대동맥류의 외과적 치료 -37례 보고 (1984-1987) - (Surgical Treatment of Aortic Aneurysm - Review of 37 cases between 1984 and 1987 -)

  • Thirty-seven patients of aortic aneurysm underwent operations during January 1984 December 1987 at our hospital. Twenty-six patients had aneurysms involving ascending aorta, three patients had aneurysms involving both ascending aorta and abdominal aorta. and eleven patients had aneurysms involving descending thoracic or abdominal aorta. Among the patients who had aneurysms involving ascending aorta, annuloaortic ectasia with aortic regurgitation were thirteen and all of these underwent ascending aorta graft replacement + AVR with composite graft. The patients who had aortic regurgitation due to ascending aortic dissection were three and all of these underwent intraluminal ringed graft insertion at ascending aorta + aortic valve resuspension. Intraluminal ringed graft insertion was safe, simple, and fast method in the operation for aortic aneurysm. Eleven patients were underwent this operation and the results were good. Major causes of death of the patients who underwent aortic aneurysm operation are underlying cardiovascular diseases or delayed rupture of the aneurysm or complications related newly appeared aneurysm. Among our patients, dissection progressions were appeared in two but neither severe nor complicated. And no patient died from delayed rupture of aneurysm or complications related newly appeared aneurysm. All patients were followed up via OPD and were controlled hypertension or heart failure if present. Operative mortality is 18.9\ulcornera in all, 23% in patients who had aneurysms involving ascending aorta and 7.6` who had aneurysms involving descending thoracic or abdominal aorta. Comparing with other reports, our operative mortality is still high but improved steadily. So we recommend aggressive surgical management of the aortic aneurysm.

매독성 대동맥류의 수술치험 -1예 보고- (Syphilitic aortic aneurysm -A case report-)

  • Syphilitic aortic aneurysm is a rare lesion today. We experienced a case of huge syphilitic ascending aortic aneurysm with aortic insufficiency. Surgical correction was done by replacement of ascending aorta with woven Dacron graft and aortic valve replacement under cardiopulmonary bypass. There is no abnormality in postoperative aortography. The postoperative course was uneventful.

  • The incidence of syphilitic aortic aneurysm was decreased now a day. We experienced a case of huge syphilitic ascending aortic aneurysm from just above portion of aortic annulus to about 1 cm below innominate artery without aortic insufficiency. Surgical correction was done by replacement of ascending aorta with woven Dacron graft under cardiopulmonary bypass. Postoperative course was uneventful and discharged 15 days after surgery with good condition.

Aortic Valve Sparing Operations: A Review

  • Aortic valve sparing operations were developed to preserve the native aortic valve during surgery for aortic root aneurysm as well as surgery for ascending aortic aneurysms with associated aortic insufficiency. There are basically two types of aortic valve sparing oprations: remodeling of the aortic root and reimplantation of the aortic valve. These operations have been performed for over two decades and the clinical outcomes have been excellent in experienced hands. Although remodeling of the aortic root is physiologically superior to reimplantation of the aortic valve, long-term follow-up suggests that the latter is associated with lower risk of developing aortic insufficiency. Failure of remodeling of the aortic root is often due to dilatation of the aortic annulus. Thus, this type of aortic valve sparing should be reserved for older patients with ascending aortic aneurysm and normal aortic annulus whereas reimplantation of the aortic valve is more appropriate for young patients with inherited disorders that cause aortic root aneurysms. This article summarizes the published experience with these two operations. They are no longer experimental procedures and should be part of the surgical armamentarium to treat patients with aortic root aneurysm and ascending aortic aneurysms with associated aortic insufficiency.

Bentall 씨 수술 치험 -1례 보고- (Bentall`s Operation of Ascending Aorta Aneurysm with Aortic Regurgitation - Report of One Case -)

  • We have experienced one case of ascending aorta aneurysm with aortic regurgitation due to atherosclerosis. The 45 year old man had been suffered from palpitation and precordial chest pain. 2-D echocardiogram and aortogram confirmed aneurysm of ascending aorta with aortic regurgitation. Atherosclerotic change was noted in the aortic wall and there was marked dilatation of the sinuses of Valsalva as well as the aortic annulus with upward displacement of coronary ostia in the operative field. The patient underwent complete replacement of the aneurysmal ascending aorta and the aortic valve with 27mm Bjork-Shiley aortic valve composite graft. We got preclotting with heparin free blood including thrombin and then autoclave at 132` for 3 minutes. The postoperative course was uneventful and the patient was discharged with good clinical result.

Marfan 증후군에 동반된 상행대동맥류와 대동맥판 폐쇄부전증의 외과적 치료 (Marfan's syndrome associated with ascending aorta aneurysm and aortic regurgitation (Report of 8 cases))

  • Eight patients received operation for ascending aortic aneurysm and aortic regurgitation associated with Marfan`s syndrome from January 1984 to July 1986 at Seoul National University Hospital. The patients` age ranged from 29 to 51 years [mean 37.3 years]. Five patients were male and three were female. All of them showed some stigmata of skeletal system in Marfan`s syndrome. Three patients had dissecting aneurysm and five patients had fusiform aneurysm of ascending aorta. Two patients had concomitant fusiform aneurysm of abdominal aorta. All patients showed aortic regurgitation of grade III to IV. One patient received insertion of intraluminal ringed graft and resuspension of aortic valve, and seven patients received modified Bentall operation [Carol`s method]. There was no hospital death and all showed functional improvement in the 7.4 patient-years follow-up period.

대동맥판부전이 동반된 상행대동맥류의 외과적 치험 (Surgical Management of Aneurysm of the Ascending Aorta with Aortic Insufficiency)

  • We retrospectively studied 34 patients who underwent operations of ascending aortic aneurysm and aortic valve replacement from August 1979 to July 1992 at the Yonsei Cardiovascular Center. Eight patients underwent supracoroanry non-composite graft replacement and separate aortic valve replacement[group I]. Twenty six patients underwent valved composite graft replacement with reimplantation of coronary arteries[group II]. Two cases in group II died within 1 month after the operation. Among the 32 survivors 28 patients have been followed up for an average of 59 months ranging from 1 months to 159 months. During the follow up periods, a pseudoaneurysm around the ascending aorta and a newly developed dissecting aneurysm in remaining aorta were noted in group II. There were 6 late deaths: 2 cases in group I and 4 cases in group II. Three cases among the 6 late deaths have stigmata of Marfan`s syndrome[1 cases in group I and 2 case in group II]. There was no statistically significant difference in actuarial survival rates between group I and group II[p > 0.05]. This study suggests that non-composite supracoronary graft interposition with separate aortic valve replacement is a safe surgical technique in patients who have normal aortic annulus and normal position of coronary ostia. However in degenerative disease such as cystic medial necrosis, composite graft replacement is recommended because this procedure eliminates entire abnormal tissue.But it seems to be important that the suture technique and strict follow-up in patients with diseased aortic wall.

  • The treatment of aortic aneurysm of ascending aorta has been fraught with difficult surgical problems. For the most part, these were resolved in 1968 with the introduction of a technique of total replacement of ascending aorta and reimplantation of the coronary arteries by Bentall and De Bono. This technique however, with all of its advantages, caries a certain problems. In chronic dissecting aneurysms, there is frequently a marked disparity in circumference between the true and false lumen distally. Distal perfusion is directed into both the true and false lumens by removing segment of the septum between the two lumens and constructing the distal graft anastomosis is to the outer layer of aortic adventitia. The distal false lumen, aortic branches and fenestrations have matured and healed in most cases. And importantly, major aortic tributaries may be solely dependent on the false lumen for perfusion. We are presenting two cases of chronic dissecting aneurysm of ascending aorta with aortic regurgitation, who have good result by surgical correction of so-called Bentall procedure with maintenance of blood flow directed into both true and false lumen.

대동맥관 폐쇄부전을 동반한 상행대동맥의 외과적 치료 (Results with Total Replacement of the Ascending Aorta and Reimplantation of the Coronary Arteries)

  • From April, 1981, to April, 1990, 20 male and 7 female patients ranging in age from 17 to 63, were operated on for aortic insufficiency with an aneurysm of the ascending aorta. Ten patients were in New York Heart Association functional class II, 7 in class III, and ten in class IV. The surgical treatment in all cases consisted of total replacement of the ascending aorta with composite graft containing a prosthetic aortic valve and reimplantation of the coronary arteries by an intermediate tube graft. In 15 patients an uncomplicated annulo-aortic ectasia existed, and in 12 an aortic dissection; three of the latter group were operated during the acute phase. 17 patients showed typical Marfan syndrome, and 3 patients showed severe ascending aortic aneurysm secondary to the aortic valve disease. The overall operative mortality was 7%[2 deaths]. Those 2 deaths occurred following emergency operation due to associated aortic dissection, but no death during elective operation. All survivors have been followed-up during a period ranging 1 to 108 month[average 34 months]. There was no late mortality. Among the survivors, clinical improvement is readily apparent[2,3 in class I, 2 in class II ]. In conclusion, the treatment of aortic insufficiency associated with an aneurysm of the ascending aorta by insertion of a composite graft and reimplantation of the coronary arteries through an intermediate Dacron tube is a reliable method with low mortality and excellent results.

Bentall씨 수술치험 2예 (Bentall's operation of ascending aorta aneurysm with aortic regurgitation in Marfan's syndrome)

  • We have experienced two cases of ascending aorta aneurysm with aortic regurgitation in Marfan`s syndrome. There were abnormal findings in cardiovascular system associated with abnormalities of skeletal systems. They had total replacement of the ascending aorta and aortic valve with Bjork-Shiley`s aortic valve composite graft and reimplantation of coronary ostia on the graft. Their postoperative courses were uneventful and discharged with good clinical results for follow up.
