The present study focuses on simulating river profile changes downstream of the Daecheong multipurpose dam by using the computer program HEC-6, which was developed by the United States Army Corps of Engineers. The dam locates at the Keum river, a typical alluvial stream, whose bed material is composed mostly of fine and medium sands. The study reveals that after the completion of dam, a 15 km long reach downstream from the regulatory dam was severely degraded by about 2~3 m. No further severe degradation of this reach is expected, however, because the river-bed of this reach has been well armored since then with gravels and cobbles. Some places in the study reach were degraded locally by 2 m, due mainly to the large-scale gravel mining activities in that reach. On the other hand, a 20 km long reach in downstream study reach is aggraded more or less by 0.5~1 m. Calculation by the computer program HEC-6 is close to measurement for the study river reach. According to the results by HEC-6, the study river reach would remain generally stable in the future, except a few places in the mid-upstream where further river-bed degradation of 1~2 m would occur and a few places in the far downstream where local river-bed aggradations of about 0.5 m would occur.
Objectives: The survival outcomes of antiretroviral treatment (ART) programs have not been systematically evaluated at the state level in India. This retrospective study assessed the survival rates and factors associated with survival among adult human immunodeficiency virus (HIV)-infected patients in Andhra Pradesh, India. Methods: The present study used data from 139 679 HIV patients aged ${\geq}15$ years on ART who were registered from 2007 to 2011 and were followed up through December 2013. The primary end point was death of the patient. Mortality densities (per 1000 person-years) were calculated. Kaplan-Meier and Cox-regression models were used to estimate survival and explore the factors associated with survival. Results: The overall median follow-up time was 16.0 months (2.0 months for the deceased and 14.0 months for those lost to follow-up). Approximately 13.2% of those newly initiated on ART died during follow-up. Of those deaths, 56% occurred in the first three months. The crude mortality rate was 80.9 per 1000 person-years at risk. The CD4 count (adjusted hazard ratio [aHR],4.88; 95% confidence interval [CI], 4.36 to 5.46 for < $100cells/mm^3$ vs. > $350cells/mm^3$), functional status (aHR, 3.05; 95% CI, 2.82 to 3.30 for bedridden vs. normal), and body weight (aHR, 3.69; 95% CI, 3.42 to 3.97 for <45 kg vs. >60 kg) were strongly associated with the survival of HIV patients. Conclusions: The study findings revealed that high mortality was observed within the first three months of ART initiation. Patients with poor baseline clinical characteristics had a higher risk of mortality. Expanded testing and counseling should be encouraged, with the goal of ensuring early enrollment into the program followed by the initiation of ART in HIV-infected patients.

To evaluate the possible pathophysiologic role of renin in acute renal failure observed in Korean hemorrhagic fever (KHF), the author measured the basal plasma renin activity (PRA) and the stimulated PRA by radioimmunoassay for angiotensin I in 15 normal controls and 42 KHF patients who are admitted in Seoul National University Hospital and Nation Army Hospital from Jan. 1975 to Jan. 1976. The results obtained were as follows: The mean basal PRA in normal control group was $2.9{\pm}2.16ng/ml/hr$ in the patients during the oliguric phase of KHF, the mean basal PRA was $4.7{\pm}2.13ng/ml/hr$, and there was statistically significant increase compared to the normal control. In the patients during the diuretic phase of KHF, the mean basal PRA was $3.4{\pm}2.09ng/ml/hr$, and there was statistically significant decrease compared to the oliguric phase of KHF. In normal control group, the mean basal PRA was $2.9{\pm}2.16ng/ml/hr$. And the PRA 1 hour after the administration of $Lasix^{(R)}$ 40 mg intravenously(stmulated PRA) was $5.3{\pm}2.20ng/ml/hr$ and there was statistically significant increasec ompared to basal level. In oliguric phase of KHF, the mean basal PRA was $4.6{\pm}2.01ng/ml/hr$. And stimulated PRA was $4.4{\pm}2.34ng/ml/hr$ and there was no significant changes. In diuretic phase of KHF, the mean basal PRA was $3.3{\pm}1.86ng/ml/hr$. And stimulated PRA was 5.2{\pm}2.58ng/ml/hr and there was statistically significant increase compared to basal level. There were statistically no significant correlations between basal PRA and stimulated PRA and serum creatinine, BUN, urine volume and peritonial dialysis.
Objective: To explore the clinical efficacy and toxic and side effects of recombinant human endostatin (rhendostatin/endostar) combined with chemotherapy in the treatment of advanced gastric cancer. Materials and Methods: A total of 70 patients with advanced gastrointestinal adenocarcioma confirmed by histopathology and/or cytological examination were divided into group A (37 patients) and group B (33 patients). Patients in group A were given intravenous drip of 15 mg endostar added into 500 mL normal saline, once every other day until the cessation of chemotherapy or patients' maximal tolerance to chemotherapy. Patients in group B received chemotherapy alone. Two groups selected the same chemotherapy regimens. FOLFIRI scheme: 90-min intravenous drip of $180mg/m^2$ irinotecan, intravenous drip of $200mg/m^2$ calcium folinate (CF) and $400mg/m^2$ 5-fluorouracil (5-Fu) on d1, and continuous intravenous pumping of 2 $400mg/m^2$ 5-Fu for 46 h. FOLFOX4 scheme: intravenous injection of $85mg/m^2$ oxaliplatin (L-OHP), $200mg/m^2$ calcium folinate (CF) and $400mg/m^2$ 5-FU on d1 for 2 h, and then continuous intravenous pumping of 2 $400mg/m^2$ 5-Fu for 46 h. XELOX scheme: oral administration of 1 $500mg/m^2$ xeloda (or tegafur 50~60 mg) in twice during d1~14 and intravenous drip of $135mg/m^2$ L-OHP on d1 for 2 h. The modified FOLFOX scheme: intravenous injection of $135mg/m^2$ L-OHP on d1 for 2 h, $200mg/m^2$ CF and 1.0 g tegafur during d1~5. Whereas, control Group B received chemotherapy regimens which were same as Group A, but no addition of endostar. Before chemotherapy, patients were given intravenous injection of 8 mg ondansetron, intramuscular injection of 10 mg metoclopramide and 20 mg diphenhydramine for prevention of vomiting, protection of liver and stomach as well as symptomatic supportive treatment. One cycle was 21 d, 4~6 cycles in total. The efficacy was evaluated every 2 cycles. Results: 32 patients in Group A could be evaluated, and the response rate (RR) and disease control rate (DCR) were 59.38% and 78.13%, respectively. 31 patients in Groups could be evaluated, and the RR and DCR were 32.26% and 54.84%, respectively. The differences between 2 groups were significant. The toxic effects include myelosuppression, gastrointestinal reaction, fatigue, cardiotoxicity and peripheral neurotoxicity. Conclusions: Preliminary observations show that endostar (once every other day) combined with chemotherapy is effective in the treatment of advanced gastrointestinal cancer, with low toxic effects, good tolerance, deserving further study.

Objective: To explore the incidence, clinical characteristics, diagnosis and treatment strategies, prognosis of patients with malignancy-associated hypercalcemia (MAH). Methods: The data of 115 patients with MAH who were treated at the Medical Oncology Department of Chinese PLA General Hospital from Jan., 2001 to Dec., 2010 was retrospectively reviewed. Survival analysis was performed using the Kaplan-Meier method and the Cox proportional hazard model with statistic software SPSS 18.0. Results: The patients had blood calcium levels ranging from 2.77 to 4.87 mmol/L. Except for 9 cases who died or were discharged within 5 days after admission, all other patients recovered to normal blood calcium level after treatment with bisphosphonates or intravenous hydration and diuretics; their survival after occurrence of MAH was from 1 day to 4,051 days, and the median survival time was only 50 days. In the log-rank test, the male, renal metastasis, central nervous system symptoms and hypercalcemia occurring over 140 days after cancer diagnosis were predictors of poor survival (P=0.002, P=0.046, P=0.000, P=0.009). In the COX analysis, being male, central nervous system symptoms and hypercalcemia lasting over 140 days after cancer diagnosis were independent prognostic factors for survival time (RR=2.131, P=0.027; RR=3.054, P=0.002; RR=2.403, P=0.001). According to these factors, a score system was established to predict the patient prognosis and adjust the treatment. Conclusion: Cancer patients with MAH have an extremely poor median survival. Some independent factors indicate poor prognosis, including male gender, central nervous system symptoms and hypercalcemia lasting over 140 days after cancer diagnosis. The prognostic score can serve as a reference for MAH prognosis and treatment, worthy of further investigation.

The purpose of this descriptive study was to assess sexual orientation and Knowledge and attitudes with regards to AIDS in military personnel, and to provide basic data for developing a further sex education program for them. The data were collected in June 2002 on 242 service members of A Branch of the Ground Army located in Chung-Chung Do Region. They responded to a survey questionnaire that consisted of 68 items: 14 items on sexual orientation. 39 items on knowledge about AIDS, and 15 items on attitudes toward AIDS. The data were analyzed using SAS program. The study results are as follows: 1. Sexual orientation of the military personnel 1) In terms of sexual orientaion, harassment and assault, only $26.4\%$ of the subjects responded as, 'know well'. Appropriate awareness of the legal liability was reported by $21.6\%$ of the subjects regarding sexual harassment and $78.1\%$ of the subjects regarding sexual assault. 2) Pre-marital cohabitant was supported by $89.2\%$ of the subjects. 3) The Korean military personnel showed liberal attitudes toward sexuality as $46.4\%$ of them reported having sexual-relationship with the dating partner is acceptable. 4) The attitudes of the most subjects toward homosexuality was found to be unfavorable. 5) Subjects were m the need of appropriate sex education. of which substantial knowledge on sexual intercourse was the most needy area. 2. Knowledge and attitudes toward AIDS of the military personnel 1) The subjects' level of knowledge on AIDS was relatively low, showing the average score of $22.31\pm7.23$ points (range: 0-39 points). The higher level of knowledge was reported by those who had high level of education, father alive. and less number of brothers in their family. 2) The subjects' attitudes toward AIDS was relatively positive. The attitude toward AIDS was also found to be more positive in those who had high level of education and less number of brothers. 3) There was a positive but weak correlation between knowledge and attitude toward AIDS. The study finding showed that the sexual orientaion of the Korean military personnel was moderately positive and liberal. However, their awareness of sexual harassment and assault and AIDS was still found to be low. As Korean enlisted military service men are likely to be in the ages with strong sexual desire, it may be important for them to have appropriate sexual orientation and attitudes toward AIDS. Therefore, it is recommended to develop further practical and sound sex education programs within the military unit in order to meet their education needs as well as to utilize sex education specialists as the instructors of those programs.
This paper aims to examine the constructional background and process of the Japanese military installations of Oiyang-po(外洋浦), especially based on the military secret documents. Furthermore, it aims to analyze the characteristics of the remains. The results are as follow; 1) The number of the Japanese military secret documents concerning with the installations of Oiyang-po, summed up to 33s. Especially, 14 documents about the expropriation of the lands and houses, and the constructions of the artillery position are reserved in "Mildae-ilgi"(密大日記)written from 1893 to 1942. 2) Imperialist Japan constructed firstly the military installations of Oiyang-po against the Russo-Japanese War. After the moving of the artillery headquarters into Masan in 1911, these installations had been maintained for the defense of Busan and Jinhae Bays. 3) As soon as 1904, the lands and houses of Oiyang-po were forcibly expropriated according to (韓日議定書). The Korean Government payed the expropriation prices to the dwellers. But the amount of money were too small and were lately payed. Moreover the dwellers' fishery right were never recompensed. 4) In 1904, the artillery headquarters and position were constructed by the 3rd Chookseong-dan(築城團) under the command of Matsui, a military engineer officer. The executional constructions were accomplished by the Japanese construction contractors. 5) After the moving of the artillery headquarters into Masan in 1911, the 3rd Chookseong-dan had usually repaired and consolidated the explosive warehouses and artillery facilities. 6) The artillery position constructed with the thick concrete walls was located at the foot of the mountain in back. It's plan was similar to the rectangular shape. It reserved six 280㎜ howizers and several explosive warehouses. 7) The reserve funds and arsenal funds were used for the constructions. And the items of expenses such as the establishments of the electric lights and communication networks, and the repairs of the explosive warehouses were mainly recoded in "Mildae-ilgi".

At the computer games, we can experience a variety of environments using a virtual object. It is similar to that be trained in War-game simulator of the defense. Actual soldiers and a computer-generated virtual group(Computer Generated Force: CGF) in 3-D virtual battlefield environment are training. However, path finding algorithm, one of the techniques of simulation models, to the current level only considers the shortest time path. So, this current level at the special situation of the army in the battlefield for selecting the optimal path is limited. The focus of this paper is to select the least-cost path using the deadline with several different mission conditions(METT+TC). For the only shortest time path algorithm and the least-cost path algorithm using dealine,($d_t$, one of METT+TC elements), Its usefulness is verifying the change of the move spent time(t) for all possible paths and the fighting power of the combat troops(Troops ability, a) through a comparison of the total cost of moves(c(t)). According to the results, when considering the deadline, the proposed algorithm saves about 62.5% of the maximum cost.
Purpose: The aim of the present exploratory study was to evaluate extraction socket healing at sites with a history of periodontal and endodontic pathology. Methods: The mandibular 4th premolar teeth in 5 adult beagle dogs served as experimental units. Periodontal and endodontic lesions were induced in 1 premolar site in each animal using wire ligatures and pulpal exposure over 3 months (diseased sites). The contralateral premolar sites served as healthy controls. The mandibular 4th premolar teeth were then extracted with minimal trauma, followed by careful wound debridement. The animals were sacrificed at days 1, 7, 30, 60, and 90 post-extraction for analysis, and the healing patterns at the healthy and diseased extraction sites were compared using radiography, scanning electron microscopy, histology, and histometry. Results: During the first 7 days of healing, a significant presence of inflammatory granulation tissue was noted at the diseased sites (day 1), along with a slightly accelerated rate of fibrin clot resolution on day 7. On day 30, the diseased extraction sites showed a greater percentage of persistent fibrous connective tissue, and an absence of bone marrow formation. In contrast, healthy sites showed initial signs of bone marrow formation on day 30, and subsequently a significantly greater proportion of mature bone marrow formation on both days 60 and 90. Radiographs exhibited sclerotic changes adjoining apical endodontic lesions, with scanning electron microscopy showing collapsed Volkmann canals protruding from these regions in the diseased sites. Furthermore, periodontal ligament fibers exhibited a parallel orientation to the alveolar walls of the diseased sites, in contrast to a perpendicular arrangement in the healthy sites. Conclusions: Within the limitations of this study, it appears that a history of periodontal and endodontic pathology may critically affect bone formation and maturation, leading to delayed and compromised extraction socket healing.

The newly developed diagnostic method with application of $^{113}Sn-^{113m}In$ cow system ($^{113}Sn:\;T\frac{1}{2}$ 118 days, $^{113m}In:\;T\frac{1}{2}$ 1.7 hrs, 390 Kev, Single ${\gamma}$) has the remarkable advantages such as increased diagnostic ability by single large dose administration of $^{113m}In$ with no subsequent radiation hazard and shortened examining time. We reformed the research of following scope with the use of developed $^{113}Sn-^{113m}In$ cow (25 mCi) generator: The sizes of particles produced under various conditions were investigated, and possibility for application to the scannings of various organs such as brain, liver, lung, bone marrow and blood pool etc. were studied. Results: $^{113m}InCl_3$ solution eluted from diluted HCl solution (pH 1.5) passed through $^{113}Sn-^{113m}In$ generator, and there can be produced various sized particles of colloidal indium. And there observed the state of distribution of $^{113m}In$ in each organ which showed many differences according to the particle sizes of colloidal indium. The results are stated as follows: 1. The adjustment of pH is the most important factor in making the desirable particle size of colloidal indium. The colloid for blood pool showed the highest level as 7.1%/gm blood, at pH 1.7, the colloid of pH 3.5 for liver scanning showed the highest level, 88.4%, in the liver, the colloid pH 6 showed the highest level, 3.1%, in the spleen, and the colloid of pH 11.0 showed the highest level, 85.3%/gm, in the lung. 2. The colloid for liver scanning made with NaCl-NaOH system showed the highest liver uptake at pH 7.2, and at either higher or lower pH than 7.2 showed decrease of liver uptake more or less. 3. The activity of $^{113m}In$ eluted through $^{113}Sn-^{113m}In$ generator indicated over 90% in the initial 4 ml, and particularly 88.1%-86.0% in the initial 2 ml. 4. The incubation time, tempertaure and mechanical irritation related to colloid formation and coating of colloid were not the definite condition of influence.
