총상으로 인한 악안면 결손을 가진 환자에 대한 가철성 보철물 수복증례 (Removable prosthetic rehabilitation in patient with maxillofacial defects caused by gunshot: A case report)

  • 악안면부의 결손은 선천적으로 결손을 가지고 있거나 외상이나 수술적인 절제 등에 의한 후천적 원인에 의해 발생할 수 있다. 특히 구강 내 결손을 가진 환자 중 상악의 결손과 연관되어 있는 경우가 높은 비중을 차지하고 있으며 보철적 치료의 필요성이 높다. 하악의 부분적 결손을 가질 경우 기능적 회복에 상당한 한계점을 보이나 양측 턱관절이 정상적으로 남아 있을 경우 국소의치를 이용한 악안면 보철물을 제작함으로써 양호한 결과를 기대할 수 있다. 본 증례의 환자는 58세 남자 환자로서 안면부 총상으로 인한 구개부 및 좌측 하악 구치부 결손을 가진 분으로 obturator와 하악 RPD 재제작을 위해 내원하였다. 환자의 상악 결손 범위는 Aramany 분류법 Class IV에 해당되는 상태이며, 하악은 Cantor와 Curtis 분류법 Type V resection 상태이다. 상, 하악의 우측 구치부가 잔존하였으나, 수술 후 악골 변형으로 인해 서로 교합되지 않았으며 치주상태 불량으로 인한 동요도도 존재하는 상태였다. Obturator의 유지를 위해 잔존 지대치를 최대한 활용하였고 안정적인 교합이 형성되도록 치아를 배열하였다. 하악 RPD는 하악 절제술로 변형된 연조직에 적합하도록 제한된 범위를 피개하는 RPD를 제작하였다. 이에 환자분의 저작, 연하, 발음 기능이 많이 개선되었으며 만족할 만한 결과를 보여주어 본 증례를 발표하는 바이다.

  • 상악의 복합조직 결손이 크게 있을 경우에 그를 재건하는데는 여러가지 방법이 있지만, 본 영남대학교 의과대학 성형외과학 교실에서는 최근 경험한 2예의 상악부 복합조직결손을 재건함에 있어서 광배근 근피 유리피판술을 이용하였던 바, 각각 16개월 및 2개월의 추적에 있어서 별다른 기능적 이상없이 만족할 만한 결과를 보았다.

  • In the present study, rotating bending fatigue tests have been carried out in three kinds of carbon steel specimens; an annealed low carbon steel, an annealed high carbon steel and quenched-tempered high carbon steel; with a small artificial surface defect that might exist in real structures. Fatigue crack lengths have been observed by a method of replication in order to investigate the growth characteristic of fatigue crack in the viewpoints of strength of materials and fracture mechanics. The main results obtained are as follows: 1) The effect of a small surface defect upon the reduction of fatigue limit is considerably large, and the rate of fatigue limit reduction grows in the following order; annealed low carbon steel(mild steel), annealed high carbon steel, quenched-tempered high carbon steel. 2) When the growth rate of surface crack length(2a) was investigated in the viewpoints of fracture mechanics based upon $ ${\Delta}K_{\varepsilon}$, the dependence of stress level and of surface defect size disappear, and there exists a linear relationships between d(2a)/dN and ${\Delta}K_{{\varepsilon}t},\;\Delta_{{\varepsilon}t}\sqrt{{\pi}a}$, on log. plot, i.e, $d(2a)/dN={C{\cdot}{\Delta}K_{\varepsilon}}^3_t$, where ${\Delta}_{{\varepsilon}t}\sqrt{{\pi}a}$ a is the cyclic total strain intensity factor range.

중탄소강의 피로크랙 개폐구의 특성 및 피로강도의 평가 (Evaluation of Fatigue Strength and Characteristics of Fatigue Crack Closure in SM35C Steel)

  • It is not clearly known how surface defects or inclusions of a medium carbon steel affect a fatigue strength. In this study, we used SM35C specimens with spheroidized cementite structure to eliminate dependence of micro structure of fatigue crack. The investigation was carried out by behavior of crack closure at non-propagation crack and effect of the fatigue limit according to the artificial defects size. Experimental findings are obtained as follows : (1) Fatigue crack initiation point of medium carbon steel with spheroidized cementite structure is at the surface defects. (2) Non-propagating crack length of smooth specimen is equal to the critical size of defect. (3) Considering the opening and closure behavior of fatigue crack, the defect shape results in various crack opening displacement, while it does not affects the fatigue limit level of medium carbon steel with spheroidized cementite structure. (4) The critical length of the non-propagation crack of smooth specimen is the same as critical size of defect in transient area which determines threshold condition in steel with spheroidized cementite structure.

하지 방사통과 심한 골다공증을 동반하는 척추전전위증을 위한 척추경 부분절제술 (Partial Pediculectomy for Spondylolisthesis with Radicular Pain Combined with Severe Osteoporosis)

  • Objective : For relief of back pain related to spondylolisthesis, fusion with or without internal fixation may be necessary. Currently, bilateral wide posterior decompression and fusion with or without internal fixation are used for treatment of spondylolisthesis. In case of severe osteoporosis, discectomy may be done without fusion to decompress the nerve roots. However, the procedure may aggravate radicular symptom because slippage may increase and thus foramen may become narrower. In these settings, inferomedial partial pediculectomy has been done in our institution, The object of this study was to evaltuate such cases with regard to per- and postoperative clinical courses. Patients and Methods : Fifteen patients complaining radicular pain with mild or no back pain underwent inferomedial partial pediculectomy. It was done in lytic spondylolisthesis after extension of isthmic defect. In degenerative spondylolisthesis of L4-5 interspace, intertransverse pediculectomy was done via paraspinal approach not to injure the posterior segment with intact isthmus. But, in degenerative spondylolisthesis of L5-S1 interspace however, partial pediculectomy was done after making artificial isthmic defect via midline approach. Then, reconstructive laminoplasty of L5 was performed to avoid iatrogenic instability resulted from artificial isthmic defect. They were followed up for average 14.4 months(3-31 months). Results : Radiating pain was relieved in all cases(100%). Postoperative result was excellent in 7 cases(46.7%), good in 7(46.7%) and fair in 1(6.6%). Only 1 case(6.7%) showed increase in slippage during follow-up period. Conclusion : Inferomedial partial pediculectomy is considered as a useful procedure for severely osteoporotic patients with spondylolisthesis with regards to keeping stability and decompressing the nerve roots.

Application of SA-SVM Incremental Algorithm in GIS PD Pattern Recognition

  • With changes in insulated defects, the environment, and so on, new partial discharge (PD) data are highly different from the original samples. It leads to a decrease in on-line recognition rate. The UHF signal and pulse current signal of four kinds of typical artificial defect models in gas insulated switchgear (GIS) are obtained simultaneously by experiment. The relationship map of ultra-high frequency (UHF) cumulative energy and its corresponding apparent discharge of four kinds of typical artificial defect models are plotted. UHF cumulative energy and its corresponding apparent discharge are used as inputs. The support vector machine (SVM) incremental method is constructed. Examples show that the PD SVM incremental method based on simulated annealing (SA) effectively speeds up the data update rate and improves the adaptability of the classifier compared with the original method, in that the total sample is constituted by the old and new data. The PD SVM incremental method is a better pattern recognition technology for PD on-line monitoring.

  • These days, citizens have made change of food life to take Western style food and to suffer from diabetes because of excessive nutrition taking, less exercise, stress and other environmental factors. They may suffer from diabetes because of genetic defect, surgery of pancreas, disinfection and medicine and others. One of ten Koreans may have symptom of diabetes to be popular. The diabetes that is a kind of metabolic disease has high blood sugar at disorder of hyper insulinism and/or defect of insulin action. Long time high blood sugar may produce chronic disease of kidney, eyes, nerve, heart and blood vessel and others. The purpose of health care of diabetes patient was to reach target blood sugar by diet, physical exercise and medicine and to prevent and delay complication. Diabetes patient shall control blood sugar to keep healthy. The blood sugar control requires time and effort, and all of the patients are difficult to make effort and to spend time. You can control blood sugar by the application. The application allows patients to control blood sugar and to save time and efforts and to make small sized input and automation of remaining area. The service was limited to blood sugar graph, and user carries smart phone to conduct test and to have difficulty. Further study needs to solve the problems and to investigate blood sugar testing not carrying smart phone and to make application of easy control of blood sugar.

여러가지 뉴럴네트웍 기법을 적용한 부분방전 패턴인식 비교 (Comparison of Various Neural Network Methods for Partial Discharge Pattern Recognition)

  • This study deals with various neural network algorithms for the on-site partial discharge pattern recognition. For the purpose, the pattern recognition has been carried out on partial discharge data for the typical artificial defect using 9 different neural network models. In order to enhance on-site applicability, artificial defects were installed in the insulation joint box of extra-high voltage xLPE cables and partial discharges were measured by use of the metal foil sensor and a HFCT as a sensor. As the result, it is found out that the accuracy of pattern recognition could be enhanced through the application of the Sigmoid function, the Momentum algorithm and the Genetic algorism on the artificial neural networks. Although Multilayer Perceptron (MLP) algorism showed the best result among 9 neural network algorisms, it is thought that more researches on others would be needed in consideration of on-site application.

A Brachial Artery Pseudoaneurysm Treated with a Bifurcated Y-Shaped Artificial Vessel Graft

  • Brachial artery aneurysms are rare diseases that may be caused by infection or trauma. We report a case of a 71-year-old man who presented with a mass in his right antecubital fossa that increased in size slowly over time. Three years ago, the patient underwent ascending and total-arch replacement with artificial vessel graft to treat aortic root and ascending aorta aneurysm. Preoperative physical examination of right upper extremity showed a nonpulsatile mass with normal pulse of axillary, brachial, and radial arteries. The mass was removed and brachial artery reconstruction was done initially using saphenous vein graft. Two months later, the patient revisited with recurrent pseudoaneurysm, involving the bifurcation point of brachial artery. Aneurysm was totally resected and the brachial artery was reconstructed by interposition graft using a bifurcated GORE-TEX artificial vessel graft. The patient healed without complication and no recurrence was observed. Artificial vessel graft is an available option for reconstruction, and revascularization of vessel defect after excision of brachial artery aneurysm may involve bifurcation point.
