Development of the Selected Multi-model Consensus Technique for the Tropical Cyclone Track Forecast in the Western North Pacific (태풍 진로예측을 위한 다중모델 선택 컨센서스 기법 개발)

  • A Selected Multi-model CONsensus (SMCON) technique was developed and verified for the tropical cyclone track forecast in the western North Pacific. The SMCON forecasts were produced by averaging numerical model forecasts showing low 70% latest 6 h prediction errors among 21 models. In the homogeneous comparison for 54 tropical cyclones in 2013 and 2014, the SMCON improvement rate was higher than the other forecasts such as the Non-Selected Multi-model CONsensus (NSMCON) and other numerical models (i.e., GDAPS, GEPS, GFS, HWRF, ECMWF, ECMWF_H, ECMWF_EPS, JGSM, TEPS). However, the SMCON showed lower or similar improvement rate than a few forecasts including ECMWF_EPS forecasts at 96 h in 2013 and at 72 h in 2014 and the TEPS forecast at 120 h in 2013. Mean track errors of the SMCON for two year were smaller than the NSMCON and these differences were 0.4, 1.2, 5.9, 12.9, 8.2 km at 24-, 48-, 72-, 96-, 120-h respectively. The SMCON error distributions showed smaller central tendency than the NSMCON's except 72-, 96-h forecasts in 2013. Similarly, the density for smaller track errors of the SMCON was higher than the NSMCON's except at 72-, 96-h forecast in 2013 in the kernel density estimation analysis. In addition, the NSMCON has lager range of errors above the third quantile and larger standard deviation than the SMCON's at 72-, 96-h forecasts in 2013. Also, the SMCON showed smaller bias than ECMWF_H for the cross track bias. Thus, we concluded that the SMCON could provide more reliable information on the tropical cyclone track forecast by reflecting the real-time performance of the numerical models.

  • Purpose: The purpose of this study was to develop the medical error reporting system and to validate an trait of error in the Operating Room. Methods: Descriptive research design was used. The subjects were 30 nurses with below 5-year-career in a University Hospital. Data was collected from 11, April until 22, April, 2005 using web-based error reporting system. Data was analyzed by mean, standard deviation, $X^{2}-test$ using SPSS WIN 10.0 program. Results: A time of medical error in operating room nursing frequent occurrence was from 12 pm. to 4pm. 'Lack of sterile materials' management' was the best frequent occurrence of medical error in operating room nursing. Conclusion: The findings of this study show that manager of healthcare organization must develop the error reporting system more familiar and ordinary. Afterward, we prevent the repetitive medical errors in nursing care through analyzing of error reporting system.

Human Resource Investment in Internal Control and Valuation Errors

  • The purpose of an internal control system is to prevent the occurrence of errors and fraud in the process of producing accounting information, thereby providing investors with reliable information. For the effective operation of an internal control system, it is necessary to secure a sufficient number of personnel and experienced staff. This study focuses on the personnel directly involved in producing accounting information, examining whether companies that invest in their internal control staff experience a mitigation in the phenomenon of valuation errors. The analysis revealed that the size and experience months of the personnel responsible for internal control have a significant negative relationship with valuation errors. This result implies that by securing sufficient personnel for the smooth operation of the internal control system and placing experienced staff within the system, investors can effectively make judgments about the intrinsic value based on quality accounting information, thereby reducing valuation errors.

Experience and Perception on Patient Safety Culture of Employees in Hospitals (환자안전 문화에 대한 의료 종사자의 인식과 경험)

  • Purpose: The objectives of this study were to understand and compare perception and experience between clinical staffs(nurses and pharmacists) and Quality Improvement managers. Method: A qualitative study was conducted with 14 clinical staffs and QI managers who are working at tertiary hospitals in Korea. Interviews were recorded and transcribed for systematic analyses of qualitative data. Results: Most critically, while QI managers acknowledged that establishment of the patient safety culture and reduction of medical errors are urgent tasks for QI effort, clinical staffs don't seem to share such perceptions. All participants agree that staff shortage and no compliance to safety procedures were major reasons for medical error occurrences. Many suggested that an organizational culture where errors were perceived as a systematic problems rather than individual failures or carelessness should be formed to promote voluntary reporting of medical errors. Conclusion: A more systematic effort and attention at the hospital leadership and public policy level should be promoted to constitute societal consensus on the urgence of promoting patient safety culture and more specific approaches to tackle the patient safety problems.

Perception and Experience of Medication Errors in Nurses with tess than One Year Job Experience (신규 간호사의 투약오류 인지 및 경험에 대한 조사 연구)

  • Purpose: This study was carried out to investigate perception and experience of medication errors by nurses. Method: Data collection through a survey was performed using structured questionnaires over the period of September 1 to October 15, 2004. Questionnaire were delivered to 222 nurses from 15 hospitals; thereafter, 205 questionnaires were responded (i.e., 92% response rate). The subject in the study was a nurse who had been working in the hospital for less than one year. Results: The average perception rate was 87.5%. The perception rates of subjects in medication errors from four areas are 62% in wrong dosage form for drug administration, 61.5% in air into an IV set, 63% in crystals in an IV lines, and 83.5% in wrong time. The experience rates of subjects in medication errors from four areas are 85.5% in wrong time, 39.5% in wrong injection site, 34.5% in omission error, and 28% in wrong patient. Conclusion: The average perception rate and experience rates of medication errors were 87.5% and 23.5%, respectively. Education about the Five right in medication and knowledges about drugs would improve the perception of medication errors of nurses whose work experience is less than one year, and prevent them from medication errors.

Verification and Comparison of Forecast Skill between Global Seasonal Forecasting System Version 5 and Unified Model during 2014 (2014년 계절예측시스템과 중기예측모델의 예측성능 비교 및 검증)

  • The comparison of prediction errors in geopotential height, temperature, and precipitation forecasts is made quantitatively to evaluate medium-range forecast skills between Global Seasonal Forecasting System version 5 (GloSea5) and Unified Model (UM) in operation by Korea Meteorological Administration during 2014. In addition, the performances in prediction of sea surface temperature anomaly in NINO3.4 region, Madden and Julian Oscillation (MJO) index, and tropical storms in western north Pacific are evaluated. The result of evaluations appears that the forecast skill of UM with lower values of root-mean square error is generally superior to GloSea5 during forecast periods (0 to 12 days). The forecast error tends to increase rapidly in GloSea5 during the first half of the forecast period, and then it shows down so that the skill difference between UM and GloSea5 becomes negligible as the forecast time increases. Precipitation forecast of GloSea5 is not as bad as expected and the skill is comparable to that of UM during 10-day forecasts. Especially, in predictions of sea surface temperature in NINO3.4 region, MJO index, and tropical storms in western Pacific, GloSea5 shows similar or better performance than UM. Throughout comparison of forecast skills for main meteorological elements and weather extremes during medium-range, the effects of initial and model errors in atmosphere-ocean coupled model are verified and it is suggested that GloSea5 is useful system for not only seasonal forecasts but also short- and medium-range forecasts.

Effect of Red Ginseng Triol Saponin Fractions on the Spatial 1 Memory Function Studied with 12-Arm Radial Maze (12-Arm Radial Maze를 이용한 쥐의 공간 인지능력에 미치는 홍삼 PT계 사포닌의 효과)

  • Male rats aged 15 months, which had been over-trained with 12 arm radial maze up to the criterion of 1 or 2 errors an average per trial, were divided into two groups and 7 months after the over-training and were studied the effect of red ginseng trial saponin fraction (PT) on the spatial memory function. The rats which could be improved the performance of 9.75$\pm$1.26 (within 185$\pm$8.2 1 sec) were classified into "normal group" (n=4) and the ones, which showed R-maze performance of 7.0$\pm$2.87 within 300 sec, "deteriorated memory group" (n=5). PT was dissolved in distilled water and injected into the deteriorated memory group intraperitoneally at 30 min before R-maze for 3 consecutive days. The injected amount of PT on the 1st day was 10mg/rat, 1mg/rat both on the 2nd the 3rd day. As results, the performance was restored to 9.4$\pm$2.0 after stopping ginseng administration, although the lower performance values (1.4$\pm$0.89, 2.8$\pm$0.83, 3.8$\pm$0.84, respectively) were exerted during the three days of PT administration than before PT administration (7.0$\pm$2.87) since 1 day after administratitan of PT. The restored performance values were continuously maintained up to the level of the "normal group". However, any lower performance values were not observed when PT was administered via intraperitoneal route to the normal group in about of 2 mg/rat except 10mg1ra1. The number of errors when the foods in arms were put at only 6 places was clearly reduced during the 9 days of consecutive administration of PT as compared with untreated control. These results indicated that PT administration may enhance their cognitive function after a long lapse of time not only in the memory deteriorated rats but also in normal ones. The reason exerted lower performance values during PT administration in this paper was discussed as compared with the results of T-maze behavior of another 3-month-aged rats. of another 3-month-aged rats.

Validation of Significant Wave Height from Satellite Altimeter in the Seas around Korea and Error Characteristics

  • Significant Wave Height (SWH) data measured by satellite altimeters (Topex/Poseidon, Jason-1, Envisat, and Jason-2) were validated in the seas around Korea by comparison with wave height measurements from marine meteorological buoy stations of Korea Meteorological Administration (KMA). A total of 1,070 collocation matchups between Ku-band satellite altimeter data and buoy data were obtained for the periods of the four satellites from 1992 to the present. In the case of C-band and S-band observations, 1,086 matchups were obtained and used to assess the accuracy of satellite SWH. Root-Mean-Square (RMS) errors of satellite SWH measured with Ku-band were evaluated to roughly 0.2_2.1 m. Comparisons of the RMS errors and bias errors between different frequency bands revealed that SWH observed with Ku-band was much more accurate than other frequencies, such as C-band or S-band. The differences between satellite SWH and buoy wave height, satellite minus buoy, revealed some dependence on the magnitude of the wave height. Satellite SWH tended to be overestimated at a range of low wave height of less than 1 m, and underestimated for high wave height of greater than 2 m. Such regional characteristics imply that satellite SWH should be carefully used when employed for diverse purposes such as validating wave model results or data assimilation procedures. Thus, this study confirmed that satellite SWH products should be continuously validated for regional applications.

Cognitive Analysis on Accident-related Human Factors during Shunting Movements (철도 입환작업 중의 인적 사고요인에 대한 인지과학적 분석)

  • Railroad shunting movements connecting and disconnecting train sets are very susceptible to human errors since they depend on human decision-making and action procedure that are variable to situation to situation. Nevertheless, in the investigation of railroad accidents, all the accident causes related with human factors have merely been categorized as 'careless treatment' of the workers without any systematic approach of behavioral sciences or the analysis of human errors. In this research, therefore, 137 accident cases occurred during railroad shunting movements and 435 accident cases occurred during driving were analyzed with a special interest of human errors. According to results, the traditional accident investigation scheme used for last several decades did not seem to be appropriate for catching up true accident causes with respect to human errors. In addition, both signal men and locomotive drivers made many mistakes in judgement/action stage while the former mainly commit judgement tasks where as the latter mainly commit cognition tasks. Ant those tasks such as 'confirmation of signal and route', 'location check-up of connected train sets', and 'route identification for a shift of track' ranked highly for accident susceptibility.
