in ultrasonography following follicle stimulating hormone(FSH) treatment in Korean native cows. The mean recovery rate of oocytes was 66.2, 52.8 and 41.7% in the FSH-OPU, non-treatment-OPU and SHD ovaries, respectively. The mean number of recorved oocytes per cow were not significantly(P<0.05) different between the FSH-OPU(14.0$\pm$11.54) and SHD(17.1i6.21) groups, but the numbers in both groups were significantly(P<0.05) higher than the number in the non-treatment-OPU(3.7$\pm$1.57) group. The mean number of usable nocytes in Grade T /11 per ovary was 6.3, 4.8 and 1.3 in the cows of the SHD, FSH-OPU and non-treatment-OPU groups, respectively. The in vitro developmental rate to the blastocyst was not significantly different between the oocytes obtained via OPU(37.1%) and SHD(29.3%). Therefore, the ultrasound-guided OPU technique can be applied to the production of excellent embryos from the high-quality cows, and for the large scale production of in vitro bovine oocytes and embryos, the SHD ovary aspiration method is valuable.

Gamma Knife Radiosurgery after Stereotactic Aspiration for Large Cystic Brain Metastases

  • Objective : Several treatment options have proven effective for metastatic brain tumors, including surgery and stereotactic radiosurgery. Tumors with cystic components, however, are difficult to treat using a single method. We retrospectively assessed the outcome and efficacy of gamma knife radiosurgery (GKRS) for cystic brain metastases after stereotactic aspiration of cystic components to decrease the tumor volume. Methods : The study population consisted of 24 patients (13 males, 11 females; mean age, 58.3 years) with cystic metastatic brain tumors treated from January 2002 to August 2008. Non-small cell lung cancer was the most common primary origin. After Leksell stereotactic frame was positioned on each patient, magnetic resonance images (MRI)-guided stereotactic cyst aspiration and GKRS were performed (mean prescription dose : 20.2 Gy). After treatment, patients were evaluated by MRI every 3 or 4 months. Results : After treatment, 13 patients (54.2%) demonstrated tumor control, 5 patients (20.8%) showed local tumor progression, and 6 patients (25.0%) showed remote progression. Mean follow-up duration was 13.1 months. During this period, 10 patients (41.7%) died, but only 1 patient (4.2%) died from brain metastases. The overall median survival after these procedures was 17.8 months. Conclusion : These results support the usefulness of GKRS after stereotactic cyst aspiration in patients with large cystic brain metastases. This method is especially effective for the patients whose general condition is very poor for general anesthesia and those with metastatic brain tumors located in eloquent areas.

  • Objectives: This clinical study reports on the effects of Yukgunja-tang on aspiration pneumonia.Method: This case examined a comatose male patient who suffers from continuous regurgitation. He showed symptoms of aspiration pneumonia with accompanying gastroesophageal reflux. We gave the patient Yukgunja-tang for 28 days and observed any changes in symptoms.Results: During the period when the patient was given Yukgunja-tang, he defecated on his own without glycerin enema, regurgitation occurred less frequently, and the aspiration pneumonia symptoms improved.Conclusion: Yukgunja-tang helped the patient's bowel movements move more quickly and smoothly through his colon. As a result, Yukgunja-tang appears to have prevented regurgitation and aspiration pneumonia.

  • There has been a marked increase in the utility of aspiration cytology for pathologic diagnosis. It may be applied to any kinds of organs and substitutes surgical biopsy. Be cause of the high risk of complication and difficulties In localization, aspiration cytology in the central nervous system (CNS) has been used with less frequency compared to other sites. However, with the advent of sophisticated imaging instruments, aspiration cytology of lesions in the CNS is being used increasingly. We present cytologic features of brain tumors as well as corresponding histologic findings. Eight types of tumors were aspirated intraoperatively and stained with the Papanicolaou method; 1 anaplastic astrocytoma, 1 glioblastoma multiforme, 1 ependymoma, 1 malignant ependymoma, 1 central neurocytoma, 1 primitive neuroectodermal tumor, 1 benign neurogenic tumor and 1 germinoma. Cytologic features of the CNS neoplasms were quite similar to those of histology except one spindle cell tumor. Reviewing various CNS neoplasms, it appears that cytology may be a useful diagnostic method.

  • The practice of oral surgery or other dentistry possesses the danger of causing the loss of foreign bodies used within the oral cavity. If such foreign bodies would be lost, they could enter the viscera through the esophagus or tracheobroncheal tree. Ingestion is four times as frequent as aspiration and 80% to 90% of ingested foreign bodies will pass through the body spontaneously. Once the aspiration or ingestion of foreign bodies is happened, a dental procedure should be discontinued immediately. If symptoms of respiratory distress, including coughing, wheezing, or stridor, are present, a patent airway should be maintained, oxygen administered, and ventilation supported if necessary. The PA chest radiograph will identify the objects in the lung, esophagus, or stomach. Ingested gastrointestinal foreign bodies may be managed by observation, endoscopy, and or surgical intervention and aspirated tracheobroncheal foreign bodies may be managed by bronchoscopy, and or surgical intervention. This case report describes the management of ingested or aspirated foreign bodies happened to the three patients during intraoral treatment. we recommend that the preventive method of ingestion or aspiration of the dental foreign bodies should be performed prior to intraoral treatment and the immedieate measures should be carried out after ingestion or asipiration of it.

  • Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is becoming a standard method for invasive mediastinal staging and for the diagnosis of paratracheal and peribronchial lesions. It is essential to understand the technical aspects of EBUS-TBNA to ensure safe and efficient procedures. In this review, we discuss the practical aspects to be considered during EBUS-TBNA, including anesthesia, manipulation of equipment, understanding mediastinal ultrasound images, target selection, number of aspirations needed per target, sample handling, and complications.

  • Fine needle aspiration cytology (FNAC) is an easy convenient non-invasive method in the diagnosis of superficial palpable masses. The cytologic findings by FNAC of reactive and neoplastic lesions in various organs including breast, lymph node, thyroid, salivary gland, etc., have been described, but, those of soft tissue lesions including proliferative fasciitis are relatively rare to find. We recently experienced a case of FNAC of proliferative fasciitis in the left back of a 72-year-old male. The FNAC smears were scant in cellularity and contained large cells with abundant basophilic cytoplasm, one to two nuclei lying at the periphery, and prominent nucleoli that resemble ganglion cells.

  • Sono-guided fine needle aspiration (FNA) of the thyroid is widely used, but the aspirated samples are typically not well preserved and low cellularity makes diagnosis difficult in many cases. The object of the current study is to evaluate the adequacy and diagnostic accuracy of the use of $SurePath^{TM}$ liquid-based cytology (SP-LBC) in the sonoguided fine needle aspiration of the thyroid nodule and to compare its use with that of the use of a conventional smear (CS). A total of 172 sono-guided FNAs of thyroid nodules from April to June, 2006 were prepared by the use of the split method with either SP-LBC or CS; the samples were stained with the use of hematoxylin-eosin (H&E) and Papanicolaou (Pap) stains. A cyto-histological correlation was performed in 69 (30 SP and 39 CS) cases that had been histologically confirmed. The rate of producing unsatisfactory slides by the use of the SP-LBC method (9.3%) was less than that of the use of the CS method (20.9%). The diagnostic accuracy of the SP method (93.3%) was better than that of the CS method (85.3%). The sensitivity and specificity of the SP method (94.4% and 92.3%) was better than that of the CS method (83.3% and 70%), respectively (p < 0.05). The CS of sono-guided aspirated specimens had some unavoidable limitations related to inadequate sampling such as a bloody background, low cellularity and an indication that some clinicians smeared many useless slides (averaging four to ten slides), and that most slides showed only blood that included few follicular cells. The SP method resulted in more thinly smeared slides and showed cleaner background and greater cellularity than the use of the CS method. Each follicular cell shows superior nuclear detail, and more distinct cytoplasmic features than with the use of the CS method. SP-LBC appears to be an easy, highly accurate, and reliable cytological method for employ for a diagnostic approach of thyroid disease and thyroid nodules. The SP-LBC method is a suitable alternative to the CS method to overcome diagnostic difficulties.

Fine needle aspiration biopsy for the diagnosis of fatty liver in cattle (Fine needle aspiration biopsy에 의한 소의 지방간진단(脂肪肝診斷))

  • This study was carried out to investigate the morbidity of fatty liver in cattle at the abattoir and on the farm, and to cytodiagnose fatty liver in cattle by fine needle aspiration biopsy. Incidence rates of fatty liver in cattle, detected macroscopically or based on hepatic lipid content by buoyancy, were 0.30% in Korean native cows, 4.70% in dairy cows, and 0.15% in dairy bull. Fatty liver was enlarged, swollen with round edges, light weight, and pale to yellow-orange color, but its color was not always correlated to the severity of fatty liver. The findings of fat infiltration of the hepatic lobule were large droplets around central vein, fine droplets in the periphery, and fat infiltration in the perivascular region execpt for most of normal liver and severe fatty liver. The sensitivty, specificity, and accuracy of cytological finding compared with hepatic lipid content by buoyancy were 94.4%, 95.2%, and 94.9% in normal cases, 64.3%, 100%, and 87.2% in mild cases, 100%, 83.3%, and 87.2% in moderate cases, and 100%, 100%, and 100% in sesvere cases, respectively. Cytological findings were well correlated with histological findings. Complications of fine needle aspiration biopsy were not recognized clinically. Consequently, the cytodiagnosis by fine needle aspiration biopsy is simple, rapid, safe, and economical method compared with histological techniques in the diagnosis of fatty liver in cattle.
