Purpose : Obesity has become a pandemic and especially the prevalence of childhood obesity has increased remarkably over the past years. Childhood obesity is often accompanied by the expected morbidities such as hyperlipidemia, fatty liver and glucose intolerance. We evaluated the pattern of alteration in the body mass index (BMI), serum adiponectin concentration, insulin resistance (HOMA-IR), and complication rates after one year follow-up. Methods : Forty two obese children (age: 8-15 years, 95th percentile of BMI for age and sex) were examined twice annually on June, 2004 and July, 2005. The body mass index (BMI) and sexual maturityrating (SMR) were determined annually by the same examiner and serum fasting glucose, liver enzyme, lipid profiles, adiponectin, insulin and HOMA-IR were measured and annual results were compared. Upper abdominal sonography was performed to detect fatty liver. Results : BMI ($25.3{\pm}2.9kg/m^2$ vs. $26.0{\pm}2.9kg/m^2$) and HOMA-IR ($3.6{\pm}1.8$ vs. $4.7{\pm}1.9$) have increased significantly after one-year follow-up. The serum adiponectin concentration ($17.2{\pm}5.2g/mL$ vs. $12.6{\pm}5.2g/mL$) has decreased after one year. The prevalence of hyperlipidemia and fatty liver were not changed significantly but that of hyperinsulinemia increased after one-year. HOMA-IR showed a positive correlation with BMI and SMR, but was negatively correlated with serum adiponectin concentration. Conclusion : Obese children in our study showed significant increment of insulin resistance during one year. These results suggest that well-organized and continuous obesity control is required especially for obese adolescent to prevent complication of obesity.
Purpose: The purpose of our study is to provide useful information for diagnostic methods of fatty liver by childhood simple obesity and to provide correlation between serum alanine aminotransferase (ALT) for screening test and abdominal computerized tomography (CT) and liver biopsy for confirmative diagnostic methods of fatty liver. Methods: Among 78 obese childrens who visited our hospital, CT was carried out in 26 childrens. Of these, liver biopsy was carried out in 15 childrens who had high obesity index or severe elevated ALT. Based on the level of serum ALT, 26 cases were classified into 3 groups, and compared with physical measurements and degree of fatty infiltration on CT and liver biopsy. Results: 1) Correlation between ALT and physical measurements: Of 26 obese children, ALT was abnormally elevated (>30 IU/L) in 17 cases (67.4%) but there was no significant correlation between ALT and physical measurements (p>0.05). 2) Correlation between degree of fatty infiltration on CT and ALT: Of 26 cases, 13 cases (50%) revealed fatty liver on CT. The degree of fatty liver on CT had significant correlation with elevation of ALT (p<0.05). 3) Correlation between the degree of fatty infiltration on liver biopsy and ALT: Liver biopsy was performed in 15 cases of which 14 cases revealed fatty liver. But one case had normal hepatic histology with severe obesity and normal ALT. Fourteen fatty liver cases on liver biopsy were classified into 3 groups by the degree of fatty infiltration and analysed with obesity index and ALT. The histologic hepatic steatosis had no significant correlation with obesity index (p>0.05), but significant correlation with ALT (p<0.05). 4) Correlation between CT and liver biopsy finding: Both CT and liver biopsy were performed in 15 cases of which 6 cases revealed normal finding on CT and 9 cases manifested fatty liver. There was significant correlation between CT and liver biopsy findings (r=0.6094). Conclusion: The results of our study suggest that abdominal CT and liver biopsy are useful and accurate methods of estimating fatty liver in the childhood obesity. But biochemical abnormalities of routine liver function tests dot not correlate well with severity of the fatty liver and liver injury.
Objectives : We investigated the effects of Ingecheonggeumdan(ICD) on body weight and examined whether blood lipid levels and visceral fat are inhibited by it in high fat diet-fed obese male mice. Methods : 8 weeks old, high fat diet-fed obese male mice were divided into 6 groups: C57BL/6N normal, control, ICD-1(150mg/kg), ICD-2(300mg/kg), ICD-3(600mg/kg) and orlistat(10mg/kg). After mice were treated with ICD and orlistat for 8 weeks, we measured body weight gain, food intake, feeding efficiency ratio, fat weight, plasma leptin and lipid levels. We also performed histological analysis for liver and fat on the mice. Results : Compared with controls, ICD and orlistat-treated mice had lower body weight gain and adipose tissue weight, the magnitudes of which were prominent in ICD-3. Compared with controls, ICD and orlistat-treated mice had lower blood leptin levels, the magnitude of which was prominent in ICD-3. Compared with controls, ICD and orlistat-treated mice had higher blood HDL-cholesterol and lower blood plasma LDL-cholesterol, free fatty acid and triglyceride levels, the magnitudes of which were prominent in ICD-3. Blood plasma AST and ALT concentrations were not changed by ICD and orlistat, indicating ICD and orlistat do not show any toxic effects. Consistent with their effects on body weight gain, the size of adipocytes and hepatic lipid accumulation were significantly decreased by ICD and orlistat. Conclusions : These results demonstrate that ICD and orlistat effectively reduce body weight gain, blood plasma LDL-cholesterol, free fatty acid and triglyceride levels and improves abdominal fat, the magnitudes of which were prominent in ICD-3.

In the elderly, a lower muscle mass and higher body fat mass are induced by a lower level of physical activity. A negatively changed body composition with an advanced age can lead to a higher falling risk and rate of diseases. On the other hand, aerobic-type exercise positively influences the body composition and hyperlipidemia in the elderly. Therefore, this study examined the effects of aquarobic training for 12 weeks on the body composition and blood lipid levels in obese old women. The subjects (n = 31, body fat: 33.42%) completed a 12 week water based aerobic training at 12 - 13 of Borg Scale of intensity (three times per week, each session: an hour). The body composition was measured by bioelectrical impedance analysis (Inbody 770-Biospace, Seoul, Korea) and the concentrations of blood lipids (high-density lipids cholesterol (HDL), low-density lipids cholesterol (LDL), triglyceride (TG) total cholesterol (TC)) were determined at pre and post training. A paired t-test was used for data analysis with ${\alpha}=0.05$. In the results, the body composition (% body fat (p < 0.05) and waist circumference (p < 0.05)) were reduced significantly. The LDL (p < 0.05), TG (p < 0.001) and TC (p < 0.05) were reduced significantly. In conclusion, a 12 week aquarobic exercise program helps improve the body composition and concentrations of serum lipids. Therefore, aquarobic exercise can enhance lipolysis using fat as energy to induce an improvement of the body composition and induce hyperlipidemia.

The purpose of this study was to evaluate the effects of low calorie meal substitute and weight control preparation containing dietary fibers, ${\alpha}$-amylase inhibitor and hydroxycitrate on the reduction of body weight and visceral fat in obese women. Sixteen pre-menoposal healthy women (age: 20∼50 y, body mass index >25) who were living in the Daejeon area participated in this study. We replaced one meal of the subject with low calorie meal substitute and fed the weight control preparation twice a day for 9 weeks. Anthropometric indices, body composition, dietary intake and stool movements were investigated every 3 weeks during the dietary intervention. The blood was collected before and after the dietary intervention. Results are as follows: 1) The subjects' body weight, body fat, BMI, waist, hip and abdominal adipose tissue decreased gradually and significantly between 3rd and 9th week after intervention. 2) The levels of fasting blood glucose, triglyceride and cholesterol, the indicators of liver dysfunction such as activities of ALT, AST and ALP, and bilirubin level were within a normal range and not affected significantly by dietary intervention. 3) Hemoglobin levels increased significantly and blood urea nitrogen level decreased. 4) Their stool movement was improved 5) Compared with the baseline values, calorie intake decreased by 17.5∼21.9% and the intakes of vitamin A, vitamin B2, folate, Ca, Fe, and Zn were below 80% of Korean RDA. In conclusion, the intake of low calorie meal substitute and weight control preparation could be effective in reduction of body weight and fat mass, improving the stool movement and the general physical symptoms.

The aims of this study were to evaluate obesity-related dietary behaviors and to determine long-term exercise effects on obesity and blood lipid profiles in elderly Korean subjects. A total of 120 subjects, aged 60-75 yr, were recruited, and obesity-related dietary behaviors were determined. An exercise intervention was conducted with 35 qualified elderly females for 6 months, and body composition and blood lipids were measured 6 times at 4 week intervals. At baseline, mean BMI ($kg/m^2$) was 24.8 for males and 23.1 for females. The females had better eating habits than the males and were more concerned with reading nutrition labels on food products (P < 0.001); they also preferred convenience foods less than the male subjects (P < 0.05). Obese individuals were more likely than overweight or normal weight individuals to misperceive their weight (P < 0.001). Those with a high BMI responded feeling more depressed (P < 0.01), lacking self-confidence (P < 0.01), and feeling isolated (P < 0.01) as well as having more difficulty doing outdoor activities (P < 0.01). After exercise, body fat (%) and WHR were significantly reduced (P < 0.05), while body weight and BMI were also decreased without statistical significance. Total cholesterol and blood HDL were significantly improved (207.1 mg/dl vs. 182.6 mg/dl, HDL: 45.6 mg/dl vs. 50.6 mg/dl, P < 0.05). Other benefits obtained from exercise were improvements in self-confidence (26.4%), movement (22.6%), stress-relief (18.9%), and depression (13.2%). In conclusion, elderly females had better eating habits and were more concerned with nutrition information and healthy diets compared to elderly males. However, misperceptions of weight and obesity-related stress tended to be very high in females who were overweight and obese, which can be a barrier to maintain normal weight. Long-term Danhak practice, a traditional Korean exercise, was effective at reducing body fat (%) and abdominal obesity, and improved lipid profiles, self-confidence, and stress.

This study is performed to evaluate the effectiveness of a weight management program for overweight and obesity office workers; comparing their changes in the anthropometry, biochemistry, nutrient intakes, and exercise performance before and after the obesity management program. This program was performed on 39 workers, who are overweight or obese, over BMI 23 for 6 months in Jeollabuk-do public institution. As the results of this study suggests, triceps thickness (TT) of male subjects was significantly decreased (P<0.05). Waist circumference (WC), and hip circumference (HC) of the total subjects were significantly decreased after the weight management program (P<0.05). The biochemical result of the Total cholesterol (TC) (P<0.05) and LDL cholesterol (LDL-C) (P<0.01) was significantly decreased in male subjects. The results of abdominal computed tomography (CT) also showed that subcutaneous fat area (SFA) of male subjects was significantly decreased after the weight management program (P<0.01). The nutrient intake's results of female subjects, who took Fe, Vit B2, and niacin, were increased significantly after the weight management program (P<0.05). The nutrition knowledge score of male subjects was significantly increased (P<0.05), and a muscular endurance of the physical test was significantly increased after the obesity management program (P<0.01). At the work place, workers need a systemic health and obesity management program to improve their work efficiency and quality of life. After this study, the program should be developed and supported continuously to maintain positive results from this study.

The study aimed to investigate the anti-obesity effects of 1% Rhus verniciflua vinegar (RV) supplementation in high-fat-diet (60% fat)-induced obese rats. A total of 50 4-wk-old male Sprague-Dawley rats were fed normal chow diet or maintained on high-fat diet (HFD) for 12 weeks to induce obesity and were then randomized into five groups as follows: normal diet+ultra-pure water (CON), HFD+ultra-pure water (OB-DW), HFD+1% acetic acid (OBAA), HFD+1% RV (OB-RV), and HFD+0.1% caffeine (OB-CF). AA was used as a control for RV, and caffeine was used as a positive control due to its weight reducing effect. After 2 months, body weight, organ and adipose tissue weights, serum lipids, hepatic lipids, adipocyte size, and cell number per spot level were analyzed. As a result, food efficiency ratio, abdominal adipose tissue weight, serum levels of total cholesterol, triacylglycerol, free fatty acids, coronary artery index, and fecal lipid were significantly reduced in the RV treatment group. In this study, we found that dietary RV improved obesity by increasing lipid excretion and reducing lipogenesis. These results suggest that RV has potential as a functional anti-obesity food.
In this study, the effect of Curcuma longa L. on obesity and insulin resistance was investigated in animals fed a moderate high fat diet. The animals used in this study were normal weight Spargue-Dawley (SD) rats and type 2 diabetic obese db/db mice. Accumulation of abdominal adipose tissue and weight gain were inhibited in the animals fed the C. longa extract. C. longa decreased fasting insulin and HOMA-IR in the SD rats, and effectively decreased blood glucose and hemoglobin A1c in db/db mice. C. longa decreased serum free fatty acid (FFA) level in the SD rats. FFA in db/db mice fed C. longa tended to decrease. C. longa significantly decreased serum triglyceride level. Our results collectively represent that C. longa prevented fat accumulation and insulin resistance in both normal weight SD rats and type 2 diabetic obese db/db mice fed a moderate high fat diet.
In order to identify an Convergence aerobic exercise's influence on obese female college students' Cardiorespiratory systems and arterial pulse wave Velocity, this study targeted female college students in D University in Y-gu, D-City, and selected total 26 ones having body fat percentage over 30% with no special disease in past and at present and no regular physical activity. After finally confirming their participation in this experiment by completely explaining the purpose of and contents of this experiment and getting the subjects' written contents to participate in the experiment, the investigator randomly arranged the subjects into the exercise group(N=13) and the control group(N=13) and conducted a 12-week aerobic exercise program to the exercise group this study got the following conclusion. First, concerning the body composition's change, the exercise group showed significant reduction in the weight, the body fat percentage, and the abdominal fat percentage. Second, for the respiratory & cardiovascular systems'change, the exercise group showed significant increase in the maximal oxygen uptake, the maximal heart rate, and the maximal breathing capacity. Third, for the arterial pulse wave Velocity's change, the exercise group showed significant reduction in the upper body(right and left hands) and lower body (right and left feet).
