Investigation on Individual Variation of Organ Doses for Photon External Exposures: A Monte Carlo Simulation Study

  • Background: The reference dose coefficients (DCs) of the International Commission on Radiological Protection (ICRP) have been widely used to estimate organ doses of individuals for risk assessments. This approach has been well accepted because individual anatomy data are usually unavailable, although dosimetric uncertainty exists due to the anatomical difference between the reference phantoms and the individuals. We attempted to quantify the individual variation of organ doses for photon external exposures by calculating and comparing organ DCs for 30 individuals against the ICRP reference DCs. Materials and Methods: We acquired computed tomography images from 30 patients in which eight organs (brain, breasts, liver, lungs, skeleton, skin, stomach, and urinary bladder) were segmented using the ImageJ software to create voxel phantoms. The phantoms were implemented into the Monte Carlo N-Particle 6 (MCNP6) code and then irradiated by broad parallel photon beams (10 keV to 10 MeV) at four directions (antero-posterior, postero-anterior, left-lateral, right-lateral) to calculate organ DCs. Results and Discussion: There was significant variation in organ doses due to the difference in anatomy among the individuals, especially in the kilovoltage region (e.g., <100 keV). For example, the red bone marrow doses at 0.01 MeV varied from 3 to 7 orders of the magnitude depending on the irradiation geometry. In contrast, in the megavoltage region (1-10 MeV), the individual variation of the organ doses was found to be negligibly small (differences <10%). It was also interesting to observe that the organ doses of the ICRP reference phantoms showed good agreement with the mean values of the organ doses among the patients in many cases. Conclusion: The results of this study would be informative to improve insights in individual-specific dosimetry. It should be extended to further studies in terms of many different aspects (e.g., other particles such as neutrons, other exposures such as internal exposures, and a larger number of individuals/patients) in the future.

The Report for Atypical Vascular Variations in the Latissimus Dorsi Myocutaneous Flaps (광배근 피판의 비전형적인 혈관 해부학적 증례에 대한 보고)

  • It has been known that latissimus dorsi(LD) myocutaneous flap based on thoracodorsal artery is one of most useful method for microreconstructive surgery and the thoracodorsal artery of this flap has constant vascular anatomy. The retrospective study for anatomy of the thoracodorsal arterial system was performed at operative cases. The aim of this study was to document the anatomical variation of this pedicle clinically. 167 LD flaps were carried out from 1983 to 2002 in our clinic. We found unusual 7(4.2%) cases compared to standard textbooks of anatomy. One case was no vascular supply to LD muscle. In 2(1.2%) cases thoracodorsal artery was a typical branch of the subscapular artery but didn't branch to LD muscle, passed to lower serratus anterior muscle, and at this point, supplied vessel to LD muscle and it's vascular diameter was about 1mm diameter. The thoracodorsal artery arose from the axillary artery in 1.8% of cases(3 cases). One case had less than 1mm vascular diameter but a branch of subscapluar artery. It should be emphasized that we must elevate the latissimus dorsi flap after accurate cognition for the anatomy of thoracodorsal artery because the thoracodorsal arterial system is almost reliable but not uniform in rare cases.

Voice quality of normal elderly people after a 3oz water-swallow test: An acoustic analysis (3온스 물 삼킴검사 이후 정상 노년층의 음질 변화: 음향학적 분석)

  • The elderly are at increased risk of developing dysphagia due to aging and illnesses. The aim of the current study was to analyze, via an acoustic study, the change in the voice quality of normal elderly people after a 3oz water-swallow test. Subjects included a group of 60 normal elderly people (age: $mean{\pm}SD=76.9{\pm}6.66$) and 60 healthy young adults (age: $mean{\pm}SD=25.1{\pm}2.36$). Every participant produced a five-second /a/ phonation pre- and post-swallowing, and the fractioned two-second sections were analyzed using the MDVP (multi dimensional voice program) analysis. The elderly group demonstrated a post-swallowing increase in the following related acoustic parameters: fundamental frequency, fundamental frequency variation, amplitude-variation, and noise in both two-second sections. However, the younger group showed an increase only in frequency related acoustic parameters (i.e., STD ) in the first two-second section. The significant changes in values in the post-swallowing parameters might indicate temporary irregularities in pitch and amplitude along with higher amounts of noise in the voice. The results could be attributed to water residues in the vocal fold and vocal tract, as well as a deterioration of the motor and sensory functions caused by anatomical and physiological changes that result from aging.

Reappraisal of anatomical diversity of lateral circumflex femoral artery with its substantial clinical applicability: cadaveric study

  • Studies reveal variations in the in the origin, number, and branching patterns of the lateral circumflex femoral artery (LCFA). The present study aimed to document such variations and their potential clinical applicability. Thirty-two femoral triangles of 16 embalmed adult human cadavers were dissected to investigate the variation in the origin, number, and branching patterns of LCFA. The main branches of the LCFA were tracked independently for numerical variations in branching pattern. The distance between the origin of LCFA and mid inguinal point (MIP) was also measured in each case. LCFA was most commonly arising from profunda femoris (PF), followed by femoral artery (FA) and common trunk of the femoral artery (CFA). Duplication LCFA was observed in 15 (46.87%) limbs, in 5 (31.25%) cases duplication was only on right side, in 4 (25%) cases duplication was only on left side and in 3 (18.75%), duplication was bilateral. Cases with duplication of LCFA, showed numerical variations with descending pattern being the most common. The average distance of LCFA1 and LCFA2 from mid-inguinal point was 5.77±1.35 cm and 6.14±2.05 cm respectively. Detailed information regarding the occurrence of duplication will be great importance for surgeons, interventional radiologists, and other medical professionals performing procedures in the femoral region. Knowledge of variation of branching pattern of LCFA is utmost important as surgeons use the descending branch of the LCFA in bypass grafting and vascular reconstruction surgeries.

Pharmacognostical Studies on Ginseng Folium (인삼잎의 생약학적 연구)

  • Ginseng Folium has been used to cure acute gastritis, alcoholism and anemia. The morphological and anatomical characteristics of the leaf and stem of Panax ginseng C.A. Meyer cultivated in Korea for 1 to 6 years are described. It is found that the vessels increase in number and diameter, and also the vascular bundles increase in number, as the plant becomes old. The result showed that the commercial &In Sam Ip& samples on the markets of Pusan, Kumsan, Taegu and Seoul have been derived from the leaf and stem of 4 to 6 years old P. ginseng plants, most of them being 5-6 years old.

Aneurysm at the Origin of the Accessory Middle Cerebral Artery - A Case Report - (부중대뇌동맥 기시부의 동맥류 - 증례보고 -)

  • A case of unruptured cerebral aneurysm at the junction of accessory middle cerebral artery and the distal portion of the $A_1$ segment of the anterior cerebral artery is reported. To the authors' knowledge, this is the first reported case of cerebral aneurysm developed at the junction of accessory middle cerebral artery, demonstrated on magnetic resonance angiography(MRA). The accessory middle cerebral artery is a rare vascular variant of middle cerebral artery. Furthermore, it is extremely rare for an aneurysm to be developed at the origin of the accessory middle cerebral artery. The development of the accessory middle cerebral artery is very important in surgery of cerebral aneurysm and collateral circulation of cerebral infarction. Review of the literature regarding the genesis and anatomical variation of the accessory middle cerebral artery is also presented.

  • Tape measurement for evaluating edema is frequently used by clinicians. The primary purpose of this study is to determine the intertester and intratester reliability of tape measurement carried out by two physical therapists on lower extremities. The intertester and intratester reliability of six anatomical landmarks measurements were determined in six healthy subjects. The measurements were taken in five sessions by each tester. The intraclass correlation coefficients (2,1) were between .87 and .99 for intertester reliability. Intratester reliability was .93 and .99 for A tester and .82 and .99 for B tester. The average range of variation was .13 to .54 in six landmarks. The reliability of six volume measurements was very high. These results support the use tape measurement as a reliable tool for measuring limb girth.

Visualization of blood sucking phenomena of a female mosquito (암 모기 흡혈과정 가시화)

  • As a carrier of malaria and sneak of blood, mosquitoes are regarded as an unpleasant insect. However, there are novel phenomena that happen inside a mosquito. Among them, we focused on the blood sucking function of a female mosquito. The main objective of this study was to investigate the mosquito's pumping mechanism in order to resolve the problem encountered when we inject or transport biologic fluids into a micro-chip. To analyze the pumping mechanism, we visualized the blood sucking process inside a female mosquito. Flow characteristics of blood flow in a proboscis were investigated experimentally using a micro-PIV velocity field measurement technique. The anatomical variation of head, thorax, abdomen which work as pumps and valves, was visualized using the syncrotron X-ray micro-imaging technique.

  • Anatomical measurements were made at the necoropsy and sacrifice in a group of 41 adult female Rhesus monkey to determine the 34 characteristcs found in tracheas snd bronchus. Table 1, 2, 3, 4, and figure 1 were therefore prepared as a guide in the further use for illustrating monkey lung research. Summary of this report is as follows: 1. Accurate Knowledge of these measurements may have some value in pulmonary dissection and reconstructive procedures. 2. These studies may provide a more accurate determination of the monkey lungs. 3. The right and left bronchus length of the monkey is shorter than fornd in man. 4. The left bronchus length is more longer than the rrght bronchas. 5. There were large variation among the weight of adult female Rhesus monkey.

Dissecting Aneurysm Associated with a Double Origin of the Posterior Inferior Cerebellar Artery Causing Subarachnoid Hemorrhage

  • Two cases of the posterior fossa dissecting aneurysm associated with a double origin of the posterior inferior cerebellar artery (DOPICA) causing subarachnoid hemorrhage are presented. After observing a relationship between the aneurysm and DOPICA on a three dimensional rotational angiogram (3DRA), the dissecting aneurysms were successfully obliterated by surgical trapping and endovascular internal trapping, respectively. This report warrants suspecting DOPICA of an associating anomaly predisposing to dissecting aneurysm in the vertebral artery-posterior inferior cerebellar artery territory and highlights the role of 3DRA in pretreatment evaluation of unusual aneurysms accompanying a particular anatomical variation.
