인삼 및 인삼 사포닌이 쥐의 건망증 및 신경세포배양에 미치는 영향 (Effects of Ginseng and Its Saponins on Experimental Amnesia in Mice and on Cell Cultures of Neurons)

  • 본 연구는 중앙대서전에 인삼의 주 효과중 건망증 방지작용이라고 수록되어 있는 것을 확인하기 위하여 행하여졌다. Step through 시험에서 $GRb_1$$GRg_1$은 기억력 획득을 용이하게 하였으며 전기충격 쇼크에 의해 야기되는 기억상실 효과를 억제하였다. 게대가 $Rg_1$은 에타놀 야기 기억재생 저해작용을 방지하였다. Stepdown test에 있어서 $Rb_1,\;Rb_2,\;Rg_1$은 전기충격쇼크에 의해 야기되는 기억보지 저해효과를 방지하였다. 또한 $Rg_1$은 에타놀에 의해 야기되는 기억재생 저해효과를 방지하였으며 단기적 기억력 획득을 용이케 하였다. Shuttle box와 lever press 시험에서도 $Rb_1$을 제외하고 나머지는 기억획득과 재생에 효과를 미치지 못했다. $Rb_1$은 shuttle box 시험에서 조건회피반응의 재생 (retrieval)을 억제하였다. 이와 같은 4가지 시험이 끝난후에 인삼의 구강투여가 진정, 진통, 해열, 진경효과와 자발 및 탐색활동에 미치는 영향을 500 mg/kg 투여 범위내에서 조사하였으나 아무런 반응도 관찰되지 않았다. 현재까지 연구결과는 $Rg_1$이 기억의 재생 및 학습반응의 획득과정에 대하여 효과가 있다는 것을 의미한다. 최근의 신경성장인자(NGF)가 성숙동물에 있어 뇌신경세포의 생존, 재생 및 조절에 미치는 작용에 관한 연구결과는 지능장해와 건망증에 대한 신경성장인자의 중요성을 시사했다. NGF에 의해 야기된 신경세포돌기 성장의 특이성에 관한 연구결과를 병아리 배배근 신경절에 있어서 NGF의 영향은 $Rh_1$에 의해 증가되었다. 다음으로 $Rb_1$은 병아리 배의 감각 및 교감신경단위에 있어서 NGF-관련 신경섬유 증가를 강화시켰다. NGF와 $Rb_1$을 함께 했을 때도 역시 쥐 대뇌피질의 신경세포 생존수를 증가시키는 경향을 보였다. NGF는 쥐의 배격부 부근의 신경세포를 배양했을 때 cholineacetyl transferase 활성을 증가시키지 않았다. 이러한 결과로 $Rb_1$은 뇌에 있어서 신경세포의 생존이나 재생에 중요한 역활을 한다는 것은 알 수 있었다.

Medetomidine의 투여가 흰쥐의 진통과 진정효과에 미치는 영향 (Effects of Medetomidine on Analgesia and Sedation in Rats)

  • Medetomdine의 투여가 흰쥐의 진통과 진정효과에 미치는 영향에 대하여 평가하였다. 실험동물은 생리식염수 1 ml/kg 투여군 (group 'Saline'), butorphanol, 2.0 mg/kg 투여군 (group 'BUT'), medetomidine, 0.2, 0.4, 0.8 or 1.6 mg/kg 투여군 (각각 group 'MED0.2', 'MED0.4', 'MED0.8' and 'MED1.6') 등 6개의 실험군으로 나누어졌다. 진통효과는 $50^{\circ}C$ hot-water tail-flick latency test로 측정하였고, 진정효과는 numerical sedation score (NSS)와 정위반사 (righting reflex)로 평가하였다. MED0.2군을 제외한 모든 medetomidine 투여군들이 Saline군에 비해 유의적인 진통효과의 증가를 보였다. MED0.2군과 비교할 때 MED0.4와 MED0.8군은 유의적인 진통효과의 상승을 보였으나, MED1.6군은 유의적인 변화를 보이지 않았다. BUT군과 모든 medetomidine 투여군은 진통효과에서 유의적인 차이를 보이지 않았다. Saline과 butorphanol의 투여는 진정효과와 정위반사의 소실을 유발하지 않았다. Medetomidine의 투여 용량의 증가는 진정효과를 증가시켰다. MED0.2군과 비교할 때 MED0.4, MED0.8과 MED1.6군의 NSS가 유의적으로 증가하였으나, MED0.4군에 비해서는 MED0.8과 MED1.6군이 유의적인 차이를 나타내지 않았다. 정위반사의 소실 시간은 MED0.2군에 비해 MED0.8과 MED1.6군이 유의적으로 감소하였다. 본 실험의 결과로 볼 때, 흰쥐에서 medetomidine 0.4에서 0.8 mg/kg 이내의 용량을 복강내 투여할 때 신뢰할 수 있는 진통효과와 진정효과를 동시에 얻을 수 있으며, 따라서 흰쥐의 진정, 진통 및 화학적 보정 목적에 적절한 용량임을 알 수 있었다.

유방암 환자의 통증, 우울 및 불안 증상 조절에 아로마 자가 치료의 효과 (The Effects of Aroma Self Massage in Hands on Pain, Depressive Mood and Anxiety in Breast Cancer Patients)

  • 목적: 아로마테라피는 진통 효과, 항우울 및 항불안 효과를 가지는 것으로 알려진 대체 의학의 한 종류이다. 본 연구는 유방암 환자에서 아로마 오일로 손 마사지를 교육시킨 후 대조군과 비교하여 활력 징후, 통증의 강도, 우울, 그리고 스트레스에 미치는 영향을 비교 분석하여 아로마 자가 치료의 효능을 검증 하고자 하였다. 방법: 20세 이상 유방암 환자 32명을 연구대상으로 하였으며, 비맹검 무작위 방법을 이용하였다. 아로마 자가-치료군(n=15)은 2주간 집에서 스스로 아로마 오일(프랑켄센스 버가못, 라벤더)을 사용하여 하루 2회 손 마사지를 시행하도록 하였고, 대조군(n=17)에게는 어떠한 중재도 하지 않았다. 두 군 모두에서 0, 1, 3 주에 3회에 걸쳐 통증 강도(Visual Analogue Scale: VAS, $0{\sim}10cm$), 불안(State-anxiety in State Trait Anxiety Inventory: STAI) 및 우울(Beck Depression Inventory Scales: BDIS), 스트레스(Brief Encounter Psychosocial Instrument: BEPSI수정판) 등을 조사하여 두 군 간의 자가치료 전, 후의 변화량 차이를 비교하였다. 또한 아로마 자가치료 후 환자의 동반 증상 변화도 설문지를 통해 분석하였다. 결과: 통증 강도는 대조군과 비교시 아로마 자가 치료군에서 의미 있게 감소하였다(VAS score $0.83{\pm}1.01\;vs\;0.38{\pm}0.86$, P=0.0046). 동반 증상의 수도(P=0.044), 우울 정도(P=0.001) 그리고 불안정도도아로마 자가 치료군(P=0.008)에서 2주 후 의미 있게 감소하였으나, 대조군은 오히려 증가하였다. 그러나, 스트레스 정도는 두 군 사이에 유의한 차이를 보이지 않았다($0.05{\pm}0.85\;vs\;0.04{\pm}0.20$ P=0.1519), 우울 정도, 불안 정도, 스트레스 정도는 자가치료 순응도가 좋을수록 감소하는 경향을 보였으나, 통계적으로 유의하지는 않았다. 아로마 자가 치료군에서 수축기 혈압은 약간 증가하였으나 통계학적 의의는 없었다($4.53{\pm}14.43mmHg\;vs\;0.0{\pm}7.22mmHg$, P=0.152). 자가 치료군에서 환자들은 두통(20%), 감각이상(6.7%), 오심(6.7%) 등의 부작용을 호소했으나, 이들 모두 일시적인 것으로 자가치료를 중단할 정도는 아니었다. 결론: 유방암 환자들에게 아로마 손 마사지 교육을 시킨 후 자기 스스로 2주간 시행한 결과, 통증강도, 우울 및 불안 정도가 의미 있게 감소하였다.

아동병동 간호사의 경력에 따른 아동 통증지식에 대한 연구 (A Study on the Knowledge Related to Children's Pain of pediatric Nurses According to their Career)

  • Purpose : The purpose of this study was to identify pediatric nurses's knowledge related to children's pain and pediatric nurses' career including educational experience and to examine whether there is a difference of their knowledge according to their career. Method : Subjects of this study was 115 pediatric nurses of one university hospital who understood purpose of this study and accepted to participate in this study. Subjects responded questionnaire by self report. The data were gathered for 7 days during the period from October 20 to October '2:7, 2003. The pediatric nurses' knowledge and attitude survey regarding pain developed by Manworren(2001) was used to identify the knowledge related to children's pain of pediatric nurses. Nurses' career consisted of level of education, presently working ward, duration of experience at clinic, pediatric clinic and presently working ward and educational experience related to children's pain during undergraduate courses, ward orientation and continuing education. Educational experience regarding children's pain was measured by whether they had education related to children's pain during undergraduate courses, ward orientation and continuing education or not and their perception on the contents of education was measured by 5 point Likert type scale. SPSS Windows was used to analyze the data. Scores of the knowledge regarding children's pain and career including educational experience of pediatric nurses were presented as mean and standard deviation. Mean percentage of correct answers responded by the subjects was presented under the category of introduction, assessment, pharmacological and nonpharmacological intervention for children's pain. The difference of scores of the knowledge regarding children's pain according to nurses' career was analyzed using t-test, ANOVA and Tukey as a post hoc. Statistical significance was accepted at the level of p<.05. Result : Pediatric nurses had deficient knowledge related to children's pain on the whole and did not trust pain complaint and underestimated the pain. They were short of concrete practical knowledge about pain assessment and pharmacological and nonpharmacological intervention for children's pain. They were excessively concerned with side effect and addiction of analgesic drugs and had a negative view point on nonpharmacological pain intervention. About 50% of the subjects received education regarding children's pain while their undergraduate courses. The number of nurses who received education regarding children's pain while their undergraduate courses was found to be greater among the recently graduated nurses. In ward orientation and continuing education, only less than 10% of the nurses received education regarding children's pain. Nurses perceived their experienced educational content deficient; experienced educational content consisted of pain introduction, assessment and pharmacological and nonpharmacological intervention for children's pain while their undergraduate course, ward orientation and continuing education. There were no significant differences of the knowledge regarding children's pain of pediatric nurses according to level of education, duration of experience at clinic, pediatric clinic and presently working ward. Scores of the knowledge regarding children's pain of nurses at neonatal ward were significantly greater than those of nurses at pediatric intensive care unit. Scores of the knowledge regarding children's pain of pediatric nurses who received education during their undergraduate course were significantly greater than those who did not receive it during their undergraduate courses. Also scores of the knowledge regarding children's pain of pediatric nurses who received one kind of educational experience were significantly greater than those who had no educational experience. Conclusion : Pediatric nurses had deficient knowledge of children's pain and underestimated the pain of children. Also they were short of concrete practical knowledge on pain assessment and intervention for children's pain.

Xylazine이 histamine 유리에 미치는 영향 (Effect of xylazine hydrochloride on histamine release)

  • It has been reported that degranulation of mast cells in rats, rabbits and dog was observed after dosing xylazine hydrochloride(Xh) which has been widely used as sedative, analgesic and muscular relaxant. Therefore, this experiment was conducted to examine the relations between Xh and histamine release and to identify the action of ${\alpha}$-adrenoceptors which exists on the suface of mast cells. 1. The content of histamine within serum was measured with HPLC by performing the O-phthalaldehyde(OPA) fluorescent derivation. The pretreatment method had a little modification from the conventional method. The pretreament was carried out in the following method. 0.2$m\ell$ of serum and 1$m\ell$ of butanol were added to mixed together and then the liquid was centrifugally separated at 4$^{\circ}C$ and 2,000 rpm for 3 minutes. 0.4$m\ell$ of 0.1N HCl and 1.6$m\ell$ of heptane were added to 0.8$m\ell$ of supernatant taken from the liquid, and they were mixed together. This mixture was also centrifugally separated at 4$^{\circ}C$ and 2,000 rpm for 5 minutes. The supernatant was thrown away and the OPA fluorescent derivation was carried out with 0.2$m\ell$ of the lower liquid then, 5 minutes after mixing 400${\mu}\ell$ of 0.1N HCl, 120${\mu}\ell$ of 1N NaOH and 40${\mu}\ell$ of 0.1% OPA in the 0.2$m\ell$ of the lower liquid,120${\mu}\ell$ of 3.57N H$_3$PO$_4$ was added to the mixed liquid, and the liquid, was mixed again and syringe-filtered. Then, the measurement was done with HPLC in the 30 : 70(ν/ν) ratio of 0.004M KH$_2$PO$_4$: CH$_3$CN, flow rate of 1.0$m\ell$/min., and a wavelength of λex= 350nm and λem=444nm at the column temperature of 27$^{\circ}C$, using the fluorescence detector. 2. The content of histamine in each laboratory animal appeared to be higher in such an order as rabbit, rat, guinea pig, dog, Korean indigenous goat, swine, Korean indigenous cattle, Holstein, and mouse, of which the individual mean values${\pm}$standard deviation were 2.0668 ${\pm}$ 0.6049. 0.4999 ${\pm}$ 0.2278, 0.4241 ${\pm}$ 0.1974, 0.1054 ${\pm}$ 0.0556, 0.1028 ${\pm}$ 0.0276, 0.0972 ${\pm}$ 0.0513, 0.0872 ${\pm}$ 0.0373, 0.0717 ${\pm}$ 0.0379, and 0.0706 ${\pm}$ 0.0366, respectively. 3. The content of histamine was measured at the moments of 15-, 30-, 60-, 120-minutes after inoamuscular injection of 20mg/100kg Xh into two to 4 years old Holstein weighing 600∼700kg. The result showed that there was a significant increase at the times of 30- and 90-minutes after injection(p<0.05). 4. Intramuscular injection of 3mg/10kg Xh was given to crossbred pug dogs weighing 2.5∼4.3kg. The content of histamine was measured at the times of 30-, 60-, 90- and 120-minutes after injection. The result revealed that there was a significant increase at the times of 60-and 90-minutes after injection(p<0.05). 5. Intramuscular injection of 10mg/$m\ell$∼25mg/$m\ell$ Xh in concentration of 0.1$m\ell$ was applied to Korean indigenous goat over 5 months old. Then, the content of histamine was measured at the times of 15-, 30-, 60- and 90-minutes after injection. A significant increase was shown at the times of 30- and 60-minutes after injection(p<0.05). 6. The content of histamine was measured at the moments of 30- and 60-minutes after intramuscular injection of 0.1-0.2$m\ell$ Xh (20mg/$m\ell$) into male rabbits weighting 2.5-4kg. A significant increase was found at the moment of 60 minutes after injection(p<0.001). 7. After administering Xh to the mast cell taken from the abdominal cavity of mouse, the content of histamine was measured. The result showed that the higher the concentration, the more significantly the content of histamine was increased(p<0.05). 8. Compound 48/80 was administered in concentration of 5$\mu\textrm{g}$/$m\ell$ and 10$\mu\textrm{g}$/$m\ell$ to the mast cell picked from the abdominal cavity of mouse. The result showed that there was a significant increase in the content of histamine in case of the concentration of 10$\mu\textrm{g}$/$m\ell$(p<0.05). It was found to be about 10,000 to 500,000 times stronger than the Xh. 9. After premedication of 1mg/kg of yohimbine hydrochloride as ${\alpha}$$_2$-adrenergic antagonist to rabbits, the Xh was administered to them. The result was that the value of histamine within serum was decreased significantly(p<0.001). 10. After premeditation of 1mg/kg of prazosin hydrochloride as ${\alpha}$$_1$-adrenergic antagonist to rabbits, the Xh was administered to them. It was found that the value of histamine within serum was decreased significantly(p<0.005). 11, Prazosin hydrochloride and yohimbine hydrochloride as ${\alpha}$$_1$-adrenergic antagonist, respectively, and ${\alpha}$$_2$-adrenergic antagonist were administerd. In this case, the value of histamine within serum was decreased significantly(p<0.0001). As the results, when the Xh is administered to various kinds of animals, the amount of histamine release within serum is increased. In view of the results so far achieved, it is concluded that Xh acted on both a$_1$-adrenoreceptor and ${\alpha}$$_2$-adrenoreceptor induces the degranulation of mast cell.
