An anatomical investigation of the suboccipital- and inferior suboccipital triangles

  • The suboccipital triangle (ST) is a clinically relevant landmark in the posterior aspect of the neck and is used to locate and mobilize the horizontal segment of the third part of the vertebral artery before it enters the cranium. Unfortunately, this space is not always a viable option for vertebral artery exposition, and consequently a novel triangle, the inferior suboccipital triangle (IST) has been defined. This alternative triangle will allow surgeons to locate the artery more proximally, where its course is more predictable. The purpose of this study was to better define the anatomy of both triangles by measuring their borders and calculating their areas. Ethical clearance was obtained from the University of Pretoria (reference number: 222/2021) and both triangles were subsequently dissected out on both the left and right sides of 33 formalin-fixed human adult cadavers. The borders of each triangle were measured using a digital calliper and the areas were calculated using Herons Formula. The average area of the ST is 969.82±153.15 mm2, while the average area of the IST is 307.48±41.31 mm2. No statistically significant differences in the findings were observed between the sides of the body, ancestry, or sex of the cadavers. Measurement and analysis of these triangles provided important anatomical information and speak to their clinical relevance as surgical landmarks with which to locate the vertebral artery. Of particular importance here is the IST, which allows for mobilisation of this artery more proximally, should the ST be occluded.

한국 여성의 피부 부위 및 연령에 따른 피부 측정 인자와 생물 인자 연구 (A Study of Skin Biophysical Parameters and Biomarkers related to the Anatomical Site and Age in Korean Women)

  • 피부는 신체에서 가장 크고 육중한 기관 중 하나로 인간의 생리 및 병리 과정에 참여한다. 피부는 자기 유지 및 치유, 기계적 및 화학적 손상 방어, 자외선 과 외부 병원성 미생물로부터의 방어, 비타민 D 합성 그리고 사회 심리적 기능을 한다고 알려져 있다. 이 연구의 목적은 한국 여성의 부위와 연령에 따라 피부 생물학 인자와 연관된 생물리학 인자의 변화를 평가하는 데 있다. 20 ~ 49세의 약 70명의 건강한 성인 여성이 이 실험에 참여하였다. 측정부위는 하박 내측과 뺨으로 진행하였다. 인체 피부의 생물리학 인자를 측정하기 위하여 여러 가지 비침습적인 방법으로 진행하였다. 피부의 생물학 인자를 분석하기 위하여 코티졸, 파이브로넥틴, 케라틴-1, 10, 11, 인보루크린, 케라틴 6를 인체의 얼굴과 하박내측으로 비교하였다. 또한 비침습적 방법으로 피부 생물리학 인자는 피부 부위와 연령에 따른 차이를 측정하였다. 측정 부위에 따른 결과, 각질층 수분량, 경피수분손실량과 피부색(L과 a값)은 유의적인 차이가 나타났다. 연령에 따른 결과, 오직 피부색에서만 연령에 따른 차이가 유의적으로 나타났다. 코티졸, 케라틴-6, 파이브로넥틴, 케라틴-1, 10, 11 은 연령과 부위간 유의적 차이가 없지만 인볼루크린은 30 ~ 39세 연령대에서 다른 연령대보다 유의적으로 가장 높았다. 이러한 결과는 개인의 피부 환경에 대한 상세한 피부 상태 변화로 설명할 수 있을 것이다.

영건의궤 기록을 활용한 조선후기 국용목재의 주요 수종 조사 및 해부학적 특징 (Major Species and Anatomical Characteristics of the Wood Used for National Use Specified in Yeonggeon-Uigwes of the Late Joseon Dynasty Period)

  • 조선후기 국용목재에 사용된 수종을 파악하기 위하여 영건의궤 조선후기 11종의 영건에 명시된 수종을 분석하였다. 주요 용도로는 궁궐, 제실, 사묘, 성곽의 수리와 보수 공사에 사용된 것으로 파악되었다. 17세기~20세기 영건의궤에 명시된 수종 중 활엽수재로는 가래나무, 느티나무, 물푸레나무, 박달나무, 오동나무, 참나무류(갈참나무, 굴참나무, 떡갈나무, 상수리나무, 신갈나무, 졸참나무)가 확인되었으며, 침엽수재로는 소나무와 잣나무가 확인되었다. 문화재를 수리할 때에는 동일 수종을 원칙으로 하기 때문에 수종 조사는 중요한 부분이다. 각 수종에 대한 목재해부학적 이미지는 실제 사용된 목부재를 대신해서 국립산림과학원 목재표본실에 소장 되어있는 같은 수종 재감을 활용하였다. 조선후기 영건의궤 편역본을 통하여 목조문화재 건축물에 사용된 주요 수종을 알 수 있었으며, 수리·보수 시 사용될 목재 수종을 판정하는 데 필요한 목재해부학적 이미지를 마련하였다.

  • When the chest roentgenogram reveals the presence of multiple pulmonary nodules, the basic investigation includes a history, physical examination, routine hematologic and urine studies, and sputum specimens to search the etiology. We have experienced a case of endometrial sarcoma with metastasis to the lung.

Taxonomic Study on the Lichen Genus Xanthoparmelia (Ascomycotina, Parmeliaceae) in Korea

  • In previous studies investigating the genus Xanthoparmelia, thirteen different species have been reported from South Korea alone. However, there currently has been no revisional study performed until now. To explore the genus Xanthoparmelia, a phenotypic analysis was performed based on morphological, anatomical and chemical characters, while an investigation of Xanthoparmelia phylogeny was based on nuclear ribosomal (nr) DNA ITS sequences. A thorough examination of the specimens deposited in the Korean Lichen Research Institute (KoLRI) confirmed that eight species of Xanthoparmelia occur inside South Korea. Our analysis further confirmed the colors of the lower surface and medullar chemistry are important taxonomic characters in Xanthoparmelia. This study also presents a detailed description of each species and a key to the genus.

Characteristics of Animal Shoulder Models for Rotator Cuff Experiments

  • Animal shoulder models are important in the investigation of the natural history of various pathologic conditions and for evaluation of the effectiveness of different treatments and biomaterials. The characteristics of animal shoulder models that may be used for rotator cuff related experiments are different with regard to the anatomy, behavioral pattern, advantages and disadvantages in application to each other. The lower primates and all the non-primate species, except the tree kangaroo, were neither functional overhead nor had a true rotator cuff. Utilizing more advanced primates, or perhaps even the tree kangaroo would be ideal and the most relevant to man as they possess a true rotator cuff. However, ethical concerns, costs, and many limitations in obtaining primates generally preclude the use of these animals for such research. Finally we should consider the differences in comparative anatomy and behavioral pattern of each animal model during performance or interpretation of rotator cuff experiments.

Anatomical Proportions and Chemical and Amino Acid Composition of Common Shrimp Species in Central Vietnam

  • This investigation was conducted to evaluate the shrimp flesh (SF) and shrimp by-product (SB) of the most abundant shrimp species (Metapenaeus affinis, Penaeus semisulcatus and Penaeus monodon) caught in Central Vietnam, with the emphasis on yield, gross and amino acid (AA) composition and effect of heat treatment. The results showed that the mean edible SF and SB (head and shells with tail) yields of the three shrimp species averaged 56.7 and 43.3%, respectively, of the total wet body weight, with the M. affinis generating the highest by-product yield (45.7%) and P. semisulcatus (40.6%) the lowest. Significant differences in dry matter (DM), crude protein (CP) and ash content were found between SF and SB. The DM content of SF (21.5%) was lower than of SB (24.9%) and the ash content (on a DM basis) of the SB in all shrimp species was more than three times that of the SF (p<0.05), whereas the CP content was almost twice as high in the SF as compared with the SB (p<0.05). The SB of the three species contained (on a DM basis) between 44.0 and 49.8% CP (p<0.05) and between 13.5 and 18.1% chitin (p<0.05). The Ca content of SB differed also between species (p<0.05). On average, the sum of AA in SB corresponded to 89.3% of the CP and essential AA accounted for about 50% of the total AA. The most abundant AA were arginine, aspartic and glutamic acids, which accounted for 33% of the total AA. Minor, but significant differences in some AA concentrations of SB between species were observed (p<0.05). With the exception of the DM and ether extract content, all other chemical constituents of entire shrimp, SF and SB were not significantly affected by heat treatment (p>0.05).

A New Septum in the Female Breast

  • Background Understanding the female breast fascial system is of paramount importance in breast surgery. Little was written about breast ligaments. Most articles refer to Cooper's work without further anatomical studies. Lately, a horizontal septum has been described conveying nerves and vessels to the nipple areola complex. Methods During the surgical dissection of the lower part of the breast, in supero-medial technique for breast reduction operations, a fascial septum between the lower two quadrants was detected. This fibrous septum was studied through anatomic dissection of breast tissues during routine breast reshaping procedures that was done on 30 female patients. Magnetic resonance imaging (MRI) was performed preoperatively in all cases and correlated with the intraoperative findings. In the other five cases, outside the clinical study, the imaging was done during routine investigation for breast swellings. Results A vertical septum was identified in the lower part of the breast, lying at the breast meridian between the two lower quadrants. It is a tough bi-laminated structure that extends from the middle of the infra-mammary crease caudally to nipple-areola complex cranially and from the pectoral fascia posteriorly to the overlying skin anteriorly. This was proved by MRI findings. Conclusions This study describes a new inferior vertical septum which separates the lower half of the breast into two definite anatomical compartments: medial and lateral.

Evaluation of Human Papilloma Virus Infection in Patients with Esophageal Squamous Cell Carcinoma from the Caspian Sea Area, North of Iran

  • Introduction: HPV has been found repeatedly in esophageal squamous cell carcinoma (ESCC) tissues. However, reported detection rates of HPV DNA in these tumors have varied markedly. Differences in detection methods, sample types, and geographic regions of sample origin have been suggested as potential causes of variation. We have reported that infection of HPV DNA in ESCC tumors depends on anatomical sites of esophagus of the patients from Mazandaran, north of Iran. Materials and Methods: HPV DNA was examined in 46 upper, 69 middle and 62 lower third anatomical sites of esophageal squamous cell carcinoma specimens collected from Mazandaran province in north Iran, near the Caspian Littoral as a region with high incidence of ESCC. HPV L1 DNA was detected using Qualitative Real time PCR and MY09/MY11 primers. Results: 28.3% of upper, 29% of middle and 25.8% of lower third of ESCC samples were positive for HPV DNA. 13.6% for males and 14.1% for females were HPV positive in all samples. Conclusions: HPV infection is about one third of ESCC in this area. Findings in this study increase the possibility that HPV is involved in esophageal carcinogenesis. Further investigation with a larger sample size is necessary.

구강재건을 위한 족배동맥피판의 해부학적 고찰 (Anatomical review of dorsalis pedis artery flap for the oral cavity reconstruction)

  • The dorsalis pedis artery (DPA) was renamed from the anterior tibialis artery after it passed under the extensor retinaculum, and DPA travels between the extensor hallucis longus and extensor digitorum longus muscle along the dorsum of the foot. After giving off the proximal and distal tarsal, arcuate and medial tarsal branches, DPA enters the proximal first intermetatarsal space via the first dorsal metatarsal artery (FDMA), which courses over the first dorsal interosseous muscle (FDIM). For detailed knowledge of the neurovascular anatomy of a dorsalis pedis artery flap (DPAF) as a routine reconstructive procedure after the resection of oral malignant tumors, the precise neurovascular anatomy of DPAF must be studied along the DPA courses as above. In this first review article in the Korean language, the anatomical basis of DPAF is summarized and discussed after a delicate investigation of more than 35 recent articles and atlas textbooks. Many advantages of DPAF, such as a consistent flap vascular anatomy, acceptable donor site morbidity, and the ability to perform simultaneous flap harvest using oral cancer ablation procedures, and additional important risks with the pitfalls of DPAF were emphasized. This article will be helpful, particularly for young doctors during the special curriculum periods for the Korean National Board of Specialists in the field of oral and maxillofacial surgery, plastic surgery, otolaryngology, orthopedic surgery, etc.
