Traditional Korean Medicine Doctors' Awareness and Utilization of the Case Report (한의사의 증례연구에 대한 인식 및 활용)

  • Objectives : The purpose of this study is to assess Traditional Korean Medicine (TKM) doctors' awareness and utilization of the case report, based on the opinion that careful observation on the case sometimes provides us with the important information as clinical trial especially in the traditional medicine research field. Methods : A questionnaire study was conducted among TKM doctors who participated in the annual continuous maintenance education (CME) held at 5 regions of the Republic of Korea. Results : Almost 60% of the respondents had read case reports published in medical journals and 67% had openly shared their clinical cases with their colleagues. Of the respondents, 18.6% had been educated on reporting cases, and only 16% had the experience of reporting cases on one's own. However, 32.6% of the respondents had the intention to report cases in the future. These results show significant differences between general physicians who holds a license but no hospital training experience and board certified TKM doctors who have training experience. Conclusions : A majority of TKM doctors have read case reports but holds little experience of having been properly trained. Through this research, it has been found that awareness of case reports is rising in hospital training. Thus, the objective of case report education for TKM doctors who have hospital training experience should be set on encouraging them to do more whereas for doctors without hospital training experience, the objective should be set on making them more exposed to case reports to heighten one's awareness.

Angiofollicular Lymph Node Hyperplasia in the Right Hilum - report of a case - (우폐문부에 발생한 Angiofollicular Lymph Node Hyperplasia -1례 보고-)

  • Angiofollicular lymph node hyperplasia is a relatively rare benign lesion. It develops most often in the mediastinum, but also it found in other area of the body, usually where lymph nodes are normally found. There are histologically divided hyaline vascular type, plasma cell type and transitional type. The lesions were discovered most often on routine roentgenograms, and complete surgical excision is the treatment of choice. Recently, the authors experienced a case of angiofollicular lymph node hyperplasia, transitional type, located in the right hilar area. The lesion was removed and the patient is in good condition postoperatively. Now we report this case with review of previously reported literatures.

  • Gliosarcoma is a rare malignant tumor of the central nervous system consist of gliomatous and sarcomatous elements. The authors report a case of gliosarcoma in the right frontal lobe. Sharp demarcation of the tumor from surrounding tissue and its superficial location may lead to complete removal and prolonged survival despite high malignancy.

  • Polypoid lesions of the esophagus occur infrequently and may be benign or malignant. Pseudosarcoma, a malignant polypoid tumor of the esophagus is rare and appears to be a distinct pathological entity; the polypoid portion is composed of sarcomatous spindle cells and the base of the polyp shows in situ or invasive squamous cell carcinoma. We experienced a case of pseudosarcoma of the esophagus and report the case with the review of literature.

  • This is a case report of extralobar pulmonary sequestration with bronchogenic cyst located in aorto-pulmonary window area of left thorax in a 17 months old male patient. In this case, a bronchogenic cyst was presented immediately adjacent to the extralobar pulmonary sequestration. Communication between the extralobar pulmonary sequestration and bronchogenic cyst was not seen grossly or microscopically. The lesion was simply excised and the postoperatively course was uneventful.

Recovery of complete left bundle branch block in dilated cardiomyopathy after optimal heart failure treatment: a case report

  • Complete left bundle branch block (CLBBB) is a significant cardiac conduction abnormality often associated with dilated cardiomyopathy (DCM). This case report highlights the improvement in CLBBB and symptom relief through reverse cardiac remodeling in a patient diagnosed with DCM following an optimized heart failure treatment regimen consisting of an angiotensin-converting enzyme inhibitor, beta-blocker, and mineralocorticoid receptor antagonist. This case highlights the potential of electrical remodeling and conduction system improvement in patients with DCM receiving optimized medical therapy.

  • We report an unusual case of respiratory depression and prolonged apnea after the second dose of 100-mg intravenous tramadol. Due to continuous pain at foley catheter keep site, intravenous tramadol was administered to the patient. Soon after the second dose of tramadol injection, the patient became apneic. The patient did not respond to verbal command and started exhibiting oxygen desaturation. The patient was quickly treated with 100% oxygen, and it took 4 hours for the spontaneous respiration to return to regular. This case report demonstrates that even two doses of tramadol administered intravenously may manifest as sudden and prolonged apnea. Respiratory depression with tramadol has been reported in patients with impaired renal functions and Cytochrome P(CYP) 2D6 gene duplication.

Occupational Therapy Intervention for Malunion of Distal Radial Fracture: A Case Report

  • Objective: Malunion of distal radial fracture can result in pain, stiffness, and decreased function of the affected limb. Occupational therapy intervention can improve outcomes in these patients, but there is limited research on the effectiveness of such interventions. Case Description: This case report describes the occupational therapy intervention and outcomes for a 44-year-old female patient with malunion of left distal radial fracture. The patient received 8 weeks of occupational therapy intervention consisting of therapeutic exercises, joint mobilization, and fine motor activities. The patient demonstrated significant improvements in range of motion, grip strength, and activities of daily living (ADL) in her left hand. The patient also reported less pain and greater ease in performing activities of daily living. Conclusion: Occupational therapy intervention can be effective in improving outcomes for patients with malunion of distal radial fracture. This case report highlights the importance of early intervention and interdisciplinary collaboration among healthcare professionals to optimize patient outcomes. Further research is needed to explore the effectiveness of occupational therapy interventions for patients with malunion of distal radial fracture.

A Case Report of Korean Medicine Therapy including Duhuo Jisheng Tang and Pharmacoacupuncture for a Chronic Inflammatory Demyelinating Polyneuropathy Patient Complaining Weakness and Numbness in Lower Extremity (하지 위약감 및 저림을 호소하는 만성 염증성 탈수초성 다발신경병증 환자에 대한 독활기생탕과 약침을 포함한 한의복합치료 증례보고 1례)

  • This case study aims to report Korean medicine treatment's response to weakness and numbness in chronic inflammatory demyelinating polyneuropathy (CIDP) patient. The patient received Korean medicine treatment during hospitalization, including Duhuo Jisheng Tang and pharmacoacupuncture. The assessment was performed using the Functional Independence Measure (FIM), Numeric Rating Scale (NRS) for numbness, and Oswestry Disability Index (ODI). After 21 days of treatment, the FIM improved from 82 to 126, NRS improved from 6 to 2, and ODI improved from 37 to 8. There were no side effects after receiving Korean medicine. This case suggests that Korean medicine treatment can induce treatment response for lower extremity weakness and numbness in CIDP patients.
