The stress analysis of supporting tissues according to crown restorative materials in Brånemark osseointegrated implant (Brånemark 골유착성 매식체의 금관 수복재료에 따른 지지조직의 응력분석)

  • This study was to analyze the stress distribution of implant and supporting tissue in $Br{\aa}nemark$ osseointegration implant. The analysis has been conducted by using the axisymmetric finite element method and type of model according to crown material. Tests have been performed at 1 kg load on central fossa of crown portion. Each type of model was designed differently according to crown material. 1) Porcelain fused to metal crown(Model A) 2) Composite resin veneered crown(Model B) 3) Acrylic resin veneered crown(Model C) 4) Type III gold crown(Model D) The displacements and stresses of implant and supporting structures were analyzed to investigate the influence of the type of crown material. The results were obtained as follows : 1. Displacement of implant was shown uniformly downward displacement in all models and abutments were observed distally downward displacement. 2. In supporting tissues, stress was concentrated on the crest of compact bone and the spongy bone below implant. 3. The PFM and the type III gold crown showed the largest concentration of stress at the crest of compact bone and the spongy bone below implant, respectively. Acrylic resin artificial teeth and composite resin veneered crown indicated almost the same distribution of stress. 4. The gold screw, the abutment screw and the top of abutment showed the concentration of stress in implants of every model.

A case of removable dentures using digital method (디지털 방식을 이용하여 제작한 양악 가철성 의치 수복 증례)

  • Generally dentures are manufactured by conventional method, however the frequency of fabricating denture using digital method is increasing due to the recent development of digital technology in dentistry. The digital method of manufacturing denture is classified into two systems; 3D scan of the impression to arrange the artificial teeth on the CAD (Computer-aided design) and 3D printing to produce the resin-based complete denture, or 3D scan of the model to design of the framework using CAD, resin pattern formation by 3D printing and casting of metal framework of complete denture or removable partial denture. In this case report, electronic surveying and design the metal framework of the dentures were performed using CAD program, and plastic resin patterns fabricated by 3D printing were casted for upper full denture and lower removable partial denture. During follow-up periods, dentures using digital method have provided satisfactory results esthetically and functionally.

PALATELESS COMPLETE DENTURE FOR RESTORING GOOD TASTES : A CASE REPORT (미각 회복을 위한 무구개 의치(Palateless Complete Denture)의 임상증례)

  • The purpose of this article is to present a clinical case report for palateless complete denture. Despite the contravacy of palatal uncoverage in upper complete denture, palateless complete denture has some merits for upper edentulous patient. Following the uncovering of the palatal portion, the patient became easy to talk and restored the lost good tastes. He is happy despite of the decrease of the retention of the upper complete denture. Palateless complete denture is a compatible alternative fir upper edentulous patients in cases of gagging, large palatal torus and restoring the lost good tastes. The clinical points are as follows : 1. The remaning alveolar ridge should be ovoid and have enough width and height for the sup-port and retention. 2. The patient must have strong wish to the palateless complete denture. 3. Palatal beading made on the palatal peripheral border give good border sealing of the palatal flange and minimaized the prominence of the denture flange 4. The peripheral border of the palatal flange should be reduced as thin as possible for more comfort. 5. Upper artificial posterior teeth should be arranged over the alveolar ridge crest and inner incline of the buccal cusp relieved for denture stability while chewing. 6. For stability of palateless complete denture, bilateral balanced occlusion should be sttained. Palateless complete denture will restore the lost good tastes and more comfortable and physiologic to upper edentulous patients and a good alternative to full palatal coverage complete denture in the properly selected cases.

THE IRRIGATING EFFECT BEFORE AND AFTER CORONAL FLARING (Coronal flaring 전, 후의 근관세척효과)

  • The objective of this in vitro study was to evaluate the efficacy of a Ca(OH)$_2$removal before and after early coronal flaring using different types of instruments. 100 plastic blocks with 30$^{\circ}$artificial curved canals were used in this study and randomly divided into a control group and 4 experimental groups(GG, OS, GT, PT Group) 20 teeth each. The canals were instrumented, and Ca(OH)$_2$was temporary filled into the each canal. Irrigation was peformed with Max-i-Probe 25- 30-gauge probes before and after recapitulation. The results of this study were as follows 1. There were no significant difference among the groups in size of irrigating needle(p<0.05). 2. There was a significant difference between before and after recapitulation regardless size of irrigating needle(p<0.05). 3. Before recapitulation, there was a significant difference between 25- and 30-gauge needle in all groups(P<0.05). 4. After recapitulation, there was a significant difference between 25- and 30-gauge needle in the control group(p<0.05). But there were no significant difference among the experimental groups. It is concluded that the effectiveness of canal irrigation was decided to the depth of irrigating needle into the canal. The effect of canal irrigation tend to facilitate by the early coronal flaring. The recapitulation was the most effective during canal irrigation regardless the size of irrigating needle. Therefore, the recapitulation is a mandatory way to facilitate the effectiveness of canal irrigation during canal enlargement.

THE INFLUENCE OF pH ON REMINERALIZATION OF ARTIFICIAL DENTAL CARIES (법랑질 인공우식의 재광화에 미치는 pH의 영향에 관한 연구)

  • Much evidence now exists from both in vivo and in vitro studies to support the claim that small caries lesions can 'heal'. But, there are still different views on the mechanism of remineralization. So in order to find the best condition for the remineralization of incipient dental caries in maximum efficient way, the author conducted the experiment which reveals the effect of pH on remineralization. 40 specimens of sound permanent teeth without demineralization or crack, $100{\mu}m$ in thickness, were immersed in lactic acid buffered demineralization solution for 4 days. Dental caries with surface zone and subsurface lesion were artificially produced. All specimens were immersed in lactic acid buffered remineralization solution of pH 4.0, 4.5, 5.0, 5.5 containg fluoride ion for 10 days. The results were obtaind by observing the specimens for every 10 days under polarized microscopy at x25. 1. Remineralization did not occur in entire depth of body of lesion at given degree of saturation and concentration of fluoride ion. 2. The pattern of remineralization has increased according to increase of pH. So it can be concluded that supersaturated solution with fluoride ion can be affected by pH in remineralization of enamel, and pH 5.5 seems to be very effective in remineralization of deep and surface zone of dental caries. However, more complex factors exist in achieving complete remineralization and further continuous researches are needed to clarify the factors.

An Evaluation of Whitening Effect and Microhardness on the Enamel Surface by White Toothpaste (미백치약으로 처리된 치아의 미백효과와 미세경도 변화)

  • The aim of this study was to evaluate the efficacy of bleached tooth with white toothpaste on the whitening effect, microhardness test. Forty-four human interior teeth were randomly assigned to two groups as follows: Group 1 was used general toothpaste as control, Group 2 exposed to white toothpaste at 3 times a day for 3 minutes. The total application time was 4 weeks. After each application, all tooth were washed with distilled water and immersed in artificial saliva. The color change and microhardness were measured using CIELab Color system and microhardness tester. The data were analyzed by student t-test, using SPSS 20.0 statistical computer program (SPSS Inc., USA). The overall color changes (${\Delta}E$) by white toothpaste have a capability of the whitening efficacy by factor of 2.3 times larger compared with general toothpaste. Moreover, the application of white toothpaste did not cause any microhardness reduction on enamel surface.

Accuracy evaluation of resin complete denture made with glass fiber mesh reinforcement before and after curing (유리섬유 보강재로 제작한 레진의치의 온성 전·후에 따른 정확성 평가)

  • Purpose: The aim of this study was to evaluate accuracy of glass fiber mesh complete denture of before and after curing. Methods: Edentulous model was selected as the master model. Ten study models were made by Type IV stone. Wax complete dentures were produced by the denture base and artificial teeth. CD and GD groups were measured six measurement distance before curing. The wax complete denture was investment after measurement is completed. Using a heat polymerization resin was injected resin. After injecting the resin it was curing. A complete denture was re-measured after curing. The measured data was verified by paired t-test. Results: Overall CD group was larger the value of the measured length. In the CD group, A-D point was larger. The smallest point was the B-D point. However, there was no statistically significant difference only C-D point(p>0.05). In the GD group, A-B point was larger. but B-D point was the smallest. A-D and B-C statistically points showed significant differences(p<0.05). Conclusion: Glass fiber mesh resin complete denture can be clinically applied to the edentulous patient.

Evaluation of diagnostic ability of CCD digital radiography in the detection of incipient dental caries (CCD 디지털 방사선사진촬영법의 초기 치아우식증의 진단능 평가에 대한 연구)

  • Purpose : The purpose of this experiment was to evaluate the diagnostic ability of a CCD-based digital system (CDX-2000HQ) in the detection of incipient dental caries. Materials and Methods : 93 extracted human teeth with sound proximal surfaces and interproximal artificial cavities were radiographed using 4 imaging methods. Automatically processed No.2 Insight film (Eastman Kodak Co., U.S.A.) was used for conventional radiography, scanned images of conventional radiograms for indirect digital radiography were used. For the direct digital radiography, the CDX-2000HQ CCD system (Biomedisys Co. Korea) was used. The subtraction images were made from two direct digital images by Sunny program in the CDX-2000HQ system. Two radiologists and three endodontists examined the presence of lesions using a five-point confidence scale and compared the diagnostic ability by ROC (Receiver Operating Characteristic) analysis and one way ANOV A test. Results: The mean ROC areas of conventional radiography, indirect digital radiography, direct digital radiography, and digital subtraction radiography were 0.9093, 0.9102, 0.9184, and 0.9056, respectively. The diagnostic ability of direct digital radiography was better than the other imaging modalities, but there were no statistical differences among these imaging modalities (p > 0.05). Coclusion : These results indicate that new CCD-based digital systems (CDX-2000HQ) have the potential to serve as an alternative to conventional radiography in the detection of incipient dental caries.

EROSION OF TOOTH ENAMEL BY ACIDIC DRINKS AND REMINERALIZATION BY ARTIFICIAL SALIVA (산성 음료에 의한 법랑질의 침식과 인공타액에 의한 재광화)

  • The purpose of study was to assess the influence of acidic drinks on the erosion of tooth enamel and the effect of fluoridated saliva on the remineralization. Twenty five drinks were sampled. The erosive potential and remineralizing effect were measured by the tooth surface microhardness test. The pH of most drinks were below pH 5.5. Reduction rates of enamel surface hardness by the flavored carbonated drink were 16.90%, 25.11%, 35.10%, and 41.62% after 5, 10, 30, and 60 minutes of demineralization, and recovery rates by remineralizaing solution were 61.52%, 67.96%, 72.13% and 75.93% after 1, 24, 48, and 72 hours of remineralization, respectively. The results suggest that the most drinks in the markets have the potential to erode the teeth and that erosion occurs fast but remineralization proceeds slowly.

Study and Effects of Bone Conducted Signal on the Implantable Microphone (골전도를 통한 생체신호가 이식형 마이크로폰에 미치는 영향 및 고찰)

  • The fully implantable hearing devices (FIHDs) have been studied to compensate the defect of conventional hearing aids. Typically, a microphone for FIHDs was implanted under the skin of the temporal bone. So, implantable microphone characteristics can be affected by the eating food, chattering teeth and moving artifact. In this paper, we fabricated the physical model that was similar to characteristics of human temporal bone and skin, and we measured implanted microphone sensitivity for effect of bone conducted noise signal. For the measurement of microphone sensitivity, we applied 1 kHz pure sounds that were transmitted to implanted microphone and sine wave vibrations of varied frequency were simultaneously transmitted through the artificial bone. As a result, sensitivity of implanted microphone can be modified by bone conducted signal and this phenomenon was confirmed at varied frequency band.
