Successful Outcome of Breast Cancer Patient Refusing Conventional Treatments: A Case Report

  • Background: Breast cancer is a treatable disease, but some women reject conventional treatment in favor of unproven "alternative therapies," which may have serious implications for their survival. Therefore, a process is needed to lead them to more appropriate treatment choices. Case presentation: Here, we present the case of a 51-year-old Korean female diagnosed with early-stage breast cancer (stage IIB, T2N1M0) in Nov. 2015. She refused a standard surgical resection together with chemotherapy and opted instead for moxibustion by nonmedical personnel. Consequently, her preference for alternative therapy without conventional treatment exacerbated her disease. Just a little over a year later, integrative cancer treatment, including chemotherapy based on histological founding, and complementary treatment, comprised of acupuncture, moxibustion, and herbal medicine, were administered for 5 months. Finally, she successfully underwent modified radical mastectomy showing a pathological complete response. She received only adjuvant chemotherapy without any alternative medicine afterwards, and she maintained a good status without recurrence. Conclusion: In the case of breast cancer patients who are resistant to surgery and chemotherapy, integrative therapy considering adverse effects from conventional treatment should be preferred to bitter opposition to alternative medicine.

  • We report an unusual case of a sigmoid colon perforation after ventriculoperitoneal shunt surgery. Distal catheters are known to cause perforation in the setting of colonoscopy. The exact pathogenesis of this complication is not clear, but it can cause serious complications. Hence, patients require prompt and aggressive management, including laparotomy with bowel wall repair, catheter removal, and antibiotic therapy.

  • Joints created with concrete construction result in serious weakness from the aspects of both structural and water-barrier function. Several case studies for the inspection of construction joints of column in concrete structure using radar. An analysis method based on radar signal processing using convolution technique is carried out with various patterns of joints in concrete. The computed results were verified by comparing the test results. As a result, radar signal analysis proposed in this study has a possibility of estimating patterns of joints with good accuracy.

  • An instance of domestic exhibition facility building case, the shortage of managing personal in construction resulted serious issue on expenses, times, quality, and safty with no satisfaction on its process and result. According to subject's speciality the CM's application methods are varied and because of its speciality, overall process of building an exhibition facility operating by its owner.   • Neonatal tension pneumopericardium is a serious disease that requires prompt diagnosis and treatment. If untreated, it may lead to cardiac tamponade and death. We report a case of neonatal tension pneumopericardium which was successfully treated by open pericardial window operation with review of literatures.

  • Percutaneous epidural neuroplasty (PEN) is a known interventional technique for the management of spinal pain. As with any procedures, PEN is associated with complications ranging from mild to more serious ones. We present a case of intracranial subdural hematoma after PEN requiring surgical evacuation. We review the relevant literature and discuss possible complications of PEN and patholophysiology of intracranial subdural hematoma after PEN.

  • Percutaneous first annular pulley (A1 pulley) release, which has been increasingly used to treat trigger fingers, has been widely established as a safe and simple procedure. Multiple studies have reported positive results of percutaneous A1 pulley release. In this study, however, we report cases of patients who developed complications after undergoing percutaneous A1 pulley release at local clinics. A total of six patients visited our hospital for infectious complications after percutaneous A1 pulley release. Various sequelae such as damage to normal structures, insufficient procedure, and tissue necrosis were observed during the exploration. A retrospective study was conducted to identify the cause and trend of the observed complications by instruments (HAKI knife or needle). In the HAKI knife group, there was a tendency for damage to normal structures, while in the needle group, an insufficient release or serious soft tissue necrosis was observed. Based on these cases, our findings confirm the existence and characteristics of infectious complications following the percutaneous A1 pulley release. We further identify that the type of instrument used predicts the nature of complications. Thus, reliable and skilled performance of the procedure by experts is essential for safe treatment.

  • Objective : We considered Astragali Radix(黃耆) can be used to cure serious atopic dermatitis. so we treated this patient with prescriptions contained Astragali Radix(黃耆). By this case, we got effective results and report this case and index of choice Astragali Radix(黃耆) Method : After treatment with prescription contained Astragali Radix(黃耆), we observe the changes and analyze results. Results : 1. Atopic dermatitis can be cured with Astragali Radix's efficiency on Sanghanron(傷寒論) 2. We considered Astragali Radix(黃耆) can be used when Chronic damage to the nasal mucosa is appeared by rhinoscopy and Sneezing, runny nose that lasts all year round is shown Conclusion : Gaeji-Ka-Hwangki-Tang(桂枝加黃耆湯) has most effect on this case. So, We need to study combination Astragali Radix(黃耆) with Glycyrrhizae Radix(甘草) and the efficacy of herbs belonged to leguminosae.

Junctional rhythm with severe hypotension following infiltration of lidocaine containing epinephrine during dental surgery

  • We experienced an unusual case of accelerated junctional rhythm with severe hypotension after infiltration of lidocaine containing epinephrine during dental surgery under general anesthesia. The patient's electrocardiogram exhibited retrograde P-waves following the QRS complex, which could be misinterpreted as ST-segment depression. As a temporary measure, administration of ephedrine restored the patient's blood pressure to normal levels. The importance of this case lies in its demonstration of an unexpected and serious side effect of commonly used epinephrine infiltration. This case also highlights the need for accurate interpretation of the electrocardiogram and comprehensive understanding of best practices for patient management.

  • Treatment of huge chronic tuberculous empyema with cardiopulmonary dysfunction. Drainage of empyemal space by closed thoracostomy in chronic tuberculous empyema is generally contraindicated because of the possibility of empyema necessitatis and ascending infection. But in case that serious cardiopulmonary dysfunction is present, drainage of empyema and decompression is necessary. We experienced a case in which chronic tuberculous empyema was big enough to cause mediastinal shifting and cardiopulmonary failure. Immediate drainage of pleural cavity with tube thoracostomy was performed. Afterward, pleuropneumonectomy was done following cyclic irrigation for one month. The patient had successful postoperative course without any evidence of complication or relapse of infection.
