약침제재별 여과속도와 약침 시술시 느끼는 통증의 상관성 분석 (Analysis the Correlation of Filtration Rate of Herbal-acupunctures and Pain Score of Herbal Acupuncture Stimulation)

  • Objectives : This study was designed to find out correlation of filtration rate of herbal-acupunctures and pain score of herbal acupuncture stimulation. Methods : Filtration rate of Hwangryunhaedoktang herbal acupuncture, Kidney yang deficiency number two herbal acupuncture, Hominis Placenta herbal acupuncture, Cervus Elaphus herbal acupuncture, BUM herbal acupuncture and Carthami Flos herbal acupuncture were measured at the korean institute of herbal acupuncture. And Hwangryunhaedoktang herbal acupuncture, Kidney yang deficiency number two herbal acupuncture, Hominis Placenta herbal acupuncture, Cervus Elaphus herbal acupuncture, BUM herbal acupuncture and Carthami Flos herbal acupuncture were injected at Xie points of the subjects. Results : Ranking of herbal acupunctures filtration rates is the same as ranking of pain score and duration time of herbal acupuncture. Correlation analysis result of herbal acupunctures filtration rates and herbal acupuncture VAS with a simple linear regression analysis is that correlation coefficient of Hwangryunhaedoktang herbal acupuncture, Kidney Yang Deficiency Number Two herbal acupuncture, Hominis Placenta herbal acupuncture, Cervus elaphus herbal acupuncture, BUM herbal acupuncture. Carthami Flos herbal acupuncture is 0.44, 0.53, 0.58, 0.76, 0.47 and 0.54. Correlation analysis result of herbal acupuncture's filtration rates and herbal acupuncture stimulation's duration time is that correlation coefficient of Hwangryunhaedoktang herbal acupuncture, Kidney Yang Deficiency Number Two herbal acupuncture, Hominis Placenta herbal acupuncture, Cervus Elaphus herbal acupuncture, BUM herbal acupuncture, Carthami Flos herbal acupuncture is -0.09, 0.11, -0.07, 0.28, -0.11 and -0.09. Conclusion : We found the correlation of filtration rate of herbal-acupunctures and pain score of herbal-acupuncture stimulation. This study help to control pain amount and pain duration time of herbal acupuncture treatment. And this study improve acupuncture theory of herbal acupuncture.

레이저자극과 수기자극에 대한 경락전기반응 특성비교 (Comparison of meridians electric response property for laser and acupuncture stimulation)

  • 최근에 레이저를 임상치료에 응용하는 사례가 증가하고 있다. 그러나 전기적 관점에서 레이저 자극이 경락에서 어떠한 전기적 반응을 유도할 수 있는지에 대한 연구가 미흡하고, 명확한 치료효과에 대한 임상보고가 발표되지 않고 있다. 본 논문에서는 레이저자극과 수기자극이 경락전위 형성에 미치는 영향을 비교 관찰하여 전기적 관점에서 침술과정 및 침술효과에 대한 객관적 근거를 제시하고자 하였다. 수양명대장경상의 삼간혈(LI3)을 각각 자극했을 경우, 삼간혈(LI3)과 합곡혈(LI4)에서의 전위변화를 측정하였다. 그 결과, 레이저 자극 시, 평균 피크전위는 $7.53{\pm}3.44{\mu}V$로 매우 낮게 나타났고, 자극전후 전위패턴에 차이가 없어서 레이저 자극에 대한 유효한 전기반응으로 간주하기 어렵다. 접지조건에 대한 수기자극에서는 평균피크전위가 $2.65{\pm}1.53mV$로 매우 높게 측정되었고, 개인별, 접지조건에 따라 전위크기와 패턴이 다양하게 나타나고 전위패턴은 주로 캐패시터의 충방전 전위와 매우 비슷하게 나타났다. 또한, 절연자침의 경우에서는 접지조건에 관계없이 평균 피크 전위가 $0.25{\pm}0.16mV$로 수기자극 전위에 비하여 매우 낮게 나타났으며, 이는 침자극에 대한 유효한 전기 반응으로 간주하기 어렵다. 따라서, 전기적 관점에서 침술과정과 침술효과는 시술자와 피시술자간의 생체이온전하의 이동에 의한 에너지 교감현상임을 확인하고, 수기자극은 경락의 전기반응을 유도하는 반면, 레이저 자극은 경락의 전기적 반응을 유도하기 어렵다는 것을 확인하였다.

약침 주입 용량 및 자입 방법에 따른 자극량 차이 연구 (Research on the Amount of Stimulus Differences According to Pharmacopuncture Injected dose and Characters Method)

  • Objectives : This study was designed to discover the difference in stimulation effect when the volume and insertion method of mountain ginseng pharmacopuncture was varied. Methods : 48 healthy people were randomly injected with 0.1, 0.3, 0.5, and 0.7 cc of mountain ginseng pharmacopuncture, perpendicularly or transversely at acupoints $LI_9$ and $LI_{10}$. The subjects noted their level of stimulation on a 100 mm visual analog scale(VAS) immediately after, 3 minutes after, and 5 minutes after the treatment. Results : There was no significant statistical difference in the stimulation strength between perpendicular injection and transverse injection pharmacopuncture. There was significant statistical difference in the stimulation strength when the volume of pharmacopuncture was varied. As the volume of pharmacopuncture increased from 0.1 cc to 0.3 cc, and to 0.5 cc, the stimulation strength increased as well. Conclusions : We found that volume of pharmacopuncture may be a controlling element for mountain ginseng pharmacopuncture. Additional study is needed for pharmacopuncture's stimulation adjusting factors.

침전극 저주파 자극이 흰쥐 중뇌수도주위 회색질의 nNOS 면역반응세포 변화에 미치는 영향 (The Effect of Needle Electrode Electrical Stimulation on the Change of neuronal Nitric Oxide Synthase Immunoreactive Cells in the periaqueductal area of the Rat)

  • Acupuncture has been used as a clinical treatment in Oriental medicine for various diseases. In the present study was carried out to investigate the effects of acupuncture and electrical stimulation on the change neuronal nitric oxide synthase(nNOS) immunoreactive cells in the periaqueductal gray(PAG) area of the male SD rats. Enhanced expression of nNOS was detected in the dorsolateral-PAG(DL-PAG) area of rat with stress by fixed body, and acupuncture and needle electrode electrical stimulation groups at Hapgok like acupoint decreased the stress-induced enhancement in the expression of nNOS. The present results demonstrate that acupuncture and needle electrode electrical stimulation is effective in the modulation of expression of nNOS in the DL-PAG area under stress conditions.

침을 이용한 전기자극 통증치료 (Electric Stimulation for Pain Relief Using Acupuncture Needles)

  • For pain relief my collegue and I used thin acupuncture needles as electrodes in electric stimulation. The needles were inserted into a trigger point and into another point located in the same muscle instead of meridian points of electro-acupuncture. Low frequency electric stimulation was given through the needles to 130 patients for 15 min. The results were as follows In 25 acute sprain patients electric stimulation was given $3.14{\pm}1.12$ times and the pain was reduced on the average by $83.00{\pm}6.77%$ (VAS). In 45 chronic sprain patients electric stimulation was given $5.51{\pm}1.38$ times and the pain was reduced on the average by $70.22{\pm}8.98%$ (VAS). In 28 myofascial pain syndrome patients electric stimulation was given $6.22{\pm}1.25$ times and the pain was reduced on the average by $66.48{\pm}8.75$(%). In 7 muscle contraction headache patients electric stimulation was given $4.14{\pm}1.57$ times and the pain was reduced on the average by $75.00{\pm}9.57%$ (VAS). In 25 radiculopathy patients electric stimulation was given $4.73{\pm}1.131$ times and the pain was reduced on the average by $21.37{\pm}9.31%$ (VAS). We he conclude that electric stimulation therapy using acupuncture needles is very effective in acute sprain, chronic sprain, myofascial pain syndrome and muscle contraction headache. Any doctor with knows anatomy and trigger points can practice this method without studying oriental medicine or difficult acupuncture techniques.

거짓침 피부접촉부 형태에 따른 침감 차이 연구 (A comparison study of acupuncture sensation scale between real acupuncture and sham needle)

  • Objectives : Proper acupuncture stimulation is associated with a characteristic set of sensation usually referred to as 'De-Qi'. In order to develop the appropriate sham acupuncture, various sensations to each stimulation should be considered through analysis of the profiles of acupuncture sensation. It was therefore investigated to compare the acupuncture sensation scale (ASS) of two types of sham acupuncture to that of the real acupuncture. Methods : Ninety-four participants (mean age 26.4, range 26-49) were asked to complete five point-Likert scale ASS developed by Vincent et al. after real or two-kinds of sham acupuncture stimulation: blunted tip sham acupuncture (BT) and round tip sham acupuncture (RT). Needling was done at LI4 acupoint on non-dominant hand and stimulated for 30 seconds with real or two-kinds of sham needle. Finger withdrawal latency (FWL) of each group was also measured to evaluate the pain sensitivity to noxious heat stimuli. Results : BT acupuncture significantly less produced penetrating, numb, intense, hurting, pulling, shock, tingling, throbbing sensation than real acupuncture stimulation. RT acupuncture significantly less produced penetrating, burning, electric, numb, intense, hurting, pulling, aching, shasharp, shock, stinging, tingling, throbbing sensation than real acupuncture. Each group did not demonstrate the differences of pain sensitivity to noxious stimuli. Conclusions : These results indicated that types of tip of acupuncture produced different kinds of acupuncture sensation. Our finding provides a general information of sensations to two kinds of sham acupuncture for development of ideal placebo sham needle.

심포경(心包經)과 삼초경(三焦經)의 목혈(木穴)과 금혈(金穴)자침이 뇌파에 미치는 영향 비교연구 (A Comparative Study of the Differences among PC9, TE3, PC5 and TE1 and Their Effects on the EEG)

  • Objective: This paper aimed to understand influences on EEG conducting acupuncture stimulation, by comparing the changes in the acupoints on the body before and after normal people are treated with acupuncture at PC9 and TE3, which are referred to as Wood points(木穴), and PC5 and TE1, which are referred to as the Metal points(金穴) among the five shu points of Yin pericardium Meridian and Yang Triple Energizer Meridian. Methods: The study was performed on 30 healthy female volunteers in their 20's. EEG was measured for 5 minutes before acupuncture stimulation was conducted on PC9, TE3, PC5 and TE1. During 20 minutes of acupuncture treatment, the same items were continuously measured to find out whether there were any changes in them, and they were measured for 5 minutes after removing the acupuncture needles in order to implement a comparative analysis. Results: Comparision of EEG data before and after the treatment at PC9 shows no significant differences in all wave. Compared with the pre-acupuncture period at TE3, $\delta-\theta$ wave decreased significantly (P<0.05) during the acupuncture stimulation periods. Compared with the pre-acupuncture period at PC5, $\delta-\theta$ wave and high $\alpha$ wave increased significantly (P<0.05) during the acupuncture stimulation periods. And Mid $\beta$ wave and high $\beta$ wave decreased significantly (P<0.05) during the acupuncture periods and the post acupuncture periods. Compared with the Pre-acupuncture period at TE1, $\delta-\theta$ wave, $\theta$ wave and high $\alpha$ wave increased significantly (P<0.05) during the acupuncture stimulation periods. And Low $\beta$ wave decreased significantly (P<0.05) during the acupuncture periods. Conclusion: When acupuncture stimulation was performed on PC9 and TE3, referred to as the "Wood points", brain waves were stabled, while when acupuncture was performed on PC5 and TE1, called the "Metal points", a brain was waked. From the findings of this study, we hypothesize that the wood properties, from which growing starts in all things, are related with fast waves of EEG, and the metal properties, which stabilize and converge in all things, are related with slow waves of EEG.

음릉천(陰陵泉)(Sp9)의 전침자극(電鍼刺戟)이 functional MRI상 뇌활성(腦活性) 변화(變化)에 미치는 영향(影響) (A fMRI study on the cerebral activity induced by Electro-acupuncture on Sp9(Yinlingquan))

  • Objective : The meridian theory in oriental medicine explains that Sp9(Yinlingquan) has been considered very important for gynecological disorders, spleen and stomach disorders, and water metabolic disorders. Functional MRI is the best method of showing the evidences of the effect of the Electro-acupuncture treatment through scientific methods. On this supposition, Electro-acupuncture stimulation on Sp9(Yinlingquan) was carried out on six healthy volunteers, using the gradient echo sequence with the 3.0T whole-body MRI system(ISOL). Methods : Electro-acupuncture stimulation on Sp9(Yinlingquan), the experiment was carried out on six healthy volunteers, using the gradient echo sequence with the 3.0T whole-body MRI system(ISOL). After the needle insertion on right Sp9(Yinlingquan), 2Hz of electric stimulation was given for 30 seconds, repeated five times, with 30 seconds' intervals. The Image analysis including motion correction, talairach transformation, and smoothing was done using SPM99. Results : The electro-acupuncture stimulation on Sp9(Yinlingquan) activates Brodmann Area 13, 19, 22, 39, 40, 47 which may be the central pathways of the electro-acupuncture stimulation on Sp9(Yinlingquan). We also hypothesize that those brain areas may be directly or indirectly related with the mechanism of the effect of electro-acupuncture stimulation on Sp9(Yinlingquan).

Acupuncture Stimulation for Motor Cortex Activities: Evidence from 3T Functional MRI Study

  • The purpose of this study was to investigate whether or not acupuncture of GB34 produces a significant response of the modulation of somatomotor areas by functional magnetic resonance imaging (fMRI) study. The acupoint, GB34, located in the back of the knee, is known to be effective in recovering motor function after stroke. Using 3T MRI scanner, functional MR imaging of the whole brain was performed in 12 normal healthy subjects during two stimulation paradigms; acupuncture manipulation on GB 34 and sham points. This study investigates the activation of the mortor cortex elicited by a soft and an intensified stimulation of GB 34. Three different paradigms were carried out to detect any possible modulation of the Blood Oxygenation Level Dependent (BOLD) response in the somatomortor area to motor stimulation through acupuncture. Group analysis from seven individuals showed that bilateral sensorimotor areas (BA 3,4,6 and 7) showed stimulation related BOLD signal contrast of approximately 6% whereas very few areas were activated when sham stimulation is given. The present study shows that acupuncture fMRI study can be safely conducted in 3T MRI environment, and acupuncture stimulation in GB34 modulates the cortical activities of the somatomotor area in human. The present findings may shed light on the CNS mechanism of motor function by acupuncture and form a basis for future investigations of motor modulation circuits in the stroke patients.

  • Objective : Recently, many studies have showed the evidences of the effect of the acupunture treatment through scientific methods. One of these methods is functional MRI. We performed electro-acupunture on Sp6 and observed the changes of brain activation using fMRI. Methods : To see the effect of electro-acupunture stimulation on Sp6, the experiment was carried out on 12 healthy volunteers, using the gradient echo sequence with the 3.0T whole-body MRI system(ISOL). After the needle insertion on right Sp6, 2Hz of electric stimulation was given for 30 seconds, repeated five times, with 30 seconds' intervals. The Image analysis including motion correction, talairach transformation, and smoothing was done with SPM99. Results : 1. Group averaged brain activation induced by bilateral eletro-acupunture stimulation on Sp6 activates Brodman Area 3, 7, 13. 2. Group averaged brain deactivation induced by bilateral eletro-acupunture stimulation on Sp6 activates Brodman Area 6, 38, 47. 3. Group averaged brain activation induced by unilateral(right side) eletro-acupunture stimulation on Sp6 activates Brodman Area 5, 6, 13, 17, 18, 19, 31, 38, 40 ptoms, back pain(32.5%) was the 4. Group averaged brain deactivation induced by unilateral(right side) eletro-acupunture stimulation on Sp6 activates Brodman Area 3, 4, 18, 21, 36, 38, 39. 5. Brain region activated by motor stimulation activates Brodman Area 3, 4, 6, 18, 19.
