Causes of age-related sarcopenia and frailty: the role of exercise and nutrition for prevention (노화 관련 근감소증과 노쇠함의 원인과 예방을 위한 운동과 영양의 역할)

  • The purpose of this study is not only to define and cause of sarcopenia and frailty due to aging, but also to explore prevention and delay through regular exercise and right nutrition. Sarcopenia and frailty are known to cause frailty, cachexia, osteoporosis, metabolic syndrome and early death due to decreased muscle mass and muscle function caused by aging. The two disease were related to nervous system damage and lack of protein synthesis within the muscles due to the increase in the amount of oxidative stress, and inadequate nutrition and lack of physical activity. It also emphasizes the role and importance of right nutrition and physical activity (such as aerobic exercise, resistance movement, etc) to prevent and improve such syndromes. Conclusions, it is in proposing the supply of high-quality protein, including regular exercise, as the most beneficial way to prevent and overcome aging-related sarcopenia and frailty.

Effectiveness of Physical Exercise in Patients Undergoing Allogeneic Hematopoietic Stem Cell Transplantation (동종 조혈모세포이식을 받은 환자에서 운동중재의 효과)

  • Purpose: To summarize and review the methodological quality of the evidence from trials examining the effectiveness of physical exercise in patients undergoing allogeneic hematopoietic stem cell transplantation (Allo-HSCT). Methods: Six randomized clinical trials (RCTs) were identified, reviewed for substantive results, and assessed for methodological quality. Results: Six trials met all methodological criteria on the modified Jadad score above 3 out of 5 points. Failure to blind the outcome assessor, and failure to describe the method of blinding of outcome assessor appropriately were the most prevalent methodological shortcomings. Various exercise modalities have been applied, differing in content, frequency, intensity, and duration. Positive results have been observed in part for a diverse set of outcomes, including physical and psychological performance. Conclusion: The trials reviewed in this study were of moderate methodological quality. They suggest that exercise in patients undergoing Allo-HSCT may be safe and feasible, and in part patients benefit from increased physical performance both during and after transplantation. Future RCTs should use larger samples, appropriate comparison groups, and a standard of outcome measures, and examine what kind of exercise intervention (aerobic vs. resistance vs. combined) is the most effective for Allo-HSCT patients. It would be necessary to define contraindication for exercise to guarantee its safety.

Morphological Changes in Quadriceps Muscles through 3-Week Combined Exercise using a Wearable Robot (EX1) in Young Adult

  • Objective: This study aims to analyze the effect of regular exercise through the combined walking-oriented aerobic and resistance exercises using EX1 in young adults. Design: Experimental one group pre and post test Methods: Participants comprised17 healthy young adults. All subjects performed a combined exercise program for 10 times using EX1. We measured quadriceps muscle thickness using ultrasound. Additionally, the hand grip strength test, and sit and reach test were performed before and after the exercise. Through paired t-test, we investigated whether there was a statistically significant difference in the measurement results after exercise program. Results: The rectus femoris muscle contraction ratio showed significant difference after exercise(P< 0.01). In the sit and reach test, flexibility showed significant difference after exercise(P < 0.01). The hand grip strength test also showed significant difference after exercise(P < 0.05). Conclusions: Healthy young adults can effectively perform various exercises commonly performed in daily life using EX1.

Theoretical Study of Effective Resistance Exercise for Sarcopenia

  • Sarcopenia is a phenomenon in which muscle function, including muscle strength, deteriorates as muscle mass decreases in the process of increasing age. The diagnosis of sarcopenia utilizes total muscle mass and limb muscle mass, and limb muscle mass is expressed as height squared, body weight, and BMI. Each divided value is used as an index, mainly less than 7.23 kg/m2 for men and less than 5.67 kg/m2 for women. Grip strength, standing up from a chair, and walking speed were mainly used as physical function factors, and grip strength less than 27 kg for men and less than 16 kg for women were used as indicators. The limb muscle mass showed a decreasing trend after peaking in the mid-20s in men, and maintaining a gradual peak in women from the mid-20s to the mid-40s, showing a more rapid decline in men. The rate of decrease in muscle mass and strength continues to increase after the age of 20, and muscle strength rapidly decreases after the age of 80. In Korean men, total muscle mass and limb muscle mass show a decreasing trend from the mid-30s, and a more markedly rapid decrease from the age of 60. For women, it remains constant from the age of 30 to the age of 50, then gradually decreases after the mid-50s, and shows a rather rapid decrease after the mid-70s, showing a more gradual decrease than that of men. Men show a sharp decrease from the mid-40s when limb muscle mass is divided by height squared, and women show a marked decrease after 70 years old when limb muscle mass is divided by height squared. Exercise for the prevention and treatment of sarcopenia results in an increase in protein assimilation hormone, an increase in antioxidant activity, a decrease in inflammation, an increase in muscle insulin sensitivity, and an increase in protein synthesis. Resistance exercise is basically used, and aerobic exercise and equilibrium A combination of exercises is effective. In addition, for a more efficient effect of sarcopenia through resistance exercise, it is necessary to supplement nutrition including protein.

Effects of Purple Sweet Potato intake and Aerobic Combined Exercise on Health Related Fitness, Blood lipid profile and Insulin resistance (자색고구마 섭취와 유산소 복합운동이 비만 여중생의 건강체력, 혈중지질 및 인슐린 저항성에 미치는 영향)

  • The purpose of this study was to investigate the effects of purple sweet potato(PSP) intake and aerobic combined exercise in obese middle school girls. Twenty-four, obese(%body fat > 30%) middle school girls composed of the purple sweet potato intake and aerobic combined exercise group(A, n=6), the aerobic combined exercise group(B, n=6), the purple sweet potato intake group(C, n=6), the control group(D, n=6). The variables of health related fitness, blood lipid profile, insulin resistance were measured in all the subjects before the start and after the end of 12 week aerobic combined exercise program(40~70%HRR, 3 times per week, 70 mins). The test data were analyzed by paired t-test and one way ANOVA, and the alpha level of p<.05 was set for all tests of significance. In the comparison within each group, %body fat[A(p<.01), C(p<.05) groups], TC[A(p<.05), B(p<.01), C(p<.01) groups] and insulin resistance[A(p<.05) group] were significantly decreased and LBM[A(p<.01), B(p<.01) groups], muscular strength[A(p<.01), B(p<.001), C(p<.05) groups], muscular endurance[A(p<.05), B(p<.001) groups], flexibility[A(p<.05), B(p<.01) groups] and cardiorespiratory endurance[A(p<.05), B(p<.001), C(p<.01) groups] were significantly increased. In the comparison between groups, A group was significantly decreased in %body fat, TC and TG than D group(p<.05). A group was significantly increased in muscular strength and cardiorespiratory endurance than C, D groups (p<.05). A group was significantly increased in muscular endurance and HDL-C than D group(p<.05). A, B groups were significantly increased in flexibility than D group(p<.05). A, B, C groups were significantly decreased in insulin resistance than D group(p<.05). In conclusion, purple sweet potato intake and aerobic combined exercise were effective in improving the health related fitness, blood lipid profile and insulin resistance in obese middle school girls.

The Effects 12 Weeks of Combined Exercise Programs on Activities of Daily Living and Quality of Living Index in the Vascular Dementia Elders (12주간 복합운동프로그램이 혈관성 치매노인의 일상생활 수행능력과 삶의 질 지수에 미치는 효과)

  • Purpose : This study was to investigate vascular dementia elders's activities of daily living, quality of life index and to determine how much vascular dementia elders is influenced by the 12 weeks combined exercise program with aerobic training and resistance exercise. Methods : Thirty patients with vascular dementia over 65 in B geriatrics hospital were recruited this study. Activities of daily living were measured by I-ADL(instrumental-activities of daily living) and B-ADL(basic-Activities of daily living) and quality of life index was measured by GQOL-D(geriatric quality of life scaledementia). Statistical analysis was used repeated one-way ANOVA to test mean difference by using SPSS 12.0 for windows. Results : After comparing the activities of daily living of experimental group that of control group according to the period of exercise, there were statistically significant differences in I-ADL, B-ADL score test and GQOL-D index test of both experimental and control groups. There was also a significant difference in comparing the results of 12 weeks exercise of the groups. Conclusion : 12 weeks combined exercise program had a good influence on vascular dementia elders's activities of daily living and quality of life index.

Effects of an exercise program on health-related physical fitness and IGF-1,C-peptide, and resistin levels in obese elementary school students

  • Childhood obesity causes a higher risk of obesity, premature death and disability in adulthood. In addition, obese children experience an increased risk of respiratory problems, hypertension, cardiovascular disease, insulin resistance and psychological effects. This study aimed to investigate how an exercise intervention affects health-related physical fitness and inflammatory-related blood factors in obese children after. We hypothesized that there would be positive effects on serum levels of insulin-like growth factor-1 (IGF-1), connecting peptide(C-peptide) and resistin, as well as in muscle and cardiovascular-related physical capacities, after an exercise intervention in obese children. Thirty-seven obese children haveperformed health-related fitness tests and provided blood samples for the analysis of changes in circulating biomarkers, both before and after an 8-week exercise intervention, which includes stretching, aerobic exercise, resistance exercise and sports games. The results indicate that exercise training beneficially affects body compositions, especially percentage body fat and muscle mass, without influencing to body weight and height. The results of the physical fitness tests show that muscle and cardiovascular capacity were increased in obese children in response to exercise training. Simultaneously, the exercise training decreased circulating levels of C-peptide, which equated to a "large" effect size. Although there were no significant effects on the levels of IGF-1 and resistin, they show a "small" effect size. Therefore, our findings suggest that the exercise intervention have beneficial effects on body composition and physical fitness levels in obese children, whichmight be associated with the decline in circulating C-peptide.

The Role of Mobility of Cognition and Emotion in Elderly : A Conceptual Review (노인의 운동성과 인지 및 정서의 관계에 대한 개념적 고찰)

  • Considering that the aged population increases and the mobility problem is pointed out as a factor that indisposes the quality of life, cognition, and mood, it is important to understand and evaluate the elderly's mobility. Factors that deteriorate mobility in the elderly include physical senility, various health changes including chronic diseases, polypharmacy as well as anticholinergics. Common mobility problems in old age are reduced gait speed, increased gait variability in walking length, careless walking, and frequent falls. Several studies have reported that decreased mobility and deterioration of gait can predict cognitive decline and emotional problems. Aerobic exercise, resistance exercise, and balance exercise are suggested as therapeutic interventions for mobility problems. Active correction for factors that reduce mobility in the elderly and prescribing physical activity can conserve the elderly's quality of life and help improve cognition and mood. There is a need for related research in the future.

A Meta-Analysis of the Effects of Exercise Programs on Glucose and Lipid Metabolism and Cardiac Function in Patients with Type II Diabetes Mellitus (제2형 환자를 위한 운동프로그램이 당대사, 지질대사 및 심폐기능에 미치는 효과에 대한 메타분석)

  • Purpose: This study was a systematic review and meta-analysis of the effects of exercise programs in patients with type II diabetes mellitus. Method: Two investigators systematically searched and reviewed English articles from PUBMED from 1988 to 2004, selecting randomized controlled trials on structured exercise programs for DM patients. Out of 87 studies identified, a meta analysis was done for eleven studies which satisfied inclusion criteria and focused on glycemic indices, lipid indices, and cardiac function indices. Results: The means and standard deviations were compared for experimental groups that received exercise-only or exercise and diet programs and control groups that received no intervention or only diet education. The groups were considered homogeneous as the p value of the Q score in each variable group was over 0.05. The experimental groups demonstrated a moderate positive effect on HbA,c and $VO_{2max}$ (d=0.55 & 0.5), and a small positive effect on fasting blood glucose and cholesterol (d=0.38 & 0.27) compared to the control groups. HDL and LDL cholesterol levels, however, showed a very low positive effect (d=0.11 & 0.12) in the experimental groups. Aerobic exercise was more beneficial than resistance exercise on $HbA_1c$ (d=0.59 vs 0.28) in the groups. Conclusions: Regular exercise has a positive effect on $HbA_1c$, fasting blood glucose, total cholesterol, HDL cholesterol, LDL cholesterol, and $VO_{2max}$ in Type 2 diabetic patients.
