Spinal Noradrenergic Modulation and the Role of the Alpha-2 Receptor in the Antinociceptive Effect of Intrathecal Nefopam in the Formalin Test

  • Background: Nefopam has shown an analgesic effect on acute pain including postoperative pain. The reuptake of monoamines including serotonin and noradrenaline has been proposed as the mechanism of the analgesic action of nefopam, but it remains unclear. Although alpha-adrenergic agents are being widely used in the perioperative period, the role of noradrenergic modulation in the analgesic effect of nefopam has not been fully addressed. Methods: Changes in the antinociceptive effect of intrathecal (i.t.) nefopam against formalin-elicited flinching responses were explored in Sprague-Dawley rats pretreated with i.t. 6-hydroxydopamine (6-OHDA), which depletes spinal noradrenaline. In addition, antagonism to the effect of nefopam by prazosin and yohimbine was evaluated to further elucidate the antinociceptive mechanism of i.t. nefopam. Results: Pretreatment with i.t. 6-OHDA alone did not alter the flinching responses in either phase of the formalin test, while it attenuated the antinociceptive effect of i.t. nefopam significantly during phase 1, but not phase 2. The antagonist of the alpha-2 receptor, but not the alpha-1 receptor, reduced partially, but significantly, the antinociceptive effect of i.t. nefopam during phase 1, but not during phase 2. Conclusions: This study demonstrates that spinal noradrenergic modulation plays an important role in the antinociceptive effect of i.t. nefopam against formalin-elicited acute initial pain, but not facilitated pain, and this action involves the spinal alpha-2 but not the alpha-1 receptor.

영선제통음(靈仙除痛飮)이 진통(鎭痛) 및 소염작용(消炎作用)에 미치는 영향(影響) (Studies on the Analgesic and Anti-inflammatory Effects of Youngsunjetong-Eum)

  • The results of the comparison among the effects of decoction, extract powder and Gamiyoungsunjetongeum are as follows: In the experiment of analgesic effect in rats, having pain caused by Acetic acid, decoction showed a considerable reduction; extract powder did not show any notability and the effect of added decoction was conspicuous. The Anti-inflammatory effect of decoction was notable during $90{\sim}240minutes$, but extract powder also did not show any notability and added decoction showed a remarkable effect. In the experiment of inhibition on heat denaturation of serum rats, decoction and extract powder showed the inhibiting effects of 18.2%, 17.9% and the added decoction was even more remarkable(28.3%).

Studies on the Pharmacological Actions of Cactus:Identification of Its anti-Inflammatory Effect

  • The ethanol extracts of Opuntia ficus-indica fructus (EEOF) and Opuntia ficus-indica stem (EEOS) were prepared and used to evaluate the pharmacological effects of cactus. Both the extracts inhibited the writhing syndrome induced by acetic acid, indicating that they contains analgesic effect. The oral administrations of EEOF and EEOS suppressed carrageenan-induced rat paw edema and also showed potent inhibition in the leukocyte migration of CMC-pouch model in rats. Moreover, the extracts suppressed the release of $\beta$-glucuronidase, a lysosomal enzyme in rat neutrophils. It was also noted that the extracts showed the protective effect on gastric mucosal layers. From the results it is suggested that the cactus extracts contain anti-inflammatory action having protective effect against gastric lesions.

The efficacy of combination treatment of gabapentin and electro-acupuncture on paclitaxel-induced neuropathic pain

  • Paclitaxel, a chemotherapeutic drug, induces severe peripheral neuropathy. Gabapentin (GBT) is a first line agent used to treat neuropathic pain, and its effect is mediated by spinal noradrenergic and muscarinic cholinergic receptors. Electro-acupuncture (EA) is used for treating various types of pain via its action through spinal opioidergic and noradrenergic receptors. Here, we investigated whether combined treatment of these two agents could exert a synergistic effect on paclitaxel-induced cold and mechanical allodynia, which were assessed by the acetone drop test and von Frey filament assay, respectively. Significant signs of allodynia were observed after four paclitaxel injections (a cumulative dose of 8 mg/kg, i.p.). GBT (3, 30, and 100 mg/kg, i.p.) or EA (ST36, Zusanli) alone produced dose-dependent anti-allodynic effects. The medium and highest doses of GBT (30 and 100 mg/kg) provided a strong analgesic effect, but they induced motor dysfunction in Rota-rod tests. On the contrary, the lowest dose of GBT (3 mg/kg) did not induce motor weakness, but it provided a brief analgesic effect. The combination of the lowest dose of GBT and EA resulted in a greater and longer effect, without inducing motor dysfunction. This effect on mechanical allodynia was blocked by spinal opioidergic (naloxone, $20{\mu}g$), or noradrenergic (idazoxan, $10{\mu}g$) receptor antagonist, whereas on cold allodynia, only opioidergic receptor antagonist blocked the effect. In conclusion, the combination of the lowest dose of GBT and EA has a robust and enduring analgesic action against paclitaxel-induced neuropathic pain, and it should be considered as an alternative treatment method.

오적산(五積散)의 소염(消炎) 진통작용(鎭痛作用)에 관한 연구(硏究) (Studies on the Anti-inflammatory and Analgesic Activities of Ohjuksan)

  • Ohjuksan has been used for the treatment of cold and pantalgia in traditional medicine. The anti-inflammatory activity of the aqueous extract from Ohjuksan (OJSE) was investigated utilizing acetic acid-induced edema and adjuvant arthritis in rats. The effects of this agent on acute toxicity and acetic acid-induced writhing syndrome in mice were also examined. OJSE did not showed acute toxicity at 2400mg/kg (p.o.) and 1200mg/kg(i.p.) body weight. It was also showed to have significant analgesic action on the writhing syndrome in mice induced by 0.7% acetic acid at 300,600mg/kg body weight. It showed anti-inflammatory activity in 5% acetic acid-induced edema and adjuvant arthritis with oral administration in rats and exhibited significant preventive effect on edema at 300 and 600mg/kg(p<0.01). In the method of adjuvant arthritis, orally administered for 19 days, it inhibited the hind paw edema in rats with 300 and 600 mg/kg body weight from 5 th day to 19th day. These results suggest that OJSE had analgesic and anti-inflammatory action.

  • Experimental studies were conducted to investigate for the effect of Tongkyu-tang on analgesic, antipyretic, sedative, anti-inflammatory, isolated ileum and anti-histamine actions. Tongkyu-Tang was composed of sixteen crude drugs including Astragali Radix, which has been used for the treatment of common cold, headache, rhinitis, nasal obstruction, rhincospasm, etc. The following results have been obtained; analgesic and antipyretic actions were obtained. Prolonged action against the hypnotic duration of pentobarbital were significantly noted in mice. Anti-inflammatory effects in the paw edema induced by 1% carrageenin and 1% dextran were significantly shown in rats. Spontaneous motilities of isolated ileum of mice were strongly suppressed, and contractions of isolated ileum of mice and guinea-pigs induced by acetylcholine chloride, barium chloride and histamine were remarkably inhibited. Antihistamine actions were noted. Considering the above experimental results, it is suggested that effects of Tongkyu-tang based on the Oriental Medical References were similarly consistent with the actual experimental results.

  • For developing plant anti-inflammatory agents, extracts from Lonicera japonica were obtained and evaluated for their anti-inflammatory and analgesic activities using acute/chronic inflammatory models and writhing tests. It was found that the extracts with a modified extraction method showed higher anti-inflammatory and analgesic activities than those of the extracts based on the ancient literatures.

  • Inflammatory mediators such as interleukin-1 (IL-1), tumor necrosis $factor-{\alpha}\;(TNF-{\alpha}),\;interferon-{\gamma}\;(IFN-{\gamma})$ and lipopolysaccharide (LPS) are thought to play major roles in joint diseases such as a rheumatoid arthritis (RA), and there is considerable evidence playing a role for these cytokines in osteoarthritis (OA). Therefore, we have studied the effects on anti-inflammation and analgesic by ethyl acetate fraction from 70% ethanol extract of Artemisia capillaries (EAC). As a positive control, apigenin, which is known as an anti-inflammatory agent as an iNOS inhibitor, was used and showed the dose-dependent inhibitory effect. EAC showed strong inhibitory efficacy against cytokine-induced proteoglycan degradation, $PGE_2$ production, nitric oxide (NO) production, and matrix-matalloproteinases (MMPs) expression in rabbit articular chondrocyte. In the writhing test induced by acetic acid, EAC $(200{\sim}400\;mg/kg)$ exhibited a dose-dependent inhibition of writhing. The results indicate that EAC have anti-inflammatory and analgesic activities, and could be a good herbal medicine candidate for curing of RA and/or OA.

근육내 전기자극에 의한 노인성 통증조절 효과 (Analgesic Effects of Intramuscular Stimulation Therapy on Pain in the Elderly)

  • The purpose of this study was to determine the effect of intramuscular stimulation (IMS) therapy in older persons with musculoskeletal pain. The subjects were 181 older persons (54 males, 127 females) with musculoskeletal pain. Intramuscular stimulation unit with needles (size $.3{\times}30$ mm) was applied for the treatment. The analgesic effects were measured by visual analog scale (VAS). Results showed that the post-treatment VAS score was significantly decreased after IMS therapy for fifteen minutes compared to pre-treatment score. In addition, the post-treatment VAS score was significantly decreased in patients with chronic pain (pain duration of one year after onset) compared to the post-treatment VAS score in patients with subacute pain (pain duration less than three months after onset). There was no significant difference in analgesic effects according to gender and age groups. It is determined from this study that IMS therapy can be beneficial for patients with chronic musculoskeletal pain in clinical setting. Further study is needed to identify whether the IMS therapy can change the pain threshold in patients with neurologic pain.
