Intrathecal Meperidine Plus Lidocaine for Prevention of Shivering during Cesarean Section

  • Background: Shivering related to spinal anesthesia may interfere with monitoring and is uncomfortable. The aim of the present study was to investigate low-dose intrathecal meperidine for the prevention of shivering after induction of spinal anesthesia in parturients with cesarean section. Methods: This was a prospective randomized, double-blind, placebo-controlled trial including 100 parturients, of American Society of Anesthesiologists (ASA) physical status I or II, scheduled for elective cesarean section under spinal anesthesia who were randomly assigned to a meperidine (0.2 mg/kg) plus hyperbaric lidocaine (5%, 75 mg, n = 50; group M) group or a placebo plus hyperbaric lidocaine (5%, 75 mg, n = 50; group L) group. Demographic and surgical data, adverse events, and the mean intensity for each parturient were assessed during the entire study period by a blinded observer. Results: There were no significant differences between the two study groups regarding the demographic and surgical data (P > 0.05). The incidence of shivering during the entire study period significantly decreased in the group of parturients who received intrathecal meperidine (P = 0.04). There were no significant differences in nausea and vomiting between the two groups. Conclusions: Low-dose intrathecal meperidine (10 mg) is safe and effective in reducing the incidence and severity of shivering associated with spinal anesthesia in parturients with cesarean section.

콩의 볶음 조건에 따른 청육장의 이화학적 특성 연구 (Physico-chemical Properties of Chungyuk-jang on Different Roasting Conditions)

  • Chungyuk-jang is one of korean traditional soy food made by boiling with meat, seafood and soybean which is fermented after roasting. To investigate the difference in the physicochemical characteristics of the Chungyuk-jang fermented with roasted soybean in different conditions, Chungyuk-jang was made from soybean roasted on the three condition, 140℃ for 21 min (CY140), 180℃ 9.5 min (CY180), 220℃ 6 min (CY220) which was decided by pre-test and compared to one made without roasting (CY0). The moisture of Chungyuk-jang was 79.98~81.87% and pH was 6.15~6.25. The lightness and yellowness of CY0 was higher than Chungyuk-jang made of roasted bean whereas redness and brown pigment was the highest on CY220. The contents of free sugar of CY140 was the highest among the treatment. The contents of amino-N of Chungyuk-jang (CY140, CY190, CY220) was higher significantly than CY0. The contents of total free amino acid and glutamic acid was highest on fermented soybean roasted for 140℃, 21 min (CY140) and followed by fermented soybean roasted on 180℃ 9.5 min (CY180), 220℃ 6 min (CY220) and CY0 (without roasting).

A first-principles theoretical investigation of the structural, electronic and magnetic properties of cubic thorium carbonitrides ThCxN(1-x)

  • Besides promising implications as fertile nuclear materials, thorium carbonitrides are of great interest owing to their peculiar physical and chemical properties, such as high density, high melting point, good thermal conductivity. This paper reports first-principles simulation results on the structural, electronic and magnetic properties of cubic thorium carbonitrides $ThC_xN_{(1-x)}$ (X = 0.03125, 0.0625, 0.09375, 0.125, 0.15625) employing formalism of density-functional-theory. For the simulation of physical properties, we incorporated full-potential linearized augmented plane-wave (FPLAPW) method while the exchange-correlation potential terms in Kohn-Sham Equation (KSE) are treated within Generalized-Gradient-Approximation (GGA) in conjunction with Perdew-Bruke-Ernzerhof (PBE) correction. The structural parameters were calculated by fitting total energy into the Murnaghan's equation of state. The lattice constants, bulk moduli, total energy, electronic band structure and spin magnetic moments of the compounds show dependence on the C/N concentration ratio. The electronic and magnetic properties have revealed non-magnetic but metallic character of the compounds. The main contribution to density of states at the Fermi level stems from the comparable spectral intensity of Th (6d+5f) and (C+N) 2p states. In comparison with spin magnetic moments of ThSb and ThBi calculated earlier with LDA+U approach, we observed an enhancement in the spin magnetic moments after carbon-doping into ThN monopnictide.

Seismic mitigation of substation cable connected equipment using friction pendulum systems

  • Power transmission substations are susceptible to potential damage under seismic excitations. Two of the major seismic failure modes in substation supplies are: the breakage of brittle insulator, and conductor end fittings. This paper presents efficient isolation strategies for seismically strengthening of a two-item set of equipment including capacitive voltage transformer (CVT) adjacent to a Lightning Arrester (LA). Two different strategies are proposed, Case A: implementation of base isolation at the base of the CVT, while the LA is kept fixed-base, and Case B: implementation of base isolation at the base of the LA, while the CVT is kept fixed-base. Both CVT and LA are connected to each other using a cable during the dynamic excitation. The probabilistic seismic behavior is measured by Incremental Dynamic Analysis (IDA), and a series of appropriate damage states are proposed. Finally, the fragility curves are derived for both the systems. It is found that Friction Pendulum System (FPS) isolator has the potential of decreasing flexural stresses caused by intense ground motions. The research has shown that when the FPS is placed under LA, i.e. Case B (as oppose to Case A), the efficiency of the system is improved in terms of reducing the forces and stresses at the bottom of the porcelain. Several parametric studies are also performed to determine the optimum physical properties of the FPS.

Experimental and numerical investigation of expanded polystyrene (EPS) geofoam samples under monotonic loading

  • The recent increase in the use of Expanded Polystyrene (EPS) geofoam in construction and geotechnical projects has driven researchers to investigate its behavior, more deeply. In this paper, a series of experimental tests to investigate the stress-strain behavior and the mechanical properties of EPS blocks, under monotonic axial loading are presented. Four different densities of cylindrically shaped EPS with different dimensions are used to investigate the effects of loading rate, height and diameter, as well as the influence of the density of EPS on the stress-strain response. The results show that increasing the height of the EPS samples leads to instability of the sample and consequent lower resistance to the applied pressure. Large EPS samples show higher Young's modulus and compressive resistance due to some boundary effects. An increase in the rate of loading can increase the elastic moduli and compressive resistance of the EPS geofoam samples, which also varies depending on the density of the samples. It was also determined that the elastic modulus of EPS increases with increasing EPS density. By implementing an efficient numerical procedure, the stress-strain response of EPS geofoam samples can be reproduced with great accuracy. The numerical analysis based on the proposed method can used to evaluate the effect of different factors on the behavior of EPS geofoam.

The Interplay between the Psychological Factors and Entrepreneurial Intention: An Empirical Investigation

  • The domain of entrepreneurship has been dotted across various paradigms. Identifying and examining entrepreneurial intention and propensity to it spontaneously require the insight from the lens of psychological approach. The aim of this study is to examine the influence and impact of psychological factors on entrepreneurial intention; as it is found that in Bangladeshi context several entrepreneurship studies are undertaken, still the exploratory research on interplay between the psychological factors (i.e., self-confidence, locus of control, need for achievement, and tolerance for ambiguity) and entrepreneurial intention has been merely found. The study has chosen Bangladeshi university students as the unit of analysis and the ultimate sample size in this research is n=265. The current paper is a quantitative study where sampling method is followed by convenience sampling technique, and study data is collected through survey questionnaire. Data has been compiled into SPSS whereas, for hypotheses assessment, Smart PLS software is applied. The results reflect that self-confidence, locus of control, and need for achievement are revealed as contributory determinants of entrepreneurial intention while tolerance for ambiguity is found as an insignificant predictor. The current research is expected to offer an in-depth understanding about the significance of psychological factors in examining entrepreneurial intention.

Effectiveness of the neutron-shield nanocomposites for a dual-purpose cask of Bushehr's Water-Water Energetic Reactor (VVER) 1000 nuclear-power-plant spent fuels

  • In order to perform dry interim storage and transportation of the spent-fuel assemblies of the Bushehr Nuclear Power Plant, dual-purpose casks can be utilized. The effectiveness of different neutron-shield materials for the dual-purpose cask was analyzed through a set of calculations carried out using the Monte Carlo N-Particle (MCNP) code. The dose rate for the dual-purpose cask utilizing the recently developed materials of $epoxy/clay/B_4C$ and $epoxy/clay/B_4C/carbon$ fiber was less than the allowable radiation level of 2 mSv/h at any point and 0.1 mSv/h at 2 m from the external surface of the cask. By utilization of $epoxy/clay/B_4C$ instead of an ethylene glycol/water mixture, the dose rates on the side surface of the cask due to neutron sources and consequent secondary gamma rays will be reduced by 17.5% and 10%, respectively. The overall dose rate in this case will be reduced by 11%.

  • Healthcare is one of the biggest industries that consume IT Infrastructure as a necessity. A reliable reach to secure private information is the essential goal. Most of the healthcare organizations either have their own custom solutions developed or third part CRM software tailored to their needs. This culture satisfies the usability internal to the organization but lacks the ability to provide public access to the users or even to take a step ahead and collaborate with like minded institutes for research and potential growth. Emergence of cloud computing has provided us the opportunity to take a step ahead and build applications available to users on a secure platform, with ability to scale the resources depending on the requirements. The effective use of hybrid and community cloud can create rock solid foundations for healthcare IT Infrastructure. These cloud deployment models not only caters the present needs of healthcare IT Infrastructure but also opens the possibilities to build applications and provide secure and reliable access beyond the limits of organizational boundaries.

Effect of Season and Fertilizer on Species Composition and Nutritive Value of Native Grasses

  • Effect of three major cropping seasons and five fertilizer treatments on botanical composition, nutritional composition and in sacco digestibility of native grasses grown in 30 experimental plots of a medium fertile land was determined. It was observed that all the major grass species were grown in all seasons but their predominancy of growth was different. During the study the predominant grass species were Panicum repens (Angta), Fimvristylis miliacea (Joina), Cyanolis axillaries (Kanainala), Cynodon dactylon (Durba) and Cyperus iria (Phulchaise) which contributed about 27, 20, 13, 11 and 9% of the total grass yield, respectively. Dry matter (DM) contents was higher in dry followed by monsoon and summer seasons (p<0.05). Crude protein (CP) content in the summer and monsoon appeared to be higher (p<0.05) than that of dry season. Organic matter (OM) and neutral detergent fibre (NDF) were higher (p<0.05) in dry and monsoon than in summer season. Application of urea fertilizer and cowdung increased 28.2% of CP content of the grasses, but decreased 19.5 and 9.8% of DM and NDF contents, respectively. The potential degradation of DM and CP of the grasses grown in summer were 4.1 and 8.4% and 3.9 and 5.8% higher than those of monsoon and dry seasons, respectively, and both of these increased (11.3 and 5.9%, respectively) with the application of cowdung and urea fertilizer.

The Influence of Optical Porosity of Tree Windbreaks on Windward Wind Speed, Erosive Force and Sand Deposition

  • The research was conducted windward of an irrigated Acacia amplicips Maslin windbreak established to protect As Salam Cement Plant from winds and moving sands. Two belts with approximate optical porosities of 50% and 20% were studied in River Nile State, Sudan. The research aimed at assessing the efficiency of the two belts in wind speed reduction and sand deposition. Research methods included: (i) estimation of optical porosity, (ii) measurements of windward wind speeds at a control and at distances of 0.5 h (h stands for windbreak height), 1 h and 2 h at two vertical levels of 0.25 h and 0.5 h, (iii) estimation of relative wind speeds at the three positions (distance and height) at windward and (iv) estimation of wind erosive forces and prediction of zones of sand deposition. Results show that while the two belts reduced windward wind speeds at the two levels for the three distances, belt II was more effective. Nearest sand deposition occurred at 2 h and 1h windward of belt II and belt I, respectively, at level 0.25 h. At level 0.5 h, sand was deposited only at 2 h windward of belt II and no sand deposition occurred windward of belt I. The study concludes that less porous windbreaks are more effective in reducing wind speed and in depositing sand in windward direction at a distance of not less than twice the belt height.
