Assessment Report of Bacterial Contamination in Some School Dining Services with Table Swabs and Air Samples (학교식당 및 교실배식 과정 전·후 미생물 오염에 관한 연구)

  • Objectives: The aim of this study is to investigate microbial contamination in the school food service environment for the assessment of microbial food safety. Methods: We collected both swab samples from tables and desks and airborne bacterial samples from an elementary school (School A) and a high school (School B). Heterotrophic plate count, total coliform, Staphylococcus aureus, and Bacillus cereus were measured with selective media to quantify microbial concentration. PCR assay targeting 16S rRNA genes was performed to identify the strains of S. aureus and B. cereus isolated. In addition, we made a food service checklist for the locations to evaluate the food service environment. A Wilcoxon test was employed to examine the differences in microbial concentration between before lunchtime and afterwards. Results: Heterotrophic plate counts showed higher levels after-lunch compared to before-lunch at School B. However, levels of S. aureus were higher in the after-lunch period (p<0.05) in both classrooms and in the cafeteria in School A. B. cereus was only sparsely detected in School B. Several samples from food dining carts were found to be contaminated with bacteria, and facilities associated with food delivery were found to be vulnerable to bacterial contamination. Although microbial concentrations in the air showed little difference between before- and after-lunchtime in the cafeteria in School A, those in classrooms were greater after-lunchtime at both schools. Conclusion: Our results suggested that the microbial safety in schools after lunchtime of concern. Necessary preventive measures such as hygiene education for students and food handlers should be required to minimize microbial contamination during food service processes in schools.

The retrospective study of marginal bone loss around dental implants according to different autogenous bone grafts (이식된 자가골의 종류와 형태에 따른 임플란트 변연골 흡수량에 관한 후향적 연구)

  • Introduction: This study examined the cumulative resorption of implants placed in a severely atrophic mandible and analyzed the radiologic bone resorption in the marginal bone, after an autogenous bone graft including both block and particulates that had been harvested from the ramus and iliac crest. Materials and Methods: A retrospective study was performed on patients who had bone grafts for augmentation followed by implant installation in the mandible area from 2003 to 2008. Twelve patients (6 men and 6 women) who received 34 implants in the augmented sites were evaluated. Cumulative radiologic resorption around the implants was measured immediately, 3 months, 6 months and 12 months after implant installation surgery. Results: The installed implant in grafted bone showed 0.84 mm marginal bone resorption after 3 months and 50% total cumulative resorption after 1 year. The mean marginal bone resorption around the implant installed in the grafted bone was 0.44 mm after 3 months, 0.52 mm after 1 year, after which it stabilized. The implant survival rate was 97% (failed implant was 1/34). Marginal bone resorption of the installed implant in the autogenous onlay block bone grafts was 0.98 mm after 3 months, which was significantly higher than that of a particulated bone graft (0.74 mm) (P <0.05). Conclusion: An autogenous graft including block type and particulate type is a predictable procedure for the use of dental implants in a severely atrophic mandible. Implant placement in augmented areas show a relatively high survival and minimal bone loss, as revealed by a radiologic evaluation.

An Acute Postoperative Intractable Hyperventilation after an Endoscopic Third Ventriculostomy

  • This report describes a rare case of postoperative hyperventilation attack after an endoscopic third ventriculostomy in a 46-year-old woman. About 60 min after the termination of the operation, an intractable hyperventilation started with respiratory rate of 65 breaths/min and $EtCO_2$, 16.3 mm Hg. Sedation with benzodiazepine, thiopental sodium, fentanyl, and propofol/remifentanil infusion was tried under a rebreathing mask at a 4 L/min of oxygen. With aggressive sedative challenges, ventilation pattern was gradually returned to normal during the 22 hrs of time after the surgery. A central neurogenic hyperventilation was suspected due to the stimulating central respiratory center by cold acidic irrigation solution during the neuroendoscopic procedure.

A study on the present status and development direction of after-school facilities in Cheonan (천안지역 방과후아동지도시설 실태 및 발전방안)

  • This study aims to identify children's after-school facilities in Cheonan, to evaluate their programs, and to further suggest desirable directions of those programs. During this study, we found that the facilities in Cheonan are much less in numbers and no better than we expected. Especially, it is shown that most of the study rooms there are evaluated as relatively the worst facilities in terms of space organization, health care and safety, daily schedule and management, and program evaluation process. In conclusion, this study proposes that such facilities should be greatly expanded and improved in the program development as well as in the working environment. We also propose to newly establish a regional center which supports and certifies educational programs of each after-school facility.

  • Reactive arthritis comprises a subgroup within infection-associated arthritides in genetically susceptible hosts. Researchers and clinicians recognize two clinical forms of reactive arthritis which occurs after genitourinary tract infection and after gastrointestinal tract infection. Chlamydia infection has been implicated as the most common agent associated with post-venereal reactive arthritis. Studies have proposed Shigella infection, Salmonella infection, or Yersinia infection as the microorganisms responsible for the post-dysenteric form. The human leukocyte antigen (HLA)-B27 antigen is the best-known predisposing factor. We report a case of HLA-B27-associated reactive arthritis after Salmonella enteritis at Pusan National University hospital.

A Study on the Educational Effects of HACCP Training for Employee Cooks of School Foodservice Operations in the Kyeonggi Area (경기 일부 지역 학교 급식 조리 종사자의 HACCP 교육이 위생 지식에 미치는 영향)

  • The purpose of this study was to investigate the effects of HACCP education on improvements of knowledge in school foodservice employees in Gyeonggi-do, particularly in Anyang, Ansan, Siheung and Hwasung City. A first and second questionnaire was answered both before and after HACCP education that was directed by a dietitian. The change in HACCP knowledge after education, as compared to before, was statistically significant(p<0.01). Specifically, the average score after education increased more than 30 points in CCP2 and CCP6, and more than 60 points in CCP3. Most of the school foodservice operations performed sanitary education once a month; however, they did not perform regular HACCP education. The fact that the level of sanitary knowledge was different after HACCP education than before suggests the possibility of improving the sanitary performance levels of foodservice employees through continual education.

Pseudoaneurysm of Anterior Tibial Artery after Ankle Arthroscopy (족관절 관절경 수술 후 발생한 전경골 동맥의 가성 동맥류)

  • Pseudoaneurysm is extremely rare complication after ankle arthroscopy with standard anteromedial and anterolateral portals. We report a case of a pseudoaneurysm of the anterior tibial artery detected at 3 months after ankle arthroscopy in a 16-year-old male. He had sustained painful swelling of his right ankle after the arthroscopic surgery, and referred to our hospital with an MRI checked postoperatively. We failed to make the diagnosis of pseudoaneurysm with the postoperative MRI, thus the patient underwent another arthroscopy which revealed massive hemarthrosis within the joint. The diagnosis was confirmed with an angiography, and the vascular lesion was ligated.

  • The purposes of this study were to investigate what kinds of idea the children have after formal instruction in school, and to investigate what is the children's conceptual change on electric current after implementation of drill and practice type CAI program about electric circuit. The CAI program was developed by KEDI research project team the author of this article was joined. The subjects were 20 eleven yaers old(elementary fifth year) children sampled from a elementary school in Seoul, Korea. They had already learnt about electric circuits for two years before implementing this CAI program according to the formal science curriculum. The pretest were accomplished before implementing the CAI program, and posttest were accomplished a week after implementation of the CAI program. The results of this study are as follows: Even though they had studied electric circuits, only 5% of the students had scientific ideas before pretest, and even after drill and practice type CAI program only 30% of the students have changed their misconceptions into scientific conceptions.

Mallory-Weiss Tear After Cardiopulmonary Resuscitation in a Patient Suffering From Acute Myocardial Infarction

  • A report of a 79 year old male patient suffering from acute myocardial infarction with Mallory-Weiss tear after successful cardiopulmonary resuscitation(CPR) by emergency medical technician in the swimming pool is presented. Successful percutaneous coronary intervention(PCI) was done after appropriate transfusion. The patient survived and discharged without major complications after admitting 11days in the hospital. Importance of CPR in AMI patient is reiterated as complication such as Mallory-Weiss tear may arise.

Evaluation of the Effects of a Smoking Prevention Program for Korean High School Students (고등학생을 위한 학교 흡연예방 프로그램 효과 평가)

  • Objectives: This study was performed to develop a smoking prevention program for Korean high school students, and to evaluate the effects of the program. Methods: A smoking prevention program, composed of five-session curriculum, was developed by modifying several smoking prevention and cessation programs based on the Social Influence Model. The program was applied to the freshmen of a technical high school. We surveyed with a questionnaire one week before education, one week after education, and two months after education. The number of participants for data analysis were 282(181 males, 101 females). Among those, 162(97 males, 65 females) students were allocated to the education group, and the other 120(84 males, 36 females) students were allocated to the control group. The effect of education was analyzed by multiple logistic regression analysis adjusted by potential confounders. Results: Among smokers, those who had education were more likely to quit smoking (OR=2.99, 95% confidence interval(CI) 0.84-10.64), and to decrease smoking frequency(OR=2.29, 95% CI 0.95-5.53) in borderline significance one week after education. However, the effect of education disappeared two months after education. The effect of education was significant(OR=9.11, 95% CI 3.22-25.76) for the increase of smoking cessation intention one week after education, and it persisted until two months after education(OR=2.94, 95% CI 1.18-7.35). Education was also a significant predictor(OR=1.97, 95% CI 0.89-4.37) for the increase of smoking cessation stage one week after education and it persisted(OR=6.39, 95% CI 2.42-16.86) after two months. Among non-smokers, those who had education were more likely to decrease smoking intention one week after education(OR=4.71, 95% CI 1.63-13.58). However, the statistical significance of education disappeared two months after education. Conclusions: The results showed that the smoking prevention program developed in this study changed smoking behaviors immdeiately after education even though the effect did not persist. However, this program was successful in increasing smoking cessation intention and stage of smoking cessation among smokers.
