Effect of the shades of background substructures on the overall color of zirconia-based all-ceramic crowns

  • PURPOSE. The objective of this study was to determine the effect of the color of a background substructure on the overall color of a zirconia-based all-ceramic crown. MATERIALS AND METHODS. Twenty one posterior zirconia crowns were made for twenty subjects. Seven premolar crowns and six molar crowns were cemented onto abutments with metal post and core in the first and second group. In the third group, eight molar crowns were cemented onto abutments with a prefabricated post and composite core build-up. The color measurements of all-ceramic crowns were made before try-in, before and after cementation. A repeated measure ANOVA was used for a statistical analysis of a color change of all-ceramic crowns at ${\alpha}$=.05. Twenty four zirconia specimens, with different core thicknesses (0.4-1 mm) were also prepared to obtain the contrast ratio of zirconia materials after veneering. RESULTS. $L^*$, $a^*$, and $b^*$ values of all-ceramic crowns cemented either on a metal cast post and core or on a prefabricated post did not show significant changes (P>.05). However, the slight color changes of zirconia crowns were detected and represented by ${\Delta}E{^*}_{ab}$ values, ranging from 1.2 to 3.1. The contrast ratios of zirconia specimens were 0.92-0.95 after veneering. CONCLUSION. No significant differences were observed between the $L^*$, $a^*$, and $b^*$ values of zirconia crowns cemented either on a metal cast post and core or a prefabricated post and composite core. However, the color of a background substructure could affect the overall color of posterior zirconia restorations with clinically recommended core thickness according to ${\Delta}E{^*}_{ab}$ values.

Fracture resistances of zirconia, cast Ni-Cr, and fiber-glass composite posts under all-ceramic crowns in endodontically treated premolars

  • PURPOSE. The aim of the present study was to evaluate the fracture resistances of zirconia, cast nickel-chromium alloy (Ni-Cr), and fiber-composite post systems under all-ceramic crowns in endodontically treated mandibular first premolars. MATERIALS AND METHODS. A total of 36 extracted human mandibular premolars were selected, subjected to standard endodontic treatment, and divided into three groups (n=12) as follows: cast Ni-Cr post-and-core, one-piece custom-milled zirconia post-and-core, and prefabricated fiber-glass post with composite resin core. Each specimen had an all-ceramic crown with zirconia coping and was then loaded to failure using a universal testing machine at a cross-head speed of 0.5 mm/min, at an angle of 45 degrees to the long axis of the roots. Fracture resistance and modes of failure were analyzed. The significance of the results was assessed using analysis of variance (ANOVA) and Tukey honest significance difference (HSD) tests (${\alpha}=.05$). RESULTS. Fiber-glass posts with composite cores showed the highest fracture resistance values ($915.70{\pm}323N$), and the zirconia post system showed the lowest resistance ($435.34{\pm}220N$). The corresponding mean value for the Ni-Cr casting post and cores was reported as $780.59{\pm}270N$. The differences among the groups were statistically significant (P<.05) for the zirconia group, as tested by ANOVA and Tukey HSD tests. CONCLUSION. The fracture resistance of zirconia post-and-core systems was found to be significantly lower than those of fiber-glass and cast Ni-Cr post systems. Moreover, catastrophic and non-restorable fractures were more prevalent in teeth restored by zirconia posts.

FRACTURE STRENGTH AND FRACTURE MODE OF RESIN ROOT ANALOGS RESTORED WITH VARIOUS POST AND CORE MATERIALS

  • Statement of Problem. Endodontically treated teeth frequently required posts and cores to provide retention and resistance form for crowns. In spite of excellent mechanical properties of metal post and core, its metallic color can be detected through all ceramic restorations occasionally. To solve esthetic problems of metal post and core zirconia post system has been introduced recently. Purpose. The purpose of this study was to examine the fracture strength and mode of resin root analogs restored with zirconia, gold and titanium posts with resin, ceramic and metal cores after cementation with metal crowns. Materials and methods. To avoid the morphological variations of natural teeth, 40 root analogs were fabricated with composite resin. Forty resin root analogs were randomly assigned to four groups according to post and core materials: Group A: cast gold post and core and complete cast crowns, as control. Group B: titanium posts (Parapost, Coltent/Whaledent Inc., NJ, USA) and composite resin cores. Group C: zirconia posts (Cosmopost, Ivoclar AG, Schaan/Liechtenstein) and composite resin cores Group D: zirconia posts and heat-pressed ceramic cores (IPS Empress Cosmo Ingots, Ivoclar AG) After thermocycling ($5^{\circ}C{\sim}55^{\circ}C$, 30 sec.), cyclic loading was applied at 3mm below the incisal edge on the palatal surfaces at an angle of 135 degree to the long axis (2Hz, 50N, 50000cycles). Fracture strength was measured by universal testing machine (Instron, High Wycombe, UK) and fracture pattern of restored resin root analogs was also evaluated. Results and conclusion. Within the limitations of this study following results were drawn. 1. Resin root analogs restored with zirconia posts and composite resins demonstrated lowest fracture strength among tested groups. 2. There was no significant difference in the fracture strength between zirconia posts and heat pressed glass ceramic cores and cast gold posts and cores 3. The fracture strength of resin root analogs restored with titanium posts and composite resin cores was lower than that of gold posts and cores. 4. The deep oblique fracture lines were dominantly observed in root analogs restored with cast gold post and core and zirconia post and heat-pressed ceramic core groups.

The effect of zirconia surface architecturing technique on the zirconia/veneer interfacial bond strength

  • PURPOSE. The purpose of this study was to evaluate the effect of the zirconia surface architecturing technique (ZSAT) on the bond strength between veneering porcelain and zirconia ceramic. MATERIALS AND METHODS. 20 sintered zirconia ceramic specimens were used to determine the optimal surface treatment time, and were randomly divided into 4 groups based on treatment times of 0, 1, 2, and 3 hours. After etching with a special solution, the surface was observed under scanning electron microscope, and then the porcelain was veneered for scratch testing. Sixty 3 mol% yttria-stabilized tetragonal zirconia polycrystal ceramic blocks were used for tensile strength testing; 30 of these blocks were surface treated and the rest were not. Statistical analysis was performed using ANOVA, the Tukey post-hoc test, and independent t-test, and the level of significance was set at ${\alpha}=.05$. RESULTS. The surface treatment of the zirconia using ZSAT increased the surface roughness, and tensile strength test results showed that the ZSAT group significantly increased the bond strength between zirconia and veneering porcelain compared to the untreated group (36 MPa vs. 30 MPa). Optimal etching time was determined to be 2 hours based on the scratch test results. CONCLUSION. ZSAT increases the surface roughness of zirconia, and this might contribute to the increased interfacial bond strength between zirconia and veneering porcelain.

Evaluation of the fracture resistance of all-ceramic zirconia posts by 3 different methods

  • Statement of the problem. All-ceramic post-and-core restorations offer a number of advantages compared with systems that use metal build-ups. In certain clinical cases, however, fractures at the joint between the post and core build-up have been reported. Purpose. The objective, therefore, is to improve the joint between the post and the core build-up. Material and methods. Three different methods were used to prepare all-ceramic post-and-core restorations; pressing IPS Empress core build-ups to CosmoPost zirconia posts, cement-ing IPS Empress core build-ups to CosmoPost zirconia posts and Celay-milling of zirconia blanks. A series of ten restorations was prepared for each of the three methods. The post-and-core complexes were tested to failure with the load applied perpendicular to the post axis. The load and deflection at fracture were recorded. Results. The highest breaking load and highest deflection were recorded for the cementing technique with values of 25.3 N and $394{\mu}m$, respectively, The corresponding values for the pressed core build-ups and the milled zirconia core build-ups were 22 N and $301{\mu}m$, and 13 N and $160{\mu}m$, respectively. All the differences are statistically significant (p=0.05). Regarding the load-dependence of the deflection, the cemented core build-ups again demonstrated the highest value with $15.5{\mu}m/N$. The difference in the values of $13.6{\mu}m/N\;and\;13{\mu}m/N$ recorded for the pressed-on and milled core build-ups, respectively, were statistically insignificant. Conclusion. In regard to the high fracture resistance of zirconia post, adhesive cementing the core build-up to the post offers a viable alternative to the conventional pressing technique. The elastic bond between the rigid high-strength zirconia post and the core build-up presents an additional advantage.

여러가지 post-and-core로 수복된 상악 중절치의 유한요소법적 연구 (FINITE ELEMENT ANALYSIS OF MAXILLARY CENTRAL INCISORS RESTORED WITH VARIOUS POST-AND-CORE APPLICATIONS)

  • 근관 치료된 치아의 수복에 있어서 파절은 가장 중요하게 고려되는 점이다. 포스트를 사용해서 수복한다는 것은 치수와 다른 단단한 물질을 근관 내에 삽입한다는 것으로 자연스럽지 못한 구조를 만들어서 고유의 응력분산을 변화시킨다. 오랫동안 수많이 in vitro 연구들이 post-and-core로 수복된 치아의 파절 저항에 대해서 이루어졌지만 어떤 것이 최상의 선택인지에 대해서는 많은 상충되는 관점들이 존재한다. 본 연구의 목적은 유한요소분석법을 사용하여 post-and-core system의 물리적인 성질이 치질의 응력분산에 미치는 영향을 분석하고 어떤 조합이 파절 저항에 도움이 되는지를 알아보는 것이다. 근관 치료된 상악 중절치를 삼차원 유한 요소법으로 Modeling하였다. 1.5 mm의 ferrule 높이를 부여하고 외관은 zirconia ceramic crown으로 지정하였다. 세가지 평행한 형태의 포스트 (zirconia ceramic, glass fiber, and stainless steel)와 두 가지 코어 (Paracore and Tetric ceram) 물질을 6개의 모델로 조합하였다. 각각의 모델은 해면골, 피질골, 치주인대, 그리고 4 mm 근관 충전을 가지도록 설계하였다. 50 N의 정적인 교합력이 치아 장축에서 60도 각도로 치관의 설면에 적용시켰다. 모델들의 응력전달 특징의 차이를 분석하였고, 결과를 나타내는 데는 Maximum von Mises stress 값을 사용하였고 최대 변위량과 정수압도 계산하였다. Glass fiber post로 수복된 경우 높은 탄성계수를 가진 레진 코어 모델 (29.14 MPa)에서 낮은 탄성계수의 코어 모델 (29.21 MPa)보다 더 낮은 응력이 발생하였다. Glass fiber post로 수복된 모델 (0.03497-0.03499 mm)은 다른 포스트로 수복된 모델들 (0.03245-0.03452 mm)보다 더 많은 최대 변위량을 보였다. 이는 glass fiber post로 수복된 치아의 경우가 상대적으로 치아에 가해지는 힘에 의해 더 많이 움직였다는 것을 보여준다. Zirconia ceramic 이나 stainless steel 과 같이 탄성계수가 큰 포스트는 응력을 증가시키지만 포스트가 스트레스를 대부분 흡수하여 치질에는 스트레스가 낮게 나타났다. Glass fiber post로 수복된 모델에서는 코어와 크라운이 만나는 순면 치경부에서 가장 높은 응력이 발생하였다.

샌드블라스팅 처리와 라이너가 지르코니아와 전장도재의 전단결합 강도에 미치는 영향 (Effect of sandblasting and liner on shear bond strength of veneering ceramic to zirconia)

  • Purpose: This study aimed to compare the shear bond strength between zirconia cores and veneer ceramics as per the sand blasting and liner treatments. Methods: The following 4 groups of zirconia-veneering ceramic specimens were prepared: (1) Group I, untreated; (2) Group II, with 110 ㎛ aluminium oxide (Al2O3) sandblasting; (3) Group III, with liner (IPS e.max ZirLiner; Ivoclar Vivadent); and (4) Group IV, with 110 ㎛ Al2O3 sand blasting and liner. Surface roughness was measured for all the prepared specimens, and the surface morphology was observed using a scanning electron microscope. All the samples (n=40) were fixed with measuring jigs, and shear bond strengths were obtained using a universal testing machine with a crosshead speed of 0.5 mm/min. The shear bond strength data were analyzed using one-way analysis of variance and t-test. The post hoc comparison was performed using the Tukey's test (α=0.05). Results: A significant difference in the surface roughness was observed between the specimens of groups I and II (p<0.05). Surface treatment with liner and sandblasting showed higher shear bond strength between zirconia core and veneering ceramic (p<0.05). Conclusion: The sand blasting and liner treatment increased the shear bond strength between zirconia core and veneering ceramic.

심미적 포스트 코어의 종류에 따른 미세누출에 관한 연구 (Microleakage of endodontically treated teeth restored with three different esthetic post and cores)

  • 연구목적: 오늘날 심미적 요구의 증가로 치아색 포스트 코어에 대한 많은 관심이 모아지고 있으며 다양한 연구가 진행 중이다. 현재 화이버 포스트 (ParaPost fiber White, Coltene Whaledent, Cuyahoga Falls, USA), 지르코니아 포스트 (CosmoPost, Ivoclar Vivadent, Schann, Germany) 등에 관한 논문은 다소 나와 있으나 요즘 여러 분야에서 관심을 받고 있는 CAD/CAM (computer-aided design/computer-aided manufacturing) 방식으로 지르코니아를 가공한 절삭형 지르코니아 포스트 코어에 대한 연구는 많지 않은 실정이다. 이 연구의 목적은 CAD/CAM을 이용한 지르코니아를 비롯하여 근관치료된 치아에서 사용된 세 가지 다른 치아색 포스트 코어에 대해 미세누출 (microleakage)을 비교해 보는 것이다. 연구 재료 및 방법: 발치된 건전한 상악 절치 27개를 채택하여 백악법랑질경계 (CEJ)에서 절단하여 근관치료를 시행하고, 세군 (각 9개)으로 나누었다. 각군들은 화이버 포스트와 콤포짓트 레진 코어 (1군), 지르코니아 포스트와 열가압형 세라믹 코어 (2군), CAD/CAM 절삭형 지르코니아 포스트 코어 (3군)로 수복하였다. $5-55^{\circ}C$ 1000회 열순환 처리 (thermocycling)를 시행하고, 2% 메틸렌 블루로 $37^{\circ}C$에서 24시간 염색하였다. 치아장축과 평행하게 순설방향으로 절단하여 미세누출을 현미경과 컴퓨터 프로그램을 사용하여 분석을 하였다. 데이터는 one-way ANOVA와 Scheffe's multiple range test (${\alpha}=0.05$)로 분석하였다. 결과: 모든 군에서 미세누출을 보였으며, 지르코니아 포스트와 열가압형 세라믹 코어군 (2군)에서 치근계면 입구에서는 다소 많은 미세누출을 보였으나 포스트 끝에 이르러서는 거의 미세누출이 없는 결과를 보였다. 화이버 포스트와 콤포짓트 레진 코어군 (1군)과CAD/CAM 절삭형 지르코니아 포스트 코어군 (3군)에서는 부위별로 균등한 미세누출을 보였다. 결론: 화이버 포스트와 콤포짓트 레진 코어군, 지르코니아 포스트와 열가압형 세라믹 코어군, CAD/CAM 절삭형 지르코니아 포스트 코어군 모두에서 미세누출을 보였고, 이 중에서 지르코니아 포스트와 열가압형 세라믹 코어군이 좀 더 큰 미세누출에 대한 저항성을 보였으며 화이버 포스트와 콤포짓트 레진 코어군, CAD/CAM 절삭형 지르코니아 포스트 코어군은 유사한 양상을 보였으나, 각 실험군 간에 통계적으로 유의한 차이는 인정되지 않았다. 사출성형 및 열간가압 소결법으로 제작된 지르코니아 세라믹 임플란트의 소결물성 및 미세구조적 결함 (Sintered Properties and Microstructural Defects of Zirconia Ceramic Implant Fabricated by Injection Molding and Hot Isostatic Pressing (HIP))

  • 3Y-TZP (3 mol% yttria-stabilized tetragonal zirconia polycrystals, 3Y-TZP) ceramics are emerging as dental implant materials due to their superior optical and mechanical properties as well as excellent biophysical properties, in spite of low bioactivity. In this study, we investigated to sintered properties and microstructural defects of dental zirconia implants fabricated by ceramic injection molding and post-HIP (Hot isostatic pressing) processing and analyzed the processing parameters related with the obtainment of its high sinterd density. Sintered and microstructural parameters, i.e, apparent density, grain size and phase composition of zirconia implants fabricated by injection molding were dependent on the fixtute size and implant type. Maximum sintered density of 99.2% and minimum grain size of 0.3-0.4 ㎛ were obtained from large-scaled 2-body sample. In 1-body ceramic implant, high sintered density of 99.5% was obtained, but it had a little monoclinic phase and wide grain size distribution.

심미 포스트가 전치에 미치는 응력과 변위에 관한 삼차원 유한요소법적 분석 (FINITE ELEMENT ANALYSIS OF THE INFLUENCE OF ESTHETIC POSTS ON INCISORS)

  • Statement of problem : Most posts are metallic, but in response to the need for a post that possesses optical properties compatible with an all-ceramic crown. an esthetic post has been developed. Although there have been many studies about the esthetic post materials, 3-dimensional finite element studies about the stress distribution of them are in rare. Purpose : The purpose of this study is to investigate comparatively the distribution of stresses of the restored, endodontically treated maxillary incisors with the esthetic post materials and the displacement on the cement layer on simulated occlusal loading by using a 3-dimensional finite element analysis model. Material and method : Four 3-dimensional finite element models were constructed in a view of a maxillary central incisor, a post, a core, and the supporting tissues to investigate the stresses in various esthetic posts and cores and the displacement on the cement layer (Model 1 ; Cast gold post and core, Model 2 ; Glass fiber post with composite core, Model 3 ; Zirconia post with composite core. Model 4 ; Zirconia post with ceramic core). Force of 300N was applied to the incisal edge and the cingulum (centric stop point) with the angle of 135-degree to the long axis of the tooth. Results : 1. The stresses and displacement on the incisal edge were higher than on the cingulum 2. The stresses in dentin were the highest in Model 2 (Glass fiber post with composite core), and the second was Model 3, the third Model 1, and the lowest Model 4. 3. The stresses in post and core were the highest in Model 4 (Zirconia post with ceramic core), and the second was Model 1, the third Model 3, and the lowest Model 2. 4. The displacement on the cement layer was the highest in Model 2 (Glass fiber post with composite core), and the second was Model 3, the third Model 1, and the lowest Model 4. Conclusion : When a functional maximum bite force was applied, the distribution of stresses or the esthetic post and core materials and the displacement on the cement layer were a little different. It seems that restoring extensively damaged incisors with esthetic post and core materials would be decided according to the remaining tooth structure.
