The Experiences of Nursing Student on Alcoholism Care (간호대학생의 알코올중독 환자간호 실습경험에 관한 연구)

  • Purpose: The aim of this study was to describe the experiences of student nurses during their clinical practice periods in an alcoholic ward. Methods: A descriptive phenomenological study design was employed. Individual interviews were conducted and audio-taped. Colaizzi's method of data analysis was applied to inductively determine themes and formulated meanings. Data saturation was achieved and methodological rigor was established. Results: Four themes were identified and these themes included anxiety of nursing practice in unfamiliar setting, sympathy with patients, learning about the inpatient alcoholics care, and self-reflection. Conclusion: Nursing students had positive and negative experiences in their clinical practice. The results of this study had important implications regarding clinical practice and research for nursing clinical education.

  • This review focused on the pharmacological treatment of alcoholism, especially alcoholism-related mental disorder. The pharmacological agent for alcoholism can be divided into the following categories : anticraving agent, aversive agent, agent to treat acute alcohol withdrawal, agent to diminish drinking by treating associated psychiatric pathology, agent to induce sobriety in intoxicated individuals. Following trends are included in new trends of pharmacological treatment of alcoholism. What are precise conditions amenable to pharmacological intervention? ; How can psychosocial and behavioral intervention be integrated with pharmacotherapy to enhance treatment outcome? ; Is the concept of "matching" specific pharmacotherapy treatment to different aspect of alcoholism more efficacious than a more generalized medicational approach to treatment? One of the most important factors for alcoholics treatment is good and proper therapeutic relationship with patients and setting up individually specialized treatment program is also important.

A Study of the Factors Influencing Alcohol Use Disorder in Elders (재가노인의 알코올 사용장애에 영향을 미치는 요인)

  • Purpose: This study was conducted in order to analyze the factors that influence alcohol use disorder in elders in KyongBuk province. The results of the study will provide basic information for the development of nursing interventions to promote proper alcohol use in elders. Methods: The subjects were 626 elderly individuals. Alcohol use disorder was defined as a score of more than 15 point on the AUDIT-K(Korean version of Alcohol Use Identification Test). Descriptive statistics, chi-square test, ANOVA and logistic regression were utilized to analyze the data. Results: Overall incidence of alcohol use disorder level; normal 73.2%, problem drinking 7.5% and alcohol use disorder 19.3%. Significant factors affecting alcohol use disorder were sex (OR=6.897), religious belief (OR=1.836), smoking (OR=2.948), liver disease (OR=4.753) and depression (OR=1.779). Conclusion: Community health care nurses perform a crucial function in the screening of elderly alcoholics. Early detection and treatment of depression in elders may help to prevent alcoholism.

  • 본 연구의 목적은 알콜중독가정에서 성장한 성인자녀들의 적응도에 영향을 미치는 요인들간의 인과관계를 설명해줄 수 있는 이론적 인과관계모델을 개발하고 그 모델의 적합 성(fit of the model)에 대해 연구되었다 본 연구의 이론적 인과관계모델은 가족 체계이론 대응이론 사회지원이론, 그리고 사회학습이론에 기초를 두고 개발되었다. 본 연구자에 의해 개발된 이론적 인과관계모델은 표보의 특성을 좀 더 적절하게 설명할 수 있고 모델의 적합 성을 증진시키기 위해 수정된 인과관계모델을 바탕으로 연구되었다. 본 연구의 결과를 요약 하면 다음과 같다. 1) 사회적지원의 이용성을 인식하는 성인 자녀들의 경우 정서적으로 불 안정한 상태를 초래했으며 ;2) 사회적지원은 삶에 대한 적응도에 긍정적인 영향을 미쳤고; 3) 정서적으로 안정된 성인자녀들의 경우 글의 삶에 좀 더나은 적응도를 나타내는데 영향을 미쳤다 본 연구에서는 또한 사회적지원과 성인자녀들의 정서적 상태간의 부정적 인관관계에 대해 논의 되었으며 본 연구의 결과를 바탕으로 상담현장에서의 실제 활용에 대해서도 논의 되었다.

Effect of Glutathione on Aldehyde Dehydrogenase Activity (알데히드 탈수소 효소 활성에 미치는 글루타치온의 영향)

  • It is known that alcoholics have significantly lower mitochondrial aldehyde dehydrogenase (ALDH)s'activity than do normal subjects or nonalcoholics with liver disease. However, there are only few reports that explain the reasons behind this reduction of ALDHs'activities. In this study, ALDH activity is inhibited by acetaldehyde, a substrate for ALDH However, the addition of glutathione (GSH) protected ALDH activities against the inhibitory effects of acetaldehyde in vitro. Furthermore, when GSH depletion is induced using diethyl maleate (DEM) in rats by 24% in cytosol and 43% in mitochondria, ALDH activities were also depressed by 31% and 63%, respectively compared to non-treated rats without significant reductions in other hepatic enzymes. These results suggest that ALDHs'activities are closely related to the concentration of acetaldehyde and/or cellular GSH contents . Therefore in alcoholic liver disease, increased productions of acetaldehyde and decreased contents of mitochondrial GSH may involved in the depression of ALDHs'activities.

  • 본 연구의 목적은 알코올 중독 가정내에서 성장한 성인자녀들의 생활만족도에 영향을 미치는 요인들간의 인간관계를 설명해 줄수 있는 원인-결과 모델을 개발하고 그모델의 적합성에 대해 연구되어졌다. 본연구의 결과를 요약하면 다음과 같다. :1) 사회적 지원의 활용성 정도에 대한 인식이 높은 자녀들의 경우 정서적으로 불안정한 상태를 보이는 경향이 있으며 ;2) 사회적 지원의 활용성에 대한 인식정도가 높은 성인자녀들의 경우 어려운 상황을 잘 극복해 나가는 성향을 보여Th ; 3) 위기 대처 능력이 뛰어난 성인자녀들의 경우 생활만족도가 높은 것으로 나타났다. ; 마지막으로 4) 정서적으로 안정된 성인자녀들의 경우 그들의 삶에 대한 적응도가 높은 경향을 보였다. 본 연구에서는 또한 사회적지원과 성인자녀들의 정서적 상태간의 부정적 상관관계에 대해 논의 되었으며 이 연구의 결과를 바탕으로 미래의 연구방향 및 상담현장에서 본 연구내용의 실제활용등에 대해서도 논의되었다.

Ethnic Differences in Cigarette Smoking Behavior: The Paso del Norte 2002 Behavioral Risk Factor Surveillance Survey

  • The goal of this study is to identify and compare predictors of smoking initiation, persistence and smoking status among Hispanics and Whites. The sample includes 1,410 Hispanic and non-Hispanic White adults living in El Paso, TX, USA and Do$\tilde{n}$a Ana and Otero counties, NM, USA from the Paso del Norte 2002 BRFSS. Whites reported higher rates of cigarette smoking and became regular smokers earlier than Hispanics. Males were twice more likely to initiate cigarette smoking and progress to regular smoking than females among Hispanics, but this gender difference among Whites was not significant. Childhood exposure to drinkers or alcoholics was an important predictor of smoking initiation and becoming a regular smoker, but only among Hispanics. Few identified ethnic differences in predictors of smoking were found. The findings underscore the importance of Hispanic norms on smoking behaviors. Prevention efforts need to address the culture as one of the important components relevant to smoking.

  • Alcoholics Anonymous(AA)/Narcotic Anonymous(NA)의 12 단계 중독 회복 프로그램과 선배 회복자의 지지는 중독 치료 모델에서 성공적인 회복에 기여하는 주요한 역할을 한다. 12단계 중독회복 프로그램의 특징은 특정 종교를 기반으로 하지 않지만 자신의 무력감과 영적 존재에 대한 인정을 바탕으로 한다. 어떤 이들에게는 성공의 요인으로 작용하는 이 특징이 일부 참여자에게 거부하게 하는 요인으로 작용한다. 또한 AA/NA의 오프라인 모임에서 발생하는 다양한 부작용으로 인해 사회적 지지가 필요하지만 사회적 지지를 포기하는 이탈이 발생하여 참석자의 회복에 지장을 주기도 한다. 이에 서구 여러 나라에서 AA/NA와 유사하지만 오프라인뿐만 아니라 온라인 모임이 가능하고, 외부의 존재에 대한 의존이 아닌 중독자 스스로가 중독을 극복하게 도와주며 다양한 형태의 중독의 문제를 다루도록 돕는 SMART(Self Management and Recovery Training) Recovery가 대안으로 부상하였다. 따라서 본 연구에서는 중독회복에 상호자조집단의 도움이 필요하지만 12단계 프로그램이나 오프라인 모임 외에 다른 대안이 없는 국내 상황에 대한 새로운 대안으로 SMART Recovery에 대한 정보와 접근방법들을 심도 있게 탐색하고자 한다.

Open Clinical Trial of Morning Light Therapy in Sleep Disturbance of Alcohol Dependent Patients (알코올의존 환자에서의 수면장애에 대한 아침 광치료의 임상시도)

  • Objectives: It is known that sedative hypnotics would make cross tolerance with alcohol and deteriorate quality of sleep in alcoholics. Light therapy is effective non-pharmacological intervention for sleep disturbance in circadian phase disorders, jet-lag, shift-work and age-related sleep disorders. Authors would investigate the effects of morning light therapy on sleep of patients with alcohol dependence during recovery state without withdrawal symptoms. Methods: 13 patients with alcohol dependence who have not any alcohol withdrawal symptom were recruited. Light therapy during 1 hour in the morning had been administered by 2500 Lux light box through serial 3 days. Sleep state of subjects were assessed by sleep log and the subjective satisfaction at sleep was by 100 mm visual analogue scale. Sleepiness, depressive mood, anxiety were evaluated by 100mm visual analogue scale at 8 AM, 2 PM and 8 PM. For assessment of performance ability that would be associated with sleepiness and vigilance, trail making test A, B and digit symbol substitution test were performed by two times on base line and 4th day. Univariate repeated-measures ANOVAs were performed for each measures except performance tests which were analysed by paired t-test. Results: Sleep latency and sleep efficiency were significantly improved with light therapy and satisfaction at sleep was. There was no significant difference in sleepiness at 2 PM with light therapy but sleepiness at 8 AM significantly decreased and at 8 PM increased. The time to complete Trail making test and digit symbol substitution test were significantly shortened at 4th day compared with baseline. Fatigue at 8 AM were not significantly changed with light therapy but at 2 PM and 8 PM significantly decreased. Depressive mood and anxiety were not significantly changed with light therapy. Conclusion: Although this study had some limitations, it showed that light therapy would be effective modality on sleep disturbance of patients with alcohol dependence who have recovered from alcohol withdrawal symptoms. It is proposed that short term light therapy could be used clinically for alcoholics with insomnia. In the future, long term controlled studies using more objective tools for sleep are required to further investigate the effect of light therapy in alcoholics.

A Discovery of the Creative Healing Properties in the Unconscious of Alcoholics: From the Perspective of Analytical Psychology (알코올 중독에서 발견되는 무의식의 창조적 치유 활동: 분석심리학의 관점에서)

  • This study is an attempt to get closer to the psychological truth of alcohol addiction. After reviewing previous related studies, the experiences of Korean alcoholics were examined, encompassing both the conscious and unconscious aspects. In order to approach the unconscious, the researcher paid attention to the series of dreams that the patients reported, from the perspective of Jungian dream analysis. Working with dreams brought considerable benefits in increasing patient's involvement in the therapeutic process and fostering therapist-patient alliance. It also helped to understand patients' conscious and unconscious aspects further. The results were as follows. On the conscious level, patients showed a long-standing alienation from their true emotional needs, a lack of sufficient experience in genuinely expressing and being accepted with one's own emotion, resulting in the need for emotional re-experience, along with the intense longing for maternal affection. This also meant that they were in need for maternal re-experience, and at the same time, for the associated paternal re-experience. Furthermore, the researcher identified that certain powerful archetypal complexes had been activated in the patient's unconscious in response to their conscious situations, triggering the destructive shadows to emerge and exert a strong influence on their conscious lives. The series of reported dreams shows how the unconscious always properly and diligently compensates, modifying the perspective and attitude of the conscious and gradually opening the doors to healing, even for alcoholic patients. This indicates that the unconscious carries out a specific and consistent guidance role so the conscious can realize and adequately respond to it, and that it also provides the creative force for transforming the conscious life. These findings suggest that alcohol addiction is a psychological condition that can jeopardize one's life by allowing the powerful energy of the unconscious to penetrate the conscious, but at the same time, have the purpose and direction to urge the creative transformation of one's conscious life. From these results, it can be concluded that the first step towards true recovery is for the individual to turn their attention towards the psychological truth occurring within themselves. In this regard, the therapist's role is crucial. This also suggests that various attempts in the field of addiction treatment will blossom their real values when they contribute, directly or indirectly, to the development of one's self-awareness since as long as one continues to turn away from the psychological truth and persist in craving for false objects, their creative healing properties in the unconscious could rather have negative and destructive impacts on their life.
