  • Klebsiella pneumoniae is an uncommon cause of septic arthritis in adults. However, late detection can cause serious complications, including joint destruction and immobility. The purpose of this study was to report a case of successfully treated septic arthritis of the hip joint (SAHJ) caused by K. pneumoniae. A 49-year-old female patient presented to our hospital with fever and progressive severe pain in the right hip area. Although there was no abnormality on plain radiographs, ultrasonography revealed diffuse swelling of the right hip joint. Under ultrasonography guidance, the hip joint fluid was aspirated, and Gram staining and culturing were performed. The patient's pain was significantly reduced after the joint aspiration. The Gram staining and culturing revealed gram-negative bacilli, which were subsequently identified as K. pneumoniae. According to the results, systemic intravenous antibiotic (ceftriaxone) was administered without complications, and the patient was discharged on oral antibiotic (ciprofloxacin). Clinical cases of septic arthritis of the knee or sacroiliac joint have been occasionally reported in adults, but cases of SAHJ are rare. Moreover, K. pneumonia-induced SAHJ has not been reported to date. Therefore, we report this very rare case and its successful treatment.

  • The purpose of this study was examine the effect of self-exercise on patients with chronic arthritis. This study was performed from 1999 to 2001 with 107 subjects. Pain, Fatigue, Flexibility and Health status were measured. Data were analysed by SPSSWIN 10. 0 Package program, frequency, independent t-test and One Way ANOVA. The results were as follows : Of the 107 patients who were sutdied, mean age was 62 years and most of common disease were osteoarthritis. 67.4%was doing self-exercise and self-exercise consisted of 17 types of exercise. Fatigue was statistically different by disease type, duration of disease, education respectively. And fatigue, left shoulder flexibility were significantly different by duration of disease. There weren't significant differences between patient with self-exercise and patient without self-exercise on pain fatigue, flexibility and halth status. These findings confirms that self-exercise do not effective in increasing health improvement for the patients with osteoarthritis. From results of this study indicate that patients with arthritis should have taken systematic exercise such as the self-help education program aquatic exercise program for arthritis patient.

  • Septic arthritis is a serious medical condition that can lead to significant complications if misdiagnosed or mismanaged. A rare case of a 1st metatarso-phalangeal joint septic arthritis in a native joint is presented in a patient with no significant risk factors. A 41-year-old patient was referred by his general practitioner owing to ongoing pain and swelling over his native 1st metatarso-phalangeal joint with difficulty on weightbearing for three months. After a series of investigations, including blood tests and a foot magnetic resonance imaging, which were inconclusive, the patient was led to the operating theatre for sampling and washout of his joint. The samples taken in the theatres revealed septic arthritis with Streptococcus mitis as the causative microorganism. The patient was treated with six weeks of oral antibiotics with a good functional outcome. This case report illuminates this rare condition and makes foot and ankle surgeons aware of its existence. A high suspicion for this condition can prevent misdiagnosis and mismanagement.

  • This report presents the case of a 14-year-old male with rheumatoid arthritis (RA) in both temporomandibular joints (TMJs), in whom a bone scan and laboratory tests were used to confirm the diagnosis. The patient visited the Department of Orofacial Pain and Oral Medicine at the affiliation hospital with a complaint of a 1-year history of bilateral TMJ pain and sound. Clinical examination revealed bilateral TMJ and masseter muscle pain during mouth opening and palpation. Radiological examination revealed no significant morphological changes in either TMJ. The patient was prescribed medications at the first visit to address the pain, inflammation, and stiffness. A bone scan and laboratory tests were planned/scheduled for differential diagnosis between simple arthralgia and osteoarthritis. The bone scan revealed increased radiotracer uptake in both TMJs. The laboratory tests revealed a RA factor of 82.4 IU/mL, which is more than four times the normal range. The final diagnoses were bilateral TMJ early rheumatoid arthritis (ERA) and juvenile idiopathic arthritis. We created a stabilization splint and referred the patient to the Department of Rheumatology for further evaluation of the ERA. After fitting of the stabilization splint and giving instructions regarding its use, the patient has been receiving monthly follow-up checks for symptoms and undergoes follow-up blood tests every 3 months. About 14 months after the initial visit, the pain had significantly decreased from a Visual Analog Scale score of 5 to 1, and the RA factor decreased to 66.6 IU/mL. A regular follow-up check will continue until the end of growth.

The experiment method was to let the patient bend the joint and lay down flat and treat with cold air from 10 cm away from joint line repeating 5 minutes treatment 1 minute rest and 3 minutes treatment. ESR and WBC cold air therapy reported with 5 days gap and each patient has been applied for 3 times. Result: The 5th day and 10th days ESR and WBC value shows when using cold air therapy, there was a reduction on rejected group but didn't last long, but 15th day it showed only on WBC it had its attention. • Objective : Septic arthritis is a disease of when bacterial infection causes inflammatory reactions to occur in the body's joints. Following damage, stiffness, and disformity may occur in the joints. Initially antibiotic treatment is used. Later treatment is the preservation and rehabilitation of the joints. Here we report a case of sequelae of septic arthritis, which is treated and managed by ways of oriental medicine. Methods : We considered that symptoms like fever, arthralgia and ankylosis were similar to Yeol-bi(熱痺) and the patient's constitution was Soyang-in(少陽人). So Yangguksanhwa-tang(凉膈散化湯) was used as medication, and acupuncture, TENS, rehabilitation treatment was carried out. Also Kinesio & Cross(Geok-ja(격자)) taping therapy was done after the contact test. Results : After 4-7 days from the end of antibiotic treatment, the patient regained movement in the joints. After 3 weeks, the patient was able to support his own weight, and one week later joints were restored to nearly normal range. Therefore sequelae of Septic arthritis is thought to be treated earlier when the above treatment is used, compared to western treatment, which mainly consist of antibiotic treatment. Conclusion : This case was managed by herbal medicine, acupuncture, TENS, rehabilitation treatment, and Kinesio & Cross(Geok-ja) taping therapy on sequelae of Septic arthritis. Earlier improvement was gained comparatively to western treatment, which mainly consist of antibiotic treatment.

  • Objectives : To examine the effects of the collaborative Oriental and Western medicine, we treated a gouty arthritis patient with acute inflammation and liver injury with a combination of Oriental and Western treatments. Methods : Acupuncture, Bangphungtongsung-San(Fangfengtongsheng-san)', 'Kangwhaljetong-Um(Qianghuochutong-yin)', and 'Sosiho-Tang(Xiaochaihu-tang) were offered to an acute gouty arthritis patient with NSAIDs, Corticosteroids and allopurinol. Laboratory data were observed for the duration of hospital days. Results : In spite of Oriental treatments, NSAIDs administration caused liver injury, but continuous Oriental treatments with small amount of Corticosteroids and allopurinol brought recovery of liver function and gouty arthritis. Confusion : Collaborative treatments of Oriental and Western medicine are better than independent Western treatment for gouty arthritis with acute inflammation and liver injury. Further studies will be required to ascertain the collaborative treatment with Oriental and Western medicine for gouty arthritis and other diseases.
