Comparison of the Pelvic Height Difference in Subjects with Lower Back Pain and in Normal Subjects in Different Postures (체중부하 자세에 따른 요통환자와 정상인의 양측골반높이에 대한 연구)

  • Background: Lumbar joint dysfunction is reported to be the main cause of lower back pain (LBP). The purpose of this study was to evaluate the effect of joint dysfunction on the postural balance of the lower hack and pelvis in different normal activities such as walking or stair management. Also it was studied whether the status of LBP (intensity and duration of LBP, length of treatment) contributes to die pelvic height difference (PHD) in various postures. Subjects: 28 patients with LBP and 32 normal adult volunteers, 60 years of age or younger, who came to the Community Health Center and orthopedic clinics in Incheon, South Korea. Methods: In order to determine the accuracy of the manual angulometer method in measuring the PHD, it was compared to the pelvic x-ray method in selected subjects. In the manual angulometer method, the arm of the angulometer was placed on the top of both iliac crests. The PHD was measured in static upright stance, then one-legged stance, on the affected leg or unaffected leg each time. Information regarding the disease status was obtained through interviews. Visual assessment scale was used to grade the intensity of LBP. Data analysis was performed using SPSS 10.0/PC program. Homogeneity between the two groups was tested by 2-test and t-test. To compare the PHD of the subgroups, we used t-test, F-test and two-way ANOVA. Relationships among dependent variables were analyzed by Pearson correlation analysis. Conclusion: In patients with LBP, lumbar joint dysfunction causes lumbar and pelvic postural asymmetry during normal activities.

Evaluation of the antegonial notch related to the mandibular morphology and the curve of Spee in anteroposterior relation (전후방적 분류에 따른 하악 형태 및 Spee 만곡에 대한 antegonial notch의 관계 평가)

  • The aim of this study was to investigate the relationship between the mandibular antegonial notch and the mandibular morphology, and the curve of Spee in anteroposterior skeletal relationship. Pre-treatment lateral cephalograms were obtained from 80(male 34, female 46) adult orthodontic patients and the samples were classified into 3 categories by ANB angle($0^{\circ}$$\leq$ANB< $4^{\circ}$ Class I, $4^{\circ}$$\leq$ANB Class II, ANB< $0^{\circ}$ Class III). The curve of Spee was measured directly from the pre-treatment mandibular study cast of each patient included in this study. Pearson correlation coefficient test and multiple regression analysis in each group revealed the following results; 1. Antegonial notch depth was positively correlated with hoth lower anterior facial height(ANS-Me) in skeletal Class I, II and III groups and Id-Me height in skeletal Class I and II groups. 2. A statistically significant negative correlation was found between the depth of the antegonial notch and the curve of Spee in the Class III group. 3. Significant relationship was not found between the antegonial notch depth and any of the other cephalometric variables such as mandibular body length(Go-Gn) and ramus height(Co-Go). As antegonial notch depth increased, more vertical growth of the mandible was observed. Antegonial notch can be used as a predictor of vertical mandibular growth in the diagnosis and treatment planning of malocclusion.

Iron Nutritional Status by ALAD Genotype and Intervention Study for Rural Area Residents (농촌 여성들의 ALAD 유전형질별 철분영양상태와 철분제 섭취에 따른 영양상태의 변화)

  • Previous studies have suggested that delta-aminolevulinic acid dehydratase (ALAD) phenotype differently affect mineral metabolism. The objective of this study was to determine the effectiveness of 6-month iron supplementation as syrup of NaFeEDTA in improvement of iron status according to ALAD genotype. One hundred thirty adult women living in rural areas of Asan were provided NaFeEDTA syrup once a week for 6 months at the dose of 64mg Fe/week. Three hundred control subjects were observed during the study period. Fasting blood was obtained for analyzing hemoglobin (Hb) and zinc protophorphyrin (ZPP) and serum was analyzed for ferritin, iron and total iron capacity (TIBC) levels before and after iron supplementation. Ninety percent of ALAD 1-1 (ALAD1) and 10% of ALAD 1-2 (ALAD2) genotype were observed in the control group. However in the intervention group, 98% showed ALAD1 while only 2% was ALAD2, which is significantly lower proportions of ALAD2 compared to the control group (p<0.01). The iron status of Intervention group significantly improved except for ferritin and TIBC regardless or ALAD genotype, while the control group did not show any changes in iron status except for ZPP. ZPP concentration of the control group significantly increased in both ALAD1 and 2 while the intervention group showed significantly decreased ZPP after supplementation in ALAD1. Iron supplementation in the form of NaFeEDTA seems to be effective in reduction of ZPP levels although ALAD2 did not show significant changes due to the small number. However, it is difficult to make a conclusion from these results, and more specified further investigation is needed with more participants.

  • An epidemiological study fur the Intestinal trematode infection of the villagers was done In Koje-myon, Kochang-gun, Kyongsangnam-do, Korea in March, 1994. Of 116 stool specimens examined, total helmlnthic ova positive cases were 13 (11.2%) and cumulative ova positive cases 21 (18.1%): Echinostoma hortense 11 cases (9.5%), Metngonimus sp. 6 cases (5.2%), and Clonorchis sinensis 4 cases (3.4%). After the treatment and purgation, variable numbers of E. hokene, 6 to 227 per person, were collected from 7 echlnostome egg Positive cases, together with M. tokohoshii In 6 cases. Three adult flukes of E. cinetorchis were collected from one person and one Stellantchasmw jnlcatus was recovered from another case. This mountainous village was proved to be one of the endemic foci of echlnostomlasis In Korea.

Influence of mobile phone texting on gait parameters during ramp ascent and descent

  • Objective: The purpose of this study was to examine the influences on gait features during mobile phone use while ramp walking. Design: Cross-sectional study. Methods: Thirty-three healthy adult subjects performed four walking conditions on an outside ramp with a 5 m length, 1.5 m width, and a $5^{\circ}$ angle. All participants were touch screen mobile phone users. Four walking conditions were used: 1) ramp ascent, 2) ramp descent, 3) texting during ramp ascent, and 4) texting during ramp descent. In conditions 3) and 4), subjects texted the words of "Aegukga"-the song of patriotism-while walking. Upon the signal of start, the subjects walked the ramp during texting. Gait parameters were measured at the length of 3 m excluding 1 m of the start and end of the total length. Each situation was repeated three times for each subject, and mean values were calculated. For gait examination, a gait analyzer was used (OptoGait). Results: Subjects ranged in age from 23 to 38 years (mean age, 27.73). Eighty-three percent of subjects in our study had experienced an accident during mobile phone use. Texting on a mobile phone while walking significantly decreased ramp gait, speed, cadence, stride length, step length, and single support (p<0.05) and significantly increased stride time, step time, gait cycle, and double support (p<0.05). There was a significant difference in cadence, step length, stride time, step time, and single support during ramp ascent and descent (p<0.05). Conclusions: Texting on a mobile phone while walking significantly decreased gait quality.

A Study of Radiation Dose Evaluation and Optimization Methods for Intra Oral Dental X-ray in Pediatric Patient (소아 구내촬영 시 방사선량 평가 및 최적화 방안에 대한 연구)

  • Although intra oral dental x-ray is a lower dose than other radiological examinations, pediatric patients are known to have a higher risk of radiation damage than adults. For this reason, pediatric dental x-ray requires management of dose evaluation and imaging conditions during the examination. In this study, the dose calculation program ALARA-Dental(child/adult) was used to evaluate the organ dose and effective dose exposed to each examination site during intra oral imaging of children during dental radiographic examination, and dose analysis according to the imaging conditions was performed. As a result, the highest organ dose distribution was shown at 0.044 ~ 0.097 mGy in all are as of the mucous membrane of oral cavity except for the maxillary incisors and canines. Also, in the case of the thyroid gland, the maxillary canine and maxillary premolar examination showed 0.027 and 0.020 mGy, respectively, and the dose distribution was 15.4% to 70.0% higher than that of the mandibular examination. As for the effective dose calculated during intra oral imaging, the maxillary anterior and canine examinations showed the highest effective doses of 0.005 and 0.004 mSv, respectively, and the maxillary area examination showed a higher dose distribution on average than the mandible.

Factors Affecting Readmission After Discharge in Stroke Patients: A Retrospective Study (뇌졸중 환자의 퇴원 후 재입원에 영향을 미치는 요인: 후향적 연구)

  • Purpose: The purpose of this study was to identify the factors affecting readmission in stroke patients. Methods: A retrospective study design was used. Participants were 3,675 adult cerebral stroke patients in the inpatient wards of the Department of Neurology and Neurosurgery of G University Hospital located in C city. Data were collected from January 1, 2016 to December 31, 2021 and data were analyzed using χ2 test, independent t-test, and multivariate logistic regression with SPSS/WIN 24.0. Results: After discharge for stroke, the readmission rate was 23.7%, and the mortality rate was 0.3%. The variables with significant differences between the readmission group and non-readmission group were age, type of stroke, surgery, ICU treatment, mRS score, blood pressure, diabetes, and heart disease. Factors influencing an readmission in stroke patients were Age 65-74 (OR 1.30, 95% CI=1.03-1.64), ≥ 75 (OR 1.28, 95% CI=1.02-1.62), mRS score 2points (OR 2.50, 95% CI=1.99-3.13), HTN status (OR 1.26, 95% CI=1.07-1.50), CVD status (OR 1.38, 95% CI=1.01-1.90), TC (OR 1.60, 95% CI=1.05-2.44). Conclusion: To lower the readmission rate of stroke patients, it is essential to control lifestyle, including whether or not to take treatment drugs, after diagnosing risk factors such as high blood pressure, diabetes, and heart disease, hyperlipidemia. Nursing interventions that can provide information on risk factor management and coping strategies are urgently needed as symptoms change. In addition, research is needed to develop and implement an intervention strategy that can improve the function of stroke patients as much as possible at home or in society so that they can lead an independent life without the help of others, and verify their effectiveness.

Endoscopic versus surgical management for colonic volvulus hospitalizations in the United States

  • Background/Aims: Colonic volvulus (CV), a common cause of bowel obstruction, often requires intervention. We aimed to identify hospitalization trends and CV outcomes in the United States. Methods: We used the National Inpatient Sample to identify all adult CV hospitalizations in the United States from 2007 to 2017. Patient demographics, comorbidities, and inpatient outcomes were highlighted. Outcomes of endoscopic and surgical management were compared. Results: From 2007 to 2017, there were 220,666 CV hospitalizations. CV-related hospitalizations increased from 17,888 in 2007 to 21,715 in 2017 (p<0.001). However, inpatient mortality decreased from 7.6% in 2007 to 6.2% in 2017 (p<0.001). Of all CV-related hospitalizations, 13,745 underwent endoscopic intervention, and 77,157 underwent surgery. Although the endoscopic cohort had patients with a higher Charlson comorbidity index, we noted lower inpatient mortality (6.1% vs. 7.0%, p<0.001), mean length of stay (8.3 vs. 11.8 days, p<0.001), and mean total healthcare charge ($68,126 vs. $106,703, p<0.001) compared to the surgical cohort. Male sex, increased Charlson comorbidity index scores, acute kidney injury, and malnutrition were associated with higher odds of inpatient mortality in patients with CV who underwent endoscopic management. Conclusions: Endoscopic intervention has lower inpatient mortality and is an excellent alternative to surgery for appropriately selected CV hospitalizations.

Prevalence of posttraumatic stress disorder in orthopedic trauma patients and a call to implement the Injured Trauma Survivor Screen as a prospective screening protocol in the United States

  • Purpose: Posttraumatic stress disorder (PTSD) is prevalent and is associated with protracted recovery and worse outcomes after injury. This study compared PTSD prevalence using the PTSD Checklist for DSM-5 (PCL-5) with the prevalence of PTSD risk using the Injured Trauma Survivor Screen (ITSS). Methods: Adult trauma patients at a level I trauma center were screened with the PCL-5 (sample 1) at follow-up visits or using the ITSS as inpatients (sample 2). Results: Sample 1 (n=285) had significantly fewer patients with gunshot wounds than sample 2 (n=45) (8.1% vs. 22.2%, P=0.003), nonsignificantly fewer patients with a fall from a height (17.2% vs. 28.9%, P=0.06), and similar numbers of patients with motor vehicle collision (40.7% vs. 37.8%, P=0.07). Screening was performed at a mean of 154 days following injury for sample 1 versus 7.1 days in sample 2. The mean age of the patients in sample 1 was 45.4 years, and the mean age of those in sample 2 was 46.1 years. The two samples had similar proportions of female patients (38.2% vs. 40.0%, P=0.80). The positive screening rate was 18.9% in sample 1 and 40.0% in sample 2 (P=0.001). For specific mechanisms, the positive rates were as follows: motor vehicle collisions, 17.2% in sample 1 and 17.6% in sample 2 (P>0.999); fall from height, 12.2% in sample 1 and 30.8% in sample 2 (P=0.20); and gunshot wounds, 39.1% in sample 1 and 80.0% in sample 2 (P=0.06). Conclusions: The ITSS was obtained earlier than PCL-5 and may identify PTSD in more orthopedic trauma patients. Differences in the frequency of PTSD may also be related to the screening tool itself, or underlying patient risk factors, such as mechanism of injury, or mental or social health.

A Novel System with EMG-controlled FES Enhanced Gait Function and Energy Expenditure for Older Adults

  • Objective: This study was conducted to analyze the effect of wearable Electromyography-controlled functional electrical stimulation (EMG-controlled FES) System on Gait Function and cardiopulmonary metabolic efficiency during walking in older adults. Design: Cross-section study Methods: Total 22 older adult participants suitable to selection criteria of this study participated in this study. The EMG-controlled FES System, which functions as a wearable physical activity assist FES system was used. All participations performed randomly assigned two conditions (Non-FES assist [NFA], FES assist [FA]) of walking. In all conditions, spatio-temporal parameters and kinematics and kinetics parameters during walking was collected via 3D motion capture system and 6 minutes walking test (6MWT) and metabolic cost during walking and stairs climbing was collected via a portable metabolic device (COSMED K5, COSMED Srl, Roma, Italy). Results: In Spatio-temporal parameters aspects, The EMG-controlled FES system significantly improved gait functions measurements of older adults with sarcopenia at walking in comparison to the NFA condition (P<0.05). Hip, knee and ankle joint range of motion increased at walking in FA condition compared to the NFA condition (P<0.05). In the FA condition, moment and ground reaction force was changed like normal gait during walking of older adults in comparison to the NFA condition (P<0.05). The EMG-controlled FES system significantly reduced net cardiopulmonary metabolic energy cost, net energy expenditure measurement at stairs climbing (P<0.05). Conclusions: This study demonstrated that EMG-controlled FES is a potentially useful gait-assist system for improving gait function by making joint range of motion and moment properly.
