A Study on the Fracture Strength of All-Ceramic Crown according to the Zirconia Coping Design using CAD/CAM System L. Bulgan, In-Ho Cho, Soo-Yeon Shin Department of Prosthodontics, Graduate School, Dankook University The fracture strength of prosthesis is important, because it affects the function, and long term success of prosthesis and teeth. The purpose of this study was to compare the fracture strength of zirconia coping designs. Experimental test group were classified into three designs according to coping design, Modified design: Zirconia coping margin was located at 1mm above the axiogingival line angle. Collarless design: The coping margin terminated at the axiogingival line angle Butt design: The coping margin was extended to the finishing line of prepared margin. A $Cercon^{(R)}$(Degussa, Germany) CAD/CAM system was used to make the zirconia coping. Fracture strength was measured using loading machine at a cross head speed of 1mm/min. The results were statistically analyzed using the one-way ANOVA and multiple comparison test. Statistical significance was set in advance at the probability level of less than 0.05. The result : I. Collarless($738N{\pm}155N$) and Modified($755N{\pm}185N$) groups showed significantly a lower fracture strength than Butt($1057N{\pm}262N$) group(p<0.05). II. There were no statistical differences of the fracture strength between Modified($755N{\pm}185N$) and Collarless($738N{\pm}155N$) groups. III. When comparing the fractured surface, all the group showed porcelain fracture, which were fractured at the labial surface of baked porcelain The butt design of the zirconia coping has higher fracture strength than modified and collarless design.
Application of ceramic materials for fabrication of dental restoration materials has been a focus of interest in the field of esthetic dentistry. The ceramic materials of choice are glass ceramics, spinel, alumina, and zirconia. The development of yttrium tetragonal zirconia polycrystal (YTZP)-based systems is a recent addition to all-ceramic systems that have high strength and are used for crowns and fixed partial dentures. Computer-aided design/computer-aided manufacturing (CAD/CAM)-produced, YTZP-based systems are popular with respect to their esthetic appeal for use in stress-bearing regions. The highly esthetic nature of zirconia and its superior physical properties and biocompatibility have enabled the development of restorative systems that meet the demands of today's patients. Many in vitro trials have been performed on the use of zirconia; however, relatively fewer long-term clinical studies have been published on this subject. The use of zirconia frameworks for long-span fixed partial dentures is currently being evaluated; in the future, more in vivo research and long-term clinical studies are required to provide scientific evidence for drawing solid guidelines. Further clinical and in vitro studies are required to obtain data regarding the long-term clinical use of zirconia-based restorations.
연구목적: 최근에는 금속도재관의 심미적 문제로 심미성과 강도가 개선된 코아용 도재 및 지르코니아를 이용한 전부도재관 시스템에 관심이 증가되고 있다. 특히 금속과 유사한 강도를 가지면서 금속의 비심미성을 극복할 수 있는 지르코니아를 이용한 보철물의 제작이 국내외에서 많이 이뤄지고 있다. 하지만 CAD/CAM 시스템으로 제작되는 여러 종류의 지르코니아를 이용한 전부도재관의 적합도에 대한 임상 연구는 부족한 실정이다. 다양한 CAD/CAM 시스템 중 Procera, Cerec inLab, Lava에 의해 제작된 지르코니아를 이용한 전부도재관의 변연 적합도와 내면 적합도를 측정하여 이를 전통적인 방식으로 주조된 금속 도재관의 적합도와 비교 평가해 봄으로써 임상 적용에 참고 자료로 활용하고자 한다. 연구 재료 및 방법: 본 연구를 위해 하악 우측 제2소구치에 해당하는 레진치를 삭제하여 인상채득 후, 금속 주모형을 제작하였다. 금속 주모형의 40개의 인상을 채득 후 각 시스템마다 10개씩 총 40개의 crown을 제작하였다. 금속도재관은 전통적인 방식으로 제작하였고 Procera, Lava, Cerec inLab crown은 zirconia core를 이용하여 CAD/CAM 시스템으로 제작하였다. 수직 변연 오차와 내면 간격을 각 그룹마다 금속 주모형에 시멘트 접착 없이 측정하였다. 그 결과는 oneway ANOVA와 Tukey's HSD test로 통계 내었다. 결과: 수직 변연 오차는 금속도재관이 $50.6{\pm}13.9{\mu}m$, Procera crown이 $62.3{\pm}15.7{\mu}m$, Lava crown이 $45.3{\pm}7.9{\mu}m$, Cerec inLab crown은 $71.2{\pm}2.0{\mu}m$로 측정되었다. 내면 간격은 금속도재관이 $52.6{\pm}10.1{\mu}m$, Procera crown이$161.7{\pm}18.5{\mu}m$, Lava crown이 $63.0{\pm}10.2{\mu}m$, Cerec inLab crown이 $73.7{\pm}10.7{\mu}m$로 측정되었다. 변연 적합도 측면에서 볼 때 Procera crown, Lava crown, Cerec inLab 그리고 전통적인 금속도재관은 임상적으로 받아들일 수 있는 적합도 범위($120{\mu}m$) 내에 있었다. 내면 적합도 측면에서 볼 때 Procera crown을 제외한 Lava crown, Cerec inLab, 금속도재관은 임상적으로 받아들일 수 있는 적합도 범위($140{\mu}m$) 내에 있었다.
Purpose: The objective of this study is to find out physical properties and the flexural strength changed by the low temperature degradation of the block which is needed to make bio-prosthetic dentistry which is better than feldspar affiliated ceramic made by building up ceramic powder and also to apply this to the clinical use of zirconia monolithic all-ceramic crown. Methods: Flexural strength of each sample was evaluated before and after the Low Temperature Degradation, and physical properties of the Tetra Zirconia Block containing 3mol % was evaluated as well. The average and standard deviation of each experimental group were came out of the evaluation. Statistical package for social science 18.0 was used for statistics. Results: The average density of the monolithic all-ceramic crown was $6.0280{\pm}0.0147g/cm$, the relative density was 99.01 %. When the sample was sintered at $1480^{\circ}C$ the diameter of average particle was $396.62{\pm}33.71nm$. All the samples had no monolithic peak after XRD evaluation but only had tetragonal peak. There were statistically significant differences in the result of flexural strength of the samples evaluated after and before the low temperature degradation, the flexural strength before the low temperature degradation was $1747.40{\ss}{\acute{A}}$, at the temperature of $130^{\circ}C$ the flexural strength after the low temperature degradation was 1063.99MPa (p<0.001). There was statistically significant difference in the result of strength of 1020.07MPa after the low temperature degradation at the temperature of $200^{\circ}C$ (p<0.001). Conclusion: The block which was made for this evaluation possesses such an excellent strength among dental restorative materials that it is thought to have no problems to use for tetragonal zirconia polycrystal.
Currently there is no dental ceramic material can be used in all dental situations need to be restored. However, in view of recent clinical reports, the most viable alternative is zirconia ceramic. Clinical success of dental zirconia restorations strongly depends on proper selection of materials, accurate laboratory procedure and final cementation, which can be achievable with the correct understanding of zirconia. As dental materials, zirconia ceramics have a very bright future, because they are being used increasingly in the anterior region as implant fixtures, as well as crown and bridge restorations and implant abutments. Many dental ceramics showing poor clinical performance have been gone from the dental market. However, in terms of outstanding mechanical properties and esthetic nature, new dental materials can replace zirconia ceramics will not be available in the foreseeable future.
PURPOSE. This study was conducted to evaluate the effects of full-coverage all-ceramic zirconia, lithium disilicate glass-ceramic, leucite glass-ceramic, or stainless steel crowns on antagonistic primary tooth wear. MATERIALS AND METHODS. There were four study groups: the stainless steel (Steel) group, the leucite glass-ceramic (Leucite) group, the lithium disilicate glass-ceramic (Lithium) group, and the monolithic zirconia (Zirconia) group. Ten flat crown specimens were prepared per group; opposing teeth were prepared using primary canines. A wear test was conducted over 100,000 chewing cycles using a dual-axis chewing simulator and a 50 N masticating force, and wear losses of antagonistic teeth and restorative materials were calculated using a three-dimensional profiling system and an electronic scale, respectively. Statistical significance was determined using One-way ANOVA and Tukey's test (P<.05). RESULTS. The Leucite group ($2.670{\pm}1.471mm^3$) showed the greatest amount of antagonist tooth wear, followed by in decreasing order by the Lithium ($2.042{\pm}0.696mm^3$), Zirconia ($1.426{\pm}0.477mm^3$), and Steel groups ($0.397{\pm}0.192mm^3$). Mean volume losses in the Leucite and Lithium groups were significantly greater than in the Steel group (P<.05). No significant difference was observed between mean volume losses in the Zirconia and Steel groups (P>.05). CONCLUSION. Leucite glass-ceramic and lithium disilicate glass-ceramic cause more primary tooth wear than stainless steel or zirconia.

PURPOSE. The aim of this in vitro study was to investigate the fracture resistance under chewing simulation of implant-supported posterior restorations (crowns cemented to hybrid-abutments) made of different all-ceramic materials. MATERIALS AND METHODS. Monolithic zirconia (MZr) and monolithic lithium disilicate (MLD) crowns for mandibular first molar were fabricated using computer-aided design/computer-aided manufacturing technology and then cemented to zirconia hybrid-abutments (Ti-based). Each group was divided into two subgroups (n=10): (A) control group, crowns were subjected to single load to fracture; (B) test group, crowns underwent chewing simulation using multiple loads for 1.2 million cycles at 1.2 Hz with simultaneous thermocycling between $5^{\circ}C$ and $55^{\circ}C$. Data was statistically analyzed with one-way ANOVA and a Post-Hoc test. RESULTS. All tested crowns survived chewing simulation resulting in 100% survival rate. However, wear facets were observed on all the crowns at the occlusal contact point. Fracture load of monolithic lithium disilicate crowns was statistically significantly lower than that of monolithic zirconia crowns. Also, fracture load was significantly reduced in both of the all-ceramic materials after exposure to chewing simulation and thermocycling. Crowns of all test groups exhibited cohesive fracture within the monolithic crown structure only, and no abutment fractures or screw loosening were observed. CONCLUSION. When supported by implants, monolithic zirconia restorations cemented to hybrid abutments withstand masticatory forces. Also, fatigue loading accompanied by simultaneous thermocycling significantly reduces the strength of both of the all-ceramic materials. Moreover, further research is needed to define potentials, limits, and long-term serviceability of the materials and hybrid abutments.
PURPOSE. The aim of the present study was to evaluate the fracture resistances of zirconia, cast nickel-chromium alloy (Ni-Cr), and fiber-composite post systems under all-ceramic crowns in endodontically treated mandibular first premolars. MATERIALS AND METHODS. A total of 36 extracted human mandibular premolars were selected, subjected to standard endodontic treatment, and divided into three groups (n=12) as follows: cast Ni-Cr post-and-core, one-piece custom-milled zirconia post-and-core, and prefabricated fiber-glass post with composite resin core. Each specimen had an all-ceramic crown with zirconia coping and was then loaded to failure using a universal testing machine at a cross-head speed of 0.5 mm/min, at an angle of 45 degrees to the long axis of the roots. Fracture resistance and modes of failure were analyzed. The significance of the results was assessed using analysis of variance (ANOVA) and Tukey honest significance difference (HSD) tests (${\alpha}=.05$). RESULTS. Fiber-glass posts with composite cores showed the highest fracture resistance values ($915.70{\pm}323N$), and the zirconia post system showed the lowest resistance ($435.34{\pm}220N$). The corresponding mean value for the Ni-Cr casting post and cores was reported as $780.59{\pm}270N$. The differences among the groups were statistically significant (P<.05) for the zirconia group, as tested by ANOVA and Tukey HSD tests. CONCLUSION. The fracture resistance of zirconia post-and-core systems was found to be significantly lower than those of fiber-glass and cast Ni-Cr post systems. Moreover, catastrophic and non-restorable fractures were more prevalent in teeth restored by zirconia posts.
Purpose: All ceramic crown, made from zirconia instead of metal for core material, is recognized the best esthetical prosthesis. Recently, high-priced zirconia blocks and expensive CAD/CAM machines come into use for making zirconia core. In this study, slip casting process is adapted to evaluate the possibility of the recycling the remained parts of zirconia block after machining. Methods: Remained zirconia blocks were reduced to powders with zirconia mortar, and screened with 180 mesh sieve. Passed powders were ball milled under various conditions to obtain the optimum zirconia slip for casting. Solid casting method was used for casting the specimens with plaster mold. Formed specimens were dried and biscuit fired at $1,000^{\circ}C$ for 1 hour. Biscuit fired specimens were finished with exact shape of square pillar. Finished specimens were fired from $1,200^{\circ}C$ to $1,550^{\circ}C$ at $50^{\circ}C$ intervals for 1 hour. Linear shrinkage, apparent porosity, water absorption, bulk density, and flexural strength were tested. Microstructures were observed by SEM. Results: Above examinations indicated that the optimum firing temperture was $1,500^{\circ}C$, and when fired at this temperature for 1 hour, apparent porosity was 0% and flexural strength was 680MPa. SEM photomicrographs showed uniform 200~300nm grain size, which is equal with microcture of sintered commercial zirconia block. when compare 24% linear shrinkage of cast specimen with 20% linear shrinkage of CAD/CAM machined block, it was estimated that the size controlling of cast core was not so difficult. Conclusion: According to the all of this experimental results, the cast zirconia core produced from the remained parts of zirconia block was possible to use for all ceramic denture.
