견관절의 회전중심점을 고려한 동작가능 영역의 생성 (Generation of a Practical Reach Envelope with the Center of Rotation of the Shoulder Joint Considered)

  • Shoulder joint is the most movable joint in human body with, at least, three degrees of freedom, since there are at least three bones and five joints involved in shoulder movement. Due to the complexity of the shoulder joint and the lack of appropriate anatomical data, modeling of the shoulder joint has been known to be extremely difficult. In many biomechanical models being used, shoulder joint is considered as a fixed point and it is also assumed that the shoulder joint does not noticeably move during the shoulder movement. However, such an assumption is not valid in real applications and causes inaccuracy, especially, in the area of workspace evaluation. The reachable area generated by a human becomes somewhat different from that of current models for those models fail to appropriately reflect the movement of shoulder joint's center of rotation. In this study, the location of the shoulder joint's center of rotation was obtained in relation to the location of humerus, on which a new model for reach envelope generation was developed for workspace evaluation. From the experiments conducted for three subjects, the initial location of the center of rotation was determined for each subject and subsequent changes in the instantaneous center of rotation were drawn as a function of flexion and abduction of the shoulder. Based on the regression analysis, the study suggested a new method for the generation of reach envelope. Comparisons were also made among real reach envelopes obtained from the experiment, the ones from the model, and the ones from the new method suggested in the study. As a result, the prediction errors incurred from the new method were significantly reduced when compared to the ones from the current approach.

Analysis According to Characteristics of 18 Cases of Brachial Plexus Tumors : A Review of Surgical Treatment Experience

  • Objective : Because the anatomical structure of the brachial plexus is very complex, surgical treatment of tumors in this region is challenging. Therefore, a lot of clinical and surgical experience is required for successful treatment; however, many neurosurgeons have difficulty accumulating this experience owing to the rarity of brachial plexus tumors. The purpose of this report is to share our surgical experience with brachial plexus tumor with other neurosurgeons. Methods : The records of 18 consecutive patients with brachial plexus tumors who underwent surgical treatment between January 2010 and December 2017 in a single institution were retrospectively reviewed. The surgical approach was determined according to the tumor location and size, and intraoperative neurophysiological monitoring (IONM) was used in most of cases to prevent iatrogenic nerve injury during surgery. In addition, to evaluate the differences in tumor characteristics according to pathologic diagnosis, the tumors were divided twice into two groups, based on two separate classifications, and statistical analysis was performed. Results : The 18 brachial plexus tumors comprised 15 (83.3%) benign peripheral nerve sheath tumors including schwannoma and neurofibroma, one (5.6%) malignant peripheral nerve sheath tumor, one (5.6%) benign tumor of non-neural sheath origin (neurogenic cyst), and one (5.6%) metastatic tumor (papillary carcinoma). The authors analyzed relationship between tumor size/location and tumor characteristic parameters such as age, size, right-left, and pathology. There were no statistically significant differences except a tendency of bigger tumor size in young age. Conclusion : For a successful surgical outcome, an appropriate surgical approach is essential, and the appropriate surgical approach is determined by the location and size of the tumor. Furthermore, applying IONM may prevent postoperative complications and it is favorable option for brachial plexus tumors surgery.

Cone beam형 전산화 단층영상을 이용한 하악전치부 영양관의 연구 (Nutrient canals on mandibular anterior region in cone beam computed tomography)

  • Purpose : To evaluate location, distribution, diameter, and length of the nutrient canals on mandibular anterior region using a cone beam computed tomography (CBCT). Materials and Methods : Mandibular CBCT was performed on 33 adults (18 males and 15 females) with no history of systemic disease, and any other dental surgery history Location, distribution, diameter, and length of the nutrient canals on mandibular anterior region were radiographically evaluated. A statistical comparison was done by SPSS. Results : In the location and distribution of nutrient canals, they were found in 6.8% at labial portion above root apex, in 93.28% at lingual portion above root apex in 46.2% at labial portion below root apex, and in 53.6% at lingual portion below root apex. Nutrient canals at lingual portion above root apex were most frequently observed between central and lateral incisors, and those at labial and lingual portion below root apex were most frequently observed between central incisors. The mean diameters of nutrient canals were 0.54 mm at labial portion above root apex, 0.61 mm at lingual portion above root apex, 0.66 mm at labial portion below root apex, and 0.76 mm at lingual portion below root apex. The mean lengths of nutrient canals were 2.63 mm at labial portion above root apex, 3.74 mm at lingual portion above root apex, 4.51 mm at labial portion below root apex, and 6.77 mm at lingual portion below root apex. Conclusion : CBCT is useful device to evaluate the anatomical structure of nutrient canals on mandibular anterior region.

사이클링 스마트웨어 제작을 위한 빕 팬츠 디자인 및 패턴 개발 (Development of Bib Pants Design and Pattern for Cycling Smart Wear)

  • In this study, a cycling smart wear for measuring cycling posture and motion was developed using a three-dimensional motion analysis camera and an IMU inertial sensor. Results were compared according to parts to derive the optimal smart device attachment location, enabling correct posture measurement and cycle motion analysis to design a pattern. Conclusions were as follows: 1) 'S-T8' > 'S-T10' > 'S-L4' was the most significant area for each lumbar spine using a 3D motion analysis system with representative posture change (90°, 60°, 30°) to derive incisions and size specifications; 2) the part with the smallest relative angle change among significant section reference points during pattern design was applied as a reference point for attaching a cycling smart device to secure detachable safety of the device. Optimal locations for attaching the cycling device were the "S-L4" hip bone (Sacrum) and lumbar spine No. 4 (Lumbar 4th); 3) the most suitable sensor attachment location for monitoring knee induction-abduction was the anatomical location of the rectus femoris; 4) a cycling smart wear pattern was developed without incision in the part where the sensor and electrode passed. The wearing was confirmed with 3D CLO. This study aims to provide basic research on exercise analysis smart wear, to expand the smart cycling area that could only be realized with smart devices and smart watches attached to current cycles, and to provide an opportunity to commercialize it as cycling smart wear.

수태음폐경 노선에 대한 고찰: 천부·협백을 중심으로 (Discussion on the Route of Lung Meridian: Focus on LU3 and LU4)

  • Objectives : An error was found in the recent standard by the World Health Organization (WHO) on the locations of the Upper arm Route of Lung Meridian (URLM) and its acupoints LU3 and LU4. This possible incorrect information is being taught throughout Korean medicine colleges nationwide, which follow WHO standards. Therefore, an investigation is required to sort out this discrepancy based on the evidence in historical documents. Methods : The location of WHO's URLM and LU3 and LU4 were compared with corresponding information in the classical literature. The anatomical structure mentioned in these classical documents was examined. Finally, an assessment was conducted on whether this structure is reflected in the WHO standards. Results : Classical literature prior to the early 20th century records the locations of the Lung Meridian and LU3 and LU4 of the upper arm to be in the artery on the medial aspect. This artery corresponds to the brachial artery. The location established by the WHO is on the anterolateral side of the upper arm, where no large arteries exist that can be found by haptic search or angiographically. The anterolateral side of the upper arm belongs to the Yang aspect, which does not coincide with the Yin aspect of Lung Meridian. Conclusions : The WHO's URLM and LU3-4 standards do not agree with the classical literature. The correct route must coincide with the brachial artery passing through the medial side of the humerus. The actual location of LU3-4 is on the medial aspect of the arm, just medial to the border of the biceps brachii muscle, on the brachial artery, 3-4 B-cun inferior to the anterior axillary fold.

내측 비복 천공지 유리피판을 이용한 수부재건 (Hand Reconstruction with Medial Sural Perforator Free Flap)

  • Purpose: The main advantages of the perforator flap are minimal donor site morbidity, preservation of any main source artery and its thin characteristics. Most perforator flaps for hand reconstruction need primary and secondary procedures such as a flap debulkiness and liposuction etc. However, flap thickness of calf area is thinner than any other perforator flaps. Methods: We performed an anatomical study and clinical application of medial sural artery perforator flap. We found that there are two or more medial sural perforators located on a straight line drawn from the mid-point of popliteal crease to the mid-point of medial malleolus. Most pathway of medial sural artery comes along with this line. It is possible to observe the first perforators almost exactly 8 cm from midpoint of popliteal crease in a distal half circle drawn with a radius of 2 cm. Results: We report 12 cases in 11 patients of hand reconstruction with medial sural perforator free flap from Febrary 2003 to Febrary 2006. Complete healing was possible in 11 cases. Total flap loss for venous insufficiency was in 1 patient. During the follow-up, good contour and full range of motion was observed on hand reconstruction with medial sural perforator free flap. Conclusion: In the authors' experience, this anatomical study made it possible to prepare a diagram of the exact location of the medial sural perforators. This flap can be used to achieve acceptable functional and aesthetic results for hand reconstruction because of its thin characteristics.

스티칭 기법을 이용한 대퇴골 방사선검사 시 확대 표식자 위치 및 스티칭 범위 변화가 영상에 미치는 영향 (The Effect of the Position Change of the Magnification Marker and the Stitching Range on the Image in the Examination of the Femur Radiography Using Stitching Technique)

  • In this study, we would like to examine the effect of the location of the magnification marker and the change in the stitching range on the magnification marker and the expansion of the anatomical structure in the examination of femoral stitching radiation using the stitching technique, and to find out the usability of the femoral stitching radiation examination in the preoperative examination for THRA. The measured values measured by 5 researchers were summed and presented through descriptive statistics. When the position of the magnification marker was changed, the average value of the measured value was in the range of 113.54 to 113.90 mm. The anatomical structures of the femur were measured in the range of 57.24 to 57.78 mm, 27.20 to 27.56 mm, and 70.11 to 70.81 mm, respectively. This showed a similar deviation from the expansion rate of previous related papers using markers. Therefore, it is considered that femoral stitching X-ray examination using magnification markers can be used for preoperative examination for THRA.

Cone beam형 전산화 단층촬영장치를 이용한 설공의 해부학적 구조 (Anatomical structure of lingual foramen in cone beam computed tomography)

  • Purpose: To evaluate whether cone beam computed tomography can depict the distribution, position, frequency, relative vertical dimension, and the diameter of the lingual foramen and direction of lingual bone canal. Materials and Methods : Cone beam computed tomography of mandible was performed on 25 males and 25 females with no history of any orthodontic treatments or any other dental surgeries. A statistical comparison was done on the mean values of males and females. Results: In the location and distribution of lingual foramina, median lingual foramen was found in all subjects and lateral lingual foramen in 58%. In the lateral lingual foramen, bilateral type was found in 28% and unilateral type in 30%. In the number of lingual foramina, median lingual foramen had two foramina and lateral lingual foramen had one foramen, mostly. In the relative mean vertical dimension of lingual foramina, median lingual foramen was 0.03±0.08, and both lateral lingual foramina was 0.20±0.04. The mean diameter of lingual foramina, median lingual foramen was 0.9mm±0.28, right lateral lingual foramen was 0.92mm±0.23, and left lateral lingual foramen was 0.88mm±0.27. The most frequent direction of the lingual bone canals, median lingual bone canal proceeded in anteroinferior direction and lateral lingual bone canal in anterosuperolateral direction. Conclusion : Cone beam computed tomography can be helpful for surgery and implantation on the mandibular area. Radiologist should be aware of this anatomical feature and its possible implications.

새 기형 85예에 대한 임상경험 (Experience with 85 Cases with Branchial Anomalies)

  • Branchial anomaly is a frequently occurring congenital abnormality in childhood. It is important for the pediatric surgeon alike to be familiar with the embryology and differentiation of head and neck structure to accurately diagnose and treat these lesions. Eighty-five patients with branchial anomaly treated at Hanyang University Hospital between 1980 and 2001 were reviewed to determine relative frequency, clinical classification and appropriate treatment. The male to female ratio of branchial anomaly was 1.2:1. The most commonly presenting age was before 1 year (32%) and the age group between 1 and 3 year (22%) followed it. According to the classification of branchial anomalies, 73 of 85 cases were second branchial anomaly, 9 had the first type and 3 did fourth type. One patient showed combined anomalies of the first and the second type. Infection sign were seen in 70% of patients at the time of the first visit to our hospital and also patients' symptoms were frequently related with the infection. Forty-one cases (48%) were fistula, 21 (25%) were cysts, 21 (25%) were sinuses, and two were only cartilage remnants. The most common type of the branchial anomalies is the second branchial fistula and the most common symptoms of the anomalies are related with infection. Initial proper diagnosis and anatomical classification of the anomalies are very important in managing the lesions. The efforts to find the exact anatomical location of the fistula or sinus tract are necessary because total excision of the lesions including those tracts is the only way to prevent recurrence.

치조골 높이 측정시 표준촬영과 교익촬영의 비교 (RELATIONSHIPS BETWEEN BITEWING AND PERIAPICAL RADIOGRAPHS IN ASSESSING CRESTAL ALVEOLAR BONE LEVELS)

  • Bitewing and periapical radiographic techniques are used in clinical and epidemiological studies to assess crestal alveolar bone levels. The purpose of the present study investigated relationships between these techniques by assessing alveolar crest location at the same site. Bitewing and periapical radiographs were available from posterior quadrants of 120 subjects. The distance from cemento-enamel junction to the alveolar crest (CEJ -CR) was measured for each proximal surface from the distal of the cuspid to the distal of the second molar. Data were arranged according to the proximal surface examined, and bitewing and periapical measurements were compared using paired tests. The obtained results were as follows; 1. In maxilla, a significant t ratio with a P value of 0.05 or less was reached for 78% and in mandible reached for 39%. 2. Pearson correlations were calculated 32 surfaces (89%) had values below 0.70 and so, data obtained by these techniques can not be used interchangeably on an individual or group basis. 3. In situations where the periapical measure was greater than the bitewing, it ranged up to 20.7% greater with a mean 9.8%. Where the bitewing was greater than periapical, it ranged up to 51. 9% greater, with a mean 18.2%. 4. The percentage difference was greater in maxillary posterior areas than in mandibular molar, premolar areas. A lower prevalence of significant differences was attributed to relatively more simple root and favorable radiographic conditions in mandibular molar and premolar areas. 5. The anatomical limitations imposed on periapical radiographic technique, most often result in somewhat foreshortened radiographic images. This situation would tend to be accentuated by the anatomical restrictions of the hard palate. 6. Consequently, since the significant differences frequently exist between measurements obtained from bitewing and periapical techniques, it is important to define which technique is used.
