Double Tilt Angle Board (DTAB)의 자체 제작에 관한 유용성 평가 (The Evaluation of Usefulness of the Manufactured DTAB (Double Tilt Angle Board) System)

  • 목 적: 본원에서는 자체 제작한 double tilt angle 방식의 board를 이용하여 비교적 table set-up에 어려움을 가진 다양한 형태의 환자를 대상으로 치료에 적용하여 유용성을 평가해 보았다. 대상 및 방법: Double tilt angle board(DTAB) 제작은 Breast board를 이용해 누운 상태에서 몸전체에 각도를 주도록 하고, 상업용 두부고정용구(Head tilt board)와 동일 방식의 Head tilt board를 10 mm 두께의 아크릴을 이용해 축소 제작하여 환자 경부에 한 번 더 각도를 줄 수 있게 이중 tilt 방식으로, 일자형과 U-자형 Aqua-plast 모두 사용할 수 있는 구조이며, 일정 간격의 Hole & Holder를 제작해 환자의 신장에 제한 없이 사용할 수 있게 하였다. 이러한 자체 제작한 보드를 SVC 환자 3명, Pacemaker 환자 2명 및 8명의 기관절제술을 시행한 환자와 같이 호흡의 불안정, 목과 가슴의 심한 압박감과 통증으로 인해 Table에 제대로 눕지 못하여(Set-up이 어려워) 치료를 시행하기 어려웠던 13명의 환자들을 대상으로 모의 치료하고 치료 시 10회의 반복적인 L-gram 촬영의 오차범위로 재현성을 평가해 보았다. 결 과: SVC 환자 1명, Pacemaker 환자 2명 및 4명의 기관절제술을 시행한 환자와 같은 호흡불안정한 총 13명의 환자를 대상으로 Simulation 해본 결과 SVC($40{\sim}56$), Pacemaker($30{\sim}68$), 기관 절제술($25{\sim}45$) 각각의 적정한 tilt angle을 이용한 환자 Set-up이 가능하였고 L-gram을 이용한 오차범위 측정결과에서도 5 mm 이내의 범위를 나타냄으로써 일반적인 원발성 종양환자와 동일한 치료 결과를 보였다. 결 론: 치료목적에 부합하는 새로운 double tilt 형태의 Board를 제작하여 불안정한 호흡과 기도유지의 어려움으로 인해 Table에 눕지 못하는 다양한 환자들을 대상으로 사용한 결과 용이한 환자 Set-up이 가능해져 목적하는 치료 계획과 치료를 시행할 수 있었다는 데 큰 의의를 두며, 앞으로 이러한 적응증을 가진 환자의 치료에 있어서 오는 시간적, 심리적인 부담감을 최소화할 것이며, 구형의 Board만을 재활용해 제작함으로써 커다란 비용절감의 경제적 효과를 보았다.

동물의 번식 효율 증진을 위한 한약제의 활용 (Application of pure organic phyto-formulations to optimize performance results in the conventional European animal breeding and production)

  • 유럽에서 돼지 번식 효율 증진과 비육을 위한 적절한 활용방법과 몇 가지 잘 조제된 한약제가 규격화되어 나왔다. 이 자료에는 한약제의 효능의 기준을 비롯하여 위생에 대해서도 제시하고 있다. 대학 및 연구기관, 임상 수의사들과 사료업체들이 수행했던 지난 수년간의 실험과 연구 결과를 토대로 돼지 번식효율 증진을 위한 한약제를 사료첨가제로 활용하여 최적화시켰다. 유럽의 수의분야에서 사용되었던 4가지의 특정 한약제를 기본으로 하여 돼지 사양에 있어서 가장 중요한 문제들의 대부분이 성공적으로 해결되었다. 즉 수정률, 호흡기질병, 설사와 스트레스에 관한 문제들인데 면역이 억압되고 저질의 자돈이 생산됨에 따라 생산성 저하에 따른 엄청난 경제적인 손실을 대처할 수 있다. 또한 일반적인 임상증상을 치료하면서 동시에 가장 중요한 것 중의 하나는 자기방어체계를 증진시킨다는 것이다. 이로써 동물의 면역력과 활력을 증가시키고 건강을 유지시켜서 폐사율을 감소시킨다. 게다가 여기에서 제시된 자료들은 일상적으로 사용되었던 항생제나 호르몬제제들의 사용을 대체할 수 있는 한약제들인데 고효율임이 증명되었다. 이러한 한약제들은 백신 프로그램이나 성장을 촉진시키는 인자들과 상호보완 상승작용도 가져 온다.

신도입 케나프 품종의 파종시기에 빠른 생육 및 수량 변동과 광합성 특성 (Growth, Yield and Photosynthesis of Introduced Kenaf Cultivars in Korea)

  • 제주에서 새로 도입한 케나프 3개 품종(Everglades-41, Chingpi-3, Tainung-2)의 파종시기별 생산성을 검토하였다. 11월 말 수확의 단위면적당 수량(지상부생중)은 Everglades-41가 다른 두 품종보다 적었는데, 파종시기별로는 Chingpi-3은 5월 11일 파종에서 높았고 파종시기가 늦어질수록 감소하였으나, Everglades-41 및 Tainung-2은 5월 21일 및 6월 1일 파종에서 높은 경향을 나타냈다. 단위엽면적당 광합성 능력은 Everglades-41이 다른 두 품종보다 높았는데, 이것은 Everglades-41의 잎모양이 타 두품종과 다른 것과 높은 광이용 효율에 기인한 것으로 생각된다. 그러나, Everglades-41의 생장량은 두 품종보다 낮았는데, 이것은 생육초기에 병충해에 의한 생육 저해에 의한 것 이외에도 광호흡(photo respiration)이 높은 점으로 보아 광호흡에 의한 물질 생산량의 손실도 고려 대상이다. 생산적인 측면을 고려 할 때 제주 지역에서는 Chingpi-3을 5월중순경 파종해서 재배하는 것이 적당할 것으로 보여지나, 본 시험에 공시한 3품종은 제주에서의 채종은 쉽지 않을 것으로 보여진다.

희토류비료 시비가 사과 과실내 축적과 수확 및 저장 중 사과품질에 미치는 영향 (The Accumulation of Rare Earth Elements Fertilizer and its Subsequent Effects on Apple Fruit Quality at Harvest and During Storage)

  • 본 연구는 희토류비료 시비가 8년생 '후지'/M.9 사과의 미량원소 변화 및 사과의 수확 품질과 5개월간 4 저장 후 과실 품질에 미치는 영향을 알아보기 위해 수행하였다. 1년차 희토류 비료의 시비는 '후지' 사과 과실내 란타늄, 프라세오디뮴, 가돌리늄 및 네오디뮴을 축적하였다. 또한 2년차 연구에서 높은 농도의 희토류비료 시비는 보다 많은 량의 희토류 성분을 과실에 축적하여 과실내 희토류의 축적은 희토류비료의 시비량에 비례하였다. 이러한 희토류비료의 시비는 과실내의 다른 미량원소인 칼슘, 마그네슘과 칼륨의 농도에는 영향을 미치지 않았다. 희토류비료 시비에 따른 과실 품질조사에 있어, 희토류비료 0.2%의 엽면살포는 수확기 사과 과피의 붉은 색을 증가시켰으나, '후지' 과실의 과중, 경도 및 산도는 변화가 없었다. 저장 사과의 희토류비료 시비효과를 조사한 결과, 희토류비료 처리된 사과의 경우 무처리에 비해 5개월 저장 후 과실의 연화 및 적정산도의 감소를 지연시키며, 호흡률과 에틸렌발생을 감소시켰다.

CFD모사기법을 이용한 가스 여과기 성능 해석 (Analysis of a Gas Mask Using CFD Simulation)

  • 화학적 무기 중 혈액작용제는 전자전달계 내 효소의 철 이온과 반응하고 세포호흡을 정지시켜 사망을 초래한다. 혈액작용제는 활성탄의 미세공보다 분자크기가 작아 화학적 흡착이 유일한 제독방법이다. 본 연구는 SG 생활안전에서 개발한 SG-1 가스 여과기를 이용하여 혈액작용제 시아노겐 클로라이드(CK) 가스의 유입에 따른 유동해석을 수행하였다. 구리, 은, 아연 및 몰리브데늄 이온이 첨착된 ASZM TEDA 활성탄을 적용하여 가스 여과기 제작 시험 규정에 따라 화학적 흡착 모사를 수행하였으며 흡착 Kinetic을 적용하기 위해 선 수행된 흡착 베드에서 CK 가스 흡착 실험 결과를 분석하였다. 화학적 흡착을 통해 발생되는 가스 여과기 내부 압력강하 및 가스 흡착 질량 등 주요 변수의 동적거동을 예측하였다. CFD에서 다공성 물질을 적용할 때 사용하는 Ergun 방정식 대신 Granular와 Packed bed를 사용하여 활성탄 적용 가능 결과를 확인하였으며 시간에 따른 흡착 및 유속에 따른 흡착의 유동 해석에 대한 동적 모사를 수행하였다.

기관내 관 제거 후 발생한 급성 호흡부전에서 비침습적 양압 환기법의 유용성 (The Usefulness of Noninvasive Positive Pressure Ventilation in Patients With Acute Respiratory Failure after Extubation)

  • 연구배경: 침습적 기계 호흡 이탈에 성공하여 기관내 관을 제거하였으나 48시간 이내에 급성 호흡 부전이 경우 또는 환자 스스로 기관내 관을 제거한 후 발생된 급성 호흡 부전은 이탈 실패의 중요한 원인이며, 발생시 기관내 관의 재삽관을 통한 호흡 보조가 표준적 치료이다. 비침습적 양압 환기법(noninvasive positive pressure ventilation이하 NIPPV)은 비 혹은 안면마스크를 통해 양압 환기를 시행하므로 기관내 삽관을 회피할 수 있다. 본 연구는 기계 호흡 이탈 후 기관내 관을 제거한 환자들에게 발생된 급성 호흡 부전 시 NIPPV 적용이 기관내 삽관을 통한 양압 환기 치료를 대체할 수 있는지를 알아보고자 하였다. 방 법: 대상은 아산 재단 서울 중앙병원 내과계 중환자실에 입원하여 기계적 환기 치료를 받고 이탈 과정에서 기관내 관 제거 후 48시간 이내에 급성 호흡 부전이 발생한 환자 21명 및 스스로 기관내 관을 제거한 후 급성 호흡 부전이 발생한 환자 7명과 기관내 관이 기도내 분비물로 막혀 기관내 관을 제거한 환자 2명등 총 31명에게 NIPPV를 적용하였다. NIPPV는 환자 상태에 따라 기계 환기 양식, 압력 보조 수준 및 흡입 산소의 양이 조절 되어졌으며, 압력 보조 8cm $H_2O$미만에서 임상적으로 안정된 상태를 유지하는 경우 완전히 NIPPV에서 이탈하였다. 성공군은 NIPPV 이탈후 48 시간 이상 자발 호흡을 유지한 경우로 정의하였고, 실패군은 NIPPV 시행 후 호흡 부전 소견이나 동맥혈 가스검사의 호전이 없어 다시 기관내 삽관을 시행하여 기계호흡을 시행한 환자로 정의하였다. 각 군에서 NIPPV적용 직전, 적용 후 30분, 6시간, 24시간, NIPPV 이탈 작전 또는 실패하여 기관내 재삽관으로 전환 직전의 심박동수, 분당 호흡수, 동맥혈 가스검사, 압력 보조 수준 및 호기말 양압등을 비교하였다. 결 과: 총 31명에서 NIPPV를 적용하였고, 이 중 성공군은 14명(45%)이었다. 성공군과 실패군을 비교 시 나이, 중환자실 입원 당시의 APACHE III 점수, 기관 내 삽관 기간, 기관내 관 제거 후 NIPPV적용시까지의 시간 및 NIPPV 시행 직전의 분당 호흡수, 심박동수, 동맥혈 가스검사, $PaO_2/FiO_2$ 등은 양군간에 유의한 차이가 없었다. 모든 환자에서 NIPPV적용 30분후부터 분당 호흡수 및 심박동수는 감소하였고 동맥혈 산소 포화도는 증가하였다.(p<0.05) 그러나, 실패군에서는 NIPPV 시행중에 상태가 악화되어 기관내 재삽관을 시행하였고, 재삽관 작전의 분당 호흡수 및 심박동수는 다시 증가하였으며 동맥혈 산소포화도는 감소하였다(p<0.05). 기저 질환이 COPD 이면서 기관내 관 제거 후 급성 호흡부전이 발생한 환자 8명에게 NIPPV 적용 시 COPD가 아닌 다른 환자들에 비해 NIPPV의 성공률이 의미 있게 높았다(62% 대 39%)(p=0.007). NIPPV를 이용한 이탈 시 실패의 원인으로는 기저 질환의 악화 없이 동맥혈 가스 소견이 악화되었던 예가 9예, 그 외 기저 질환 악화 5예, 마스크 부적응이 2예, 기도내 분비물 축적이 1예였다. 결 론: NIPPV는 침습적 기계 호흡이탈과정에서 특히 기저질환이 COPD인 환자들의 경우 기관내 관 제거 후 발생한 급성 호흡 부전 시 기관내 관의 재삽관을 피할 수 있는 유용한 치료적인 방법으로 사료된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The authors suggests that this system based on down-sized computer could provide a progressive approach to total LIS based on local area network, and the implemented system could serve as a model for other hospital's LIS for routine hematology. 30609 T00401030609 ^x To develop an artificial bone substitute that is gradually degraded and replaced by the regenerated natural bone, the authors designed a composite that is consisted of calcium phosphate and collagen. To use as the structural matrix of the composite, collagen was purified from human umbilical cord.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.
