Growth observation and orthodontic treatment of a hemifacial microsomia patient treated with distraction osteogenesis

  • Hemifacial microsomia (HFM) patients may experience emotional withdrawal during their growth period due to their abnormal facial appearance. Distraction osteogenesis at an early age to improve their appearance can encourage these patients. Some abnormalities of the affected side can be overcome by distraction osteogenesis at an early age. However, differences in the growth rate between the affected and unaffected sides during the rest of the growth period are inevitable due to the characteristics of HFM. Therefore, re-evaluation should be performed after completion of growth in order to achieve stable occlusion through either orthognathic surgery or camouflage orthodontic treatment. An eight-year-old patient visited the clinic exhibiting features of HFM with slight mandibular involvement. He received phase I treatment with distraction osteogenesis and a functional appliance. Distraction osteogenesis was performed at the right ramus, which resulted in an open bite at the right posterior dentition. After distraction osteogenesis, a functional appliance and partial fixed appliance were used to achieve extrusion of the affected posterior dentition and settlement of the occlusion adjustment on the unaffected posterior dentition. The patient visited the clinic regularly for follow-up assessments, and at the age of 20 years, he showed facial asymmetry of the mandible, which had deviated to the right side. He received orthodontic treatment to improve the occlusion of his posterior dentition after the growth period. Without orthognathic surgery, stable occlusion and a satisfactory facial appearance were obtained through camouflage orthodontic treatment.

Is Rectosigmoid Vaginoplasty Still Useful?

  • Background The ideal vaginoplasty must be successful functionally as well as have a natural appearance, and also must retain its functionality and appearance over the long term. Conventional vaginoplasty techniques have functional limitations and are associated with recurrent complications, but rectosigmoid vaginoplasty is known to have a high satisfaction rate due to its functional similarity with the vagina. We conducted the present study to assess the usability of rectosigmoid vaginoplasty over the course of long-term follow-up. Methods From March 1992 to February 2014, 84 patients were treated with rectosigmoid vaginoplasty; 44 had gender identity disorder, 29 had vaginal agenesis, 8 had female pseudohermaphroditism, and 3 had gynecologic malignancies after radical pelvic surgery. This retrospective study was based on a review of the patients' records, clinical examinations, complications, and questionnaires about appearance, function, and sexual intercourse. Results All patients who underwent rectosigmoid vaginoplasty were discharged within 2 weeks without surgical flap loss. The early complications were partial flap necrosis, difficulty in defecation, mucous hypersecretion, and postoperative ileus. The late complications were vaginal introitus contracture, vaginal prolapse, and difficulty in urination. The mean length and diameter of the neovagina 3.4 years after rectosigmoid vaginoplasty were 13.2 cm and 3.8 cm, respectively. On questionnaires about satisfaction, 70% of patients reported excellent satisfaction, 11% good, 12% fair, and 7% poor. Conclusions Rectosigmoid vaginoplasty is useful, safe, and well-accepted operative method with good functional and cosmetic results, such as natural lubrication and adequate vaginal length and width obtained without requiring the use of a dilator.

An Anthropometric and Three-Dimensional Computed Tomographic Evaluation of Two-Point Fixation of Zygomatic Complex Fractures

  • Background Maintaining stability and restoring the aesthetic appearance are the fundamental goals when managing zygomatic fractures. We aimed to evaluate the stability and anthropometric outcomes of zygomatic fracture patients who underwent two-point fixation involving the infraorbital rim and zygomaticomaxillary buttress via the transconjunctival and gingivobuccal approaches without any skin incisions. Methods We examined 15 zygomatic fracture patients who underwent two-point fixation during a 3-year period. Stability was evaluated using three-dimensional facial bone computed tomography. Superoinferior and anteroposterior displacement of the zygoma was quantified. The aesthetic appearance of the periorbital region was evaluated using indirect anthropometry with standardized clinical photographs. The ratios between the eye fissure height and width, and lower iris coverage ratio were used to evaluate aesthetical changes. The bony displacement and aesthetic ratios were analyzed using Wilcoxon or Friedman tests. The correlation between the preoperative zygoma position and anthropometric values was analyzed. Results The positions of the zygoma were similar to those on the contralateral side at the long-term follow-up. The preoperative anthropometric measurements on the fractured side differed from those on the contralateral side, although these values were close to the normal values at the long-term follow-up. Furthermore, we noted that the anteroposterior displacement strongly positively correlated with the lower iris coverage rate (Spearman's coefficient=0.678, P=0.005). Conclusions Two-point fixation of zygomatic fractures achieved stable outcomes on long-term follow-up, and also appeared to be reliable in restoring the aesthetic appearance of the periorbital region.

A STUDY OF AMELOBLASTOMA ON THE RELATIONSHIP BETWEEN HISTOPATHOLOGIC PATTERNS AND RADIOGRAPHIC CHARACTERISTICS (법랑모세포종의 조직병리학적 분류에 따른 방사선학적 소견에 관한 연구)

  • The purpose of this study was to evaluate the correlationship between histopathologic types of ameloblastoma and their radiographic appearances. The materials for this study consisted of 106 patients diagnosed as ameloblastoma both radiographically and histologically. The obtained results were as follows: 1. The incidence of ameloblastoma in male(60cases, 56.6%) was slightly higher than that in female (46 cases, 43.4%). The average age was estimated as 30.7 years with a range from 6 to 76 years. The second decade revealed the highest rate. 2. 106 ameloblstomas were histopathologically classified as 36 unicystic, 28 plexiform, 20 follicular, 14 acanthomatous, 7 granular cell, and 1 basal cell ameloblastoma. 3. Unilocular, soap-bubble appearance and scalloped margin were the radiographic appearances frequently seen in unicystic ameloblastoma. The predominant radiographic appearance of plexiform ameloblastoma showed unilocular radiolucency with scalloped margin. 4. 19.8%, 21 cases of ameloblastoma in this study showed containing tooth in their tumor mass by radiography. 5. Root resorption occured in 37 cases(34.9%) and tooth displacement in 7 cases(6.6%). Root resorption and tooth displacement occured in same patient were 24 cases(22.6%). 6. Recurrence occured in 21.7% and average year between initial treatment and recurrence were 2 years.

An Experimental Study on the Arc Stability Improvement of Underwater Wet Welding with Flux Ingredients (피복성분에 의한 수중용접봉의 아크 안정성 개선에 관한 실험연구)

  • Underwater wet bead-on-plate welds were experimentally performed on 11mm thick SS400 steel plate as base metal by using six different types of flux coated electrodes of 3.2mm diameter. Two kinds of different flux coated wet arc electrodes (UW-1, UW-2) were individually designed flux materials, three kinds of the electrodes (E4301, E4311, E4313) were terrestrial electrodes and the another one (TN20) was an imported underwater wet welding electrode. As results, the individually designed flux coated underwater electrode, UW-2 and TN20 had a good starting and the excellent arc stability compared with other electrodes. No significant difference of bead appearance could be detected, but the slag detachability of TN20 electrodes was relatively undesirable. By rapid cooling rate, the hardness value and the portion of martensite of HAZ were increased, but it was considerably maintain stable for TN20 and UW-2 electrodes. The individually designed flux coated electrode, UW-2 could be used in practice for underwater bead welds.

Effect of Sulfate on the Reaction of 3CaO.SiO2 Formation in Presence of Clinker Melt and Its Appearance in Clinker ($SO_3$가 클링커 액상이 존재하는 $3CaO.SiO_2$ 생성반응에 미치는 영향과 클링커안에서의 존재상태)

  • Effect of sulfate on the reactionof C3S formation in presence of clinker melt and its appearance in clinker were investigated, using (NH4)2SO4 and CaSO4 as sulfate sources. When (NH4)2SO4 and CaSO4 were added, both showed the similar results, 1.0wt% of sulfate could promoted the reaction of C3S formation, however for its content of more than 2.0wt%, the formation of C3S was prevented. Residual limit of sulfate to C3S formation is about 1.4wt%. Appearances of sulfate were C4A3l and CaSO4 in interstitial phase. For the addition of (NH4)2SO4 or CaSO4 of 20wt%~4.0wt%, C3S grains showed the hypertrophic growth. We might consider that, because sulfate reduced the surface tension and viscosity of the clinker melt, C3S crystals were precipitated below 140$0^{\circ}C$, and the crystlas of C3S were coalesced and linked in the same crystallographical direction with increasing temperature becuase of their rapid growth rate.

  • Performances of Taekwondo fabrics were evaluated in respect of fiber component. yarn count, weave structure and wearing comfort. As a beginning step, we investigated the present situation of Taekwondo wears by questionnaire from pro and amateur Taekwondo players. Samples employed in this study were cotton/nylon blend fabric that was newly woven for this study as well as fabrics of current Taekwondo wears for sale in the market. Their fundamental properties measured were such as air permeability, water vapor transport. wickability, absorption rate, Qmax values, thermal conductivity, durability, hand value, and etc. In addition, subjective wearing sensations were evaluated using Taekwondo wears made of those fabrics. From the results of the objective measurement and the subjective wearing test, we estimated the total fitness of fabrics as a Taekwondo wear. From the questionnaire we could see that pro players and amateurs wanted highly absorbing, quick drying, and soft-tough and complained abrasive surfaces and static elasticity of current fabrics. In view of the results so far achieved, nylon blended fabrics newly woven in this study, showed better comfort-related properties from both of the objective and subjective tests. It was also represented that finer yams enhanced water absorption and touch, and fabrics with rough surface such as honeycomb weave was superior in wearing comfort as well as aesthetic appearance.

  • This study was investigated the personal rating reason for preference, and opinion for improvement on the Korean traditional foods. Questionnaire were answered by male and female university students at 8 universities in Seoul and the results are summarized as follows. 1) General trend of subjects showed preference for traditional food in order of nutrition, appearance, taste, economics, and cooking method. While the value of food itself such as nutrition, appearance, taste was rated high, practical side such as economics, and cooking method was not so agreed. Especially female students set as lower value on cooking method. 2) As a reason for rating it low, male and female students answered without distinction that traditional food are laborious and much time is needed for cooking. 3) For the idea of improving traditional food, all students had same opinion that standardized cooking method should be developed for convenient use at home, and secondly, special salestore of traditional food should be promoted. Particularly more male students had the opinion that traditional cooking method should be succeeded to at home than female students. Summarizing some significant findings from the analysis of data, all students prefer traditional food itself relatiovely. But generally they rate it low because traditional food is difficult to cook and there are not so many chances of eating. Therefore, it is regarded that standardized cooking method and promoted special salestore for more chance of eating will activated the interest of young generation, and hence contribute to succession of tradition.

A Study on the Utilizing of Cool-season Turfgrass of Golf Courses in Korea (우리나라 골프 코스에서 한지형 잔디의 활용방안)

  • This study was carried out to investigate the utilizing of cool-season turfgrass in areas, excluding greens, of Korean golf courses. Data collected from 120 golf courses were subjected to frequency and T-test analysis using SPSSWIN. The results obtained were as follows: 1) seventy eight golf curses were utilizing cool-season turfgrass in areas except of the greens. At thirty five golf courses (46.0%) of them, the area utilized appeared tee, green collar, green approach etc. (tee>green collar>green approach). At 37 golf courses(48.7%), a mix of Kentucky Bluegrass and Perennial Ryegrass was utilized and the ratio of the mix was 70:30(v/v). At 57 golf courses(76.0%), seed sowing was utilized. 2) In Korean golf courses, the cognition of utilizing cool-season turfgrass depended on the existence of the practice. The cognition of the experienced was more´ affirmative´ than that of the inexperienced. 3) In the experienced, the preference was determined by turfgrass quality and good appearance and recuperative rate of cool-season turfgrass. In the inexperienced, the preference was determined by turfgrass quality god appearance. 4) The experienced recommended the mix of Kentucky bluegrass and Perennial Ryegrass. 5) It seems that the golf courses having used cool-season turfgrass have difficulties in the maintenance in summer while the golf courses without using cool-season turfgrass have difficulties in the maintenance in summer and the selection of turfgrass variety.

A Study on the Association between Sasang Constitutions and Body Composition in Teenagers (청소년의 사상체질과 체성분과의 상관성에 관한 연구)

  • Purpose In Sasang Constitution Medicine, persons are divided into four types such as Taeyangin, Taeumin, Soyangin and Soeumin according to their appearance, moral nature and characteristic of constitutional symptoms. In teenager group, diagnosis of constitution based on their appearance was so difficult that we adopted new diagnostic method which measures and compares body compositions. Methods 180 middle school students who live in Pusan were studied from 20th of March, 1999 to 20th of March, 2000. After practicing Questionnaire(I), QSCC(Questionnaire of Sasang Constitution) I , QSCCII, we have measured the height and body compositions with Inbody 3.0, five times. Results 1. The results of Sasang Constitution analysis showed 91 students were classified as Taeumin(50.3%), 75 as Soyangin(41.4%), 14 as Soeumin(7.7%), and 1 as Taeyangin(0.6%). 2. The level of total body water, muscle area, BMR(Basal Metabolic Rate), BCM(Body Cell Mass), and AMC(Arm Muscle Circumference) were increased during spring and summer. That of body weight, fat body weight, fat body weight ratio, waist hip ratio, and BMI(Body Mass Index were increased during fall and winter. 3. In Taeumin and Soyangin groups, the changed capacity of total body water, muscle area and BMR were increased with statistical significance. 4. Waist hip ratio of Soeumin group was reduced more than those of Taeumin, Soyangin groups with statistical significance.
