A Systematic Review of Herbal Medicine Treatment for Knee Osteoarthritis (퇴행성 슬관절염의 한약제제 치료에 대한 체계적 문헌고찰)

  • Objectives This study reviewed recent clinical research trends regarding the effectiveness of herbal medicine treatments for knee osteoarthritis. Methods We reviewed 4 different online databases (PubMed, China National Knowledge Infrastructure [CNKI], National Digital Science Library [NDSL], Oriental Medicine Advanced Searching Integrated System [OASIS]) from January 1, 2015 to August 31, 2019. Results Thirty-two randomized controlled trial papers were selected in this review. Most of them were conducted during 12 weeks, used Western Ontario and Mcmaster Universities Arthritis Index. In 22 of the papers, the effectiveness in the intervention groups was significantly higher than that in the control groups statistically (p<0.05). 5 studies reported intervention group was not inferior to the control group. Conclusions Most of studies showed herbal medicine treatments were statistically effective to knee osteoarthritis. More scientific and systematic clinical studies should be actively conducted in the future, and the results of this study could be used as basic data in the future clinical studies on herbal medicine treatment for knee osteoarthritis.

A Review Study on Qigong Interventions for Performance Enhancement in Korean Journal Articles: A Preliminary Study for Developing Qigong Therapy for Elite Athletes (수행능력 향상을 위한 국내 기공 연구 고찰: 스포츠 현장에 적용 가능한 기공 프로그램 개발을 위한 예비 연구)

  • Objectives: To investigate the research trends of qigong interventions to make influence on performance enhancement. Methods: Korean databases were searched for papers related to studies of qigong interventions to assist subjects perform better. Results: Twelve articles published in Korean Journals from 2000 to 2017 were reviewed. Among the retrieved papers, eight were applied with breathing exercises, including Dan-jeon breathing training, one applied with brain breathing meditation with DoIn exercise, one applied with qigong meditation, one applied with qi-training, and one applied with brain breathing training. RoBANS was used for assessment. The quality of selected articles was generally low. 10 of the 12 articles on qigong reported that experimental group showed statistically significant increase of performance enhancement both directly and indirectly. Conclusions: Reviewing 12 articles, this study established that qigong interventions might be beneficial for enhancing performance especially in sports. However, more scientifically designed study on qigong therapy are needed to prove the efficacy and effectiveness.

The Effect of Banhabaekchulcheonma-tang on Benign Paroxysmal Positional Vertigo: A Systematic Review Using the CNKI Database (양성 돌발성 두위 현훈에 대한 반하백출천마탕(半夏白朮天麻湯)의 효과 : 체계적 문헌고찰 - CNKI 검색을 중심으로)

  • Objectives: This study examines the effect of Banhabaekchulcheonma-tang on Benign Paroxysmal Positional Vertigo (BPPV) through a systematic review of randomized controlled trials (RCTs) in the Chinese National Knowledge Infrastructure Database (CNKI). Methods: RCTs were included that tested the effect of Banhabaekchulcheonma-tang on BPPV. A study of the literature in the CNKI was performed for papers published from January 1, 2011 to June 30, 2021. Results: A total of eight RCTs met the inclusion criteria. All studies identified the effect of Banhabaekchulcheonma-tang in the treatment of BPPV. The effectiveness of treatment was higher in the intervention group than in the control group, as shown through evaluation indicators, including the total effective rate, dizziness handicap inventory (DHI), and blood lipids. Conclusion: Treatment with Banhabaakchulcheonma-tang was found to be effective in treating BPPV. However, due to the low quality of available studies, the significance of this conclusion is somewhat limited. Further clinical studies are needed on the development of diagnosis and treatment methods for BPPV based on Korean medicine.

A Case Report of a Patient with Charcot-Marie-Tooth Disease Complaining about Lower Limb Pain (하지통증을 호소하는 샤르코마리투스 질환 환자에 대한 복합적 한방처치 1례)

  • Objective: This study was performed to report on the effects of traditional Korean Medicine as a treatment for a patient with Charcot-Marie-Tooth disease, with lower limb pain as the chief complaint. Methods: We treated this patient with traditional Korean medicine and measured symptom severity using the Visual Analogue Scale (VAS), and the Oswestry Disability Index (ODI) as well as measuring the length of the patient's self-exercising time. Results: After about 7 weeks of treatment, most pathological symptoms had decreased. There was a gradual decline in the VAS and a gradual increase in the length of the self-exercising time. Although there was an increase in the ODI scale during the first 2 weeks, it eventually showed a decline in the rest of the period. Conclusions: Traditional Korean medicine can be a solution for the patients with Charcot-Marie-Tooth disease suffering from peripheral neurological symptoms.

A Case Report of Worsening Alzheimer's Disease Symptoms after COVID-19 Infection That Were Treated with Yigan-san-gami (COVID-19 감염 후 알츠하이머 치매증상의 악화에 대한 억간산 가미 치험 1례)

  • Introduction: In this case report, we describe the clinical effectiveness of Yigan-san-gami in improving symptoms in a patient with worsening Alzheimer's dementia after COVID-19 infection. Case presentation: Symptoms of dementia worsened in a patient with Alzheimer's disease after COVID-19 infection. Western medicines were added following neurological consultation. However, there was no medicinal effect. The patient was treated with Korean medicine (Yigan-san-gami) three times a day. We used daily charts, the Korean Mini-Mental State Examination-2 (K-MMSE-2), Clinical Dementia Rating (CDR), and Short Version Geriatric Depression Scale (SGDS) to assess the patient's symptoms. Cognitive decline and behavioral and psychological symptoms of dementia (BPSD) improved following the treatment with Yigan-san-gami. Conclusion: The results suggest that Yigan-san-gami may be effective in improving symptoms of Alzheimer's disease that worsen after COVID-19 infection.

The Experimental Study on the Immuno-regulatory effect of ASARI HERBA CUM RADICE Herbal- acupuncture at Pyesu(BL13) on OVA-induced asthma in mice (폐유(肺兪) 세신약침(細辛藥鍼)이 OVA-induced Asthma Mouse Model의 면역조절작용(免疫調節作用)에 미치는 영향)

  • Objective : The aim of this study was to investigate the asthma-suppressive and immune-regulatory effect of AHCR-HA(ASARI HERBA CUM RADICE Herbal-acupuncture) at Pyesu(BLl3) on OVA(ovalbumin)-induced asthma in mice. Methods : C57BL/6 mice out of all the experimental groups, except the Normal group and the AHCR-HA group, were sensitized and challenged with OVA The mice in the AHCR-HA group and the OVA-AHCR-HA group were treated with AHCR-HA(1%) at Pyesu(BL13). The mice in the OVA-Saline group were injected with saline at Pyesu(BL13). The mice in the OVA-Needle-Prick group were treated with a single prick with an injection needle at Pyesu(BL13). AHCR-HA saline injection and needle prick were administered for 8 weeks, three times a week. Result : 1. The populations of granulocytes, CD3e-/CCR3+ cells, CD69+/CD3e+ cells, CD4+ cells and CD23+/B220+ cells in the OVA-induced asthmatic mouse lungs decreased significantly by AHCR-HA. 2. The lung weight, total cells in lung, total leukocytes in BALF, eosinophils in BALF, collagen accumulation in the lung sections of the OVA-AHCR-HA group decreased significantly. 3.The concentrations of IL-4, IL-5, IL-13, IgE in BALF and serum of the OVA-AHCR-HA group decreased significantly. 4. The numbers of Gr-1+/CD11b+, CCR3+, CD3e+, CD19+, CD3e+/CD69+cells in the OVA-AHCR-HA group decreased significantly. 5. The mRAN expressions of $TNF-{\alpha}$, IL-5, IL-4 and IL-13 in lung of the OVA-AHCR-HA group decreased significantly. 6. The AHCR-HA group didn't show any considerable difference from the Normal group. The OVA-saline group and the OVA-Needle prick group showed suppressive effects on OVA-induced asthma however they were not statistically significant. Conclusion : These results suggest that AHCR-HA at Pyesu(BL13) is considered to be effective in treating asthma and to be put to practical use in the future asthma clinic.

A Study on the Effect of Luffae Fructus Retinervus Herbal-Acupuncture at ST36 on Collagen-induced Arthritis in Mice (족삼리(足三里) 사과락약침(絲瓜絡藥鍼)이 생쥐의 Collagen-induccd Arthritis에 미치는 영향)

  • Objective : The aim of this study is to observe the effect of Herbal-acupuncture(HA) with Luffae Fructus Retinervus Herbal-Acupuncture Solution(LFR-HAS) at ST36(Joksamni) on Collagen Ⅱ -induced arthritis(CIA) in mice. Methods : DBA1/J mice were immunized with bovine type Ⅱ collagen(CⅡ) on days 0 and 21 to induce an arthritis. The mice were divided into 5 groups. They were Normal group(wild type), Control group(CIA), Saline group(CIA +saline injection), Needle Prick group(CIA +single Prick with an injection needle) and LFR-HA group(CIA +LFR-HA treatment). The saline injection, needle prick and LFR-HA were made on the right ST36(Joksamni) of mice for 5 weeks, 3 times a week beginning 4 weeks after the booster immunization. Results : 1. The highest synovial rate of lung fibroblasts was measured in the 1% LFR-HAS. 2. TNF-${\alpha}$ expression of survival cells from CIA mouse joint was significantly reduced in the 1% LFR-HAS. 3. The incidence of arthritis and the spleen weight of CIA mouse were significantly reduced by the Luffae Fructus Retinervus Herbal-Acupuncture (LFR-HA) at ST36. 4. The concentrations of IL-6, INF-${\alpha}$, INF-${\alpha}$, IgG, IgM, and anti-collagen Ⅱ in the CIA mouse serum were significantly reduced by the LFR-HA at ST36. 5. The histological examination showed that, in the LFR-HA group, the cartilage destruction and the synoviocyte proliferation in the CIA mouse joint were not significant compared to the control group, and the collagen fiber was similarly expressed as the normal group. 6. In the LFR-HA group, the ratio of CD3e+ to CD19+ cell, and the ratio of CD4+ to CD8+ cell in the lymph node were similarly maintained as those of the normal group. 7. CD69+/CD3e+ and CD11a+/CD19+ cells in the CIA mouse lymph node were significantly reduced by the LFR-HA at ST36. 8. CD11b+/Gr-1+ cells in the CIA mouse joint were significantly reduced by the LFR-HA at ST36. Conclusions : These results indicate that Luffae Fructus Retinervus Herbal-Acupuncture (LFR-HA) at ST36 may regulate the immune system and have a therapeutic effect on Collagen-induced arthritis(CIA) in mice.

Use of SNOMED CT to Represent Traditional Korean Medicine Concepts : A Semantic Characterization of Migraine-Related Concepts from Korean Medicine Clinical Practice Guideline (SNOMED CT를 활용한 한의약 개념 매핑 : 한의임상진료지침에서 도출된 편두통 관련 개념의 의미론적 표현)

  • 목적 : 본 연구는 한의약에서 사용하는 용어가 SNOMED CT로 매핑 가능한지 여부를 조사하고, 한의약 용어를 표현하기 위해 기존 SNOMED CT 온톨로지를 개선할 수 있는 방안을 제안하는 것을 목표로 하였다. 방법 : 선행 연구의 매핑 가이드라인에서 제시된 7단계 과정을 수정하여 활용하였다. 매핑의 목적 및 범위 정의, 용어 추출, 개념 추출, 매핑을 위한 소스 용어 작업, SNOMED CT 개념 검색, 매핑 관계 분류 및 매핑 검증의 과정을 수행하였다. 매핑의 목적은 한의약 임상 아이디어를 표현하는 표준 용어로서 SNOMED CT를 평가하는 것이고, 범위에는 편두통 환자 관리의 평가, 진단, 치료 및 예방을 포함하였다. 결과 : 총 546개의 용어가 추출되었다. 중복된 용어를 제거한 후, 271개의 개념이 SNOMED CT 매핑에 사용되었다. 이중 43.2%는 SNOMED CT 개념과 의미론적으로 동등하게 매핑되었고(117개 개념), 39.1%는 SNOMED CT 개념이 더 포괄적인 의미를 가지도록 매핑되었다(106개 개념). 상대적으로 포괄적인 의미를 가지는 SNOMED CT 개념에 매핑된 한의약 개념 106개 중 19개는 SNOMED CT 후조합을 이용하여 의미론적으로 동등하게 표현이 가능하였다. 나머지 17.7%의 한의약 개념은 SNOMED CT에 매핑할 수 없었다. 결론 : 본 연구는 한의약에서 사용되는 개념을 SNOMED CT에 매핑하여 한의약 용어를 표준화하였다. 연구 결과를 바탕으로, 한의약에서 사용되는 용어를 표준의료용어로 표현하기 위하여 SNOMED CT에 새로운 개념과 속성을 추가하는 것을 제안한다.

A clinical study of East-West Pain Treatment on Chronic Headach Patients (만성두통환자에 대한 동서협진의 임상적 연구)

  • Objective : Because the cause and etiology of chronic headache are not yet fully explained, the treatment of this symptom is not simple. In order to study the effectiveness of East-West pain treatment on chronic headache, we evaluated its effect of pain alleviation and quality of life improvement on chronic headache patients who were treated with nerve block and acupuncture, and only acupuncture for eight weeks. Methods : 92 clinical experiment participants were gathered and through a questionnaire patients who experienced headache for more than 4 hours a day and more than 15 days per month were qualified as chronic headache patients. The qualified patients were classified into two groups, nerve block and acupuncture group(EW group, n=49) and acupuncture group(E group, n=43). Treatment was applied 2 times a weeks for 8 weeks. The acupoints, GV20, HN23, ST8, HN46, TE17, GB20, LI20O, LI11, LI14, ST36, and LR3 were stimulated for 20 minutes. The effects of two groups were analyzed using VAS scores and BPI(Brief Pain Inventory).

Efficacy of Bee-venom Acupuncture on Central Post Stroke Pain -Single-blind Randomized Controlled Trial- (중풍 후 중추성 통증환자에 대한 봉독약침의 임상효과 -단일맹검 무작위배정 환자 대조군 비교연구-)

  • Objectives : This study was designed to evaluate objectively the efficacy of bee-venom acupuncture on Central post-stroke patients. Methods : After screening, 25 patients recruited with eligible criteria. Among them 1 dropped out in treatment group, 4 in control group. We applied bee-venom acupuncture 6 points of body($LI_{15}$, $GB_{21}$, $LI_{11}$, $GB_{31}$, $ST_{36}$, $GB_{39}$) 2 times a week for 3 weeks for treatment group, and normal saline for control group in the same way. After 3 weeks treatment and 2 weeks follow up we measured VAS, categorial rating scale as a pain assessment also MBI, Modified Rankin Scale as a stroke recovery. Results : 1. Treatment group showed significant difference in VAS, categorial rating scale after treatment and 2 weeks follow up, compared with baseline. The control group showed significant difference between baseline and after treatment But the 2 weeks follow up, there is no significant difference compared with baseline. Also there's no significant difference in categorial rating scale. 2. MBI showed significant difference in treatment group between baseline and after treatment, but the control group did not have any difference. There's no significant difference in Modified Rankin Scale for treatment group and control group. 3. There's no significant difference between control group and treatment group. This is because of the small sample size in the study, Thus it seems to reflect individual difference strongly. Conclusions : There are positive effect on CPSP patients in pain and stroke recovery treating with bee-venom acupuncture than control group.
