Evaluation of Waist Circumference Cut-off Values as a Marker for Fatty Liver among Japanese Workers

  • Objectives: Metabolic syndrome has received attention as a risk factor for cardiovascular disease, with particular importance attached to visceral fat accumulation, which is associated with lifestyle-related diseases and is strongly correlated with waist circumference. In this study, our aim is to propose waist circumference cut-off values that can be used as a marker for fatty liver based on a sample of workers receiving health checkups in Japan. Methods: This study was conducted in a total of 21,866 workers who underwent periodic health checkups between January 2007 and December 2007. The mean age of the subjects was 47.4 years for men (standard deviation [SD]: 8.0) and 44.7 years for women (SD: 6.9). Evaluation included abdominal ultrasound and measurement of waist circumference, body mass index, fasting blood glucose, triglycerides, high-density lipoprotein cholesterol, and blood pressure. Results: Based on receiver operating characteristic curve analysis, the optimal waist circumference cut-off values were shown as 85.0 cm (sensitivity 0.72, specificity 0.69) for men and 80.0 cm (sensitivity 0.75, specificity 0.78) for women. Conclusion: Abdominal ultrasound is the most efficient means of diagnosing fatty liver, but this examination seldom occurs because the test is not routinely performed at workers' health checkups. In people found to have a high risk of fatty liver, recommendations can be made for abdominal ultrasound based on the waist circumference cut-off values obtained in this study. That is, waist circumference can be used in high risk individuals as an effective marker for early detection of fatty liver.

  • The relationship of abdominal obesity ("AO") with co-morbidity and mortality is well established. This study assessed the factors associated with AO, which was defined as a waist circumference ≥90 cm for men and ≥85 cm for women, in the group aged over 65 years. A total of 1,435 subjects were analyzed among the participants of the 2014 Korean National Health and Nutrition Examination Survey. Statistical methods for a complex sample were applied by using a SPSS program (ver. 25.0). AO was more frequently found in females. The results of the logistic regression analysis showed that heavy drinking (OR: 1.53), no weight training (OR: 0.68), stressful mental status (OR: 0.61), bad health-related quality of life (by EQ_5D, OR: 1.45), hypertension (OR: 2.18), prediabetes (OR: 1.94), diabetes (OR: 1.63), low HDL-cholesterol (OR: 1.86), anemia (OR: 0.55), and heavy energy intake (OR: 1.41) were significantly related with the prevalence of AO after adjustment for gender. Heavy drinking (OR: 1.89), bad self-rating of health status (OR: 1.72), low HDL-cholesterol (OR: 1.85), heavy energy intake (OR: 1.79), low intake of riboflavin (OR: 1.60) were still significantly related with the prevalence of AO after adjustment for gender and body mass index (BMI), this study suggests that certain characteristics of health habits, mental health status, and chronic diseases may be associated with AO. This study did not establish the existence of relationship between nutrient intakes, except for riboflavin, and risk of AO, but this study suggests that prospective research is needed to establish causal connections among those factors.

Prevalence of hyperuricemia and its association with metabolic syndrome and cardiometabolic risk factors in Korean children and adolescents: analysis based on the 2016-2017 Korea National Health and Nutrition Examination Survey

  • Purpose: Investigating the prevalence of hyperuricemia and its association with metabolic syndrome (MetS) and cardiometabolic risk factors (CMRFs) in Korean children and adolescents. Methods: This cross-sectional survey used data from the 7th Korea National Health and Nutrition Examination Survey (2016-2017); 1,256 males and females aged 10-18 years were included. Hyperuricemia was defined as serum uric acid levels were >6.6 mg/dL at 10-11 years of age (both sexes), >7.7 mg/dL for males at 12-18 years of age and >5.7 mg/dL for females at 12-18 years of age. MetS was defined by the International Diabetes Federation criteria. Logistic regression analysis was used to analyze hyperuricemia-associated risk factors. Results: The prevalence of hyperuricemia was 9.4% (male, 8.4%; female, 10.5%) (P<0.281). After adjusting for sociodemographic factors and health behaviors in multivariate analysis (model 1), the odds ratio (OR) for hyperuricemia of MetS was 3.05 (95% confidence interval [CI], 1.17-7.92; P=0.022). After adjusting for the same variables in model 1 plus obesity and all MetS components (model 2), only abdominal obesity was significant, and the OR for hyperuricemia was 3.38 (95% CI, 1.72-6.63; P<0.001) After adjusting for the same variables in model 1 plus body mass index (BMI) z scores and all MetS components except abdominal obesity (model 3), only BMI z scores was significant, and the OR for hyperuricemia was 1.59 (95% CI, 1.34-1.89; P<0.001). Conclusion: MetS, abdominal obesity, and BMI z scores were CMRFs significantly associated with hyperuricemia in Korean children and adolescents. Therefore, attention should be paid to hyperuricemia in patients with obesity or MetS.

A Case of Advanced Gastric Cancer with Multiple Leptomeningeal Metastasis (진행성 위암의 추적 관찰 도중 다발성 수막내 전이가 발견된 환자 1례)

  • Leptomeningeal carcinomatosis occurs in approximately 5% of patients with cancer. The most common cancers involving the leptomeninges are breast, lung cancer and melanoma. However, gastric adenocarcinoma has been rarely reported with leptomeningeal carcinomatosis. The presenting manifestations are usually headache, visual disturbances and seizures. We report a case of leptomeningeal metastasis that presented as a gastric cancer. A 75-year old man was transferred to our hospital for further evaluation and treatment after being diagnosed with adenocarcinoma through endoscopic biopsy during a regular health examination. An abdominal computed tomography (CT) showed AGC, stage IA (cT1N0M0), while an endoscopic examination showed AGC, Borrmann type 2. The patient is currently under observation after undergoing radical subtotal gastrectomy with gastroduodenostomy and subsequent administration of oral chemotherapeutic agents. As an abdominal CT response assessment performed after surgery revealed new metastasis to the liver, the patient received palliative chemotherapy as recurrence was suspected. After receiving chemotherapy in the order of DP (Cisplatin + Docetaxel), FOLFIRI (5-FU + Leucovorin + Irinotecan), an abdominal CT response assessment showed complete response. Since decreased mentality maintained throughout the follow up period based on outpatient clinic, brain MRI was performed and revealed multiple leptomeningeal metastasis. The Patient died 2 days after the diagnosis.

A Canine Lymphoma Patient Diagnosed by the Cytologic Examination of Transudative Ascites (누출액성 복수의 세포학적 검사로 진단된 림프종 1예)

  • A 12-year old neutered male Yorkshire terrier dog was presented to the Veterinary Medical Teaching Hospital of Seoul National University with a history of chronic intermittent diarrhea, vomiting, anorexia and weight loss of 2-months duration. On presentation, he was very cachexic and had ascites. Abnormal findings on a complete blood count and chemistry profile included mild anemia, leukocytosis, panhypoproteinemia, hypocholesterolemia, decreased blood urea nitrogen (BUN) and increased serum bile acids. Radiographic findings indicated microhepatica. Peritoneal fluid analysis was consistent with transudates (total protein < 2.5 g/dl, total nucleated cell count = 2,200/ul) and cytologic examination of the fluid revealed neoplastic lymphoblasts. From these findings hepatic dysfunction and protein-losing enteropathy were attributable to abdominal lymphoma. This case suggests that cytologic examination is important in diagnosing underlying diseases of ascites, even if it is transudative effusion.

Abrupt Hard Mass Caused by the Push of an Ice Cream Stick after Gastric Perforation in a Dog

  • A neutered male Maltese, 11-year-old, presented for hard mass at right flank suddenly. Patient vomited and had anorexia before the presentation, but it is improved after. On blood tests, there were no remarkable findings. On physical examination, firm mass ($2.9{\times}2.6cm$, firm) was detected. No remarkable finding was shown in radiography except for right upper-medial abdominal subcutaneous soft tissue mass. Punch biopsy was performed for histopathologic examination. During the punch biopsy, ice-cream stick ($11.5{\times}1.2cm$) appeared from the hole on right flank. Additionally gastric perforation was detected on ultrasonography. Emergency surgery was performed for the perforation. On histiopathologic examination of the mass, marked, diffuse, neutrophilic and mild eosinophilic dermatitis/cellulitis with no infectious agents was observed.

Prevalence and Related Factors of Metabolic Syndrome among Korean Older Adults (건강검진 수진 노인의 대사증후군 유병상태 및 관련 요인)

  • Objectives: The purpose of this study is to identify prevalence and related factors of the elderly, who took health examination, with metabolic syndrome. Methods: The health examination and lifestyle survey were performed for 21,512 adults at 60 years of age or older who took health examination in H health promotion center during January-March 2009. Results: The prevalence of metabolic syndrome for the subject was 24.0%. Of the subject with metabolic syndrome, the prevalence of the diseases was obesity 60%, abdominal obesity 78.5%, hypertension 82.6%, dyslipidemia 89.7% and diabetes 51.9%. In comparison of the relationship between metabolic syndrome and other diseases, the male subject with metabolic syndrome were significantly higher in BMI, waist circumference, systolic/diastolic blood pressure, hemoglobin, AST, ALT, $\gamma$-GTP, TG, AC glucose, creatinine than normal male(p<0.001). In comparison of the relationship between metabolic syndrome and lifestyle, more drinking frequency and amount in male and more drinking frequency in female were associated with increased risk of metabolic syndrome(p<0.01). Regardless of exercise intensity, practice of exercise contributed to reduce the risk of metabolic syndrome(p<0.01). Conclusion: In conclusion, TLC program, focused on lifestyle behaviors which is strongly associated with the prevalence of metabolic syndrome, should be developed for the improvement of life quality in the elderly with metabolic syndrome.

Characteristics of Acupuncture at LU10 on Abdominal Thermography of Health Subject (어제(魚際)(LU10) 자침(刺鍼)이 복부(腹部) 한열변화(寒熱變化)에 미치는 영향(影響))

  • This study was examined for characteristics of acupuncture of LU10 on the abdominal thermography of health subject. The volunteers who participating in this study had taken rest for 20 - 30 mins in room temperature $(23-25^{\circ}C)$ before the examination and informed them what to prohibit smoking, drinking and administration of drug for the previous day. The thermography of abdomen including a below part of the chest was taken using Infra-Red Imaging System (IR 2000, MEDI-CORE Co., Korea) by time interval of 15 minutes at 15 min before, just before and 15 min after, 30 min after and 45 min after acupuncture stimulation. Acupuncture was applied to both LU10 for 30 mins. The results showed that acupuncture of LU10 for 30 mins had more potencies of changes on all the ROIs of abdominal thermography than those of control group. Also, those responses were significantly clear at the A1, A3, A6, A7 and A9 areas. It was observed that the quantities of thermal changes following acupuncture of LU10 been increased significantly comparing that of control group at the all ROIs (region of interest). These effects have more potencies at the upper (A1 and A2) and below (A7, A8, A9) abdominal areas than midline ones (A4, A5, and A6). These results suggest that acupuncture of LU10 having the characteristics of controls of chill and fever (寒熱) may modulate thermal distributions and changes of abdominal areas including the below of chest.

Simultaneous Surgery on Jejunum perforation with Pelvic Ring Fracture: A Case Report

  • Patients with pelvic bone fractures with gastrointestinal perforations are reported in 4.4% of the cases and in very rare cases jejunum (0.15) is involved. However, intestinal perforations are often undiagnosed on the first examination before peritonitis is evident. We are presenting a report where a patient with anteroposterior compression injury, who was expected to undergo an internal fixation procedure, did not show any jejunum perforations on abdominal CT or other physical exams but was found on abdominal CT 1 week after right before surgery, therefore excision and anastomosis surgery, pelvic open reduction and internal fixation was simultaneously done with favorable results. In our case, we present a 61 year old male patient with liver trauma, adhesion at the abdominal cavity, with a past history of gallbladder excision, but without abdominal pain, fever, or infection symptoms. Therefore, this was a case that was difficult to initially diagnose the patient with jejunum perforation and peritonitis. The diagnosis was further supported during laparotomy when peritonitis around the area of intestinal perforation was observed. Generally, it is understood that pelvic bone fracture surgery is not immediately done on patients with peritonitis. However, this kind of patient who had peritonitis with intestinal adhesion and other complications could undergo surgery immediately as infection or other related symptoms did not coexist and the patient was rather stable, and as a result the treatment was successful.

Effects of Zusanli (ST36) Acupuncture on Abdominal Thermography of Health Subject (족삼리(足三里) (ST36) 자침(刺鍼)이 복부(腹部) 한열변화(寒熱變化)에 미치는 영향(影響))

Observed the thermography classified by ROI, however, it was failed that acupuncture of ST36 could modulate the specific areas concerning to the abdominal pathway of Stomach Meridian. These results suggest that acupuncture of ST36 may modulate thermal distributions and changes of abdominal areas including the below of chest.
