Recent Trends in Treatment for Acute Alcohol Intoxication in Traditional Chinese Medicine (급성 알코올중독증의 치료에 대한 중의학의 최근 동향)

  • Objectives : This study was performed to review the research trends in treatment for acute alcohol intoxication in traditional Chinese medicine. Methods : 19 studies were selected by searching CNKI(China National Knowledge Infrastructure) from 2006 to 2011, and we performed a systemic review of them. Results : All studies are controlled clinical trials using pharmacopuncture therapy. Each experimental group was given various kinds of pharmacopuncture therapy, whereas each control group was given another western medicine or treatment. Emergent treatment was used in both groups. The results of 15 studies showed that pharmacopuncture therapy has an effect on the treatment of acute alcohol intoxication. However the quality of these clinical trials wasn't high. Conclusions : It seems that the researches for acute alcohol intoxication have been performed actively in traditional Chinese medicine. We hope that our study can activate clinical research for this disorder in Korean traditional medicine.

Effect of Puffer Fish Extract on the Hepatic Alcohol Metabolizing Enzyme System in Alcohol-Treated Rat (Alcohol 섭취 쥐에서 Alchol 대사효소계에 미치는 복어추출물의 효과)

  • The present study was undertaken to clarify the effect of Puffer fish skin extract (PF) on the hepatic alcohol metabolism in rats. It was observed that alcohol concentration in blood had been markedly decreased by the pretreatment of PF for two weeks. Activities of alcohol dehydrogenase (ADH) and microsomal ethanol-oxidizing system (MEOS) were significantly incrased (more than 20% of control) by pretreatment of PF for two weeks and acute alcohol intoxication (5 g/kg) on final day. When rats were fed with subacute toxic state by alcohol (25v/v % , once a day for six weeks), activities of ADH and MEOS were significantly increased by additional treatments of PF for final two weeks. But the catalase activity was not affected by any of both case. And also activities of ADH and MEOS in vitro were not changed . These results suggest that PF treatemnt prompted the recovery from alcohol intoxication.

Effect of Acute Ethanol Intoxication on Hepatic Rhodanese Activity in Rats with Extrahepatic Cholestasis

  • Liver and serum rhodanese activities were determined in acute ethanol intoxicated rats with extrahepatic cholestasis induced by common bile duct ligation (CBD) to manifest the biochemical background of alcohol drinking hazard under the hepatobiliary disease. Liver cytosolic and microsomal rhodanese activities and these Vmax values in CBD ligated rats with acute ethanol intoxication were found to be decreased much more than that in CBD ligation alone. However, the difference of Km value on above hepatic enzyme was not found between the experimental groups. On the other hand, serum rhodanese activity in CBD ligated rats with acute ethanol intoxication was greater increased more than that in CBD ligation alone. These results indicate that the biosynthesis of the hepatic rhodanese decreases and the serum rhodanese activity increases in cholestasis combined with acute ethanol intoxication, reflecting damage of aggravated hapatocytic membrane. Accordingly, the resulting data supported the fact that alcoholic drinks were enzymologically harmful to the hepatobiliary disease.

  • The protective effect of EtOAc fraction of Limonium tetragonum extract (EALT) against alcohol-induced hepatotoxicity was assessed following acute ethanol intoxication in Spraque-Dawley rats. EALT (200 mg/kg p.o.) was administrated once before alcohol intake (8 g/kg, p.o.). Blood ethanol concentration, and the activities of alcohol metabolic enzymes, alcohol dehydrogenase (ADH) and aldehyde dehydrogenase (ALDH) in the liver were measured. Also, the formation of malondialdehyde (MDA) and the activities of antioxidant enzymes, superoxide dismutase (SOD), glutathione peroxidase (GSH-px), catalase were determined after acute alcohol exposure. Pretreatment of rats received ethanol with EALT significantly decreased blood ethanol concentration and elevated the activities of ADH and ALDH in liver. The increased MDA level was decreased, and the reduced activities of SOD, GSH-px and catalase were markedly preserved by the treatment with EALT. This study suggests that EALT prevent hepatic injury induced by acute alcohol which is likely related to its modulation on the alcohol metabolism and antioxidant enzymes activities.

Effect of Ginseng on the Alcohol Metabolism in Alcohol Treated Rat (알톨 대사에 미치는 인삼의 영향)

  • After pretreatment with ginseng followed by induction of acute intoxication of alcohol, the activities of alcohol dehydrogenase (ADH), microsomal ethanol-oxidizing system (MEOS) and aldehyde dehydrogenase(Ald DH) increased respectively compared to the groups treated with alcohol alone. In case that ginseng was given to rats fed with 5% alcohol instead of water for 60 days, the activities of ADH and MEOS increased compared to the groups treated. On the contrary, the activity of Ald DH in mitochondrial fraction decreased to an extent of about 35% in chronic alcoholism, but after pretreatment of ginseng the activity was restored to the control level. On the other hand, the catalase activity was not significantly affected by either treatment. Ginseng butanol fraction significantly increased the serum isocitrate dehydrogenase activity which is inhibited by alcohol-treated in rat. Alcohol-induced lactate dehydrogenase activity was decreased to control level in liver by ginseng treatment. And the serum level of lactic acid also decreased by ginseng treatment in alcohol-intoxicated rat. Ginseng butanol fraction markedly decreased the xanthine oxidase activity in the ethanol-treated rat liver. It was also observed that ginseng reduced the blood concentration of uric acid on experimentally reduced hyperuricemia by alcohol treatment. Uricase activity was not affected by either treatment. Ginseng butanol fraction decreased the hepatic aniline hydroxylase activity which was induced by alcohol-treated rat. These results suggest that the treatment with ginseng can be promoted the recovery from alcohol intoxication and some therapeutic effect on alcoholinduced metabolic disease.

Clinical Characteristics of Acute Drug Intoxication in Emergency Department

  • The number of acute drug intoxication who visiting to emergency department which is located near urban and rural area concomitantly is inclined to increase slightly, The purpose of this study was the investigation of methods and strategies of management of acute drug intoxication in emergency department Clinical trials were done on 92 cases of acute drug intoxication visiting to Emergency Department of Chungnam National University Hospital during 4 months from April to July 2003. 1. The total number of acute drug intoxication during this period was 92 patients, which presented 0.95% of all the emergency department visiting patients during the study. The intentional ingestions were observed in 84 cases (91.3%). 2. The maximal point of age distribution was 4th decade. The number of that was 21 cases (22.8%). 3. The number of patients who had ingested benzodiazepines and doxylamine succinates was 26 cases (28.2%), organophosphorus was 20 (21.7%), paraquat was 10 (10.8%), others were 36 (39.3%). 4. Five kinds of po1ydrug ingestion was observed in 3 cases, four kinds was observed in 3, three kinds was observed in 5 and co-ingestion of alcohol was observed in 28 cases (30.4%). 5. The mortality was occurred in 4 cases, 2 of them were caused by paraquat, 1 of them was caused by organophosphorus (OP), 1 of them was caused by acetic acid. 6. The gastric larvage as a mean of treatment modality was done in 57 cases (61.9%). The use of activated charcoal was done in 8 (8.69%). The maintenance of tracheal intubation was done in 6 (6.52%). 7. The mean observational period in emergency department was 8 hours 42 minutes in benzodiazepine and doxylamine succinate group, 21 hours 46 minutes in OP, 20 hours 39 minutes in other germicidal except OP, 23 hours 9 minutes in paraquat group. Without a drug information and intoxication center in Korea, We should minimize the exhaustion of medical resources by establishment of determinant criteria which can be seen in relatively less-complicated cases of acute drug intoxication. Thus, we should consider the introduction of simple toxicology treatment protocol and toxicologic observation unit in emergency department as possible means to reduce economical and social burdens.

The Effect of Puffer Fish Extract on the Acetaldehyde Metabolism in Rat (흰쥐에서 Acetaldehyde 대사에 미치는 복어추출물의 영향)

  • The present stduy was undertaken to investigate the possible effect of Puffer fish skin extract (Pf) on the heptic acetaldehyde metabolism . It was obsrved that PF markedly decreased the acetaldehyde levels in blood and liver. The activity of mitochondrial aldehyde dehydrogenase (Ald DH) increased by induction of acute intoxicatiion of alcohol (5 g/kg) was further increased through pretreatment with PF for 2 weeks. When PF was given to rat fed with 25% alcohol solution instead of water for 6 weeks. the activity of Ald DH in mitochondrial fraction decreased to about 28% compared with sucrose-treated group. But after pretreatemnt of PF, the activity was restored to the normal level. By the treatment with disulfiram (300 mg/kg, once a day for 3days) was restored to the control after the pretreatment with PF. And also mitochondrial Ald DH activity in vitro was not changed. All these observations suggest that reduction of acetaldehyde levels are partly due to increase activity of mitochondrial Ald DH. Therefore, the recovery from intoxication of acetaldehyde may be enhanced by treatment with PF.

Effects of Puerariae radix herb-acupuncture on nitric oxide synthase expression in the hippocampus of alcohol-intoxicated Sprague-Dawley rats (갈근약침(葛根藥鍼)이 알콜 중독(中毒) 백서(白鼠) 해마(海馬)의 Nitric Oxide Synthase에 미치는 영향(影響))

  • Objective: The purpose of this study was to determine the effects of Puerariae radix(PR) herb-acupuncture on nitric oxide synthase(NOS) expression in hippocampus of alcohol-intoxicated Sprague-Dawley(SD) rats. Methods: SD rats were randomly assigned into 6 groups; normal group, control group, alcohol with herb-acupuncture group(0.3, 3, 30 and 300mg/kg PR). Normal groups were received with NaCl, while alcohol intoxication groups were injected intraperitoneally with alcohol(2g/kg) twice per day for 3 days. Herb-acupuncture groups were injected on Zhongwan(CV12) for 5 consecutive days. For the detection of NADPH-d-positive cells in hippocampus, immunohistochemistry was performed. Results: n control group, a significant decrease in NADPH-d-positive cells was observed compared to normal group. In alcohol with herb-acupuncture group, NADPH-d-positive cells increased significantly compared to control group. Conclusions: The present results revealed that NOS expression is enhanced in the hippocampus of SD rats through PR herb-acupuncture in an acute alcoholic intoxication condition.

Protective Effects of Food Including Hovenia dulcis on Acute Alcohol Intoxication (급성 알코올 중독에서 헛개나무 추출물을 포함한 식품 조성물의 보호 효과)

  • Over-consumption of alcohol leads to many side-effects such as malnutrition, liver disease, and neuronal disorders and many investigators have tired to identify methods for preventing the side-effects of drinking. In this study, we demonstrated the protective effect of a new food component, SAC-1, containing Hovenia dulcis Thumb and Lonicera caerulea Thumb extract against the side-effects of drinking. We observed that blood alcohol concentration, glutamic oxaloacetate transaminase, lipid peroxidation, and total glutathione level decreased significantly in plasma and liver of mice fed the SAC-1 extract before alcohol intoxication. In particular, SAC-1 had more of a protective effect than that of Hovenia dulcis Thumb extract alone. These results suggest that SAC-1 should further be developed to treat alcohol detoxification and stimulate antioxidative potentials.

Pharmacological Studies on Butanol Fraction of Puerariae Radix (갈근 Butanol Fraction의 약리 작용에 관한 연구)

  • Effective BuOH fraction was extracted from Puerariae radix (Pueraria thunbergiana), and several pharmacological activities were examined. From several pharmacological examinations, it was found that BuOH fraction has an antipyretic activity against typhoid vaccine pyrexia in rabbit, a papaverine-like antispasmodic activity on isolated ileum and jejunum of rat, an analgesic effects on mice treated by 0.7% HAc, and an anti-inflammatory activity of carrageenin edema in rat. It was found that BuOH fraction has detoxication effects on acute and subacute toxic rat induced by alcohol in consequence of biochemical tests of serum. Therefore it was considered that BuOH fraction has preventive and therapeutic effects in alcohol intoxication.
