구순열비변형 환자에서 비교정술에 대한 비교 연구 (A COMPARATIVE STUDY ON THE CORRECTION METHODS OF NOSTRIL IN PATIENTS WITH CLEFT LIP NASAL DEFORMITY)

  • The secondary correction of cleft lip nasal deformity (CLND) presents difficult surgical problems. Characteristically, nostrils are asymmetric. The present study was aimed to examine and compare the effect of Straith's alar web Z-plasty, Millard's alar web Z-plasty, alar web excision, and lateral V-Y advancement of the alar base for augmentation of the nostril with or without lengthening the columella in CLND. The subjects were 28 patients with unilateral cleft lip, who had secondary nostril correction. The nostril correction methods were Straith's alar web Z-plasty, Millard's alar web Z-plasty, alar web excision, and lateral V-Y advancement of the alar base. Facial photographs were taken before and 20 days after the operation. By using Adobe photoshop, the columella length and the nostril width were measured from the facial frontal photograph and Worm's eye view. The degree of improvement was calculated and statistically analyzed. The degree of improvement of the columella length using Straith's alar web Z-plasty was 70.20%. And then Millard's alar web Z-plasty was 55.01%, alar web excision was 39.93%, and lateral V-Y advancement of the alar base was 16.38% in order. The degree of improvement of the nostril size using lateral V-Y advancement of the alar base was 55.26%. And then alar web excision was 52.72%, Millard's alar web Z-plasty was 34.86%, and Straith's alar web Z-plasty was 16.06% in order. Straith's alar web Z-plasty and Millard's alar web Z-plasty resulted in elongation of the columella, equalization of asymmetrical nostril, and enlargement of small nostrils. Alar web excision enlarged nostrils and restored symmetry. Lateral VY advancement of the alar base increased nostril width and enlarged nostrils. These results indicate that the correction of nostrils improve the shape and the symmetry of the nostrils in CLND.

  • Z-correction is new method to be used when measuring pattern registration of photomasks. The method is based on measurement of the plate profile in the Zaxis and takes into account the impact on the registration deviations caused by plate support, contamination as well as the photomask flatness itself. Z-correction further facilitates a more neutral way of judging the overlay properties between individual photomasks within a mask set.

Comparison between Z-plasty and V-Y Advancement for the Surgical Correction of Cryptotia

  • Background: Cryptotia correction by V-Y advancement of a temporal triangular flap was introduced in 2005. However, despite the several advantages of V-Y advancement, visible scars at the donor site are problematic. As a result, a Z-plasty technique was considered for skin deficiency in mild cases. Therefore, we introduce a new surgical scheme for cryptotia correction based on considerations of techniques and complications that arose in our clinic. Methods: Between 2000 and 2013, 26 patients (35 cases) of cryptotia were treated. Seventeen patients had unilateral cryptotia and nine had bilateral cryptotia. Two corrective methods were used, Z-plasty or V-Y advancement, based on the severity. In mild cases, Z-plasty was used for correction and in severe cases, V-Y flap advancement was used for more skin supplement. Results: Follow-up periods ranged from 6 months to 1.5 years. The results obtained were relatively favorable. Nine cases of mild deformity were corrected by Z-plasty, and the other 26 cases with mild or severe deformities were corrected by V-Y advancement. In Z-plasty cases, there was one hypertrophic scar and in V-Y advancement cases, seven resulted in visible scarring and three in skin sloughing. Conclusion: The main advantage of Z-plasty is a lower likelihood of visible scarring at the donor site. In mild cases, Z-plasty may be a good alternative, but in severe cases, V-Y advancement is probably the best option for more skin supplement.

DXA를 이용한 전완부와 요추부 골밀도 검사의 보정계수 및 상관관계 연구 (The Correlation Analysis and Correction factor of BMD in Forearm and Lumbar with DXA)

  • 전완부와 요추부의 골밀도 검사를 통해 얻은 골밀도 값, T-score 와 Z-score 수치간의 상관관계 및 보정함수를 구해 어느 한 부위의 결과로서 다른 부위의 결과를 유추하는데 목적이 있다. 환자 66명은 연령별로 11명씩 20대에서 70대까지 환자들로 구성하였고 측정된 전완부와 요추부의 골밀도와 T-score와 Z-score를 조사하여 세가지 사항들에 대해 각각 상관관계가 있는지 평가하고 그 상관관계를 구하여 보정계수를 찾는다. 골밀도의 상관계수는 R=0.769 이고 보정계수 식은 Y=1.541X + 0.133 이다. T-score의 상관계수는 R=0.768 이고 보정계수식은 Y=0.715X - 0.4 이다. Z-score의 상관계수는 R=0.635 보정계수식은 Y=0.751X - 0.162 이다. 상관관계와 보정계수식를 통해 어느 한 부위의 결과로서 다른 부위의 결과를 유추할 수 있는 임상적 유용성이 있을 것으로 사료된다.

Correction of Unilateral Nostril Hypoplasia with Z-Plasty in a Child

  • Unilateral nostril hypoplasia is an extremely rare congenital malformation of unknown etiology, and only a few cases have been reported in literature. Owing to variability and complexity of the deformity, surgical correction of unilateral nostril hypoplasia represents one of the most significant reconstructive challenges to reconstructive plastic surgeons. We report a 7-year-old Vietnamese child with nasal and periocular deformity resembling a craniofacial cleft. Grossly, the right nostril was patent but with alar rim deformity, and the left nostril was not readily identifiable. A dystopic medial canthus was present on the left side as well. Closer inspection and palpation of the left side of nose revealed a patency through the soft tissue and underlying bony structure, Thus, a new alar rim were reconstructed with an irregularly shaped Z-plasty to create patency on the involved side. Simulatneously, a second Z-plasty was performed to address the medial canthal deformity. Postoperative appearance and function was sastisfactory at one-year follow up visit. In the treatment of patients with nostril hypoplasia, a careful preoperative physical examination is a prerequisite, and Z-plasty can be a valuable option for surgical correction.

설유착증 환자에서의 Z-Plasty를 이용한 치료증례 (TREATIMENT OF ANKYLOGLOSSIA USING Z-PLASTY TECHNIQUE: A CASE REPORT)

  • Ankyloglossia, or tongue-tie, is a congenital condition which occurs as a result of fusion between the tongue and the floor of the mouth. Ankyloglossia often results in malocclusion with an anterior "open bite" deformity, early prognathism, swallowing problem, speech disorder, and periodontal problem. Generally lingual frenectomy is used for treatment of ankyloglossia, but incomplete operation and simple frenectomy may produce a scar contracture resulting in a more deformed ankyloglossia than was present initially. The Z-plasty is used for the correction of scar contractures and the replacement of missing tissue and this procedure is ideally suited for the treatment of an ankylosed frenum. Most authors advise postponement of any decision for surgical correction of tongue-tie until the age of 4 years, unless the child is having much difficulty with sucking or swallowing. We treated 4 patients with ankyloglossia using Z-plasty technique. As a result, we found out that it was effective for correction of movement limitation of tongue, prevention of relapse. Further, periodic check ups are needed for evaluation of relapse, improvement of speech, and other functions of the tongue.

중복 Z-성형술을 이용한 큐피드활의 교정 (SURGICAL CORRECTION OF A CUPID'S BOW USING A DOUBLE Z-PLASTY: REPORT OF A CASE)

  • 이차 구순비성형술을 받고자 본과에 내원한 편측성 완전 구순구개열을 가진 29세 남자 환자에서 상방으로 반전된 큐피드활과 적순부의 휘파람 변형을 교정하기 위해 중복 Z-성형술을 시행하였다. 수술 후에 반전된 큐피드활이 정상적인 형태로 재건되고 휘파람 변형이 없어졌으며 상순의 긴장도가 완화되었다. 중복 Z-성형술시 적순부 내에서만 피판을 형성하므로 피부에는 새로운 반흔이 생기지 않아 심미적으로 만족스러웠고 상순결절은 더욱 볼록해 졌다. 수술 4년경과 후 큐피드활은 부드러운 곡선으로 좌우 대칭을 이루고 매우 자연스럽게 보였으며 장기적인 추적관찰 결과 양호한 결과를 보였다. 중복 Z-성형술은 반전된 큐피드활의 교정시 다른 수술방법에 비해 술식이 간단하고 주위조직 손상이 적으며 휘파람 변형의 교정도 가능한 좋은 방법이라고 사료된다.

  • The Z-plasty has been known as a most common surgical treatment of congenital circumferential constriction band syndrome of the hand. There were thirteen patients of congenital circumferential constriction band syndrome of the hand. All patients underwent Z-plasty under microscope. we did microsurgical dissection to minimize vascular, neural and lymphatic injury, and then considering secondary correction and scar contracture, tried to preserve as much subcutaneous fat and skin flap as possible without any excision. There were less skin necrosis and lymphedema as a result of vascular compromise. Using microscope offers several advantages. First, preventing vascular, neural and lymphatic injury. Second, getting an accurate suture approximation. finally, preserving as much subcutaneous fat and skin flap as possible without any excision. The outcome of digit growth and contour can be excellent than we expected. As a results, we believe that correction of congenital circumferential constriction band syndrome of the hand under the microscope have better results.
