Speech Stimuli on the Diagnostic Evaluation of Speech with Cleft Lip and Palate : Clinical Use and Literature Review (구개열 환자 말 평가 시 검사어에 대한 고찰 : 임상현장의 말 평가 어음자료와 문헌적 고찰을 중심으로)

  • Differential diagnosis of articulation and resonance problems in the cleft lip and palate speech is required for evaluating various factors contribute to speech problems such as VPI, dental occlusion, palatal fistulae, learning. However, validity of speech stimuli is current issue to evaluate accurately each problem in cleft speech. This study was conducted to investigate speech stimuli using in the clinical setting and review the literatures and articles published 1990 to 2005 for helping develop standardized speech samples. The results were recommendation to evaluate properly velopharyngeal function when conducting a diagnostic evaluation as follows : 1) In identification hypernasality, the speech stimuli should be included low pressure consonants to eliminate effects of nasal emission, compensatory articulation. 2) Speech stimuli should be consist of visual, front sounds to eliminate compensatory articulation and to stimulate easily. 3) Regarding early diagnosis and treatment, speech stimuli need to develop for infants and preschooler. 4) Stimulus length on nasalance scores should be at least 6 syllables. 5) In phonetic context on nasalance scores, /i/ vowel should be take into consideration excluding paragraph. 6) Connected speech stimuli should be developed for evaluating intelligibility and VP function.

Speech Improvement of the Patients Performed Primary Palatal Repair over 4 Years Old (4세 이후에 구개성형술을 시행받은 환자의 발음개선)

  • Time to time, we face patients who missed the proper time for primary palatal repair. Although we do not have enough available documents, it is important to establish efficacy of palatal repair in patients more than 4 years old. From May 1995 to March 2005, we selected 14 patients who underwent palatal repair in more than 4 years old patients and they are able to tolerate speech articulation tests. Out of 14 patients 5 males an 9 females in sex, aged form 4 to 50 years old. 6 patients with incomplete cleft palate and 8 patients with submucous cleft palate. Double reversing Z-plasty(n=5), pushback palatoplasty(n=4), two flap palatoplasty(n=2), von Langenbeck palatoplasty(n=2), and intravelar veloplasty(n=1) were performed. Preoperative and postoperative speech articulation test, "Simple method of speech evaluation in Korean patients with cleft palate", were conducted. Satisfaction rate was sorted into 5 levels. There is no significant statistical correlation in the speech improvement, satisfaction rate, patients sex, cleft type and operative method. But there is significant statistical correlation between the speech improvement and patienet's age. There were better result in younger patient group than aged patients group.

The Acoustic Characteristics of Articulation and Phonation in Peritonsillar Abscess (편도외 농양 환자의 발화시 조음 및 음성의 변화)

  • Background and Objectives: The voice changes can occur in peritonsillar abscess and the labeling of this changes as a "muffled voice". The aim of this study was to investigate the changes in acoustic feature of voice before and after treatment in patients with peritonsillar abscess. Materials and Method: 12 patients with peritonsillar abscess were enrolled in the study. Acoustic analysis on sustained Korean vowels /a/, /i/ and /u/ were performed before and after treatment. Results: In patients with peritonsillar abscess, the first formant frequency (F1) and second formant frequency (F2) of /a/ were decreased. There was tendency of articulation of back-low vowel /a/ as back-high vowel /u/. F1 of /i/ and /u/ were increased, while F2 were decreased. There was tendency of articulation of front-high vowel /i/ as back-low vowel /a/. The third, forth, fifth formant frequency (F3, F4, F5) of /a/, /i/ and /u/ were decreased although statistically not significant. Conclusion: The anatomical and functional changes of oropharynx by peritonsillar abscess can cause changes in resonance and speech quality. We suggest that these changes could be the cause of 'muffled voice' in patients of peritonsillar abscess.

The Effects Where the Stroke Shoes Which Use Functional Electric Stimulation Goes Mad to Walking of the Hemiplegia (기능적 전기자극 치료기를 이용한 중풍구두가 편마비 환자의 보행에 미치는 영향)

  • Purpose: An objective analysis and observations were to be done on hemiplegia patients that are wearing a walking support device, Stroke shoes. Their improvements in walking pace, the reduction of distance between the two knee joint, the increase of curve angle of the knee joint and their steps and the reduction of ankle joint upon swing phase were analyzed using a 20 walking analyzer. Methods: An examination was carried out to see the patients' communication skill and independent walking and then let them walk with the Stroke shoes on to get results before and after wearing it. Simi Reality Motion Systems GmbH (Germany, 2007) was used to analyze the results regarding knee joint and ankle joint angle changes of sagitta plane and coronal plane, stepping distances, distances between the knees and walking pace. Results: 1. The articulation angle of ankle joint during swing phase decreased and knee joint has shown a statistically significant increase in such value(p<0.05). 2. Only knee joint showed a significant increase in articulation angle during heel strike(p<0.05). 3. Knee joint showed a significant increase in articulation angle during toe off(p<0.05). 4. The distance between the two knees as well as their foot steps significantly decreased compared with when Stroke shoes were not worn(p<0.05). 5. Stroke shoes with FES have shown positive effects on the patients in improving their walking styles overall. (p<0.05). Conclusion: There was an improvement in rotation walking pattern by a reduction in the distance between the knees after wearing Stroke shoes with FES. Plantar flexion reduced that occurred in ankle joint during walking and flexion angle increased in knee joint, both of which improved foot drop which was a major problem in hemiplegia patients. Also it is believed that the device will have some positive influences on knee joint stiffening paralysis to aid in improving inefficient walking phases.

Effects of Listener's Experience, Severity of Speaker's Articulation, and Linguistic Cues on Speech Intelligibility in Congenitally Deafened Adults with Cochlear Implants (청자의 경험, 화자의 조음 중증도, 단서 유형이 인공와우이식 선천성 농 성인의 말명료도에 미치는 영향)

  • The current study investigated the effects of experience of deaf speech, severity of speaker's articulation, and linguistic cues on speech intelligibility of congenitally deafened adults with cochlear implants. Speech intelligibility was judged by 28 experienced listeners and 40 inexperienced listeners using a word transcription task. A three-way (2 $\times$ 2 $\times$ 4) mixed design was used with the experience of deaf speech (experienced/inexperienced listener) as a between-subject factor, the severity of speaker's articulation (mild to moderate/moderate to severe), and linguistic cues (no/phonetic/semantic/combined) as within-subject factors. The dependent measure was the number of correctly transcribed words. Results revealed that three main effects were statistically significant. Experienced listeners showed better performance on the transcription than inexperienced listeners, and listeners were better in transcribing speakers who were mild to moderate than moderate to severe. There were significant differences in speech intelligibility among the four different types of cues, showing that the combined cues provided the greatest enhancement of the intelligibility scores (combined > semantic > phonological > no). Three two-way interactions were statistically significant, indicating that the type of cues and severity of speakers differentiated experienced listeners from inexperienced listeners. The current results suggested that the use of a combination of linguistic cues increased the speech intelligibility of congenitally deafened adults with cochlear implants, and the experience of deaf speech was critical especially in evaluating speech intelligibility of severe speakers compared to that of mild speakers.

  • The purposes of this study were to investigate the characteristics of speech rate and pause in patients with flaccid dysarthria. For this purpose, 15 patients with flaccid dysarthria and 15 normal speakers matched in gender and age participated as subjects. The overall speech rate, the articulation rate, the inter-sentence and the intra-sentence pause duration and pause frequency were measured during reading the standardized passage "Autumn"(Kim, 1996). As a result, the overall speech rate and articulation rate of patients with flaccid dysarthria were significantly slower than normal speakers and intra-sentence pause duration and frequency of patients with flaccid dysarthria were significantly higher than normal speakers, but those of inter-sentence weren't. The results from this study provides the speech rate index of flaccid dysarthria and indicates that to control the speech rate of flaccid dysarthria have regard to not only the overall speech rate and the articulation rate but also the intra-sentence pause duration and the frequency.

  • Objectives This study aimed to report the effectiveness of abdominal Daoyin and lymphatic circulation technique in conjunction with whole-body articulation mechanics technique therapy in treating indigestion. Methods Three patients who suffered from dyspepsia and abscess after consuming foods incompatible with their constitution, following a 10-year implementation of a constitutional diet at a local Korean Medicine clinic, visited a Korean Medicine Hospital. The patients underwent treatment with abdominal Daoyin and lymphatic circulation technique. The effects of these techniques were analyzed using a retrospective record analysis method. Results Following the implementation of abdominal Daoyin and lymphatic circulation technique for indigestion, the numeric rating scale scores improved significantly. Case 1 improved from 9 to 2, case 2 from 6 to 0, and case 3 from 5 to 1. Conclusions While acknowledging the limitations of being a case report involving only three patients and the absence of specialized outcomes for digestive issues, this report may provide a preliminary basis for considering the efficacy of Chuna manual therapy in indigestion treatment.

  • Recently, due to the saturation of ground structures and the overpopulation of pipeline facilities requires to development of underground structures as an alternative to ground structures. Thus, mechanized tunnel construction of the shield TBM method has been increasing in order to prevent vibration and noise problems in construction of the NATM tunnel for the urban infrastructure construction. Tunnel construction plan for the tunnel line should be formed in a sharp curve to avoid building foundation and underground structures and it is inevitable to develop a shield TBM technology that suits the sharp curve tunnel construction. Therefore, this study is about the structural stability technology of the articulation jack, shield jack and skin plate for the shield TBM thrust in case of the mechanized tunnel construction that is a straight and sharp curve line. The construction case study and shield TBM operation principle are examined and analyzed by the theoretical approach. The torque of the cutter head, the thrust of the articulation jack and the shield jack, the amount of over cutting for curve is important respectively in shield TBM construction of straight and sharp curve line. In addition, it is very important to secure the stability of the skin plate structure to ensure the safety of the inside worker. This study examines the general structure and construction of the equipment, experimental simulation was carried out through numerical analysis to examine the main factors and structural stability of the skin plate structure. The structural stability of the skin plate was evaluated and optimizes the shape by comparing the loads of the articulation jack by selecting the virtual soil to be applied in a straight and sharp curve line construction. Since the present structure and operation method of the shield TBM type in domestic constructions are very similar, this study will help to develop the localized shield TBM technology for the new equipment and the vulnerability and stability review.

  • Multi-Response Surface Optimization aims at finding the optimal setting of input variables considering multiple responses simultaneously. The Weighted Mean Squared Error (WMSE) minimization approach, which imposes a different weight on the two components of mean squared error, squared bias and variance, first obtains WMSE for each response and then minimizes all the WMSEs at once. Most of the methods proposed for the WMSE minimization approach to date are classified into the prior preference articulation approach, which requires that a decision maker (DM) provides his/her preference information a priori. However, it is quite difficult for the DM to provide such information in advance, because he/she cannot experience the relationships or conflicts among the responses. To overcome this limitation, this paper proposes a posterior preference articulation method to the WMSE minimization approach. The proposed method first generates all (or most) of the nondominated solutions without the DM's preference information. Then, the DM selects the best one from the set of nondominated solutions a posteriori. Its advantage is that it provides an opportunity for the DM to understand the tradeoffs in the entire set of nondominated solutions and effectively obtains the most preferred solution suitable for his/her preference structure.

Phonetic analysis of Korean elementary students who had overseas study at early ages (조기 유학 후 귀국한 초등학생의 발음 이상에 대한 음성학적 연구)

  • Purpose : The number of the repatriated Korean students who had overseas study at early ages is increasing. They received foreign education, they can speak international languages, but they have many difficulties in articulation and intonation of the Korean language. This study aims to measure closure and aspiration duration, length of consonants, length of subsequent vowels, and ratio of consonants against subsequent vowels in vowel-consonant-vowel (VCV) syllables. Methods : This study compares the acoustic and phonetic characteristics of repatriated and native students, the ratio of articulation error of Korean plosives, the closure and aspiration duration, and the ratio of the aspiration duration against the closure duration. Results : The ratio of articulation error of Korean plosives between repatriated and native students is 19% and 2%, respectively. The closure duration was significantly longer in repatriated students than in native students. The aspiration duration was significantly longer in repatriated students than in native students. No difference was found in the ratio of aspiration duration against closure duration between the native and repatriated students. Conclusion : This study can be a good reference for estimating the phonetic difficulties of Korean elementary students who had overseas study at early ages.
