Tenosynovial Giant Cell Tumor Showing Severe Bone Erosion in the Finger: Case Report and Review of the Imaging Findings and Their Significance

  • We report a case of tenosynovial giant cell tumor with severe bone erosion in the right fifth finger of a 46-year-old man. Throughout this case review, we describe the imaging findings of tenosynovial giant cell tumor with severe bone erosion and review the literatures regarding osseous lesions caused by tenosynovial giant cell tumor and their significance related to the differential diagnosis and patient treatment.

A Case Report for Adult Acne treated by Daesiho-tang based on Shanghanlun provisions with Extrusion therapy (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 따른 대시호탕(大柴胡湯) 투여와 압출치료를 병행하여 호전된 성인 여드름 증례보고)

  • Objectives : The purpose of this case report is to report the effect of Daesiho-tang on adult acne by Shanghanlun provisions with extrusion therapy. Methods : According to diagnostic system based on Shanghanlun provisions, the patient was diagnosed with Taeyang-byung Gyeolhyung and treated by Daesiho-tang with extrusion therapy. The result of administrations were evaluated by Korean Acne Grading System(KAGS), Korean Skindex-29, and change of clinical symptoms. Results : After administration of Daesiho-tang for 150 days with weelky extrusion therapy, KAGS improved from grade 4 to grade 1, Skindex-29 improved from 67.93 to 15.51, and patient was very satisfied with the treatment. No serius treatment-related adverse events were reported. Limitations : This case is only one case that the level of evidence is weak. Two interventions were used together and the duration of photo was uneven. Conclusions : The patient almost recovered from acne treated by Daesiho-tang according to diagnostic system based on Shanghanlun provisions with extrusion therapy. This is the first case report that suggests '熱結在裏' on the number 136 provision of Shanghanlun means fever that is always present in chronic skin lesions.

  • This is a case report of hemangiopericytoma on lower lip. Hemangiopericytomas are very rare tumors of vascular origin featuring pericytes distributed about vascular spaces. Approximately one third occur in the head and neck, but the oral hemangiopericytomas are very rare and occur on tongue, mandible, maxilla and lip etc. The histogenesis is uncertain and there is no feature to distinguish the benign from the malignant variants. Complete excision is mandatory to avoid recurrence. A review of the literature and a report of a case of the oral hemangiopericytoma is presented.

Case Report for a Refractory Recurrent Aphthous Stomatitis Treated with Jibaekpalmi-hwon

  • Objectives: This report reports a clinical case of a patient with recurrent aphthous stomatitis (RAS) for 10 years who was completely cured using Korean medicine. Methods: A male patient with terribly severe RAS was hospitalized, and then treated with Jibaekpalmi-hwon, indirect moxibustion (KI 1 and CV 4) and acupuncture. The clinical outcome was observed by self-reported VAS measurement and macroscopy. Results: The severity of stomatitis became improved gradually and completely recovered during 17 hospital days, and RAS has not recurred for three months as an outpatient. The blood test including complement activity was in the normal range. Conclusions: This case report provides information about a therapeutic effect of Jibaekpalmi-hwon and Korean medicine on RAS.

  • Pleomorphic adenoma is the most common intraoral salivary gland tumor and characteristically presents as slow painless growth. The cheek are rarely affected site of all pleomorphic adenoma arising from minor salivary glands. In this report, we present the case of a patient with pleomorphic adenoma that affects the cheek. The purpose of this paper is to report this case and review the features of the pleomorphic adenoma occuring in this site.

  • Lymphangiohemangiomas of the mediastinum are exceedingly rare and few cases have been published in the English literature. This report may be the only reported case in which lymphangiohemangiomas were found bilaterally. We report a case of a 7-year-old boy with an incidental finding of an abnormal mediastinal shadow on a chest X-ray. The chest CT showed a large mass in the left superior mediastinum and another in the right posterior mediastinum. The left mass had anomalous venous channels connected to the left innominate vein, and the right mass to the left atrium. We performed an excision of the mass in the left side first and then the right side one month later. Anomalous venous channels were dissected carefully and ligated. There were no complications and no signs of recurrence 30 months after the operation.

  • Chylothorax after thoracic surgery is a rare complication, and treatment for refractory chylothorax is challenging. We report a case of chylothorax after cardiothoracic surgery in an infant after failure of conservative management and thoracic duct ligation. The patient underwent chemical pleurodesis with a Viscum album extract. The treatment was successful and chylothorax did not recur.

Uterine leiomyosarcoma in a captive female capybara (Hydrochoerus hydrochaeris): a case report

  • A captive female capybara (Hydrochoerus hydrochaeris) of unknown age discharged a bloody mass from the vaginal region. A histopathology examination revealed the mass to be a reproductive leiomyosarcoma, and an ovariohysterectomy was performed. The histopathology examination confirmed that the excised tissue was a uterine leiomyosarcoma. The purpose of this report is to describe clinical history and histopathological diagnosis of leiomyosarcoma in capybaras. This report is novel because it describes the first diagnosis of uterine leiomyosarcoma in a capybara. Since clinical data about capybaras are rare, this case report will help to diagnosis and treat reproductive diseases of this species.

A Case Report of Traditional Korean Medicine Treatment for Broca's Aphasia Associated with Cerebral Hemorrhage after Middle Cerebral Artery Infarction (중대뇌동맥 뇌경색 이후 발생된 뇌출혈에 동반된 브로카실어증(Broca aphasia) 환자에 대한 한의치료 증례보고 1례)

  • Objective: The aim of this case report was to present the effects of traditional Korean medicine on a patient with Broca's aphasia associated with cerebral hemorrhage after middle cerebral artery infarction. Methods: The Korean version of the Western Aphasia Battery (K-WAB) and the Evaluation of Articulator Performance were used to evaluate the language functions of the patient. Herbal medication and acupuncture were administered to improve the patient's symptoms. Results: The inpatient and outpatient treatments improved the patient's symptoms. The aphasia quotient (AQ) score of the K-WAB test before treatment was 6.4, but it increased to 21.4 after treatment. The Evaluation of Articulator Performance score improved by 0.0 points before treatment, by 1.0 points after 9 days of treatment, and by 1.5 points after 42 days of treatment. Conclusions: This case report suggest that Korean medical therapy can be effective in improving the language functions of patients with Broca's aphasia.

A Case Report of Rhabdomyolysis Patient Treated with Traditional Korean Medicine (한방치료로 호전된 횡문근 융해증 환자 치험 1례)

  • Objectives : The purpose of this study is to report a case of traditional Korean Medicine effect on a patient with rhabdomyolysis who appealed leg pain, numbness and weakness. Methods : A patient received traditional Korean Medicine including acupuncture, herbal medicine, moxibustion, and cupping therapy daily. We evaluated the clinical results by observing the patient's symptoms, NRS(Numerical Rating Scale), MMT(Manual Muscle Test) grade and DITI(Digital Infrared Thermal Imaging). Results : After traditional Korean Medicine, the patient's pain and numbness decreased from NRS 10 to 1~2 and weakness improved from Gr3- to Gr4. And CPK(creatine phosphokinase) numerical value decreased from 330(IU/L) to 164 and body heat status improved. Conclusions : Traditional Korean Medicine can be effective in reducing rhabdomyolysis patients' symptoms and changing examination numerical value. And further studies on traditional Korean Medicine of rhabdomyolysis should be carried out.
