A Case Report of Herpes zoster with acute pain by Yongdamsagan-tang(longdanxiegan-tang) (용담사간탕(龍膽瀉肝湯)을 이용한 대상포진(帶狀疱疹)의 급성기(急性期) 통증(痛症)에 대한 치험(治驗) 1례(例))

  • Herpes zoster is a viral disease characterized by unilateral radicular pain and vesicular eruptions that are generally limited to the dermatome innervated by single spinal or cranial sensory ganglion. The disease causes severe pain and in particular, put elderly patients in great risks and further it develops postherpetic nenralgia. Nowadays western medicine use antiviral durg(ex: acyclovir), analgesics, nerve block and etc for treatment of herpes zoster and pain control, but the effect is not much satisfactory. A 73-year-old patient, admitted to our hospital because of severe migrain. Two days later, her illness was diagnosed as herpes zoster so we administrated Yongdamsagan-tang. After the six-day of treatment, all symptoms improved, especially headache was cleared since the four day long administration of Yongdamsagan-tang.

The Study on the Composition of Yongdamsagan-tang(龍膽瀉肝湯)'s Essential Oil Obtained by Supercritical Carbon Dioxide Extraction (초임계 유체 추출법을 이용한 용담사간탕(龍膽瀉肝湯) 약침액(藥鍼液)의 정유(精油)성분에 대한 연구(硏究))

  • Objectives : This study was performed to analyze the effective components of essential oil obtained from Yongdamsagantang, which has been efficacious against leukorrhea in gynecologic diseases. Methods : I obtained the essential oils of Yongdamsagan-tang by hydrodistillation extraction method and supercritical fluid extraction(SFE) method, and then I analyzed those by GC/MS(Gas Chromatography/Mass Spectrum). Results : 1. The optimum SFE(Supercritical Fluid Extraction) condition was obtained in the following experiment conditions: pressure 200atm, temperature $45^{\circ}C$, duration of extraction 25minutes. 2. With GC(Gas Chromatography) and GC/MS(Gas Chromato- graphy/Mass Spectrum) analysis, I identified 37 compounds in the Yongdamsagan-tang's essential oil obtained through the SFE method. The main compounds were as follows : 3-Methyl-but-2-enoic acid,2,2-dimethyl-8-oxo-3,4-dihydro-2H,8H -pyrano[3,2-g]chromen-3-yl ester(49.81%), (Z)-6-Pentadecen-1 -ol(3.19%), (-)-Spathulenol(2.40%). 3. I identified 4 compounds in the Yongdamsagan-tang's essential oil obtained through the hydrodistillation method. The main compounds were as follows : 3-Methyl-but-2-enoic acid, 2,2-dimethyl-8-oxo-3,4-dihydro-2H,8H-pyrano [3,2-g]chromen-3-yl ester(2.61%). 4. 3-Methy I-but-2-enoic acid, 2,2-dimethyl-8-oxo-3,4-dihydro-2H,8H-pyrano[3,2-g] chromen-3-yl ester, all were identified in both the SFE method and the hydrodistillation method, but the others were not identified in common. 5. I also conducted an additional test in order to examine the essential oil's antimicrobial action against bacteria. Both MIC(Minimum Inhibitory Concentrations) and MBC(Minimum Bactericidal Concentrations) were $0.125mg/m{\ell}$ against N. meningitidis, however MIC and MBC were $1.0mg/m{\ell}$ in antimicrobial action against 12 different genera of bacteria.

Quantitative Analysis of the Marker Constituents in Yongdamsagan-Tang using Liquid Chromatography-Electrospray Ionization-Tandem Mass Spectrometry (LC-ESI-MS/MS를 이용한 용담사간탕의 주요 성분 분석)

  • Yongdamsagan-tang has been used to treat the urinary disorders, acute- and chronic-urethritis, and cystitis in Korea. In this study, an ultra-performance liquid chromatography-electrospray ionization-mass spectrometry (UPLC-ESI-MS/MS) method was established for simultaneous analysis of the 20 bioactive marker compounds, geniposidic acid, chlorogenic acid, geniposide, liquiritin apioside, acteoside, calceolarioside B, liquiritin, nodakenin, baicalin, liquiritigenin, wogonoside, baicalein, glycyrrhizin, wogonin, glycyrrhizin, wogonin, saikosaponin A, decursin, decursinol angelate, alisol B, alisol B acetate, and pachymic acid in traditional herbal formula, Yongdamsagan-tang. Chromatographic separations of all marker compounds were conducted using a Waters Acquity UPLC BEH $C_{18}$ analytical column ($2.1{\times}100mm$, $1.7{\mu}m$) at $45^{\circ}C$ using a mobile phase of 0.1% (v/v) formic acid in water and acetonitrile with gradient elution. The MS analysis was performed using a Waters ACQUITY TQD LC-MS/MS coupled with an electrospray ionization source in the positive and negative modes. The flow rate was 0.3 mL/min and injection volume was $2.0{\mu}L$. The correlation coefficient of 20 marker compounds in the test ranges was 0.9943-1.0000. The limits of detection and quantification values of the all marker components were 0.11-6.66 and 0.34-19.99 ng/mL, respectively. As a result of the analysis using the optimized LC-ESI-MS/MS method, three compounds, geniposidic acid (from Plantaginis Semen), alisol B (from Alismatis Rhizoma), and pachymic acid (from Poria Sclerotium), were not detected in this sample. While the amounts of the 17 compounds except for the geniposidic acid, alisol B, and pachymic acid were $0.04-548.13{\mu}g/g$ in Yongdamsagan-tang sample. Among these compounds, baicalin, bioactive marker compound of Scutellariae Radix, was detected at the highest amount as a $548.13{\mu}g/g$.

A Case Study of Soeumin patients with Herpes zosters (소음인 대상포진 환자의 치험 1례)

  • Objectives : This study reports a case of Soeumin patients with Herpes zostersMethods : We examined progress of Soeumin Herpes zoster patient's symptoms (skin blisters, pain, insomnia, etc.) and complications when during and after the treatments.Results : Soeumin Herpes zoster patient was diagnosed as Yang Depletion. Yongdamsagan-tang, Sweet BV, were effective for decrease the patient's skin blisters and pain. Soeumin Bojungikgi-tang prevented the patient's aftereffects.Conclusions : Soeumin Herpes zoster patient who had severe pain, blisters, and sleep disorder, was expected high risk of postherpetic neuralgia. But diagnosed Yang Depletion symptomatology and treated with Soeumin Bojungikgi-tang, patient recover completely without sequelae.

Anti-inflammatory Effects in LPS-treated RAW 264.7 Cells and the Influences on Drug Metabolizing Enzyme Activities by the Traditional Herbal Formulas, Yongdamsagan-Tang and Paljung-san

  • Objectives: Yongdamsagan-tang (YST) and Paljung-san (PJS) in traditional medicine and finasteride in modern medicine are used to treat benign prostatic hyperplasia (BPH). In recent, the use of combination herbal remedies with conventional drugs has been increasing. Therefore, we investigated the anti-inflammatory effects of these drugs to treat BPH and the influence of herbal formulas on finasteride metabolism. Methods: The inhibitory effects of the herbal formulas and finasteride on the production of inflammatory mediators and cytokines were determined in lipopolysaccharide (LPS)-treated RAW 264.7 cells. Additionally, the influence of herbal formulas on activities of human drug metabolizing enzymes (DMEs) was assessed using human microsomal enzymes. Results: We observed that YST, PJS and finasteride inhibited the production of nitric oxide (NO), prostaglandin E2 (PGE2) and interleukin-6 (IL-6) in RAW 264.7 cells. The half maximal inhibitory concentration (IC50) of YST on PGE2 production was calculated to be below 25 ㎍/mL. YST inhibited the activity of uridine diphosphate-glucuronosyltransterase (UGT) 1A4 with an IC50 value of 49.35 ㎍/mL. The activities of cytochrome P450 (CYP) 1A2, CYP2B6, CYP2C19, CYP3A4, and UGT1A1 were inhibited by PJS (IC50 < 100 ㎍/mL, each). Although PJS and YST inhibited the activities of CYP3A4 and UGT1A4, respectively, these formulas may not influence the metabolism of finasteride because the IC50 values of herbal formulas on DMEs are too high to affect metabolism. Conclusions: Our results suggest that the combination of finasteride and YST or PJS might not influence their drug metabolism and that the drugs may have synergistic effects against BPH.

Effects of Yongdamsagan-tang on the Progression of the Estradiol Valerate-induced Polycystic Ovaries and on the Conception in Rats (용담사간탕(龍膽瀉肝湯)이 Estradiol Valerate로 유발된 흰쥐의 다낭성 난소 발달과 수태에 미치는 영향)

  • Objectives: This study was designed to investigate the effects of Yongdamsagantang on the polycystic ovary(PCO) induced by estradiol valerate(EV) in rats. Methods: After administrating Yongdamsagan-tang to PCO induced rats, we measured the weight of body, ovaries, adrenal glands, and uterus of rats. The observation through naked eye and histopathological observation of ovaries were evaluated. Also, the number of follicle and corpora lutea and content of androstenedione(ADD) and total estrogen were evaluated. The expressions of nerve growth factor(NGF) and corticotropin releasing factor(CRF) were analyzed by immunohistochemistry. The breeding rate and number of implantation with normal male rats were evaluated. Results: - The weight(mg) of ovaries in YST treated group($73.8{\pm}7.6$) was significantly increased(p<0.001) compared with control group($54.3{\pm}4.5$). - The number of mature follicles in YST treated group($7.3{\pm}2.4$) was significantly increased(p<0.01) compared with control group($3.5{\pm}1.2$). - The number of atretic follicles in YST treated group($9.0{\pm}1.5$) was significantly decreased(p<0.01) compared with control group($13.4{\pm}3.8$). - The number of cystic follicles in YST treated group($3.1{\pm}1.1$) was significantly decreased(p<0.01) compared with control group($6.0{\pm}2.0$). - The number of corpora lutea in YST treated group($3.8{\pm}2.1$) was significantly increased(p<0.001) compared with control group($0.3{\pm}0.7$). - The expression of NGF-immunoreactive cells in the ovarian granulosa cells in YST treated group was lesser observed than control group. - The expression of NGF-immunoreactive cells in the adrenal cortex in YST treated group was lesser observed than control group. - The breeding rate in YST treated group(100 %) was significantly increased (p<0.05) compared with control group(50 %). - The number of implantation in YST treated group($6.4{\pm}4.7$) was significantly increased(p<0.05) compared with control group($1.4{\pm}2.6$). Conclusions: We concluded that Yongdamsagan-tang activates the maturation of follicles, normal ovulation, breeding rate and number of implantation. Therefore, this may be effective for the treatment of anovulation, amenorrhea and sterility of PCOS patients.

Treated One Case of Cholangitis Due to Caroli's Disease (Caroli's Disease로 인한 담관염 환자 치험 1례)

  • Caroli's disease is a non-obstructive dilatation of the intrahepatic bile ducts. It is a rare congenital disorder that classically causes saccular ductal dilatation, which usually it segmental dilatation. Caroli's disease is associated with recurrent bacterial cholangitis and stone formation. A 57 years-old female inpatient suffering from cholangitis due to Caroli's disease was admitted at Wonkwang University Jeonju Oriental Medicine Hospital. The patient was treated with herbal medicine such as Yongdamsagan-tang and acupuncture. As result, there was improvement of symptoms. The process and content of treatment and the patient's recovery over general symptoms is described.

A Case Report of One Polycythemia Vera Patient Treated with Oriental Medicine in Conjunction with Conventional Medicine (한양방치료를 병행 후 호전된 진성적혈구증가증 환자 1례)

  • Objective: This study reports one case of polycythemia vera patient treated with oriental medicine in conjunction with conventional medicine. Methods: One patient who was diagnosed with polycythemia vera was treated using different modalities of herbs, acupuncture, and pharmacopuncture. An herbal formula, Yongdamsagan-tang was given three times a day with daily acupuncture treatment session, in addition to Soyeom and Jahageo pharmacopuncture injection which were administered three times a week. After treatment, patient's symptoms and blood test results were observed and analyzed. Results: After ten days of treatment, the patient had shown improvements in symptoms of headache, dizziness, tinnitus, and itchiness. Two months later, laboratory findings continued to be kept within the normal range and the intervals of phlebotomy therapy has extended to three months. Conclusions: This case study shows minimum evidence of safety and efficacy of oriental medicine for managing symptoms of polycythemia vera. Considering this is only a short term, single case study, further research is needed to confirm these results.

A Case Report of Vaginitis in Menopausal Woman Treated by Herbal Medicine (한약치료로 호전된 폐경 여성 환자의 질염 1례)

  • The purpose of this study is to report the effects of Korean medicine treatment, especially herbal medicine on vaginitis in menopausal woman. The patient suffered from vaginitis was treated with herbal medicine such as Yongdamsagan-tang and Jibaekjihwang-tang. The effect of treatment was evaluated through questionnaire. After treatment, symptoms of vaginitis were reduced. This case report shows that herbal medicine treatment may be effective for vaginitis.

Evaluation of Vasodilative Effects on 10 Traditional Herbal Formulas for Treatment of Hypertension (고혈압 치료를 위한 상용하는 한약 처방 10종의 혈관이완 평가)

  • Objectives : Hypertension (high blood pressure), one of the world's major chronic diseases, has a high mortality rate due to its high prevalence and complications, but its control rate is low. The proper management and control through appropriate exercise, diet management, and optimal drug choice can reduce the risk of death from hypertension. Although various antihypertensive drugs are used to treat hypertension, they also have numerous adverse effects. Alongside increased interest in the use of Traditional Herbal Formulas (THF) for hypertension treatment, the purpose of this study was to examine the vasodilative effects of 10 THF in the rat thoracic artery pre-contracted by potassiumchloride (KCl). Methods : THF were extracted with distilled water for 2 hours. The rat thoracic artery was suspended and contracted by KCl in the organ bath which contained 10 ml Krebs Henseleit (KH) buffer. THF extracts were added in a dose-dependent increase (10-1,000 ㎍/mL) to examine vasodilative effects. The vasodilative effects produced by THF were expressed as the percentage in response to KCl-induced contraction. Results : Among the 10 THF, Banhasasim-tang, Buhnsimgieum, Sagunja-tang, and Samul-tang showed vasodilative effects. And, Sipjeondaebo-tang, Ssanghwa-tang, Ojeok-san, Onkyung-tang, Yongdamsagan-tang, and Hyangsayukgunja-tang showed no significant vasodilative effects. Also, in co-administration with amlodipine, Banhasasim-tang showed higher vasodilative effects than amlodipine alone, and Buhnsimgieum showed greater vasodilative effects at low concentrations, but inhibited amlodipine's vasodilative effects at high concentrations. Conclusion : As a result of these studies, they will be expected to provide useful data to establish guidelines of combined administration of THF and western antihypertensive drugs for the treatment of hypertension.
