한국 노인의 활동제한 유병율 (Prevalence of Activity Limitation in Korea's Older Adults Population)

  • 본 연구는 우리나라 65세 이상 노인인구의 활동제한 유병율, 활동제한 사유, 일반적 특성과 활동제한 유병율과의 관련성 등을 확인하기 위한 자료분석 연구이다. 본 연구자료는 우리나라 질병관리본부에서 수행한 국민건강영양조사 제7기 1차년도 (2016년) 원시자료로부터 추출되었다. 본 연구 대상자는 2016년 1월부터 12월까지 수행된 국민건강영양조사에 참여한 대상자 중 만 65세 이상의 노인인구 1,578명 이었다. 수집된 자료는 SPSS/WIN 8.0 version 프로그램을 이용하여 빈도분석, 교차분석 및 로지스틱 회귀분석 등으로 분석하였다. 2016년도 우리나라 65세 이상 노인인구의 활동제한 유병율은 남성이 17.3%, 여성이 21.1%, 전체는 19.6% 이었다. 활동제한 사유는 등과 목의 문제(19.8%)와 관절염 및 류마티즘(17.1%)이 가장 높게 나타났고, 활동제한 유병율은 연령 및 교육수준과 통계적으로 유의한 관련성이 있었다. 연령에 따른 활동제한 유병율은 65-69세 그룹에 비해 70-74세 그룹은 1.33배(OR 1.33; 95% CI 0.89-2.00, p<0.05), 75-80세 그룹은 1.69배(OR 1.69, 95% CI 1.34-2.50, p<0.05)가 더 높게 나타났다. 교육수준에 따른 활동제한 유병율은 대학졸업 대상자에 비해 초등학교 졸업이하 대상자가 2.30배가 더 높게 나타났다 (OR 2.30; 95% CI 1.28-4.14, p<0.05). 결론적으로 우리나라 65세 이상 노인인구의 활동제한 유병율에 대한 주요 영향요인은 연령, 교육수준, 그리고 등과 목의 문제와 관절염 및 류마티즘 등과 같은 근골격계 질환이었다. 이러한 결과는 우리나라 65세 이상 노인인구의 활동제한 유병율의 감소를 위한 근거 자료로 활용될 수 있을 것으로 사료된다.

지역사회건강조사 지표를 이용한 고지혈증 유병율의 지역 간 변이와 위험 요인의 융복합적 분석 (Convergence analysis for geographic variations and risk factors in the prevalence of hyperlipidemia using measures of Korean Community Health Survey)

  • 본 연구는 고지혈증 유병률의 지역 간 변이 정도와 위험 요인을 규명하여 지역별 특성에 맞는 고지혈증 관리 사업을 지원하기 위한 기초자료를 제공하기 위해 수행되었다. 이를 위해 질병관리본부의 2012년도 시군구 지역사회건강조사 249건의 자료를 이용하여 단순 상관관계 분석, 단계적 회귀분석, 의사결정나무 등의 기법으로 분석하였다. 249개 시군구 지역의 고지혈증 유병률은 9.2%였고, 변동계수는 28.3%였다. 남동부 해안지역에 비해 수도권과 내륙지방의 고지혈증 유병률이 높았다. 의사결정나무 모형이 회귀모형에 비해 예측력이 좋았는데, 지역의 임금근로자 비율, 스트레스 인지율, 고혈압, 협심증, 관절염 유병률이 높은 지역일수록 고지혈증 유병률이 높은 것으로 나타났다. 따라서 사회 역학적 관점에서 지역사회의 개입이 가능한 지점을 중심으로 고지혈증 유병률을 감소시키기 위한 전략 마련이 필요하다.

  • 자해 행동으로 인하여 하순이 심하게 손상된 Lesch-Nyhan 증후군 환아를 대상으로 소아 정신과와의 협진을 통한 약물 요법과 soft mouthguard를 이용한 보존적 치료로 단기간에 만족할 만한 결과를 보였다. 결론적으로 Lesch-Nyhan 증후군 환아의 자해를 예방하는 표준화된 방법이 있는 것은 아니지만 발치나 교정적 수술 등의 침습적인 방법 이전에 약물치료와 장치물 치료 등의 보존적 치료를 통한 접근이 먼저 있어야 한다. 그리고, 보존적 치료의 성공을 위해서는 정신과 전문의와의 협진이 요구되며 환자에 대한 보호자의 적극적인 관심과 구강 위생 관리가 반드시 필요하다.

노봉방(露峰房)의 Peroxynitrite 제거 효과 (Peroxynitrite Scavenging Activity of Vespae Nidus)

  • Objectives : Peroxynitrite($ONOO^-$), superoxide anion(${\cdot}{O_2}^-$) and nitric oxide (NO) is a cytotoxic species that can oxidize several cellular components such as proteins, lipids and DNA. It has been implicated in the aging process and age-related disease such as Alzheimer's disease, rheumatoid arthritis, cancer and atherosclerosis. The aim of this study was to investigate scavenging activities for $ONOO^-$ and its precursors, NO and ${\cdot}{O_2}^-$ of Vespae Nidus. Methods : Dichlorodihydrofluorescein diacetate (DCFDA), 4,5-diaminofluorescein (DAF-2) and dihydrorhodamine 123 (DHR 123) were used to investigate scavenging activities of $ONOO^-,\;NO,\;{\cdot}{O_2}^-$. Six-months-old ICR mice were used. After mice were injected with lipopolysaccharides(LPS), kidney organization was evaluated. Three comparison groups of ICR mice were used : a normal group, an experimental group that was fed Vespae Nidus extract and then injected with LPS, and a control group that was injected with LPS. Scavenging activities of $ONOO^-,\;NO,\;{\cdot}{O_2}^-$ in these groups were measured in the same way. Results : Vespae Nidus markedly scavenged authentic $ONOO^-,\;{\cdot}{O_2}^-$ and NO. It also inhibited $ONOO^-$ induced by ${\cdot}{O_2}^-$ and NO which are derived trom SIN-1. Furthermore, it inhibited $ONOO^-,\;{\cdot}{O_2}^-$, and NO generation by Vespae Nidus in LPS-treated ICR mouse kidney postmitochondria. Conclusions : These results suggest that Vespae Nidus might be developed as an effective $ONOO^-,\;{\cdot}{O_2}^-$, and NO scavenger for the prevention of the aging process and age-related diseases.

  • 견봉-쇄골 관절 낭종은 비교적 드물게 발생되나, 대부분의 경우 만성적인 회전근 개 질환과 퇴행성 견봉-쇄골관절염에서 동반된다. 이러한 병변에 대한 임상적 해석, 치료적 접근 및 치료 후 합병증을 최소화하기 위해서는 견관절 질환과의 병태생리학적 기전을 이해하는 것이 필수적이라 할 수 있다. 저자들은 단순 흡입술 후 재발되는 견봉-쇄골 낭종에 대한 추가적인 단순 절제술 이후 재발성 무균성의 누공이 합병된 증례와 함께 초음파 검사상 누공을 가진 낭종성 병변과 동반된 광범위한 회전근 개의 파열을 진단하였던 예를 경험하였기에 보고하고자 하며, 문헌고찰을 통하여 견봉-쇄골 관절 낭종의 발생 기전을 고려한 임상적 접근 및 치료적 방법들을 재조명하고자 한다.

Omega 3 fatty acids as a host modulator in chronic periodontitis patients: a randomised, double-blind, palcebo-controlled, clinical trial

  • Purpose: Periodontitis is an infectious disease caused predominantly by gram-negative anerobes. The host inflammatory response to these bacteria causes alveolar bone loss that is characterized as periodontitis. Omega-3 fatty acids (${\omega}$-3 FAs) have anti-inflammatory properties, thus have been used to treat some chronic inflammatory diseases such as cardiovascular disease and rheumatoid arthritis. We aimed to evaluate the effect of dietary supplementation with ${\omega}$-3 FAs as a host modulating agent in patients with chronic periodontitis. Methods: Sixty otherwise healthy subjects with moderate and severe chronic periodontitis were enrolled in our randomised, double-blind, placebo-controlled trial. The control group (CG, n=30) was treated with scaling and root planing (SRP) and given a placebo; the treatment group (TG, n=30) was treated with SRP and dietary supplementation of ${\omega}$-3 FAs (one 300 mg tablet daily for 12 weeks). Periodontal clinical parameters and serum C-reactive protein (CRP) levels were evaluated in all patients at baseline, a 6-week and 12-week period after treatment. Results: A significant reduction in the gingival index, sulcus bleeding index, pocket depth, and clinical attachment level was found in the TG compared to the CG at a 12-week period. However, no statistically significant changes in serum CRP levels were found. Conclusions: Our findings suggest that ${\omega}$-3 FAs can successfully reduce gingival inflammation, pocket depth, and attachment level gain. Dietary supplementation with ${\omega}$-3 FAs may have potential benefits as a host modulatory agent in the prevention and/or C management of chronic periodontitis.

생강(生薑)의 Peroxynitrite 제거 기전 (Peroxynitrite Scavenging Mechanism of Zingiberis Rhizoma)

  • Objectives : Peroxynitrite($ONOO^-$), formed from the reaction of $O2^-$ and NO, is a cytotoxic species that can oxidize several cellular components such as proteins, lipids and DNA. It has been involved in the aging process and age-related diseases such as Alzheimer's disease, rheumatoid arthritis, cancer and atherosclerosis. The aim of this study was to investigate scavenging activities of $ONOO^-$ and its precursors, NO and $O_{2^-}$ and its scavenging mechanism of Zingiberis Rhizoma (ZR). Methods : To investigate scavenging activities of $ONOO^-,\;NO,\;O_{2^-}$ and its scavenging mechanism, we used fluorescent probes like DCFDA, DAF-2 and DHR 123. The $ONOO^-$ scavenging activity on ZR was assayed by measuring oxidized dihydrorhodamine 123 (DHR 123) by fluorometry. The scavenging efficacy was expressed as IC50, showing the concentration of each sample that is required to cause 50% inhibition of DHR 123 oxidation. In a separate study, the protective effect of ZR on $ONOO^-$-induced nitration of bovine serum albumin was investigated through immuno-assay with a monoclonal anti-nitryrosine antibody, and a horseradish peroxidase-conjugated anti-mouse secondary antibody from sheep. Results : ZR markedly scavenged authentic $ONOO^-,\;O_{2^-}$ and NO. It also inhibited $ONOO^-$ induced by $O_{2^-}$ and NO which are derived from SIN-1. The data demonstrated that ZR led to decreased $ONOO^-$ mediated nitration of tyrosine through electron donation. It also inhibited the nitration of bovine serum albumin induced by $ONOO^-$ in a dose-dependent manner. Furtheremore, it blocked LPS-induced ROS and RNS generation. Conclusions : These results suggest that ZR can be developed as an effective $ONOO^-$ scavenger for the prevention of aging process and age-related diseases.

오자환(五子丸)의 Peroxynitrite 제거 작용 (Peroxynitrite Scavenging Mechanism of Ojawhan)

Due to the lack of endogenous enzymes to thwart $ONOO^-$ activation, developing a specific $ONOO^-$ scavenger is remarkably important. The aim of this study was to investigate scavenging activities of $ONOO^-$ and its precursors, NO and $O_2^-$ and its scavenging mechanism of Ojawhan. Methods : To investigate scavenging activities of $ONOO^-$, NO, $O_2^-$ and its scavenging mechanism using fluorescent probes, DCFDA, DAF-2 and DHR 123. The $ONOO^-$ scavenging activity on Ojawhan was assayed by measuring oxidized dihydrorhodamine 123 (DHR 123) by fluorometry. Oxidative stress was induced by strong oxidants t-butyl hydroperoxide (t-BHP). Endothelial cell (YPEN-1) was used for detection of intracellular oxidative stress. Results : Ojawhan markedly scavenged authentic $ONOO^-$, $O_2^-$ and NO. It also inhibited $ONOO^-$ induced by $O_2^-$ and NO which are derived from SIN-1. Furthennore, ${\underline{Ojawhan}}$ blocked lipopolysaccharide (LPS)-induced $ONOO^-$, $O_2^-$ and NO generation utilizing kidney homogenates of LPS-injected mouse and inhibited t-BHP-induced ROS and $ONOO^-$ in endothelial cell culture system. Conclusions : These results suggest that Ojawhan be developed as an effective $ONOO^-$ scavenger for the prevention of $ONOO^-$ involved diseases and age-related diseases.

신약 개발을 위한 참당귀(Angelica gigas Nakai) 추출 Decursin과 Decursinol Angelate의 약리 작용 (Pharmacological Effect of Decursin and Decursinol Angelate from Angelica gigas Nakai)

  • Traditionally, Cham dang-gui (Angelica gigas Nakai) is one of the most popular herbal medicines in Asian countries including Korea. A. gigas has been used as a functional food product for treatment anemia, women's health care, a sedative, an anodyne or a tonic agent. Decursin and decursinol angelate isolated from the roots of A. gigas are pyranocoumarin compounds. Recently, as the global herbal medication market is increasing, investigations about pharmacological effects of decursin and decursinol angelate are rapidly increasing. We summarized previous studies about pharmacological effects of decursin and decursinol angelate, and reviewed relation with pharmacological effects of decursin and decursinol angelate on human disorder, focused on the approach for new drug development. Pharmacological effects of decursin and decursinol angelate were classified as anti-tumor activity, anti-bacterial activity, improvements of the circulating system, inhibition of cytochrome P-450 activity, anti-inflammation activity, anti-oxidant activity and cognitive-enhancing activites. The activity of A. gigas with improvement of the circulating system may have wide therapeutic potential for circulatory diseases, including diabetes, hyperlipidemia and atherosclerosis. Also, anti-inflammation activity A. gigas may be beneficial for the treatment and prevention of asthma, atopic dermatitis and rheumatism arthritis. This relation could potentially lead to the development of herbal new drugs. In order to development a new drug containing decursin and decursinol angelate, it is also necessary to consider the safety profile, and the information in this review would contribute to development a new drug from herbal medicine.

실험동물을 이용한 화분섭취의 면역안전성 평가 (Immunotoxicological Evaluation of Pollen Intake Using Mice Model)

  • Pollen has been used for Prevention or treatment of certain diseases such as diabetes, arthritis, or cancer in traditional medicine. In addition, pollen is under investigation as a host cell for a gene expression. This study was undertaken to evaluate the immunologic safety of pollen intake. BALB/c mice were administered with 500, 50,5, or 0.5 mg/kg bw of lily pollen for five times a week for four weeks through gastric intubation. Comparing the control mice administered with distilled water, no significant changes were observed in body weight gain, weight of liver, spleen, lung, and his-topathological findings of liver and kidney of the mice groups administered with the pollen. Plasma level of IgG1, IgG2a, and IgE was not different among the groups. When splenic B lymphocytes were stimulated in vitro with lipopolysaccharides for 7 days, level of IgGl and IgGwa produced in the culture supernatants was not significantly different among the groups. Furthermore, no significant alteration was observed in IL-4 and $IFN{\gamma}$ producing ability with splenic T lymphocytes stimulated in vitro with phytohemagglutinins for 48 hours between the pollen-administered and the control mice. Overall, this study suggests that the lily pollen intake is Inducing no significant modulation of humoral and cell-mediated immunity in mice.
