The purpose of the current study was to investigate histologic changes in the alveolar bone of the lower molar region subsequent to the loss of their opposite molars, and to characterize chemical alterations by utilization of histochemical procedures. Twenty five rats(Sprague Dawley), approximately 150-200gm body weight, were used in this experiment. In the treated animals, upper molars were removed. The animals were decapitated by groups at the following intervals after teeth removals: 10th, 20th, 50th, 70th and 100th day. The normal, untreated rats were used as controls. The molar region of lower jaw, including the intact alvelar bone and teeth was dissected and specimens were decalcified in 3% formic acid. After the tissues were fully decalcified, the specimens were embedded in celloidin and sectioned in mesiodistal plane. These sections were stained in the following staining methods. Mallory azan stain and hematoxylin-eosin stain were utilized for structural evaluation. Polysaccharides were demonstrated by means of the PAS reaction. Acidmucopolysaccharides were studied by means of the colloidal iron stain. Alloxan-Schiff reaction was used for protein. The results were as follows: 1) In the control animals, bone resorption was noted in the distal alveolar bone proper and bone apposition was shown in the mesial alveolar bone proper. But in the treated animals, bone apposition was observed on the mesial and distal walls of the alveolus and osteoclastic activity was not noted in any walls. 2) Bone apposition was most prominent from the 10th to 20th day after treatment. 3) Appositional growth of cementum along the surface of root was prominent from the 50th to 70th day after treatment. 4) In the area where osteoblastic activity was apparent, osteoblasts were stained strongly in the PAS and alloxan-Schiff reaction. A plastic resorption line showed strong alloxan-Schiff reaction. 5) In the colloidal iron stain, the alveolar wall adjacent to the cementum apposition area was stained more strongly than the other areas.
The purpose of this experiment was to examine the antimicrobial effect of the natural flavonoid hesperidin on dental caries and alveolar bone resorption in the albinorats. Twenty five day old male rats were fed with the experimental diets for 42 days. At the end of the 42 day experimental period, the molar tooth occlusal surface was examined by a dissecting microscope. The sulcular caries lesions were recorded: the first molar caries incidence was higher than that of the second molar and the third molar. Alvelolar bone resorption was measured on the buccal and lingual aspects of each molars. Three measurements were taken on the first molar (mesialpoint, midpoint, distalpoint). The results of this experiments, showed that hesperidin is effective in reducing dental caries and alveolar bone resorption.

Purpose : To determine whether peri-implant crestal bone loss could be affected by systemic disease, primary ISQ value, implantation method (submerged vs. non-submerged), surface treatment, and bone density Materials and methods : Patients who underwent fixture installation from June 24, 2005 to October 23, 2008 at Seoul National University Bundang Hospital were evaluated. A total of 157 patients (male: 52, female: 85) had 346 fixtures installed. Among them, 49 patients had periapical radiographs taken 1 year after prostheses were first set. A total of 97 fixtures were implanted. In particular, 30 fixtures were installed in patients with systemic diseases such as diabetes mellitus, cardiovascular disease, hypertension, and liver disease. The immediate stability of implants was measured with $Osstell^{tm}$. Implant surface treatment was classified into two groups (RBM, Cellnest (Anodized)), and bone density, into four groups (D1~D4). The bone resorption on the mesial and distal areas of fixtures was measured with periapical radiographs using the paralleling technique, and the mean value was calculated. The length determination program in IMPAX (AGFA, Belgium) was used. Results : At least 332 out of 346 (96%) installed GS II implants were successfully osseointegrated 1 year after prostheses were first set. The mean value of the bone resorption of the installed GS II implants was 0.44mm. The minimum value was 0mm, and the maximum value, 2.85mm. There was a statistically significant difference between the implantation methods (submerged, non-submerged) with regard to the amount of alveolar bone loss 1 year after prostheses were first set (p<0.05). Non-submerged implants showed less crestal bone loss. Note, however, that other variables had no correlation with crestal bone loss (p>0.05). Conclusion : There was a statistically significant difference between the 1-stage method and 2-stage method with regard to the amount of alveolar bone loss 1 year after prostheses were first set. Systemic disease, primary ISQ value, surface treatment, and bone density were not associated with alveolar bone loss. Other variables were assumed to have a correlation with alveolar bone loss.
교정치료에 의해 빚어지는 치조골 소실의 정도를 좌, 우측 제1대구치의 근, 원심면에서 치료 전, 후의 파노라마 필름을 이용하여 비교평가하고 치조골 소실에 미치는 영향 요소들을 알아보고자 하였다. 고정식 교정장치 치료를 받은 216명의 환자들은 104명의 성장군과 112명의 성인군으로 나뉘어 졌으며 4개 소구치의 발거가 이루어진 경우는 각각 50명씩이었다. 치료 전, 후 파노라마 사진에서 상하 좌우 제1대구치의 근심 및 원심 부위의 치조골 높이를 백악법랑질경계에서 치조골의 최첨점까지 치아 장축에 평행하게 계측하여 다음과 같은 결론을 얻었다. 1. 치료 시작시, 성인군은 성장군보다 유의하게 치조골 수준이 낮았다. 2. 치료 후 치조골 수준은 치료 전 수준보다 유의하게 낮았다. 3. 성장군과 성인군 간에 치조골 변화량의 유의한 차가 없었다. 4. 성인발치군은 비발치군보다 유의하게 더 많은 치조골 소실을 모든 원심면에서 보였다. 5. 상악은 하악보다 통계적으로 유의하게 더 많은 치조골 소실을 근심면에서 보였다. 6. 성인군은 원심면에서 근심면보다 유의하게 더 많은 치조골 소실을 보였다.
In order to observe the responses of the periodontal tissue on the tension side following the experimental tooth movement, 35 Guinea pigs were divided into the control group (5 animals) and 6 experimental groups (3 movement groups and 3 retention groups) consisting of each 5 animals. The experimental tooth movement of Guinea pig's upper incisors installing open helical loop were carried out by rendering continuous force : 5g (1st groups) 35g (2nd groups), 100g (3rd groups), respectively for 7 days. 3 movement groups (15 animals) were sacrificed soon after the continuous force, and 3 retention groups (15 animals) were sacrificed after the retention period of another 7 days. The following results were obtained from the observation of the surrounding tissues of teeth on the tension side through light microscopy any transmission electron microscopy. 1. The vessel walls in the experimental groups were thinner than those of the control group, the number of blood vessel had the tendency to increase. The greater the strong force applied to each group, the more the destruction of cells and fibers was found and the more the number of the red blood cell of vessel outside appeared. 2. New collagen fibers were produced from fibroblasts in the 1st groups (light force), but were produced rather less in the 2nd groups (medium force) and the 3rd groups (heavy force). 3. In the forming patterns of the new alveolar bone of the 3rd groups (heavy force), the bone trabeculae were formed towards the direction of the force to be applied, but the new alveolar bone in the 1st groups (light force) was produced evenly throughout the all surfaces of the alveolar bone rather than the patterns of bone trabeculae ; therefore, the patterns of new alveolar bone were observed differently according to the magnitude of the force applied. 4. In the retention group, it was observed that the collagen fibers were produced from the osteoblasts in the marginal areas of the periodontal ligaments being widely opened and were deposited on the alveolar bone surface but the production of collagen fibers from the osteoblasts in the other area of the periodontal ligaments was almost ceased, and a rest line on the new alveolar bone surface was found.
Purpose: Many researches showed loss of alveolar bone in fresh extraction socket and even in case of immediate implant placement. The aim of this study was to evaluate the effect of non-resorbable barrier membrane on the change of buccal and lingual alveolar bone in immediate implant placement into periapically infected extraction sockets. Materials and methods: Immediate implants were placed into artificially induced periapical lesion of mandibular premolars after complete debridement using buccal bone defect made by a 6mm trephine bur in 4 mongrel dogs. Before flap repositioning, a non-resorbable barrier membrane was placed on the buccal defect in the experimental group. No membrane was placed in the control group. In 12 weeks after placement, the dogs were sacrificed and undecalcified histologic specimens were prepared. The vertical distance from the smooth-rough surface interface(SRI) to gingiva, 1st bone contact and bone crest were measured in buccal and lingual side. The horizontal thicknesses of gingiva and bone at 0, 1, 2 and 3mm below SRI were measured. Results: The buccal bone was resorbed more than lingual bone in both groups and there was statistical significance(p<0.05). The distances from SRI to 1st bone contact were $2.45{\pm}2.35\;mm$ in experimental group and $4.49{\pm}3.10\;mm$ in control group. In all vertical level, lingual bone was thicker than buccal bone(p<0.05). Conclusion: Buccal bone was reduced more than lingual bone in immediate implant placement into periapically infected extraction sockets. Placement of non-resorbable barrier membrane reduced the buccal bone resorption. However there was no statistical significance.
There're many cases that should be reconstructed with post and core when clinical crown is destructed. But this post and core restoration may cause damaging stress on the teeth. Previous finite element study was restricted to normal bone model relatively close to cemen-toenamel junction. Moreover, the test of a model with diminished bone support was rare. The purpose of this study is to test the effects of alveolar bone loss on the magnitude, stress distribution and displacement of post reconstructed teeth. In this study, it was assumed that the coronal portion of upper incisor was severely destructed. After conventional endodontic treatment, it was restored with post and core. The PFM restoration was made on it. This crown was cemented with ZPC. Alveolar bone was classified by 4 types of bone, such as normal, 2 mm, 4 mm, 6 mm bone, according to the bone loss. Meanwhile, the material of post are divided into 2 types of materials, such as gold, co-cr. Force was applied to two directions. One was fuctional maximum bite force (300 N) applied to the spot just lingual to the incisal edge with the angle of 45 degree to the long axis of the tooth, and the other one was horizontal force (300 N) applied to the labial surface. The results analyzed with three dimensional finite element method were as follows : 1. Stress was concentrated on the adjacent dentin of the post apex, one third portion of the post apex and the labial & lingual mid-portion of the root in all case. The stress of middle third of the root was apparently concentrated on the labial aspect. 2. The stress on adjacent dentin of the post apex and one third of the post apex increased as alveolar bone height moved apically. This increase was dramatic beyond 4 mm bone loss model. 3. The stress of the post apex was spreaded to the middle third of the post and greater than gold post in the case of metal post. 4. The displacement of the neck of post was the greatest in one of the post-cement interface and this increased as alveolar bone height moved apically. Besides the displacement of the metal post is slightly lower than one of the gold post.
The purpose of this study was to compare effects of the bioceramics on healing processes of the alveolar bone defects in dogs. Five adult dogs aged 1 to 2 years were used in this study. Experimental alveolar bone defects were created surgically with a #1/2 round bur at the furcation area of the buccal surface of the mandibular 3rd, 4th premolars and 1st molar. Fifteen experimental alveolar bone defects were devided into three groups according to the type of graft materials. The groups were as follows : 1) flap operation with dense hydroxyapatite( DHA group ) 2) flap operation with porous hydroxyapatite( PHA group ) 3) flap operation with natural coral ( NC group ) At 1, 2, 4, 6, and 12 weeks, dogs were serially sacrificed and specimens were prepared with Hematoxylin-Eosin stain and Mallory stain for light microscopic evaluation. The results of this study were as follows : 1. In every group, inflammatory cell infiltrations were seen at 1st weeks due to surgical trauma, however inflammatory response owing to graft materials were not seen. 2. In every group, the appearance of connective tissue around graft materials was loosely formed at the initial stages, however the connective tissue was densely formed at 2 weeks. 3. The presence of osteocytes were observed at 2 weeks in the natural coral group, however the osteocytes were appeared at 6weeks in the dense hydroxyapatite group. 4. A new bone was formed from the base and walls of the defect and gradually expanded toward the graft materials. 5. A resorption of the natural coral occurred irregularly at the periphery of the material, therefore the size and shape of the natural coral were reduced at 6 weeks. 6. At 12 weeks, the porous hydroxyapatite and natural coral were surrounded by newly formed bone most completely, however dense hydroxyapatite was surrounded by newly formed bone in part.
Evaluation of the methods of processing allogenic bone must be considered in order to make an effective choice of graft materials in oral surgery. Allograft materials processed by the tissue banking industry have varying capacities of bone reconstruction. The biological function of processed bone can be affected by many factors, like particle size, processing parameters, and inclusion or exclusion of mineral and moisture. For example, freeze drying step offers a safe and economical means for packaging, shipping, storage, and preservation of homologous bone. Demineralization of cortical bone using hydrochloric acid can produce a uniform demineralized surface with a capacity for osteoinduction. The objectives of this review were to evaluate the processing methods for allogenic bone and to characterize processed materials for grafting. It is important to understand the biological, biomechanical healing of different types of allografts to make the right choice for allogenic bone on each clinical application and to achieve a successful outcome for alveolar bone reconstruction in oral surgery.
Loss of dentition can lead to not only compromised esthetics and functions of the patient, but also alveolar bone resorption. Bone grafting with prosthetic reconstruction of the gingiva can be selected for the treatment, and it provides many benefits as prosthetic gingival reconstruction does not require a complicated surgical process and is available within a short period of time, with stable clinical results. However, conventional porcelain fused to metal prosthesis has certain limits due to its size, and deformation after several firing procedures. In this clinical report, the author would like to introduce a patient with severe alveolar resorption who was treated with gingiva-shaped zirconia/titanium CAD/CAM implant fixed prosthesis for esthetic and functional rehabilitation. Clinical reports Clinical report 1, 2 : A case of loss of anterior dentition with atrophied alveolar bone. Implant retained zirconia bridge applied with Procera implant bridge system to simulate the gingiva. Upper structure was fabricated with zirconia all ceramic crown. Clinical report 3, 4 : A case of atrophied maxillary alveolus was reconstructed with fixed implant prosthesis, a CAD/CAM designed titanium structure covered wi th resin on its surface. Anterior dentition was reconstructed with zirconia crown. Conclusion and clinical uses. All patients were satisfied with the outcome, and maintained good oral hygiene. Zirconia/titanium implant fixed prosthesis fabricated by CAD/CAM system was highly accurate and showed adequate histological response. No critical failure was seen on the implant fixture and abutment overall. Sites of severe alveolar bone loss can be rehabilitated by implant fixed prosthesis with CAD/CAM system. This type of prosthesis can offer artificial gingival structure and can give more satisfying esthetics and functions, and as a result the patients were able to accept the outcome more fondly, which makes us less than hard to think that it can be a more convenient treatment for the practitioners.
