Influence of Chlorpromazine(CPZ) and Iproniazid on Acute Carbon Tetrachloride(CCl4) Poisoning (사염화탄소(四鹽化炭素)의 독성(毒性)에 미치는 Chlorpromazine 및 Iproniazid의 영향(影響))

  • In the sequence of carbon tetrachloride hepatotoxicity, increased serum levels of a number of enzymes have been demonstrated in experimental animals. These observations, therefore, have served to help in detecting hepatic injury. The serological influence of chlorpromazine (CPZ) and iproniazid on the acute $CCl_4$ poisoning was executed in this investigation taking use of 6 albino rabbits (around 2 kg b.w.) in each group. By measuring of blood sugar level (Nelson-Somogyi method), S-GOT and S-GPT activities (Reitman-Frankel method), the pharmacological effects of the drugs was evaluated setting pretreated groups against the control. The results obtained were summarized as follows: 1. The intramuscular injection of $CCl_4$ led to increase the blood sugar level in first 3 hours and, after that, to decrease reasonably. But CPZ-pretreated group showed a tendency of increasing in compare with the control, and iproniazid-pretreated group inhibited evidently. 2. In S-GOT activity, the increased level was induced by $CCl_4$ in control. And CPZ-pretreated group showed a increased level until first day and decreased rapidly. But this property inhibited inhibited significantly by pretreating with iproniazid. 3. Although a single dose of $CCl_4$ increased the S-GPT activity, the more increasing trend was observed in CPZ-pretreated group. But these tendencies depressed remarkably in the iproniazid-pretreated group. It seemed to be attributed not to defend the $CCl_4$ toxicity but to be suppressed the enzyme systems in the liver by iproniazid that the blood sugar level and serum transaminase activities was decreased significantly in pretreating with iproniazid.

Comparison of Prescription Patterns and Clinical Features according to Clinical Departments in Sedative-hypnotic Intoxication (진정수면제 중독 환자의 처방과에 따른 처방 및 임상양상 비교)

  • Purpose: The purpose of this study was to compare prescription patterns and clinical features according to clinical departments in sedative-hypnotic intoxication. Methods: This was a retrospective study of histories, substances of poisoning, acquisition routes, clinical courses, and outcomes of patients treated for acute intoxication in a single emergency medical center from January, 2011 to December, 2013. Results: A total of 769 patients were treated for acute intoxication, 281 patients ingested sedative hypnotics during the study period. Among 281 patients, 155 patients were prescribed by psychiatric department and 80 patients were prescribed by non-psychiatric department. Benzodiazepines were more likely to be prescribed by psychiatrists, and zolpidem was preferred by non-psychiatrists (p<0.001). Non-psychiatrists were more likely to prescribe short acting benzodiazepines than psychiatrists (p<0.001). However, there was no statistically significant difference in the clinical outcomes, including prevalence of admission to ICU, ventilator care, and length of stay in ICU. In patients prescribed by non-psychiatrists, there were more patients prescribed without psychiatric diagnosis and diagnosed as major depression disorder after hospitalization. Conclusion: To promote rational prescribing of sedative hypnotics, proper psychiatric evaluation should be performed before prescribing, and educational programs including the contents of interactions and side effects of sedative hypnotics are needed.

Detection of Cyanide and Arsenic from Oral Tissues of Acute Poisoned Rabbits (급성중독가토의 치아 및 악골에서의 청산 및 비소검출에 관한 실험적 연구)

  • The purpose of this study was detection of the poison from the acute cyanide and aresenic poisoned rabbits, The author administered KCN and $AS_2O_3$ to rabbits and caused acute poisoning, then analysed the teeth, dental pulp and jaws of the rabbits chemicotoxicologically and observed the specimen histopathologically. 1. In subcutaneausly injected group of KCN, a large amount of cyanide was detected in blood and lung and a small amount of cyanide was detected in teeth and dental pulp, but was not detected in jaws. 2. In orally administered group of KCN a large amount of cyanide was detected in blood, lung and dental pulp and a small amount of cyanide was detected in teeth and jaws. 3. In orally administered group of $AS_2O_3$, arsenic was detected markedly in teeth and jaws, but was detected a little in dental pulp. 4. In orally administered and heat-treated group of KCN, the author could detected cyanide in teeth, dental pulp and jaws. 5. In suvcutanelusly injected group of KCN, orally administered group of KCN and orally dministered group of $AS_2O_3$, histopathologic findings showed the congestion and hemorrhage in dental pulp. 6. In orally administerd group of $AS_2O_3$, the congestion and hemorrhage in buccal mucosa were found and the basal cell degeneration and fibrosis were found in palatal mucosa.

Ultrastructural Alterations of Rabbit Liver after Overdose of Nitrate (질산염과잉투여(窒酸鹽過剩投與)에 의(依)한 간장(肝臟)의 변화(變化)에 관한 전자현미경적연구(電子顯微鏡的硏究))

  • In order to know the morphological changes of liver in nitrate poisoning, the ultrastructural studies were carried out on the rabbit liver after potassium nitrate was administered orally at lethal dose, in single treatment, as acute case and at two different levels. 1.0 and 0.5g/kg of body weight daily for 43 and 60 days as chronic case, respectively, The results were summarized as followings: 1. In the hepatic cells of acute case, mitochondria were swollen, disappearance of cristae and variable in shape. Dilatation of rough endoplasmic reticulum and vacuoles containing degenerated cell organells were observed. Glyogen particles were decreased in number. Degenerated Kupffer cells were often seen in acute case. 2. In the hepatic cells of chronic case, there were increase of smooth endoplasmic reticulum, marked enlargement of rough endcplasmic reticulum, detachment of membrane bound ribosome and some rough endoplasmic reticulum changed into smooth endoplasmic reticulum. Secondary lysosome, abundant glycogen paricles and myelin-figure structures were also observed in the cytoplasm of the hepatic cells. The endothelial cells were proliferated in the area of the necrotic cells.

Comparison of Acute Symptoms Between a Alpine Agricultural Workers and General Workers in Gangwon-do (강원도 일부지역 고랭지 농업 농민과 일반 농업 농민의 농약중독 증상 비교)

  • Objectives: This study was performed to evaluate the pesticide exposure status and acute pesticide poisoning symtoms among agricultural workers at Gangwon-do province alpine area. Alpine area was defined as the area higher than 400m. Methods: We analyzed 257 interviwed questionnaire about pesticide exposure, acute pesticide poisoning symptoms and other variables. Results: The result was shown that agricultural workers at alpine area used more pesticide than general agricultural workers for annual usage days(35.9 days vs 14.4 days), daily usage hours(6.7 hrs vs 2.8hrs, p<0.05). But there was no difference between general and alpine agricultural worker's herbicide exposure. Moreover, the alpine agricultural worker's acute pesticide poisoning symtom score was higher than general agricultural worker's. These difference was also found at result of regression analysis, under control the age, sex, monthly income. Most frequently suffered symptom was headache and dermatological problems. The symptom prevalence of dermatological problem, headache, general weakness, eye irritation, nausea were higher among alpine agricultural workers than general agricultural workers. Conclusions: As a result, agricultural workers at alpine area were more exposed to pesticide and suffered from pesticide poisoning symptoms. To prevent the symptoms and disease from pesticide exposure among agricultural workers at alpine area, more research and political effort will be needed.

Respiratory Failure following Tetramine poisoning after Ingestion of Sea Snail: A Case Report (소라 섭취 후 테트라민 중독에 의한 호흡부전 1례)

  • Tetramine toxicity due to sea snail ingestion is generally mild and has a good prognosis. Tetramine toxicity acts on the acetylcholine receptor, affecting the neuromuscular junction and autonomic nervous system. A 78-year-old female patient visited the emergency room with vomiting and dyspnea after eating sea snails. At the time of admission, the vital signs recorded were 140/80 mmHg-105/min-24/min-36.5℃, and 90% oxygen saturation. Arterial blood test revealed hypercapnia (pCO2 58.2 mmHg) and respiratory acidosis (pH 7.213, HCO3- 22.5 mmol/L), whereas other blood tests showed no specific findings. Due to decreased consciousness and hypoxia, endotracheal intubation and mechanical ventilation were administered to the patient. Successful weaning was accomplished after 12 hrs, and the patient was discharged without any further complications. Although tetramine toxicity rarely results in acute respiratory failure due to paralysis of the respiratory muscle, caution is required whilst treating the patient.

PREVENTIC Poisoning Occurred in three German Shepherd Dogs (German Shepherd의 프리벤틱 중독 3례)

  • Three German Shepherd dogs (5 years old, 30 kg, male; 4 years old, 23 kg, female; 4 years old, 24 kg, female) with history of acute vomiting, hemorrhagic diarrhea, anorexia and dullness were admitted to veterinary clinic in the military working dogs training corp (MWDTC). All of them showed same symptoms after ingestion the $'PREVNTIC^{(R)}'$ collar to control ecto-parasites infection in field. One dog was operated by gastrotomy and the $5{\sim}6$ pieces of the $'PREVNTIC^{(R)}'$ collar were removed. The dog was completely cured by intensive care within 2 hours after $'PREVNTIC^{(R)}'$ collar ingestion. However, two dogs were operated by gastrotomy and intensive care over 24 hours after $'PREVNTIC^{(R)}'$ collar ingestion but died. In autopsy, the $5{\sim}6$ pieces of hard materials were found in the stomach, and severe thickness, congestion hemorrhage and swelling of the stomach walls and mucous membrane were observed. In these cases, it was suggested that the first aid of treatment such as gastrotomy within two hours after $'PREVNTIC^{(R)}'$ collar ingestion as soon as possible was helpful for attenuating of toxicosis.

Radiographic Diagnosis of 'Rubber Jaw Syndrome' Secondary to Chronic Renal Failure Due to Ethylene Glycol Intoxication in a Dog (개에서 Ethylene glycol 중독에 의한 만성신부전증의 속발성 'Rubber jaw syndrome'의 방사선학적 진단례)

  • A 9-month-old, intact female mixed dog was referred to Veterinary Medical Teaching Hospital of Gyeongsang National University with symmetrically enlarged and protruded upper jaw. The patient was diagnosed as acute renal failure due to ethylene glycol poisoning and was treated for 1 month in a local animal hospital. In spite of treatment, the patient proceeded to chronic renal failure. Also, the patient's upper jaw begun to enlarge continuously. To evaluate this upper jaw, radiographic examination was performed. Skull radiographs revealed thickening of maxilla, decreased bone opacity, cortical thinning, loss of lamina dura and periodontal space in the maxilla. Diagnosis of rubber jaw syndrome is based on clinicial signs, radiographic findings and laboratory evidence of chronic renal failure due to ethylene glycol poisoning.

A Case of Green Tobacco Sickness (GTS) in Elderly Patient (담뱃잎농부병으로 진단된 고령 환자 1례)

  • Green tobacco sickness is an illness caused by dermal exposure to nicotine. The common symptoms of the disease include dizziness, headache, nausea, vomiting, severe general weakness, fluctuations of blood pressure or heartbeat, abdominal cramping, chills, increased sweating, salivation, and difficulty breathing. A 79-year-old female arrived at the emergency room for an evaluation of sudden onset dizziness. Magnetic resonance imaging and angiography of the brain did not show any relevant abnormal findings. Four days later, with supportive care, she said that she had harvested green tobacco for six hours on the day of admission and the tobacco harvest was the first time in her life. She sweated excessively during the hot and humid weather and the tobacco leaves were wet from rain the night before. The serum cotinine tested at five days of admission was 16ng/ml. She was diagnosed with acute nicotine poisoning by her clinical symptoms and the half-life of cotinine in the blood.

Acute methemoglobinemia after a normal diet (일상적인 음식 섭취 후 발생한 급성 메트헤모글로빈혈증)

  • Methemoglobin is generated by the oxidation of ferrous iron to ferric iron within a hemoglobin molecule. Methemoglobin is unable to bind and transport oxygen, resulting in methemoglobinemia, which can lead to fatal tissue hypoxia. The most common cause of methemoglobinemia is poisoning by oxidizing agents such as dapsone, benzocaine, and primaquine. However, methemoglobinemia can also be caused by normal dietary sources. We present two cases of methemoglobinemia that developed after a normal diet in two male patients. In this case report, the patients suddenly developed dyspnea and cyanosis after eating the same meal. They had no history of suspected poisoning, such as the use of drugs, exposure to chemicals, or gas inhalation. Their symptoms did not improve even after a high dose of oxygen was administered; further, an abnormal 'oxygen saturation gap' was observed. Because of CO-oximetry, the methemoglobin levels of the patients were 50.0% and 46.6%, respectively. We administered methylene blue (1 mg/kg), and the patients recovered completely without any complications. Emergency physicians should, therefore, be aware that methemoglobinemia can also be caused by normal dietary sources. In addition, if the source and route of contamination are unclear, an epidemiological investigation should be conducted.
