The present study was conducted to investigate the anti-allergic activity of Gunggwihyangso-San(GHS). We investigated the anti-allergic effects of GHS in RBL-2H3 basophilic leukemia cells by compound 48/80, a mast cell degranulator and compound 48/80 induced anaphylactic shock in mice. Gunggwihyangso-San significantly inhibited ${\beta}$-hexosaminidase and histamine release from compound 48/80 stimulated RBL-2H3 cells. In addition, GHS effectively inhibited anaphylactic shock in mice by 50% at a dose 80 mg/mouse versus PBS treated control after the I.p injection(8 mg/kg) of compound 48/80. The in vitro anti-inflammatory activities of GHS in LPS-stimulated RAW 264.7 cells were investigated. GHS inhibited NO production in LPS-stimulated RAW 264.7 cells and effectively dowregulated the expression of iNOS mRNA and iNOS protein expression in LPS-stimulated RAW 264.7 cells. These result provide evidences that GHS may be beneficial in the treatment of allergic inflammtory disease.

Although allergic reactions are not rare complications in drug use, anaphylaxis or anaphylactoid reactions to some widely used drugs can embarrass clinicians because anaphylaxis is not easily diagnosed at the time of the event and treatment is unfamiliar to many. Lidocaine is a very popular drug in dental procedures and anaphylactoid reaction to it has been rarely reported. Clinicians who use lidocaine daily should, however, be aware of the possibility of anaphylaxis after its use. Once it occurs, anaphylaxis can be fatal, but if it is quickly diagnosed or suspected, treatment is simpler than most clinicians believe. An 86-year-old woman experienced an anaphylactic reaction 30 min after local infiltration of lidocaine for retraction of retained teeth. The dentist called an anesthesiologist for assistance. Fortunately, an anaphylactic reaction was quickly suspected and after subsequent rapid treatment with the administration of fluid and drug therapy, the patient recovered completely.

The flowers of Lonicera japonica Thunb. (Caprifoliaceae) have been used as a traditional effective drug in treating bacillary dysentery, purulence. However, the exact role of Lonicera japonica in allergic reaction has not been clarified yet. Immediate hypersensitivity, popularly known as allergy, is a major clinical problem in humans. It has been found that the histamine release from mast cells is an essential step in the pathological process of immediate hypersensitivity. In this study, the effect of aqueous extract of Lonicera japonica (AELJ) on allergic reaction was investigated. AELJ inhibited the compound 48/80-induced anaphylactic reactions and antigen-induced passive cutaneous anaphylaxis (PCA). AELJ in vitro exhibited a dose-dependent inhibition of degranulation in RPMC stimulated by compound 48/80. AELJ also suppressed the morphological changes and the increase of intracellular free calcium level induced by compound 48/80. These results suggest that inhibitory effect of AELJ on allergic reaction may be mediated through the decrease of intracellular free calcium levels, and AELJ importantly contributes to the treatment of anaphylaxis and may be useful for other allergic disease.
This study was conducted to investigate antigenic potential of DA-3030, a recombinant human granulocyte-colony stimulating factor, in guinea pigs and mice. In the active systemic anaphylaxis test, the guinea pigs sensitized with 1.25 or 12.5 $\mu\textrm{g}$/head of DA-3030 alone did not show any anaphylactic reaction. In the homologous passive cutaneous anaphylaxis reaction, anti-DA-3030 antibody was not detected in guinea pigs sensitized with 1.25 or 12.5 $\mu\textrm{g}$/head of DA-3030 alone. On the other hand, the guinea pigs sensitized with 12.5 $\mu\textrm{g}$/heed of DA-3030 incorporated in Freund's complete adjuvant(FCA) or 1 mg/head of ovalbumin incorporated in FCA showed anaphylactic reaction. Anti-DA-3030 antibody was also detected in those guinea pigs. In immunodiffusion test using the sera sensitized with DA-3030 incorporated in FCA, precipitating antibodies were detected only in the sera sensitized with DA-3030 or DA-3030 incorporated in FCA showed. In 24-hour heterologous PCA reaction with sera of C57BL/6 mice immunized with 1.25 or 12.5 $\mu\textrm{g}$/head of DA-3030 alone, none of the sera showed positive reaction. But sera of the animals immunized with 12.5 $\mu\textrm{g}$/head of DA-3030 incorporated in aluminum hydroxide gel(Alum) or 5 $\mu\textrm{g}$/head of ovalbumin incorporated in alum showed positive PCA reaction. DA-3030 did not cause anaphylactic shock or passive cutaneous anaphylaxis in guinea pigs and mice when given alone although DA-3030 incorporated in FCA or Alum induced anaphylactic shock and passive cutaneous anaphylaxis. From these results, it may be concluded the DA-3030 does not induce systemic allergic reaction when administered alone in its clinical use.
We compared the effect between CAS and WAS(root, stem) on mast cell-mediated allergic reaction. CAS, WAS-root and WAS-stem, significantly inhibited compound 48/80-induced systemic allergic reaction(1g/kg) and histamine release from RPMC(1mg/ml). CAS, WAS-root and WAS-stem also inhibited passive cutaneous anaphylactic reaction. In addition, IgE-induced $TNF-{\alpha}$ secretion from RBL-2H3 was inhibited by pretreatment of CAS, WAS-root or WAS-stem$(0.01{\mu}g/ml)$. Taken together, inhibitory effect on mast cell-mediated allergic reactions of WAS-root is greater than those of WAS-stem but less than those of CAS.

Generalized anaphylaxis is a most dramatic and acutely life-threatening allergic reaction and may cause death within a few minutes. Differential diagnosis of anaphylaxis is made by clinical signs, such as, mental change, respiratory distress, hypotension, hypoglycemia, urticaria and angioedema. Especially, insulin reaction, myocardial infarction and vasovagal syncope are considered as differential diagnosis. In cases of fatal anaphylaxis, respiratory and cardiovascular disturbances predominate and are evident early in the reaction. This is a case report of the intensive care of anaphylactic shock after intravenous injection of the penicillin in a old medically compromised patient with the maxillary osteonecrosis. The anaphylactic shock symptoms, such as, unconsciousness, respiratory disorder, no pulsation on carotid artery and cardiopulmonary arrest are occurred in intravenous injection of augmentin 1.2 g after the skin test. In spite of immediate emergency cares, such as intravenous injection of epinephrine, endotracheal intubation, cardiopulmonary resuscitation, and continuous intensive care, the patient is expired in 58 hours after anaphylactic shock attack.

Experimental studies were done to research the dinical effects of Juglandis Semen aqua-acupuncture and acupuncture(BL 13) on the anti-allergic response. anaphylaxis provoked by the compound 48/80, delayed type hypersensitivity response to picryl chloride and SASC and inflammation response to egg albumin. The following results have been obtained: Juglandis Semen aqua-acupuncture and acupuncture(BL13) group were increased the survival rate in compound48/80 induced systemic anaphylactic reaction. Picryl chloride induced contact dermatitis and delayed type hypersensitivity in SRBC challenged mouse were significantly decreased in Juglandis Semen aqua-acupuncture and acupuncture (BL13) group. Inflammation response - WBC, CRP and Nitric Oxide in egg albumin induced allergic rat were significantly decreased in Juglandis Semen aqua-acupuncture and acupuncture(BL13) group. According to the above results, Semen aqua-acupuncture and acupuncture(BL13) both depress the allergy reaction.

Objective : Anal Therapy is another way of taking medicine and applies to each field of clinical treatment extensively. Sinpo-tang(SPT) has been used for the treatment of a allergic rhinitis. In this study, the auther investigated the anti anaphylactic action of Sinpo-tang by anal therapy was investigated on cutaneous allergic reaction models. Methods : Results : 1. Sinpo-tang (0,001-0.1 g/kg) dose-dependently inhibited the compound 48/80-induced ear swelling response in mice. Inhibitory effect of Sinpo-tang was significant (P < 0.05) at the doses of 0.01, 0.1 g/kg. 2. Sinpo-tang (0.001-0.1 g/kg) inhibited the cutaneous allergic reaction activated by anti-dinitrophenyl IgE in rats. Of special note, Sinpo-tang (0.1 g/kg) inhibited the cutaneous allergic reaction by $68\%$. 3. This inhibitory effect of Sinpo-tang was confirmed by observation of alcian blue/nuclear fast red stained-mast cells in the cutaneous tissue. 4. Sinpo-tang (0.01-1 g/L) dose-dependently inhibited the compound 48/80-induced histamine release from the peritoneal mast cells. Conclusions :These results indicate that anal therapy of Sinpo-tang may be beneficial in the treatment of mast cell-mediated anaphylaxis by inhibition of histamine release from mast cells in vivo and in vitro.

Generalized anaphylaxis is a most dramatic and acutely life-threatening allergic reaction. Most fatalities from anaphylaxis occur within the first 30 minutes postantigenic exposure. The mechanism of generalized anaphylaxis is the reaction of IgE antibodies to an allergen that causes the release of histamine, bradykinin, and others. These chemical mediators cause the contraction of smooth muscles of the respiratory and intestinal tracts, as well as increased vascular permeability. Four major clinical symptoms are recognized: skin reactions, smooth muscle spasm (gastrointestinal and genitourinary tracts and respiratory smooth muscle), respiratory distress, and cardiovascular collapse. Epinephrine is the drug of choice for the management. Its syrnpathomimetic effects directly counteract most aspects of the attack. Respiration must be immediately supported by the establishment of a patent airway along with artificial ventilation. The circulation should be supported and the existing hypotension overcome by placing the victim in a position to allow gravity to aid venous return and by administering intravenous fluids, vasopressors, and corticosteroids. When an imperceptible pulse is evident, external cardiac compression must also be instituted. This is a case report of anaphylactic shock care during general anesthesia, possibly due to penicillin, pancuronium and others.
