국내 민간병원에서 치료한 다제내성결핵 환자의 치료 결과 및 예후 인자 (Treatment Outcomes and Prognostic Factors in Patients with Multidrug-Resistant Tuberculosis in Korean Private Hospitals)

  • Background: The increasing rate of drug-resistant tuberculosis (TB) is a threat to the public health and TB control. In Korea, about 75~80% of TB patients are treated in private hospitals and the rate has been continuously increasing since 2000. Methods: On a retrospective basis, we enrolled 170 newly diagnosed with or retreated for multidrug-resistant TB (MDR-TB) in 2004 from 21 private hospitals. We extracted the following demographics and treatment history from patient medical records: initial treatment outcomes, cumulative survival rates, treatment outcomes, and prognostic factors. Results: Of the 170 patients, the majority were male (64.1%), the mean age was 44.5 years old, and mean body-mass-index was $20.2kg/m^2$. None of the patients tested positive for HIV. Eleven (6.5%) were confirmed to have extensively drug-resistant TB (XDR-TB) at treatment initiation. Treatment success rates were not different between XDR-TB (36.4%, 4/11) and non-XDR MDR-TB (51.6%, 82/159). Default rate was high, 21.8% (37/170). Far advanced disease on X-ray was a significant negative predictor of treatment success; advanced disease and low BMI were risk factors for all-cause mortality. Conclusion: In private hospitals in Korea, the proportion of XDR-TB in MDR-TB was comparable to previous data. The treatment success rate of MDR-/XDR-TB remains poor and the failure rate was quite high. Adequate TB control policies should be strengthened to prevent the further development and spread of MDR-/XDR-TB in Korea.

EDXRF에 의한 용접흄 중의 중금속의 비파괴 정량 (The Non-Destructive Determination of Heavy Metals in Welding Fume by EDXRF)

  • The EDXRF(Energy Dispersive X-ray Fluorescence Spectrometer) technique was applied to the determination of heavy metals in welding fume. The EDXRF method designed in this study was a non-destructive analysis method. Samples were analyzed directly by EDXRF without any pre-treatment such as digestion and dilution. The samples used to evaluate this method were laboratory samples exposed in a chamber connected with a welding fume generator. The samples were first analyzed using a non-destructive EDXRF method. The samples subsequently were analyzed using AAS method to verify accuray of the EDXRF method. The purpose of this study was to evaluate the possibility of the non-destructive analysis of heavy metals in welding fume by EDXRF. The results of this study were as follow: 1.When the samples were collected under the open-face sampling condition, a surface distribution of welding fume particles on sample filters was uniform, which made non-destructive analysis possible. 2. The method was statistically evaluated according to the NIOSH(National Institute for Occupational Safety and Health) and HSE(Health and Safety Executive) method. 3. The overall precision of the EDXRF method Was calculated at 3.45 % for Cr, 2.57 % for Fe and 3.78 % for Mn as relative standard deviation(RSD), respectively. The limits of detection were calculated at $0.46{\mu}g$/sample for Cr, $0.20{\mu}g$/sample for Fe and $1.14{\mu}g$/sample for Mn, respectively. 4. A comparison between the results of Cr, Fe, Mn analyzed by EDXRF and AAS was made in order to assess the accuracy of EDXRF method. The correlation coefficient between the results of EDXRF and AAS was 0.9985 for Cr, 0.9995 for Fe and 0.9982 for Mn, respectively. The overall uncertainty was determined to be ${\pm}12.31%$, 8.64 % and 11.91 % for Cr, Fe and Mn, respectively. In conclusion, this study showed that Cr, Fe, Mn in welding fume were successfully analyzed by the EDXRF without any sample pre-treatment such as digestion and dilution and a good correlation between the results of EDXRF and AAS was obtained. It was thus possible to use the EDXRF technique as an analysis method of working environment samples. The EDXRF method was an efficient method in a non-destructive analysis of heavy metals in welding fume.

  • Background: Though clinical practice guidelines (CPGs) for cardiac rehabilitation (CR) are an effective and widely used treatment method worldwide, they are as yet not widely accepted in Korea. Given that cardiovascular disease is the second leading cause of death in Korea, it is urgent that CR programs be developed. In 2008, the Government of Korea implemented CR programs at 11 university hospitals as part of its Regional Cardio-Cerebrovascular Center Project, and 3 additional medical facilities will be added in 2019. In addition, owing to the promotion of CR nationwide and the introduction of CR insurance benefits, 40 medical institutions nationwide have begun CR programs even as a growing number of medical institutions are preparing to offer CR. The purpose of this research was to develop evidence-based CPGs to support CR implementation in Korea. Methods: This study is based on an analysis of CPGs elsewhere in the world, an extensive literature search, a systematic analysis of multiple randomized control trials, and a CPG management, development, and assessment committee comprised of 33 authors-primarily rehabilitation specialists, cardiologists, and thoracic surgeons in 21 university hospitals and 2 general hospitals. Twelve consultants, primarily rehabilitation, sports medicine, and preventive medicine specialists, CPG experts, nurses, physical therapists, clinical nutritionists, and library and information experts participated in the research and development of these CPGs. After the draft guidelines were developed, 3 rounds of public hearings were held with staff members from relevant academic societies and stakeholders, after which the guidelines were further reviewed and modified. Results: CR involves a more cost-effective use of healthcare resources relative to that of general treatments, and the exercise component of CR lowers cardiovascular mortality and readmission rates, regardless of the type of coronary heart disease and type and setting of CR. Conclusion: Individualized CR programs should be considered together with various factors, including differences in heart function and lifestyle, and doing so will boost participation and adherence with the CR program, ultimately meeting the final goals of the program, namely reducing the recurrence of myocardial infarction and mortality rates.

  • Purpose: To evaluate the efficiency of the electronic foot function index (eFFI) through a prospective, random based, multi-institutional study. Materials and Methods: The study included 227 patients ranging in age from 20 to 79 years, visited for surgery in different 15 institutes, and agreed to volunteer. The patients were assigned randomly into a paper-based evaluated group (n=113) and tablet-based evaluated group (n=114). The evaluation was done on the day of hospital admission and the method was changed on the second day of surgery and re-evaluated. PADAS 2.0 (https://www.proscore.kr) was used as an electronic evaluation program. Results: There were no differences in age and sex in both groups. The intraclass correlation coefficient (ICC) evaluation revealed an eFFI ICC of 0.924, showing that both results were similar. The evaluation time was shorter in the tablet-based group than the paper-based group (paper vs tablet, $3.7{\pm}3.8$ vs $2.3{\pm}1.3minutes$). Thirty-nine patients (17.2%) preferred to use paper and 131 patients (57.7%) preferred the tablet. Fifty-seven patients (25.1%) found both ways to be acceptable. Conclusion: eFFI through tablet devices appears to be more constant than the paper-based program. In addition, it required a shorter amount of time and the patients tended to prefer the tablet-based program. Overall, tablet and cloud system can be beneficial to a clinical study.

  • Lachnospiraceae bacterium KGMB03038 (=KCTC 15821)은 건강한 한국인의 대변 시료로부터 분리된 Lachnospiraceae과, Clostridia 강, Firmicutes 문에 속하는 신속 균주이다. 본 연구에서는 PacBio Sequel 플랫폼을 이용하여 KGMB03038 균주의 유전체를 해독하고 분석하였으며 그 결과, 47.8% G + C 함량을 가진 3,334,474 bp 길이의 완전한 하나의 유전체 컨티그를 얻었다. 이 유전체는 3,099개의 단백질 암호화 유전자와 12개의 rRNA 유전자, 54개의 rRNA 유전자, 그리고 4개의 ncRNA 유전자를 포함하고 있다. 이 유전체 분석 결과, KGMB 03038 균주가 탄수화물의 가수화와 아미노산의 생합성에 관련되어 있는 중요 유전자들을 가지고 있음을 확인하였다.

  • 본 연구에서는 건강한 한국인 분변으로부터 Ruminococcus sp. KGMB03662 균주를 분리하고 유전체서열을 PacBio Sequel 플랫폼을 사용하여 분석하였다. 염색체의 크기는 2,707,502 bp로 G + C 구성 비율은 43.09%, 총 유전자수는 2,484개, 단백질 코딩 유전자는 2,367개, rRNA는 14개 및 tRNA는 53개로 구성되었다. 본 유전체로부터 가수분해효소, 지방산생합성 및 대사와 항생제생합성 및 내성 관련 유전자를 확인하였다. 이러한 유전체의 분석은 KGMB03662 균주가 사람의 건강 및 질병에 관여할 것으로 여겨진다.

염촉매 전기분해 공기살균기의 효능 평가 (Efficacy and Safety Evaluation of an Air Sterilizer Equipped With an Electrolytic Salt Catalyst for the Removal of Indoor Microbial Pollutants)

  • 최근 현대인들의 다중이용시설 사용 빈도가 증가하면서 실내 공기 질, 특히 실내공기 오염원인 공기매개 전염성 세균 및 바이러스 불안에 대한 관심이 높아져 다양한 형태의 공기청정기와 공기살균기의 보급이 확산되고 있다. 따라서 공기 내 미생물을 살균할 수 있는 기술에 대한 연구 필요성이 대두되고 있다. 본 연구에서는 염촉매 전기분해 공기살균기를 개발하여 공기살균기의 항균활성과 인간 세포주(HaCaT, BEAS-2B, THP-1)에 대한 세포독성을 통한 안전성을 조사하였다. 공기살균기에서 분무되는 증기를 각각 1, 3시간 동안 분무한 결과, Staphylococcus aureus는 각각 24.01, 57.34%, Bacillus subtilis는 10.89, 57.76%, Escherichia coli는 30.79, 36.59%, Pseudomonas aeruginosa는 34.80, 49.51%, Salmonella typhimurium는 39.40, 73.98%, 진균인 Candida albicans는 36.59, 53.05%의 높은 항균활성을 보여 염촉매 전기분해 공기살균기의 살균효능이 모든 종류의 시험균주에서 시간 의존적인 항균활성을 보였다. 공기살균기의 분무액을 0.1, 0.3, 1.0%의 농도로 처리하고 세포 생존율을 측정한 결과, 1.0%의 농도에서도 BEAS-2B 세포는 93.88%, HaCaT 세포는 97.01%, THP-1 세포는 86.56%로 관찰되어 인체 정상 세포주에서는 유의적인 세포독성을 나타내지 않았다. 또한 분무액을 1.0%의 농도로 처리하고 현미경으로 관찰한 세포주의 형태학적 변화, PCR을 이용한 세포사멸관련 유전자(Bcl-2, Bax)의 발현 분석, FACS를 이용한 세포 내 ROS의 생성 변화 등에서 분무액을 처리하지 않은 대조군과 비교하였을 때 유의적인 변화가 없었다. 따라서 시험균주에 대한 항균활성, 3가지의 인간 세포주에 대한 세포독성, 세포의 형태적 변화, 유독한 반응성 산소(ROS) 생성, 세포사멸관련 유전자의 발현 등에서 유의미한 변화가 확인되지 않아 공기살균기의 살균 효능과 사용상 안전성을 확인할 수 있었다. 최종적으로 다중이용시설에서의 공기살균기의 효능을 조사하기 위하여 밀폐된 실내에서 공기살균기를 20시간 동안 가동한 후, 공기 중의 낙하균을 포집하여 배양한 결과, 총세균은 89.4%, 대장균과 진균은 100.0% 제거되는 것을 확인하였다. 결론적으로 개발된 염촉매 전기분해 공기살균기는 인체에 대한 독성이 없으면서 실내 오염원인 미생물들을 효과적으로 제거할 수 있음을 확인하였다.

개정된 아틀란타 분류법에 근거한 초기 CT에서의 괴사성 췌장염의 예측 (Prediction of Necrotizing Pancreatitis on Early CT Based on the Revised Atlanta Classification)

  • 목적 간질부종성 췌장염으로 진단된 환자군에서 괴사성 췌장염으로의 진행을 예측할 수 있는 입원 당시 초기 임상소견 및 CT 소견을 알아보고자 한다. 대상과 방법 간질부종성 췌장염으로 진단되어 입원 당시 및 14일 이내 추적 조영증강 CT를 시행한 178명의 환자를 대상으로 하였다. 두 명의 영상의학 전문의가 추적 CT를 분석하여 간질부종성 또는 괴사성 췌장염을 결정하였다. 입원 당시 혈액검사 소견도 기록하였다. 간질부종성-간질부종성 췌장염 환자군과 간질부종성-괴사성 췌장염 환자군 간에 임상소견, CT 소견 및 혈액검사 소견들을 비교하였다. 다변량 분석도 시행하였다. 결과 간질부종성-간질부종성 췌장염 환자군은 112명, 간질부종성-괴사성 췌장염 환자군은 66명이었다. 알코올성 췌장염의 비율은 간질부종성-괴사성 췌장염 환자군이 더 높았다. 입원 당시 CT 소견 중 췌장주위 액체저류, 췌장실질의 비균질성은 간질부종성-괴사성 췌장염 환자군에서 더 흔하게 나타났다. 입원 당시 혈액검사 소견 중 혈청 C-반응성 단백 수치 및 백혈구수가 간질부종성-괴사성 췌장염 환자군에서 더 높게 나타났다. 다변량 분석을 시행했을 때 췌장주위 액체저류와 췌장실질의 비균질성 소견이 두 환자군을 구별하는데 유의한 인자로 나타났다. 결론 초기 CT상 간질부종성 췌장염으로 진단된 환자군에서 CT 소견 중 췌장주위 액체저류, 췌장실질의 비균질성은 괴사성 췌장염으로의 진행을 예측하는 데 도움이 된다.
