Analysis for the Idiopathic Facial Palsy Inpatients According to Distribution of Sasang Constitution, Hyungsang Classification and Assessment Tools (특발성 안면신경마비 환자에 대한 사상체질.형상별 분포 및 평가도구에 따른 분석)

  • Objectives : The purpose of this study is to analyze the distribution, relationship, prognosis and improvement score of idiopathic facial palsy inpatients according to constitutional differentiation ; Sasang constition, Hyungsang classification, Sasang constition combined with Hyungsang classification. Methods : A study was done on 102 patients who were diagnosed and treated as idiopathic facial palsy from April 2012 to Nomember 2012 at the Department of Acupuncture and Moxibution Medicine, Dong-eui Oriental University Hospital. Medical records of inpatinets who underwent facial ENoG, NET test were analyzed. Changes of HBGS grade and Yanagihara's score were also evaluated. We classified inpaients acording to Sasang constitution by consulting to Department of Sasang constitutinal medicine, and to Hyungsang classification(Dam-body, Bangkwang-body) by measuring under medical system of 3D facial shapes(RS-400FL). The constitutional differentiation and general characteristics were initially analyzed, and valuse on ENoG, NET were correlated with constitutinal differentiation. Results of HBGS and Yanagihara's score were also correlated with constitutinal differentiation. Results : 1. Each 39.2 percent of idiopathic facial palsy inpatients were the Taeeumin and Soyangin, 21.6 percent were the Soeumin. 2. 75.5 percent of idiopathic facial palsy inpatients were the Bangkwang-body, 24.5 percent were the Dam-body. 3. 34.3 percent of idiopathic facial palsy inpatinets were the Taeeum-Bangkwang, 21.6 percent were the Soyang-Bangkwang, 19.6 percent were the Soeum-Bangkwang, 17.6 percent were the Soyang-Dam, 4.9 percent were the Taeem-Dam, 2 percent were the Soeum-Dam. 4. By sex, the each ratio of Taeeumin, Soeumin, Bangkwang-body, Taeeum-Bangkwang, Soeum-Bangkwang, Soyang-Bangkwang was higher in female, that of Dam-body, Taeeum-Dam, Soyang-Dam was higher in male. 5. By relations between Sasang constitution and Hyungsang classification, the each ratio of Taeeumin and Soeumin was higher in Bangkwang-body. 6. By values on ENoG and NET, evaluations of HBGS's grade and Yanagihara's score, there were no significant difference. Conclusions : In idiopathic facial palsy inpatients, the proportion of Taeeum and Soyang among the Sasang constitution was higher, that of Dam among the Hyungsang classification was higher, that of Taeeum-Bangkwang among the Sasang combined with Hyungsang was the highest. It would seem that Bangkwang-body, female were closely related to Taeeumin, Soeumin. Also, Dam-body were closely related to male. But when comparing groups, there was no statistically significant difference in prognosis and improvement.

  • Objectives This study aimed to investigate the effect of SJS non-resistance technique-facial palsy (SJSNRT-F) using Korean medicine treatment on facial nerve palsy through three case studies. Methods SJSNRT-F, with other Korean medicine treatments, was performed on three patients who underwent treatment at OO Korean Medicine Hospital from March 2021 to August 2021. Additionally, the House-Brackmann grading system (H-B grade) and Yanagihara's unweighted grading system (Yanagihara's score) were evaluated in these patients. Results There was a significant increase in the Yanagihara scores. Except for one patient who was discharged early, two patients showed improvement in the H-B grade. Conclusions This study suggests that Korean medicine treatment including SJSNRT-F may affect the treatment of facial nerve palsy. However, better clinical studies are needed to confirm the same.

A Comparative Study of the Effects of Jungsongouhyul Pharmacopuncture Treatment and Bee Venom Pharmacopuncture Treatment on Peripheral Facial Paralysis (말초성 안면신경마비에 대한 중성어혈약침과 봉약침 효과 비교 연구)

  • Objectives : This study was designed to compare the effects of Jungsongouhyul pharmacopuncture treatment with bee venom pharmacopuncture treatment in hospitalized patients with peripheral facial paralysis. Methods : This study was done on a total of 41 patients with peripheral facial paralysis who were admitted into the Korean Medicine Hospital from February 1st, 2013 to April 30th, 2014. Wedivided patients into two groups. The Jungsongouhyul group was treated by Jungsongouhyul pharmacopuncture once a day and the bee venom group was treated by bee venom pharmacopuncture once a day. To compare the therapeutical effects of the two treatments, we used Yanagihara's unweighted grading system, House-Brachmann grading system, lip-length & snout indices and facial disablity index twice - before initial treatment and after final treatment. Results : Both Yanagihara's score and House-Brachmann grading system score improved in each group. However, there were no significant differences in improvement between the bee venom group and the Jungsongouhyul group. Conclusions : Jungsongouhyul pharmacopuncture treatment appears to be as effective as bee venom pharmacopuncture treatment to improve symptoms of peripheral facial paralysis.

  • Objective : This study was designed to evaluate the effect of treatment of Bell's palsy patients with postauricural pain by Anti-inflammatory pharmacopuncture Methods : The clinical comparison studies were carried out 30 cases of Bell's palsy patient with postauricular pain. Anti-inflammatory pharmacopuncture. We divided into two groups. One was control group that was treated only Acupuncture and Herbal therapy and The other was a Anti-inflammatory pharmacopuncture group and the conclusions for treatment are as follows. Results : 1. Regarding alleviation of pain by VAS, Anti-inflammatory pharmacopuncture Group II showed significant decrease of VAS in the 5days treatment. 2. They were not significant score within two groups after 5th days treatment. 3. As a result of evaluation by using Yanagihara score, they were not significant score within two groups after final treament.

The clinical observation on Bell's palsy according to facial nerve grading system (안면신경평가기준에 따른 구안와사(口眼喎斜)의 임상관찰)

  • Background and Objective : The evaluation of facial nerve function is a complex procedure. The House-Brackmann facial nerve grading system(H-B grade)and the Yanagihara grading system(Y-system) have been recommended as universal standards for assessing the degree of facial nerve palsy. The purpose of this study is to compare the effectiveness of Y-system and H-B system and to know the advantages of each system for each stage of incomplete recovery or sequelae of facial paralysis. Materials and Methods : A comparison between H-B grade and Y-system was studied with 137 evaluations of 61 cases of incomplete recovery or sequelae of peripheral facial paralysis. Each case was graded by using H-B system as a gross system and Y-system as a regional system before treatment, after 12 weeks and 6 months Results and Conclusions : The range of score in the Y-system 0-6, 8-16, 14-22, 24-34, 32-38, 38-40 were matched with grade VI, V, IV, III, II and I in the H-B system. The percentage of H-B grade III was the greatest among 137 evaluations and y-system showed the greatest score range. H-B system is easy to use but Y-system is more objective, quantitative and convenient to use in the incomplete recovery or sequelae state.

Effect of Acupuncture Complex Therapy on Three Patients with Bell's Palsy During Chemotherapy (항암치료중 구안와사가 발생한 환자 3례에 대한 고찰)

  • Objectives : The aim of this study was to investigate the effect of oriental medical treatment on Bell's palsy patients receiving chemotherapy. Methods : A clinical study was done on 3 patients who were diagnosed and treated as Bell's palsy at the Dep. of Acupuncture and Moxibustion College of Oriental Medicine Daejeon University. The effects of combined treatment of acupuncture, herbal medicine and physiotherapy were evaluated with Yanagihara's unweighted grading tool. Results : 1. There was significant improvement on Yanagihara score after treating with Gunbiikqi(健脾益氣) method, which was based on pattern differentiation as Hur(虛證). 2. Three patients commonly complained heaviness, dyspepsia, anorexia after receiving chemotherapy and oriental medical treatment showed significant improvement on the symptoms. 3. All three cases received herbal medicine treatment and liver function test. Three cases showed normal range of liver function during and after the treatment. Conclusions : Oriental-medicine treatment did not damage in liver function and showed significant improvement on Bell's palsy in patients receiving chemotherapy. But it is considerd that we need to keep limits in getting enough patients.

Five Clinical Cases of Facial Chuna Manual Therapy with Korean Medicine Treatment for Acute Bell's Palsy

  • This study individually analyzed the effects of Korean Facial Chuna Manual Treatment (K-FCMT) combined with Korean medicine (KM) treatment (acupuncture, electroacupuncture, pharmacopuncture, and herbal medicine) on five patients with acute Bell's palsy who visited Dongsuwon Korean Medicine Hospital between August 1 and 31, 2022. During inpatient treatment, two of the five patients received K-FCMT 5-6 times a week, and the other three received the same frequency during outpatient treatment for approximately 2 weeks. Patients with a House-Brackmann grading scale (HBGS) score of ≤4 and Yanagihara unweighted grading system (Y-score) ≥7-8 began to recover after the 2nd-3rd sessions of K-FCMT, which entered the recovery phase quickly. As patients entered the recovery phase (7-9th sessions of K-FCMT), symptoms improved to HBGS scores of 1-2 and Y-scores of 35-40 points. This study suggests the possibility of applying K-FCMT combined with KM treatment to patients in the acute stage of Bell's palsy.

  • Objectives: The purpose of this study is to investigate the effectiveness of using Korean medical treatment for peripheral facial nerve palsy. Methods: We primarily treated the patient's peripheral facial nerve palsy during hospitalization (06/28/21~07/13/21) without any other steroids or antiviral drug treatments and additionally treated their spinal stenosis, using Korean medical treatments in both cases (herbal medicine, acupuncture, and herbal acupuncture therapy). The House-Brackmann grading system and Yanagihara's unweighted grading system were used to assess facial symptom changes, and the Numerical Rating Scale was used to evaluate the back pain caused by the spinal stenosis. Results: Following treatment, the patient's House-Brackmann and Numerical Rating Scale scores decreased, while their Yanagihara unweighted score improved. Conclusions: This study suggests that Korean medicine may be effective in the treatment of facial nerve palsy.

A Comparative Study on the Effect of Cupping Therapy Combined with Korean Medicine Treatment in Peripheral Facial Paralysis

  • Background: The aim of this study was to compare the effects of different cupping therapy methods combined with Korean medicine treatments for peripheral facial paralysis. Methods: 105 patients treated for peripheral facial paralysis with cupping therapy and other Korean medicine treatment at Gil Korean medicine hospital, Gachon University between May 19, 2014 and June 30, 2018 were selected and their medical charts retrospectively analyzed. 48 patients who met the inclusion criteria were divided into 2 groups: wet cupping (WC) therapy, or dry cupping (DC) therapy combined with Korean medicine treatment. The duration of treatment ranged from 2 weeks for inpatient treatment to 2 months for outpatient treatment. Both WC and DC therapy were performed on TE13, 3 times per week during the treatment period. The effect of cupping therapy was evaluated by using the Gross Grading System of the House-Brackmann (HB score) and the Yanagihara's Unweighted Grading System (Y score). Results: For both WC and DC treatment of symptoms related to peripheral facial paralysis, HB scores showed a significant decrease and Y scores showed a significant increase from baseline to end of treatment, indicating a beneficial improvement in patient symptoms for both WC and DC. Conclusion: In this study, both DC and WC treatment had significant improvements on peripheral facial paralysis symptoms, with WC having significantly greater beneficial effects than DC.

Clinical Case Study of Facial Nerve Paralysis with Sensorineural Hearing Loss and Tinnitus Caused by Traumatic Temporal Bone Fracture (난청과 이명을 동반한 외상성 안면신경마비 치험 1례)

  • Objectives : The aim of this report was to investigate the effects of Korean medical treatment on facial nerve paralysis with sensorineural hearing loss and tinnitus caused by traumatic temporal bone fracture. Methods : We treated a patient with acupuncture, herbal medicine and physiotherapy. The effect of these treatments was evaluated by House-Brackmann facial grading scale, Yanagihara's unweighted grading system and by Digital Infrared Thermographic Image. Results : After 21 days of Korean medical treatment, House-Brackmann facial grading scale changed from III to II and Yanagihara's unweighted grading score increased from 14 to 27. Digital Infrared Thermographic Image also improved. Conclusions : These results suggest that Korean medical treatments were effective in treating facial nerve paralysis with sensorineural hearing loss and tinnitus caused by traumatic temporal bone fracture. We hope that a more efficient application of this treatment will be the result of clinical data accumulated in future studies.
