The Effect of Dopaminergic Activity on Aldosterone Secretion in Edematous State (Dopaminergic Activity가 부종상태에서 혈청 Aldosterone 분비에 미치는영향)

  • To evaluate the effect of dopaminergic activity on aldosterone secretion, the plasma renin activity, serum cortisol and aldosterone were measured by radioimmunoassay in 6 normal controls and 12 patients who had hyponatremia and generalized edema or ascites with possible condition with secondary aldosteronism before and after(15, 30, and 60 min) 15 mg of metoclopramide by iv bolus injection and same method with 500 mg of L-dopa by per oral in 6 normal controls and 12 patients with edema ascites. The result were as follows; 1) The basal level of PRA was higher in patients rather than normal controls but PRA was not influenced by MC or L-dopa administration on both normal controls and patients group. 2) The serum cortisol level was significantly elevated at 30 min after MC injection compared with basal level in normal controls but no significant change was noted in patients group. After L-dopa administration the serum cortisol level was not changed in both normal controls and patients group. 3) The serum aldosterone level was significantly elevated in 15, 30 and 60 min after MC injection in normal controls, and there also same tendency of aldosterone secretion was noticed in patients group. On the other hands, there was no changes in aldosterone level in both normal controls and patients group with L-dopa administration. Above result means that MC stimulate aldosterone secretion by dopaminergic antagonist and aldosterone secretion in normal subject is controlled by maximal tonic dopaminergic inhibition. In edematous patients, however, both of the dopaminergic inhibitory and stimulating effect of PRA, ACTH etc on the aldosterone secretion seems to be variable.

Enhanced Expression of Aldosterone Synthase and Adrenome-dullin in Left and Right Ventricular Hypertrophy in Rats

  • The pathophysiological implications of aldosterone and adrenomedullin in the cardiac ventricular hypertrophy were examined. Male Sprague-Dawley rats were treated with deoxycorticosterone acetate (DOCA)-salt and monocrotaline (MCT) to selectively elicit left and right ventricular (LV, RV) hypertrophy, respectively. The mRNA expression of aldosterone synthase and adrenomedullin in LV and RV was determined by reverse transcription-polymerase chain reaction. The expression of aldosterone synthase and adrenomedullin was increased in LV, while not altered significantly in RV of DOCA-salt-treated rats. On the contrary, the expression was not significantly altered in LV, but increased in RV of MCT-treated rats. The enhanced expression of aldosterone synthase may be causally related with the development of ventricular hypertrophy, and the increased expression of adrenomedullin may act as a counter-regulatory mechanism.

  • Acute cadmium exposure has been shown to increase sodium reabsorption in kidney through increase in aldosterone secretion in human and rodents. However, the antinatriuresis is not completely explained by hyperaldosteronism. Moreover, it is still controversial that the increase in plasma aldosterone concentration is mediated by the renin-angiotensin-aldosterone system(RAAS).(omitted)

  • Responses of atrial natriuretic peptide (ANP), aldosterone and renin release to acute volume expansion were compared in normotensive Wistar and spontaneously hypertensive rat (SHR) fed low or high-sodium diet (2 or 25 mmol Na/100 g diet). Experimental diets were fed for 6 weeks from 7-week-old and the growth rate was similar in all groups. In the morning of the experiment, catheters were inserted under ether anesthesia in femoral artery for pressure recording and blood collection, femoral vein for saline infusion, and bladder for urine collection. Then, the rats were placed in restraining cages. When the rats were recovered from anesthesia and the arterial pressure became stabilized, control urine and blood samples were collected. Then, 0.9% saline was infused for 30 min for volume expansion (3% BW). Arterial pressure was significantly higher in the high-sodium SHR but there was no difference between the two groups of Wistar rats. Control plasma levels of Na, K, ANP, renin activity, and hematocrit were not different among the 4 groups. However, plasma aldosterone level was significantly higher in the low-sodium groups. Wistar low-sodium rats showed approximately two times higher plasma aldosterone level than the SHR counterpart. Volume expansion produced a marked increase in plasma ANP level, especially in the high-sodium groups. The low-sodium groups of both strains showed approximately two-fold increase in plasma ANP level. Following a volume expansion plasma aldosterone level and renin activity decreased in all groups. There was a significant logarithmic positive correlation between plasma renin activity and aldosterone concentration. The low-sodium rats produced a greater increase in aldosterone release by small increase in plasma renin than did the high-sodium rats. The low- and high-sodium rats produced a similar degree of diuresis and natriuresis after volume expansion. However, SHR produced a greater natriuresis than did the Wistar rats. The above results indicate that regulatory mechanisms of ANP, aldosterone and renin release are different between the normotensive and hypertensive rats, and between the low- and high-sodium groups.

1 Case of Primary Aldosteronism Caused by Adrenal Hyperplasia (부신증식으로 인한 원발성 Aldosterone증 1예)

  • Primary aldosteronism is a disease that the stimulus for the excessive aldosterone production resides within the adrenal gland. It was first described by conn in 1955. And many cases were reported by physicians at present in the world. But it is relatively rare in Korea, probably due to lack of attension and medical facilities. Only about 13 cases have been reported at present. The clinical, biochemical features in 1 case of primary aldosteranism caused by adrenal hyperplasia that was diagnosed at Yeungnam University Hospital was observed and the following result were obtained. 1. Clinical feature: The present case was 27-year-old woman who was admitted due to general weakness and easy fatigability. The above mensioned chief complaints occurred 8 months prior to admission when she delivered of second baby by cesarian section. Symptoms such as above chief complaints, intermittent muscle paralysis and cramping were noticed. Trousseau's sign was also present. The average blood pressure ranged from 170/90 to 200/120. 2. Biochemical abnormalities: Severe hypokalemia lower than 2.5 mEq/L was presented and 24 hours urine potassium showed markedly increased urinary loss.(228 mEq/day). Plasma renin activity was decreased under normal range with furosemide administration.(Basal renin; 0.01ng/ml/hr, stimulated renin 0.12ng/ml/hr). Saline suppression test revealed markedly elevated levels of aldosterone higher than normal range. (Basal aldosterone; 320.68pg/ml stimulated aldosterone; 451.86pg/ml). And posture test showed decreased plasma renin activity and increased plasma aldosterone level. - PRA(ng/ml/hr)=Bsal: 0.05(0.15~2.33), Stimulated: 0.22(1.31~3.95) - Aldosterone(pg/ml)=Bsal: 242.77(10~160), Stimulated: 432.09(40~310) 3. Adrenal CT scan revealed no abnormal findings. 4. Treatment and course: Spironolactone was given at OPD with regular follow-up. Her blood pressure ranged from 150/90 to 160/100 and symptoms were improved. The effect of treatment was satisfactory and further follow up would be performed.

Effect of Adrenal Hormones and Diets on Diabetic Rats (부신홀몬과 식이가 당뇨병 백서에 미치는 영향)

  • For assessment of the effects of adrenal hormones and diets on diabetic condition, adrenalectomized. Sprague-Dawley rats were fed either high carbohydrate diet, high protein diet, or high fat diet with administration of aldosterone or corticsterone. Adrenalectomy of the rats reduced serum glucos, insulin, total cholesterol, and body weight in all types of diet feeding. In comparison with adrenalectomized control group, corticoste5rone-administered group showed significant increase of serum glucose when fed high fat diet or high protein diet ; however, aldosterone-administered group with high fat diet or high protein diet ; however, aldosterone-administered group with high hat diet showed significant decrease o serum glucose. Serum lipid was increased significantly in cortricosterone-administered group with feeding of high fat or high protein diet. There was no significant effects of aldosterone administeration on parameters determined in this study.

  • To evaluate the acute and chronic effects of potassium on aldosterone response to different amounts of sodium intake, two series of experiments were conducted. In the first series of experiments, when the plasma K level was increased acutely by KCI infusion $(20\;{\mu}g/kg/min\;for\;20 min)$, plasma aldosterone concentration increased in both low Na and high Na groups. However, the aldosterone response to K infusion was significantly greater in the low Na than in the high Na groups. In the second series of experiments, rats fed a high K diet chronically showed a significantly higher plasma K level than those fed a low K diet. However, plasma Na level was maintained relatively constant independent of the Na intake. Both the plasma renin activity (PRA) and aldosterone levels were inversely related to the Na intake. There was a highly positive correlation between aldosterone level and PRA over a wide range of sodium intakes. However, the slope of the correlation line was distinctly steeper in the K-repleted than in the K-depleted rats. The above results indicate that the adrenotropic action of acute K load was augmented in the presence of high plasma renin levels. However, when plasna K level was elevated chronically by a high-K diet, aldosterone secretion was markedly stimulated, although the plasma lenin levels were suppressed.

  • The aim of the present experiments was to investigate the effect of Yukmijihwangtang water extracts on the plasma renin activity and plasma levels of atrial natriuretic peptide and aldosterone in rats. The results of this study were as follows: 1. Plasma renin activity decreased significantly after the administration of Yukmijihwangtang water extracts 1.5ml/kg. 2. Plasma levels of atrial natriuretic peptide (ANP) decreased significantly after the administration of Yukmijihwangtang water extracts. 3. Plasma levels of aldosterone increased significantly after the administration of Yukmijihwangtang water extract.

  • To evaluate the effect of Choakwiyeum(CKY) and Yukmijihwang-tang(YJT) water extract on the renal function and the levels of plasma aldosterone, this experiment was performed in the rabbbits. These result indicate that increase in urine volume(UV) after administration of CKY and YJT water extract is related to increase in glomerular filtration rate(GFR), free water clearance(FWC) and renal plasma f1ow(RPF) and also to decrease in plasma aldosterone concentration(PAC), which seems to be affected by renin-angiotensin-aldosterone system, and that reduced blood pressure seems to be caused by reduced peripheral resistance.
