Although prior researches have been investigated the impact of insured cost(direct cost) on employers and employees, little work has attempted to categorize the items of unsecured cost(indirect cost) by accidents. On this basis, the goal of this study was to achieve a better understanding of the nature of accident cost of unsecured cost. Specifically, this study aimed to categorize the unsecured cost items according to the domestic industry circumstances and use these results for a basis of other accident cost related studies. The results of this study are as follows: (1) accident development steps were categorized as twelve items for improved management according to each step of accident development (2) the points of occurrence and termination of the unsecured cost were identified for the improved management according to each step of accident development and (3) characteristics of each item in unsecured cost were studied and identified for a better control of accident costs. These results provide a basis for further researches on the unsecured cost.

SiC fiber-reinforced SiC matrix composite is a promising accident-tolerant fuel cladding material to improve the safety of light water nuclear reactors. Compared to the current zirconium alloy fuel cladding as well as metallic accident-tolerant fuel cladding, SiC composite fuel cladding has exceptional accident-tolerance such as excellent structural integrity and extremely low corrosion rate during severe accident of light water nuclear reactors, which reduces reactor core temperature and delays core degradation processes. In this paper, we introduce the concept, technical issues, and properties of SiC composite accident-tolerant fuel cladding during operation and accident scenarios of light water nuclear reactors.

The accident risk at the construction workplace associated with agricultural engineering is comparatively higher than those of other fields due mainly to its complex work types and processes. Agricultural engineering deals with a variety of agricultural infrastructures from irrigation and drainage facilities to giant-scale coastal reclamation land infrastructures. The characteristics that most agricultural projects have conducted on a small-scale even worsen the situation drawing low attentions to risk management. Therefore, systematical risk assessment that focuses on details of agricultural construction work process is required in order to enhance safety management capacity and to prevent repetitive accidents ultimately. This study aims to categorize construction work types and processes of agricultural construction works, and to quantitatively assess the accident risk of them based on accident analysis. Regarding classification of construction works, actual 827 accident cases were thoroughly reviewed and coded by their construction site, facility and work type, project scale and so on. Most accidents (71.8 % of total cases) occurred in small-scale construction workplaces with less than 5 billion Korean won project budget. And those accidents related to agricultural infrastructure project (37.4%) and agricultural water development project (22.4%). In terms of work types, accidents frequently took place in form-work followed by pipe installation work, steel bar work and concrete work. The potential risks were compared with actual outbreak of accidents based on Analytic Hierarchy Process (AHP). The results show that the potential conditions of accident expected to be took place is somewhat different from the actual conditions where accidents actually happened. This implicates that risk management manuals or education needs to be adjusted by reflecting unexpected circumstances. Overall, this study is meaningful in that the results could be foundations as to strengthen risk management capacity for agricultural engineering projects.
The Accident Tolerant Fuel (ATF) is a new concept of fuel, which can not only withstand the consequences of the accident for a longer time, but also maintain or improve the performance under operating conditions. ISAA is a self-developed severe accident analysis code, which uses modular structures to simulate the development processes of severe accidents in nuclear plants. The basic version of ISAA is developed based on UO2-Zr fuel. To study the potential safety gain of ATF cladding, an improved version of ISAA, referred to as ISAA-ATF, is introduced to analyze the station blackout accident of PWR using ATF cladding. The results show that ATF cladding enable the core to maintain a longer time compared to zirconium alloy cladding, thereby enhancing the accident mitigation capability. Meanwhile, the generation of hydrogen is significantly reduced and delayed, which proves that ATF can improve the safety characteristics of the nuclear reactor.
Incident investigation is regarded as a means to improve safety performance. For the prevention of industrial accidents, measures such as providing safety education, enhancing management interest and participation, establishing a safety management system, and conducting inspection of the work site are necessary. In particular, accident investigation activities, which are an important element of safety management, help to prevent similar accidents, thereby minimizing damage and enhancing work safety. They are critical for understanding business-related incidents and the vulnerabilities and opportunities associated with them. Therefore, it is clear that accident investigation activities are important for accident prevention. The primary focus of many incident investigation processes is on identifying the cause of an event. While considerable research has been conducted on potential accident investigation tools there has been little research on including the views and experiences of practitioners in the accident investigation process. In this study, a questionnaire survey was conducted among safety managers in the domestic manufacturing/construction industry to understand the practice of accident investigation. The investigation pertained to companies' accident investigation systems, the competence of investigators, and the identification and recommendations of the cause of accidents. From the analysis results of accident investigations, investigators' competence, the difficulty level of investigations, and the root causes of accidents were identified from the viewpoint of the participants of the accident investigations. In particular, the development of standardized and simple accident investigation methods and their dissemination to companies were found to be necessary for activating the root cause of accidents. Based on this, it can be used as basic data for the development of root cause analysis investigation techniques that are easily applicable to organizations.
This study develop accident response program for the hazardous materials(HAZMAT) transport vehicle with materials database and guidelines for accident response. Report documents of industrial and occupational accidents have continuously been accumulated in private and public institutes. Amongst others, information on narrative-texts of accidents such as accident processes and risk factors contained in disaster report documents is gaining the useful value for accident analysis. Despite this increasingly potential value of analysis of text information, scientific and algorithmic text analytics for safety management has not been carried out yet. Thus, this study aims to develop data processing and visualization techniques that provide a systematic and structural view of text information contained in a disaster report document so that safety managers can effectively analyze accident risk factors. To this end, the risk factor map using text mining and self-organizing map is developed. Text mining is firstly used to extract risk keywords from disaster report documents and then, the Self-Organizing Map (SOM) algorithm is conducted to visualize the risk factor map based on the similarity of disaster report documents. As a result, it is expected that fruitful text information buried in a myriad of disaster report documents is analyzed, providing risk factors to safety managers.
Background: Korea Atomic Energy Research Institute is developing a fission product transport module for predicting the behavior of radioactive materials in the primary cooling system of a nuclear power plant as a separate module, which will be connected to a severe accident analysis code, Core Meltdown Progression Accident Simulation Software (COMPASS). Materials and Methods: This fission product transport (COMPASS-FP) module consists of a fission product release model, an aerosol generation model, and an aerosol transport model. In the fission product release model there are three submodels based on empirical correlations, and they are used to simulate the fission product gases release from the reactor core. In the aerosol generation model, the mass conservation law and Raoult's law are applied to the mixture of vapors and droplets of the fission products in a specified control volume to find the generation of the aerosol droplet. In the aerosol transport model, empirical correlations available from the open literature are used to simulate the aerosol removal processes owing to the gravitational settling, inertia impaction, diffusiophoresis, and thermophoresis. Results and Discussion: The COMPASS-FP module was validated against Aerosol Behavior Code Validation and Evaluation (ABCOVE-5) test performed by Hanford Engineering Development Laboratory for comparing the prediction and test data. The comparison results assuming a non-spherical aerosol shape for the suspended aerosol mass concentration showed a good agreement with an error range of about ${\pm}6%$. Conclusion: It was found that the COMPASS-FP module produced the reasonable results of the fission product gases release, the aerosol generation, and the gravitational settling in the aerosol removal processes for ABCOVE-5. However, more validation for other aerosol removal models needs to be performed.
Purpose: The purposes of this study were to provide information about safe actions during the practice of dental technology, promote belief in safe actions among the students of dental technology, and reinforce their knowledge about how to act safety. Methods: Open questions were asked to 120 sophomores majoring dental technology to understand the target group's belief about safe actions. Following the instructions by Ajzen & Fishbein, the collected data were grouped according to similar beliefs. Those beliefs of the highest frequency were given a priority and got to represent the belief level of the target group. Even with belief changes, however, they would not be able to practice safe actions if they had no knowledge about how to do safe actions. For desirable behavioral changes among the students, the students were then asked to discuss the current state of safety accidents and issues focusing on the materials and processes used in the practice and give presentations regarding the subjects to identify risk factors. Then the identified risk factors were used to devise the program contents so that the students could practice safe actions through accurate behavioral approaches and belief changes by educating them about how to act safely by the practice processes and the involved materials and devices. Results: Not only do the students of Dept. of Dental Technicians have difficulties with the complex practice processes while practicing to make dental prosthesis, but they are exposed to the risk of accidents as they're poor at dealing with the materials and devices. Although there is a definite need for accident prevention education programs before practice, the reality is that the school doesn't the schoosuch education. Based on the judgment that an education program designed to reinforce belief for behavioral changes would generate effgrams results, an education program was developed by applying the Theory of Planned Behavior(TPB). Along with their beliefs, the current state of accidents n. Basepractice, demand for safety education, and practice level of safe actions were used to device the contents of accident prevention education and to develop an accident prevention education program for dental technology. Conclusion: The study presents an accident prevention education program developed to reinforce dental technology students' beliefs and knowledge and to help them do safe actions during practice.
This paper describes the development process, the innovative techniques used and insights gained from the latest integrated, full scope, multistate Level 2 PSA analysis conducted at the Leibstadt Nuclear Power Plant (KKL), Switzerland. KKL is a modern single-unit General Electric Boiling Water Reactor (BWR/6) with Mark III Containment, and a power output of $3600MW_{th}/1200MW_e$, the highest among the five operating reactors in Switzerland. A Level 2 Probabilistic Safety Assessment (PSA) analyses accident phenomena in nuclear power plants, identifies ways in which radioactive releases from plants can occur and estimates release pathways, magnitude and frequency. This paper attempts to give an overview of the advanced modeling techniques that have been developed and implemented for the recent KKL Level 2 PSA update, with the aim of systematizing the analysis and modeling processes, as well as complying with the relatively prescriptive Swiss requirements for PSA. The analysis provides significant insights into the absolute and relative importances of risk contributors and accident prevention and mitigation measures. Thanks to several newly developed techniques and an integrated approach, the KKL Level 2 PSA report exhibits a high degree of reviewability and maintainability, and transparently highlights the most important risk contributors to Large Early Release Frequency (LERF) with respect to initiating events, components, operator actions or seismic component failure probabilities (fragilities).
