On the Cross of Disability and Old Age (장애인에서 노인으로: 장애와 노령의 접점에서 살펴본 장애인의 나이 들어감에 관한 연구)

  • While there have been numerous studies on various issues of aging of the individuals and the population, little is known about the process and consequences of aging of the disabled. Aging is irreversible for the disabled as well as for the not-disabled. With the advances of age, the disabled confront double jeopardy from disability and old age. This study aims to explore the meaning of aging and death for 40s and 50s living with disability for more than 20 years. It utilizes the data set collected through focus groups discussions of the disabled The cases in our analysis are composed of the disabled capable of articulating their feelings and thoughts. Thus, they are selected from the less privileged ones. The analysis shows that aging of the disabled, often involving secondary disability, overwhelms their current disability. Nonetheless, it does not necessary mean that they become an ordinary elderly person. Rather they still feel exclusion from the elderly for their disability. As getting older they turn to face social barrier for both disability and old age. Also, they are trying to live their present lives in full while accepting the presence of death Our findings are hoped to help to understand how the disabled get old and view their death in their own way.

Which Index for Muscle Mass Represents an Aging Process?

  • Background: Although studies and interest in sarcopenia have increased, it is still a matter of debate which muscle mass index better represents the aging process. We compared 3 indices for muscle mass (appendicular skeletal muscle mass [ASM]/weight, $ASM/height^2$, and the body mass index [BMI]-adjusted muscle mass index [ASM/BMI]) to determine which better reflected the aging process in terms of the decline in bone mineral density (BMD), visual acuity (VA), hearing power, renal function, pulmonary function, and handgrip strength. Methods: We performed a retrospective cross-sectional study using the Korea National Health and Nutrition Examination Survey in the Korean population. Between 2008 and 2011, a total of 14,415 men and 17,971 women aged 10 years or older participated in the study. We plotted the changes in the 3 indices of muscle mass and compared these with changes in BMD, VA, hearing power, renal function, pulmonary function, and handgrip strength according to each age group. Results: The ASM/BMI showed similar changes in terms of surrogate markers of the aging process, while the ASM/weight and $ASM/height^2$ showed no correlation. Conclusions: Among muscle indices for sarcopenia, only the ASM/BMI represented the aging process.

Long-term administration of red ginseng non-saponin fraction rescues the loss of skeletal muscle mass and strength associated with aging in mice

  • Background: Sarcopenia is a new and emerging risk factor aggravating the quality of life of elderly population. Because Korean Red Ginseng (RG) is known to have a great effect on relieving fatigue and enhancing physical performance, it is invaluable to examine its potential as an anti-sarcopenic drug. Methods: Anti-sarcopenic effect of non-saponin fraction of Korean Red Ginseng (RGNS) was evaluated in C2C12 myoblasts treated with C2-ceramide to induce senescence phenotypes, and 22-month-old mice fed with chow diet containing 2% RGNS (w/w) for 4 further months. Results: The RGNS treatment significantly alleviated cellular senescence indicated by intracellular lipid accumulation, increased amount of lysosomal β-galactosidase, and reduced proliferative capacity in C2C12 myoblasts. This effect was not observed with saponin fraction. In an aged mouse, the 4-month-RGNS diet significantly improved aging-associated loss of muscle mass and strength, assessed by the weights of hindlimb skeletal muscles such as tibialis anterior (TA), extensor digitorum longus (EDL), gastrocnemius (GN) and soleus (SOL), and the cross-sectional area (CSA) of SOL muscle, and the behaviors in grip strength and hanging wire tests, respectively. During the same period, an aging-associated shift of fast-to slow-twitch muscle in SOL muscle was also retarded by the RGNS treatment. Conclusions: These findings suggested that the long-term diet of RGNS significantly prevented aging-associated muscle atrophy and reduced physical performance, and thus RGNS has a strong potential to be developed as a drug that prevents or improves sarcopenia.

  • Today, a standard of living by the development of economy and science-technology is rapidly changing, and the average life span of the people has been lengthened by the improvement of medical services. As a result, the aging population has increased, and the Korea is likely to the aging society in 2020. However, the quality of the residential care facilities for the eldery stands for an elementery stage. Under such circumstance, the purpose of this study is to suggest the design guidelines to improve the quality of housing through the case studies of the spatial composition of the residential care facilities for the elderly people.

  • The purpose of this study is to identify challenges related to social, economic and environmental change for fishing villages and suggest the developmental direction of fishing village to actively respond to new changes. This study mainly focuses on fishing village fraternities. Its current problems are summarized as follows: shortage of labor, population aging, entry barrier against newcomers, insufficient guidance and supervision functions, lack of funds of new projects and inadequate statistics and data. The fishing village community in the modern meaning of the term can be understood as an economic and flexible social-cultural community based on openness, cooperation instead of exclusiveness and closeness, which aligns fisheries with local industries. Thus, in order for the fishing village community to improve, the followings areas required. First, strengthening capacity of the fishing village community. Second, establishing the database system and the third, reorganizing the fishing village community into a sea village community.

  • Infrastructures in Japan constructed mostly in high economic growth period become elder & elder, and the troubles & accidents caused by the aging increase. Though investment for the renewal is necessary, the shortage of public fund delays the action. Besides, we expect the decrease of the population that means the decrease of the engineers who take care of social infrastructures. Thus, it is necessary for us to develop Advanced Management system of social Infrastructures (AMI) to realize the efficient and economical operation. Our concept of AMI consists of using ICT, PI (Public Involvement) and establishment of O&M diagnosis system. We expect AMI will support to realize the appropriate repairing, preventive maintenance based on the actual performance, accidents & dangerous experience and education & training of the workers. In this paper, development of AMI for the waste disposal facility as a first example of infrastructures will be shown.

A review of osteosarcopenic obesity related to nutritional intake and exercise

  • Recently, osteosarcopenic obesity (OSO) has been identified and notified world wide. Therefore, this study reviewed OSO related to lifestyle factors such as nutritional intake and exercise. Due to aging, OSO may be initiated by dietary factors and obesity related factors. Reduced muscle mass and increased fat mass may negatively impact bone health causing OSO. The complication of OSO development should be related to dietary imbalance combined with declined exercise and this may contribute to induce OSO by decreasing bone mass, muscle mass, and increasing obesity with aging. To prevent OSO, reaching peak bone mass and building optimal muscle and fat mass through exercise would be recommended. For treating OSO, balanced dietary intake and regular exercise through a whole life would be needed. In addition, sufficient carbohydrate and fat intake for minimizing protein catabolism would be recommended to prevent OSO. The combination of aerobic exercise and resistance training also would be an effective intervention for OSO population.

  • The purpose of this study is to present an appropriate approach to urban regeneration by analyzing Gwangju Metropolitan City. For the study, residential area of Asia Culture Complex was analyzed based on GIS. The areas concerned were categorized and looked into by their Situation based on the analysis of urban infrastructure state. However, as the architectural space within a city are limited, and accurate assessment of them is needed. The residential areas have the following characteristics. They become an aging and dilapidating residential and socially underprivileged-concentrated area. It is necessary to set up a direction for a future-oriented planning for the development of these areas. Along with a decreasing population, it is important to improve the urban architecture environment for the elderly in an aging society. The economic growth of a city if necessary for the future. Therefore, the criteria for the assessment is required. And the subsequent setting-up and planning of a future direction for a city is badly needed. After that, an accurate assessment of a city called for. City space allows for various interpretations. As the rich are getting richer and the poor are getting poorer, a wealth is concentrated on one district or an area.

  • There are an increasing number of healthcare facilaties, especially branch offices, in rural areas to serve the aging population living there. However, there has been a gradual decline in the ratios of recognition, satisfaction and utilization by people who live in the regions. A significant reason of declining the ratios should be the population decline, but the most of population hierarchy shows the groups of elderly people over 60. This result appears to be limited to visit the public health centers. According to the result of population hierarchy, a branch office of public health center has been re-established as a complex welfare facility which can be fulfilled in the functions of basic medical supports and cultural supports. This research is focused on collecting the meaningful information of the status of physical facilities and utilization with 15branch offices of public health care centers in the rural regions near the city of Ik-San city. In addition, this research has a purpose of getting fundamental data for future architectural plans of the branch offices in rural regions with the results about the status of facility operation systems and users' needs.

Review on Advance Directives (생명연장술 사전선택(Advance Directives) 개념 정립을 위한 문헌 고찰)

  • Rapid progress in modern medical technology has made it possible to sustain life and/or delay death using 'heroic' treatments. The availability of life-sustaining treatment brings several issues in end-of-life care such as 'dying with dignity' and an radical increase in health care costs. The use of Advance Directives(AD) have been widely heralded by health care providers, gerontologists, and advocacy groups as means of protecting patients' right to accept or refuse life-sustaining treatment in end-of-life care. The use of AD can not only improve patients' autonomy and quality of life but also bring efficiency in distributing health care resources. The proportion of older persons in Korean population has been increasing. Those 65 years of age or over were about 7 percent of the population. Death and dying is not limited to older persons, but it is more prevalent among them. In conjunction with an aging population and the increasing prevalence of death, the issues of death and dying will become crucial in near future in terms of 'dying with dignity', 'autonomy', and 'self-control'. This paper attempts to explode and establish the concept of advance directives (AD) based on literature review. Data sources are computer searches with the MEDLINE database. Due to the lack of prior study on AD for a Korean cases, studies abroad are reviewed. This paper suggests the need for future study on the possibility of the use of AD in Korea.
