연령과 손잡이에 따른 정상인들의 Motor Activity Log-28과 Actual Amount of Use Test 연구 및 Actual Amount of Use Test의 평가자간 신뢰도연구 (Studies on Motor Activity Log-28 and Actual Amount of Use Test, Actual Amount of Use Test Inter-rater Reliability in Healthy Individuals: Age Dependence and Handedness)

  • Background: Spontaneous use of the upper extremities on the affected side of patients with stroke is a meaningful indicator of recovery and may vary by the age or dominant hand of patients. No prior study has reported changes in actual amount of use test (AAUT) and motor activity log (MAL)-28 according to age and handedness in healthy adults, and AAUT inter-rater reliability for assessment of healthy adults. Objects: This study aimed to (1) research the differences in AAUT and MAL-28 according to age and handedness in healthy adults, and (2) determine the inter-rater reliability of the AAUT. Methods: Seventy healthy adults participated in this study. The MAL-28 was assessed by dividing 61 subjects into young right-handed ($n_1=20$), young left-handed ($n_2=21$), and older right-handed ($n_3=20$) groups. The AAUT was assessed by dividing 63 subjects into young right-handed ($n_1=25$), young left-handed ($n_2=18$), and older right-handed ($n_3=20$) groups. Student's t-test and the Wilcoxon signedrank test were used for statistical analysis. Results: The Amount of Use (AOU) scale values for each group showed no significant differences between age groups and handedness groups in the MAL-28 (p>.05). The AAUT AOU scale value showed significant differences regarding dominant handedness in the AAUT (p<.05), but no significant differences according to age (p>.05). (2) Inter-rater reliability of the AAUT was excellent, except few items (item 9, 11, and 12). Conclusion: Although both the MAL-28 and the AAUT measured how much participants used their dominant arms in healthy subjects, the AAUT only showed significantly higher dominant arm use in left hander than the right hander. In addition, the inter-rater reliability of the AAUT was excellent. Current results can be utilized as a basic information when clinicians develop rehabilitation strategies, and AAUT was shown to be a reliable evaluation tool for measurement of upper extremity use in Korean adults, based on the reliability demonstrated by this study.

지혜의 연령차와 중노년기 심리적 적응과의 상관 (Age Differences of Wisdom and Its Correlation with Successful Aging among Middle-aged and Elder Adults)

  • 본 연구는 청년집단, 중년집단, 노년집단 간 지혜의 차이를 탐색하고 중노년기 성인의 적응과 지혜의 관계를 지혜 요인별로 확인하였다. 청년기, 중년기, 노년기 성인 623명의 자료를 분석한 본 연구의 주요 결과는 다음과 같다. 첫째, 중년기 성인과 노년기 성인의 지혜 총점은 대학생 집단보다 더 높았다. 지혜 요인별로 살펴보면 '인지적 역량' 요인과 '긍정적 인생태도' 요인은 연령집단에 따른 차이가 없었고, '절제와 균형' 요인과 '공감적 대인관계' 요인은 중노년 집단이 청년집단에 비해 더 높았다. 둘째, 중노년기 성인의 지혜는 삶의 만족, 안녕감, 죽음 수용, 정적 정서경험, 자존감, 자아통합, 생성감, 긍정적 인생회고와 정적 상관을 보인 반면, 우울, 죽음공포, 부적 정서경험, 지각된 스트레스와는 부적 상관을 보였다. 본 연구의 결과를 지혜의 생태학적 타당성의 측면에서 논의하였으며, 마지막으로 본 연구의 의의, 한계점과 후속연구에 대해 논의하였다.

Growth and $\beta$-Glucosidase Activity of Bifidobacterium

  • $\beta$-Glucosidase was known to be involved in the mutagenic activation of $\beta$-glucosides. The level of $\beta$-glucosidase in the feces of adults was 2.7 times higher than that of infants. There was no difference in the percentage of $\beta$-glucosidase positive strains among Bifidobacterium isolates between adults and infants, corresponding to 90 and 92$%$, respectively. However, the strains from adults showed 1.9 times higher enzyme activity than those from infants when grown in Brain Heart Infusion medium. $\beta$-Glucosidase negative strains could not ferment $\beta$-glucosidase substrates, such as cellobiose, salicin, naringin, esculin and arbutin. Presence of $\beta$-glucosidase in Bifidobacterium did not alter the degree of growth in reconstituted skim milk. The $\beta$-glucosidase level was much lower in milk and vegetable medium, although cells grew above $10^8$cfu/ml, than in BHI medium. This study suggests that metabolic activation of the $\beta$-glucosides by Bifidobacterium $\beta$-glucosidase varies significantly depending on types of growth medium.

한국성인의 만성 폐쇄성 폐질환 유병률, 위험요인 및 삶의 질 (Chronic Obstructive Pulmonary Disease in Korea: Prevalence, Risk Factors, and Quality of Life)

  • Purpose: This study seeks to examine prevalence, risk factors, and quality of life of Korean adults with Chronic Obstructive Pulmonary Disease (COPD). Methods: From the database of the Fourth Korean National Health and Nutrition Examination Survey (KNHANES IV-1, 2008), the researchers selected 1,458 adults over the age of 45. The original study was a population-based epidemiological survey of health and nutrition with a stratified multistage clustered probability design. Prevalence of COPD was computed on the basis of the sampling weight. Data were analyzed using descriptive statistics, ${\chi}^2$ test, t-test and multiple logistic regression with the SPSS WIN 18.0 and SAS Ver. 9.1 program. Results: The prevalence of COPD was 18.0% among people older than 45 yr. The prevalence of current smokers was 19.7% in this population and 26.3% in individuals with COPD. Age, gender, education, and smoking levels were found to be risk factors for COPD. Significant difference in quality of life was founded between adults with COPD and the healthy controls. Conclusion: The results of this study indicate that COPD is a highly prevalent disease in Korea. To reduce the prevalence of COPD and improve health-related quality of life in patients with COPD, nursing interventions must focus on prevention of risk factors.

한국 정상 노인층의 삼킴장애지수와 후두 기능에 따른 삼킴 특성 (Dysphagia Handicap Index and Swallowing Characteristics based on Laryngeal Functions in Korean Elderly)

  • Larynx plays an important role in phonation and protection of the respiratory tract during swallowing. The reduced anatomical and physiological function in elevation of larynx and glottis closure can cause problems in voice and swallowing. The present study investigated the Korean version of handicap index of dysphagia in elderly Koreans. Therefore, 60 normal elderly Koreans ranged from 65 to 95 and 20 normal Korean young adults aged from 20 to 25 were participated in this study to compare total (T), physical (P), functional (F), and emotional (E) index scores between two groups as well as among sub groups (60s, 70s, 80s) in elderly. For swallowing, total and sub dysphagia handicap index (DHI) scores, voice quality during /a/phonation following swallowing (saliva and water), intensity of coughing, and L-DDK were measured. The results showed that functional (P), physical (P), emotional (E) scores as well as total (T) score were significantly different between young adults and old adults in DHI(p<.05). Additionally, there was a negative correlation between total DHI score and intensity of coughing (r=-.51) as well as L-DDK (r=-.70). These findings suggest that a slow rate in vocal fold adduction and reduced intensity of coughing in the elderly affect swallowing function. Thus, recently translated Korean version of DHI may be useful as supplement in evaluating the swallowing problems in elderly people.

Effect of Action Observation Training Using Y-Balance on Balance Capability in Young Adults

  • Purpose: This study examined the effects of action-observation training using the Y-Balance on the balance ability of young adults. Methods: Thirty-four healthy adults were randomized into an action-observation group (n=17) or a control group (n=17). All subjects performed the Y-Balance test before and after watching the video. The action observation group watched a video of someone performing a Y-Balance test, and the control group watched a video of scenery unrelated to the training. The subjects were measured through a Y-Balance test for both the length of the legs extended in three directions and the Y-balance composite score. Results: A significant difference in the Y-balance composite score was observed between the two groups. A part of the direction of the extended leg in the action observation group was increased significantly (posteromedial direction of the right leg, posterolateral direction of the right leg, posteromedial direction of left leg) compared to the control group. Conclusion: These results suggest that action observation training only could help improve balance.

Effect of postural change on shoulder joint internal and external rotation range of motion in healthy adults in their 20s

  • Objective: We aimed to investigate differences of range of motion in measuring shoulder internal rotation (IR) and external rotation (ER) resulting from posture change in manual scapular stabilization in prone, hook-lying, sitting, and standing positions in healthy young adults. Design: Cross-sectional study. Methods: This study included healthy young adults who agreed to participate after a thorough explanation about the study purpose and methods. A clinometer was used to measure shoulder rotation. Measurements of shoulder rotation according to postural change were performed in prone, hook-lying, sitting, and standing positions. The repeated measures analysis of variance was used to compare between-group differences in postural change. Results: The lower the posture, the greater the average value of IR angle. In contrast, the higher the posture, the greater the average value of the external rotation angle. In active and passive IR with posture change, there was difference in average value but with no statistical significance. In active and passive ER, there was a statistically significant difference between prone and sitting, prone and standing, hook-lying and sitting, hook-lying and standing, and sitting and standing position (p<0.05). Conclusions: Our findings suggest that postural change should be considered in order to increase the strength or range of motion of the internal and external rotation of the patient's shoulder joint.

Effects of Low Visual Acuity Simulations on Eye-Hand Coordination and Brainwaves in Healthy Adults

  • Objective: In general, macular degeneration, cataracts and glaucoma generally cause visual injury in clinical settings. This study aimed to examine the effects of low visual acuity simulations on hand manual dexterity function and brainwaves in healthy young adults. Design: Cross-sectional study design Methods: This study was an observational, cross-sectional study. Seventy healthy young adults participated in this study. To evaluate the effects of low visual acuity simulations on hand function and brain waves, this study involved four different visual conditions including (1) normal vision, (2) simulated cataracts, (3) simulated glaucoma, and (4) simulated macular degeneration. The hand function was measured to use the Minnesota manual dexterity test (MMDT), and the brainwaves was also measured to use the electroencephalography. Results: In hand function, placing and turning performance on the MMDT in the normal visual condition was significantly different than that in the cataract and macular degeneration conditions (p<0.05), and the placing performance was significantly differred in the normal condition than that in the simulated glaucoma. However, turning was not significantly different in the normal condition than that in the simulated glaucoma. The alpha, beta, and gamma waves did not significantly differ among the four visual conditions (p>0.05). Conclusions: The results suggest that limited visual information negatively affects the ability to perform tasks requiring arm-hand dexterity and eye-hand coordination. However, the effectiveness of low visual acuity on the brainwaves should be further studied for rehabilitative evidence of visual impairment.
