A Study for International Standards of Midwife Education and Improvement of the Level of the National Examination (조산교육의 국제 표준화 및 국가시험의 수준향상을 위한 연구)

  • The primary purpose of this study was to suggest midwife education programs which could be recognized and exchanged internationally by examining and analyzing both domestic and foreign midwife education programs. The secondary purpose of this study was to offer a plan to raise the level of national examination. Specific aims of this study were as follows: 1) to identify the international standard of the education and practices of midwives 2) to analyze both domestic and foreign midwife education programs 3) to offer a new curriculum for educating midwives 4) to suggest a prerequisite to raise the standards of the national examination 5) to suggest subjects for the national examination The results of this research were as follows: 1. The concept of midwife and midwifery practices recognized internationally by WHO and ICM(International Confederation of Midwives) was identified. In addition, Core Competencies for Basic Midwifery Practice suggested by ACNM(American College of Nurse-Midwives) of the U.S.A. were examined. 2. Midwife education programs of the U.S.A., Sweden, Australia, and Japan were investigated and analyzed. In addition, the midwife education program stated in the public health related law of this country as well as curriculums of institutions for midwife education were also investigated and analyzed. 3. As for the midwife education system, both a graduate program for midwife education in the college of nursing sciences and a postgraduate professional midwife education program centered medical institutions were suggested. 4. A new curriculum that could promote more international exchanges and extend the role of midwives was suggested after studying both domestic and foreign midwife education programs. 5. A prerequisite to raise the level of national examination for midwives was suggested. In addition, subjects for the examination which could evaluate the applicant's comprehensive thinking ability were presented with its respective range and ratio. A midwife is a medical professional who has a nursing license and is licensed nationally as a midwife with an additional year of education. An effort to extend a midwife' role and to improve its service is imperative. The laws related midwives should be revised in regard to education, service, and the national examination to the level of developed countries so that international recognition can take place. In addition, midwife curriculum and its service should be evaluated periodically. A system must be established to renew midwife licences.

A USEFULNESS OF KEDI-INDIVIDUAL BASIC LEARNING SKILLS TEST AS A DIAGNOSTIC TOOL OF LEARNING DISORDERS (학습 장애아 진단 도구로 기초 학습 기능 검사의 유용성에 관한 연구)

  • The purpose of this study was to examine usefulness of KEDI-Individual Basic Learning Skills Test as a diagnostic tool of learning disorders(LD). Learning disorder group consisted of two subgroups, verbal learning disorder group(VLD, n=34) and nonverbal learning disorder group(NVLD, n=14). Comparison group consisted of Dysthymia Disorder subgroup(n=11) and Normal subgroup(n=20). Performance of intelligence test and achievement test was examined in all 4 subgroups. In KEDI-WISC, VLD subgroup revealed primary problems in vocabulary, information and verbal-auditory attention test. NVLD group revealed primary problems in almost all performance tests such as visual acuity, psycho-motor coordination speed and visual-spatial organizations ability subtest. In KEDI-Individual Basic Learning Test, VLD group revealed primary problems in phonological coding process, word recognition and mathematics. For successful classification of LD children, the importance of achievement test and intelligence test was discussed by discriminant analysis and factor analysis. The results indicate that KEDI-Individual Basic Learning Skills is of considerable usefulness in diagnosing LD, but must be used in subtests, and additional tests must be conducted for thorough exploration of LD.

  • Growth charts are curves or tables that facilitate the visualization of anthropometric parameters, and are widely used as an important indicator when evaluating the growth status of children and adolescents. The latest version of the Korean National Growth Charts released in 2007 has raised concerns regarding the inclusion of data from both breastfed and formula-fed infants, higher body mass index (BMI) values in boys, and smaller 3rd percentile values in height-for-age charts. Thus, new growth charts have been developed to improve the previous version. The 2006 World Health Organization Child Growth Standards, regarded as the standard for breastfed infants and children, were introduced for children aged 0-35 months. For children and adolescents aged 3-18 years, these new growth charts include height-for-age, weight-for-age, BMI-for-age, weight-for-height, and head circumference-for-age charts, and were developed using data obtained in 1997 and 2005. Data sets and exclusion criteria were applied differently for the development of the different growth charts. BMI-for-age charts were adjusted to decrease the 95th percentile values of BMI. Criteria for obesity were simplified and defined as a BMI of ${\geq}95th$ percentile for age and sex. The 3rd percentile values for height-for-age charts were also increased. Additional percentile lines (1st and 99th) and growth charts with standard deviation lines were introduced. 2017 Korean National Growth Charts are recommended for the evaluation of body size and growth of Korean children and adolescents for use in clinics and the public health sector in Korea.

A Study on the Relationship of Gender Equity within Family and Second Birth (둘째 출산 계획의 결정요인과 가족내 성 형평성)

  • This research explores the relationship between gender equity within family and second birth in Korea. For a factor analysis on the intention of having additional children for women with one child, I have used the data from the 2005 National Survey on Marriage and Fertility Trend" of the Korea Institute for Health and Social Affairs. Among 516 samples(year of 20-44), women planning to have second birth are 54.7%, and others have answered that they have no intention to have second birth. As a result, main causes affecting progressing to second birth seemed to be family planning on family cycle, such as controlling age gaps between first and second child, composing children genders as they wish etc. The variable of "gender equity between couples" in progressing to second birth are of significant only with married working women group. In case of married working women, more the husband's houseworking hours increase, the more prone to progress to second birth. However, in case of housewives, gender equity level, such as husband's housework hours or share of housework are not of significant influence in progressing to second birth. Not only working mothers but also housewives need the realization of gender equity within the family and the expansion of social support system for work-family life balance.

Convulsions with Noroviral Gastroenteritis in Children at a Single Center in Korea (단일 센터에서 경험한 소아에서의 노로바이러스 위장관염과 연관된 양성 경련)

  • Purpose: Viral gastroenteritis is a common disease in infants and children. Seizures can be associated with viral gastroenteritis as benign convulsions with mild gastroenteritis (CwG). After the development of the rotavirus vaccination, norovirus has become inreasingly significant in children. We retrospectively analyzed the clinical features in a pediatric population presenting with seizures and confirmed enteral viral infections, especially norovirus infections. Methods: We retrospectively reviewed the medical records of pediatric patients aged <15 years admitted due to convulsions and gastroenteritis to the Department of Pediatrics of Myongji Hospital between July 2014 and June 2016. Results: A total of 46 patients (24 male and 22 female) were included. Norovirus was detected in 21 (45.7%) patients, adenovirus in three (6.5%), rotavirus in two (4.3%), astrovirus in one (2.2%), and none of agents were detected in 19 (41.3%) patients. Patients in the norovirus gastroenteritis (NGE) group had a higher incidence of diarrhea than that among in the non-norovirus gastroenteritis (NNGE) group (61.9% vs 28.0%; P<0.05). Twelve patients experienced status epilepticus, including five (23.8%) in the NGE group and seven (33.0%) in the NNGE group (P=0.837). Seizures were effectively terminated by intravenous benzodiazepines in 8 (66.7%) of 12 patients. Additional long-acting antiepileptic drugs such as fosphenytoin or levetiracetam were required in three (25%) of 12 patients. Conclusion: Patients with CwG with and without noroviurs infection did not differ in terms of clinical features. However, status epilepticus was not uncommon among patients with CwG by definition.

A Survey on the Parents' Perceptions of and Attitudes toward Game Use among Teenagers in Korea (청소년 게임이용에 대한 학부모의 인식 조사 연구)

  • The present study explored Korean parents' perceptions of and behavior toward game use among teenagers in Korea. A total of 600 Korean mothers of teenagers residing in Seoul and five other metropolitan areas participated in the survey. The survey was constructed based on five categories of variables, including the overall perception of games and game use, specific attitudes toward game use, cognitions about and attitudes toward game addiction, factors predicting parental monitoring of children's game use, and views and opinions about what needs to be done to promote healthy game cultures as well as to prevent problematic game use among teenagers in Korea. Results indicate that the respondents' overall perceptions of and attitudes toward games and game use are negative. In contrast, attitudes toward game use of the respondent's own child are contingent upon various comparison standards. Results also indicate that the respondents tend to overestimate the possibility that their own child is addicted to games, and their perceptions of game addiction are based on a narrow range of behavioral symptoms. Additional analyses indicate that parental monitoring of teenagers' game use can be predicted by the theoretical model driven from Ajzen(1991)'s theory of planned behavior. Finally, results also indicate that, in order to deal with the problems associated with teenagers' game use, proactive approaches to promote healthy game cultures as well as various initiatives to prevent problematic game use are necessary. Implications of the findings and future direction were discussed.

  • Countries around the world are calling for stronger law enforcement to combat the production and distribution of child sexual exploitation images, such as child grooming. Given the scale and importance of this social problem, it requires extensive cooperation between law enforcement, government, industry, and government organizations. In the wake of the Nth Room Case, there have been some amendments to the Enforcement Decree of the Telecommunications Business Act regarding additional telecommunications services provided by precautionary operators in Korea. While Naver and others in Korea use Electronics and Telecommunications Research Institute's own technology to filter illegal images, Microsoft uses its own PhotoDNA technology. Microsoft's PhotoDNA is so good at comparing and identifying illegal images that major global operators such as Twitter are using it to detect and filter images. In order to meet the Korean government's testing standards, Microsoft has conducted more than 16 performance tests on "PhotoDNA for Video 2.0A," which is being applied to the Bing service, in cooperation with the Korea Communications Commission and Telecommunications Technology Association. In this paper, we analyze the cases that did not pass the standards and derive improvement measures related to adding logos. In addition, we propose to use three video datasets for the performance test of filtering against illegal videos.

Investigating Occupational Therapy Students' Awareness of Developmental Rehabilitation Services and the Accreditation of Sensory Developmental Rehabilitation Service Providers (작업치료 전공 학생들의 발달재활서비스 및 감각발달재활서비스 제공인력 자격인정에 대한 인식도 조사)

  • Objective : This study sought to explore occupational therapy students' awareness of Developmental Rehabilitation Services (DRSs) and the certification of Sensory Developmental Rehabilitation Service Providers (SDRSPs). Moreover, it also aimed to examine the differences in students' awareness based on general characteristics. The findings were intended to provide a foundation for research on sensory developmental rehabilitation. Methods : An online survey was conducted among 199 occupational therapy students nationwide from October 4 to November 5, 2022. Frequency analysis was used to assess the general characteristics and additional items. Both descriptive statistics and frequency analysis were employed to assess the students' awareness of DRSs and SDRSP certification. Any differences in the students' awareness based on their general characteristics were examined using t-tests and an analysis of variance. Results : The students' awareness of DRSs was moderate (4.50 ± 2.40), while their awareness of SDRSP certification was also moderate (4.22 ± 2.55). The awareness of DRSs varied significantly based on the students' academic year and child-related practice experience. Similarly, the awareness of SDRSP certification differed significantly based on the students' academic program, academic year, recognition by the related department, and child-related practice experience. Conclusion : The occupational therapy students expressed a strong intention to provide DRSs and become certified as SDRSPs. However, their awareness levels were moderate. These findings emphasize the importance of educational efforts and the provision of relevant information about DRSs and SDRSP certification in universities to foster the development of competent SDRSPs.

Single-stage Laryngotracheal Reconstruction in the Children with Laryngotracheal Stenosis (소아 후두기관협착 환자에서 single-stage 방법의 후두기관재건수술)

  • Background and Objectives: Laryngotracheal stenosis and its reconstruction in children is a highly challenging field to airway surgeons, and the way of stenting after a reconstructive surgery is still controversial. The aims of this study were to analyze the single institutional experiences of laryngotracheal reconstruction (LTR) in the pediatric patients with laryngotracheal stenosis and to compare the outcomes of single-stage LTR (SSLTR) with conventional two-stage LTR (TSLTR) in these patients. Materials and Methods: Medical records of 14 children (mean age 4.1 years) were reviewed, who received 20 LTR including 6 revisions for their moderate to severe subglottic stenosis and/or combined posterior glottic stenosis. Of these 20 LTR, tracheostoma was temporarily maintained after LTR in 12 cases (TSLTR) or not in the other 8 cases (SSLTR). Results: Overall decannulation rate of LTR that were performed before and after the year of 2003 was 40% (4/10) and 70% (7/10) respectively. Decannulation rate was 42% (5/12) in TSLTR group and 75% (6/8) in SSLTR group (P = 0.197). Mean interval to decannulation after LTR was 9.8 months and 7.2 days in TSLTR and SSLTR groups respectively (P = 0.004). A number of additional touch-up procedures that were required after LTR was 4 in TSLTR and 2.7 in SSLTR group (P = 0.238). Major complication rate was similar in both groups (33% in TSLTR and 38% in SSLTR, P = 0.910). Conclusion: A laryngotracheal reconstruction in children is a technically demanding procedure and its outcome is largely dependent on the surgeon's experience. Albeit there was a tendency that SSLTR ofters a higher decannulation rate, less additional touch-up procedures and similar complication rates, a shorter interval to decannulation after LTR was the only advantage that was confirmed as statistically significant in this study.

The survey of use of after-school courses and specialization programs at Kindergarten (유치원 방과후과정과 특성화활동 이용 현황)

  • The purpose of this study was to investigate how many kindergarten parents use after-school programs and specialization activities and to suggest effective policy improvement plans based on the results. After 693 parents were interviewed by the investigators, the collected data were presented by calculating the percentages, averages, and standard deviations. First of all, the results of after-school are presented. First, about 38.2% of the surveyed parents spent additional expenses to use after - school program, and the average of additional costs was about 62,850 won. When the cost burden and satisfaction were evaluated as 5 points, it was rated as normal. Second, the main reason for participating in the after-school program was both parents were working, followed by the parents' wishes for their children to spend time with their friends. Next, the results for specialization activities at kindergartens are presented. First, the proportion of children using after-school specialization activities was 68.2%, of which 79% were expenditures, the number of activities was 2.69, and the average cost was 83,540 won. Second, the most preferred after-school specialization activity by age group was English, and the most common reason for this was that the children liked to participate. Improvement of the educational contents suggested as the most important thing to improve after-school specialization activities. Lastly, the results showed that after-school specialization activities reduced parents' spending on private education. On the basis of the results, some discussions are presented included the development after-school programs focusing on the different needs of children and parents accordingly.
