Self-Monitoring of Blood Pressure and Feed-back Using APP in TReatment of UnconTrolled Hypertension (SMART-BP): A Randomized Clinical Trial

  • Background and Objectives: Self-monitoring of blood pressure (SMBP) is a reliable method used to assess BP accurately. However, patients do not often know how to respond to the measured BP value. We developed a mobile application-based feed-back algorithm (SMBP-App) for tailored recommendations. In this study, we aim to evaluate whether SMBP-App is superior to SMBP alone in terms of BP reduction and drug adherence improvement in patients with hypertension. Methods: Self-Monitoring of blood pressure and Feed-back using APP in Treatment of UnconTrolled Hypertension (SMART-BP) is a prospective, randomized, open-label, multicenter trial to evaluate the efficacy of SMBP-App compared with SMBP alone. Patients with uncomplicated essential hypertension will be randomly assigned to the SMBP-App (90 patients) and SMBP alone (90 patients) groups. In the SMBP group, the patients will perform home BP measurement and receive the standard care, whereas in the SMBP-App group, the patients will receive additional recommendations from the application in response to the obtained BP value. Follow-up visits will be scheduled at 12 and 24 weeks after randomization. The primary endpoint of the study is the mean home systolic BP. The secondary endpoints include the drug adherence, the home diastolic BP, home and office BP. Conclusions: SMART-BP is a prospective, randomized, open-label, multicenter trial to evaluate the efficacy of SMBP-App. If we can confirm its efficacy, SMBP-App may be scaled-up to improve the treatment of hypertension.

지하수 중 라듐-226의 분석방법 및 환경 특성에 관한 예비 연구 (A Preliminary Study for the Analytical Method and Environmental Characteristics of Radium-226 in Groundwater)

  • $^{226}Ra$ in groundwater could be analyzed by various methods. LSC (liquid scintillation counter) is used to measure its activity of Ba co-precipitates with Hisafe III scintillation cocktail solution. Counting efficiency was obtained using NIST $^{226}Ra$ standard solution in triplicate and calculated $^{226}Ra$ concentration using the efficiency values. $^{226}Ra$ values of 19 groundwaters having Gross-${\alpha}$ concentrations of more than 5 pCi/L ranged from ND (${\leq}$ 0.1 pCi/L) to 1.18 pCi/L. Geologic settings of the 19 areas are composed of granitic rocks of Pre-Cambrian and Jurassic and Cretaceous, gneiss (schist) of Pre-Cambrian, and volcanic rocks of Cretaceous. No relationship was shown among $^{226}Ra$ a concentrations and in-situ water quality data, and Gross-${\alpha}$, uranium, radon concentrations.

시험관내 배양된 제대혈 모세포에서의 신경항원 발현 (Neural Antigen Expressions in Cultured Human Umbilical Cord Blood Stem Cells in vitro)

  • Objectives : Cord blood stem cells have been widely used as donor cells for bone marrow transplantation recently. These cells can give rise to a variety of hematopoietic lineages to repopulate the blood. Recent observations reveal that some bone marrow cells and bone marrow stromal cells(MSCs) can grow to become either neurons or glial cells. It is, however, unclear whether or not there exists stems cells which can differentiate into neurons in the blood during the early stages of postnatal life. Methods : Human cord blood stem cells were prepared from human placenta after full term delivery. To induce neuronal differentiation of stem cells, ${\beta}$-mercaptoethanol was treated. To confirm the neuro-glial characteristics of differentiated stem cells, immunocytochemical stain for NeuN, neurofilament, glial fibrillary acidic protein(GFAP), microtubule associated protein2(MAP2) was performed. RT-PCR was performed for detecting nestin mRNA and MAP2 mRNA. Results : We showed in this experiment that neuro-glial markers(NeuN, neurofilament, MAP2, GFAP) were expressed and axon-like cytoplasmic processes are elaborated in the cultured human cord blood stem cells prepared from new born placenta after full term delivery. Nestin mRNA was also detected in fresh cord blood monocytes. Conclusions : These results suggest that human cord blood derived stem cells may be potential sources of neurons in early postnatal life.

Eupatilin downregulates phorbol 12-myristate 13-acetate-induced MUC5AC expression via inhibition of p38/ERK/JNK MAPKs signal pathway in human airway epithelial cells

  • Chronic inflammatory airway diseases, such as chronic rhinosinusitis, chronic obstructive pulmonary disease, and asthma, are associated with excessive mucus production. Hence, the regulation of mucus production is important for the treatment of upper and lower airway diseases. Eupatilin is a pharmacologically active ingredient obtained from Artemisia asiatica Nakai (Asteraceae) and exerts potent anti-inflammatory, anti-allergic, and anti-tumor activities. In the present study, we investigated the effect of eupatilin on phorbol 12-myristate 13-acetate (PMA)-induced MUC5AC and MUC5B expression in human airway epithelial cells. We found that eupatilin treatment significantly inhibited PMA-induced mucus secretion in PAS staining. In addition, qRT-PCR results showed that eupatilin dose-dependently decreased the mRNA expression of MUC5AC in human airway epithelial cells. Western blot and immunofluorescence assay also showed that PMA-induced protein expression of MUC5AC was inhibited by eupatilin treatment. Finally, we investigated MAPKs activity after stimulation with PMA using western blot analysis in human airway epithelial cells. The results showed that eupatilin downregulated the levels of phosphorylated p38, ERK, and JNK. In summary, the anti-inflammatory activities of eupatilin, characterized as the suppression of MUC5AC expression and secretion in human airway epithelial cells, were found to be associated with the inhibition of p38/ERK/JNK MAPKs signaling pathway of MUC5AC secretion.

  • 토목시설물 건설사업에 BIM(building information modeling)이 적극적으로 도입되면서 시설물의 생애주기 동안의 활용도를 극대화할 수 있는 방안이 모색되고 있으며, 그 중, 시설물 공사 중 건설장비의 동적 거동을 반영한 검토 사례는 드물고 관련 기술 수준도 낮은 상황이다. 본 연구에서는 BIM 기반으로 구성된 가상현장 내에서 동적인 가상건설 장비 모델을 이용하여 공사단계에서 발생하는 장비운용의 안전성과 효율성을 검토할 수 있는 시뮬레이션 시스템을 개발하고 이를 실제 토목 현장에 적용하여 그 효용성을 분석하였다. 개발된 시뮬레이션 시스템이 건설장비에 대한 효과적인 3D 라이브러리 구축을 통해 공사관리자가 최적의 장비를 선정하고 경제적인 공사를 실현하기 위한 계획수립을 지원할 뿐 아니라 안전한 공사가 수행되도록 다각도의 분석을 가능케 해준다는 것을 확인할 수 있었다.

  • The intensive habitats loss of natural organisms as a consequence of anthropogenic activities has lead to the use of alternative habitats for species conservation. We reviewed the current status of alternative habitats and suggest the improvement of alternative habitats. Most of alternative habitats regarded in this study are not following the pre-arranged consultation at environmental impact assessment. These alternative habitats are rendered useless due to the insufficient consideration of ecological characteristics of species and lack of detailed plans. A number of alternative habitats are influenced by disturbance such as environmental pollution and construction. Post-monitoring of alternative habitats are needed to estimate immigration rate of species. Post management is also needed to assess the status of population stability. Overall, low effectiveness of alternative habitats is presented in this study. According to the status survey, methods for improvement of alternative habitats are required such as detailed guidelines, establishment of post-monitoring system, improvement of habitat restoration techniques, and guidelines for management and operation of alternative habitats.

스테레오 정합을 위한 고성능 하드웨어 구조 (High-Performance Hardware Architecture for Stereo Matching)

  • 본 논문에서는 실시간으로 스테레오 정합을 수행하기 위한 하드웨어 구조를 제안한다. 스테레오 정합의 연산을 분석하여 중간 연산 결과를 재사용하여 연산량과 메모리 접근수를 최소화한다. 이러한 동작을 수행할 수 있는 스테레오 정합 연산 셀의 구조를 병렬적으로 확장하여 탐색 범위 내의 모든 비용함수를 동시에 연산할 수 있는 하드웨어의 구조를 제안한다. 이러한 하드웨어 구조를 확장하여 2차원 영역에 대한 비용함수를 연산할 수 있는 하드웨어의 구조와 동작을 제안한다. 구현한 하드웨어는 FPGA 환경에서 최소 250Mhz의 클록 주파수에서 동작이 가능하고, 64화소의 탐색범위를 적용한 경우에 $640{\times}480$ 스테레오 영상을 약 813fps의 성능으로 처리할 수 있다.

  • Imaging plays a key role in the diagnosis and characterization of thyroid diseases, and the information provided by imaging studies is essential for management planning. A referral guideline for imaging studies may help physicians make reasonable decisions and minimize the number of unnecessary examinations. The Korean Society of Thyroid Radiology (KSThR) developed imaging guidelines for thyroid nodules and differentiated thyroid cancer using an adaptation process through a collaboration between the National Evidence-based Healthcare Collaborating Agency and the working group of KSThR, which is composed of radiologists specializing in thyroid imaging. When evidence is either insufficient or equivocal, expert opinion may supplement the available evidence for recommending imaging. Therefore, we suggest rating the appropriateness of imaging for specific clinical situations in this guideline.

FDS를 이용한 소화모형 화재거동의 시뮬레이션 (Simulation of Wood Crib Burning Behaviors by Using FDS)

  • In this work wood crib burning behaviors have been simulated by using the FDS(Fire Dynamic Simulator) program. Wood cribs are regularly stacked arrays of wood sticks, and available for the performance rating of fire-extinguishers. On the basis of an angle iron supporter 26 layers of wood sticks have been stacked up. Each layer consists of 5 or 6 wood sticks which are placed in parallel, with a constant distance, and in alternating rows. They are laid between the horizontally adjacent sticks at the before last layer. The wood crib is ignited instantaneously by an amount of burning gasoline below. A comprehensive simulation of such a practical sophisticated combustion is still too difficult to realize with any currently available computer, although the performance of modern processors is getting better everyday. We could carry it out here through parallel computing on the HPC(High Performance Computing) cluster as the feasible alternative. At last the validation has been executed by means of temperature distribution data measured by the thermal video camera.

  • 본 논문에서는 BGR과 ADC를 사용하여 Temperature Compensation 기능을 가진 Relaxation Oscillator를 제안한다. Relaxation Oscillator는 전류조절을 통해 주파수를 결정한다. 제안하는 Relaxation Oscillator는 온도에 따른 출력 주파수를 보상하기 위하여 온도에 따른 ADC 및 BGR의 출력 코드를 사용하여 전류를 조절한다. 제안하는 Relaxation Oscillator는 CMOS 0.35 ${\mu}m$ 공정으로 설계되었으며, 면적은 $240{\mu}m{\times}210{\mu}m$ 이다. 전류 소모는 공급전압인 5 V에서 600 ${\mu}A$이며, 온도에 대한 출력 주파수는 ${\pm}1%$이내의 정확도를 가진다.
