Factors to consider for surgical in elderly patients with oral cancer

  • At present, in Korea, due to developments in medicine and technology and an increasing mean lifespan, the expected lifetime for elderly people is increasing (at 70 years, the expected additional lifespan is 14.8 years for males and 18.3 years for females). However, among all causes of death, malignant neoplasm is ranked first for all ages. Further, the incidence rate of oral cancer tends to increase with age. Thus, oral and maxillofacial surgeons have increased opportunity to experience the 'oldest old' patients with oral cancer. Elderly patients commonly have several comorbidities, poor general condition, limited socioeconomic support, fear of various postoperative complications, and perception of short for the rest of their life. In this situation, the patient, caregiver, and surgeon often choose undergraded treatment rather than standard treatment for oral squamous cell carcinoma owing to patient age. In elderly patients with oral cancer, ablation of tumor or reconstructive surgery is challenging for surgeons. Oral and maxillofacial surgeons must evaluate carefully the patient's medical condition and make a decision regarding treatment plans after sufficient discussion with patient and caregivers. We review the literature to consider the factors involved for deciding on a treatment plan regarding surgery in elderly patients with oral cancer.

  • This paper describes a method for steady state hot gas flow analysis considering nozzle ablation for the nozzle of $SF_6$ gas circuit breaker. In order to take account of the effect of ablated nozzle material on the hot gas flow. the PTFE mass concentration equation is added to the established equations for hot gas flow analysis. The steady state simulations were carried out under the condition of DC current flows and the results are presented.

  • Trigeminal neuralgia is a condition characterized by excruciating paroxysms of pain in lips, gums, cheek, or chin, and very rarely, in the distribution of the ophthalmic division of the trigeminal nerve. There are many treatments of trigeminal neuralgia, such as, medical treatment, electrical stimulation, radiation therapy and ablative procedures. Infraorbital nerve block with pure ethyl alcohol is an ablation procedure for trigeminal neuralgia. We injected pure ethyl alcohol into the infraorbital foramen for pain control. The results were as follows; 1) The infraorbital nerve block with pure alcohol was an a simple and an effective method. 2) Complication, included a mild sensory deficit and mild edema over the infraorbital area.

  • There is many considerations of the composition development of propellant, performance analysis according to temperature, ablation condition of heat-resistant material, etc. in the solid rocket motor development process. Performance analysis of the motor according to nozzle expansion ratio is one of this process and an important factor to decide the motor performance. A Study is verified through anlaysis, motor manufacture and test.

  • We discussed the thermal analysis for a recording media with the variation of the laser pulse duration, the laser power and the temperature distribution in order to optimize the Te-based antireflection structure from the computer calculations. In the case that the radial heat diffusion is negligible, we can calculate the maximum temperature of the recording layer at the center of the spot by the Simple Model. The temperature profile of the recording layer is obtained from the Numerical Model by considering the total specific heat and the latent heat. As a result, the effect of the heat sinking acting as a thermal loss for the hole formation could be minimized by introducing the pulse with the hole formation duration(.tau.) below the thermal time constant(.tau.$_{D}$) of a dielectric layer. These requirments can be satisfied by using the dielectric thickness of the 2nd ART(Anti-Reflection Trilayer) condition or the dielectric materials with a low thermal diffusivity.y.

  • The ignition of aluminum particles under high pressure and temperature conditions is studied. The laser ablation method is used to generate aluminum particles exposed to pressures ranging between 0.35 and 2.2 GPa. A continuous wave $CO_2$ laser is then used to heat surface of the aluminum target until ignition is achieved. We confirm ignition by a spectroscopic analysis of AlO vibronic band of 484 nm wavelength. The radiant temperature is measured with respect to various pressures for tracing of required heating energy for ignition. Then the ignition temperature is deduced from the radiant temperature using the thermal diffusion equation. The established ignition criteria for corresponding temperature and pressure can be used in the modeling of detonation behavior of heavily aluminized high explosives or solid propellants.

  • Syringoma is a benign eccrine sweat gland tumor that predominantly appears in females during puberty with multiple smooth papules measuring 1-2 mm in diameter. Common locations are on lower eyelids and cheeks. Vulvar syringoma is quite a rare, occurring condition with only a few cases reported. Here, we are reporting a case of 31-year-old female with vulvar syringoma associated with pruritus. The lesion was treated with carbon dioxide (CO2) laser ablation and 50% trichloroacetic acid (TCA) chemical peeling. Jaungo was used for wound care after laser abrasion. The combination treatment was effective for removing syringoma lesions. Post laser management with fusidic acid cream and jaungo cream was sufficient to prevent infection and promote wound healing.

Pediatric Finger Warts Treated Using Taeeumjowi-tang: a case report

  • Warts caused by the human papillomavirus (HPV) are generally treated with cryotherapy, CO2 laser ablation, interferon injections, and bleomycin injections. However, it is sometimes difficult to treat children because the treatment can be painful. In addition, recurrence may occur after treatment. In this study, warts completely disappeared following the administration of herbal medicine in two children, with warts in multiple parts of the hands and around the nails. Two pediatric patients visited the hospital for treatment of warts around their fingers and nails. Both patients received Taeeumjowi-tang (TJT) as a decoction for 60 days. TJT was performed twice per day for the 11-year-old patient and once per day for the 7-year-old patient. Patient progress was observed monthly, and the visual condition of the warts was photographed during the visits. After approximately two months of treatment, the warts disappeared from the fingers and nails of both patients. This case study suggests that the oral administration of TJT may be effective for pediatric patients with warts. Further studies are required to determine the efficacy and safety of these therapies.

Zero-Dimensional Modeling of Plasma Generator in Electrothermal Gun (전열포 플라즈마 생성장치의 영차원 해석모델)

  • This paper introduces a zero-dimensional modeling on the plasma generation in electrothermal gun operation. The plasma generator consists of alumina bore and aluminum electrodes which is electrically powered by outer pulse forming network and, traditionally, its numerical simulations have employed time-dependent one-dimensional governing equations. However, by assuming isothermal approximation along the bore and choked bore exit condition, present analysis simplifies the mass and energy equations into zero-dimensional approximation of plasma conditions coupled with mass ablation model and plasma property evaluation. The numerical results show good agreement with the corresponding one-dimensional computations and thus verify the present modeling approach.

  • For the c-axis oriented epitaxial YBa$_2$Cu$_3$O$_{7-{\delta}}$ thin film on r-cut sapphire substrate it is necessary to deposit buffer layers. The CeO$_2$buffer layer was deposited on sapphire substrate using RF magnetron sputtering system. We investigated XRD pattern of CeO$_2$thin films at various sputtering conditions such as sputtering gas ratio, sputtering power, target to substrate distance, sputtering pressure and substrate temperature. The optimum condition was 15 mTorr with deposition pressure, 1:1.2 with $O_2$and Ar ratio and 9cm with target to substrate distance. The CeO$_2$(200) peak was notable for a deposition temperature above 75$0^{\circ}C$. The YBa$_2$Cu$_3$O$_{7-{\delta}}$ was deposited on CeO$_2$buffered r-cut sapphire substrate using pulsed laser ablation. The YBa$_2$Cu$_3$O$_{7-{\delta}}$CeO$_2$(200)/A1$_2$O$_3$thin film was exhibited a critical temperature of 89K.xhibited a critical temperature of 89K.
