인지기능에 비만 역설은 존재하는가?: 고령화연구패널자료(2006-2016)를 이용하여 (Does the Obesity Paradox Exist in Cognitive Function?: Evidence from the Korean Longitudinal Study of Ageing, 2006-2016)

  • Background: There have been many studies on the associations between body mass index (BMI) and cognitive function. However, no study has ever compared the associations across the methods of categorizing BMI. In this study, we aimed to fill the gap in the previous studies and examine whether the obesity paradox is valid in the risk of cognitive function. Methods: Of the 10,254 people aged 45 and older from the Korean Longitudinal Study of Ageing from 2006 to 2016, 8,970 people were finalized as the study population. The dependent variable was whether a person has a normal cognitive function or not, and the independent variables of interest were BMI categorized by the World Health Organization Western Pacific Regional Office (WHO-WPRO) method, the WHO method, and a 10-group method. Covariates included sociodemographic factors, health behavior factors, and health status factors. A generalized linear mixed model analysis with a logit link was used. Results: In the adjusted model with all covariates, first, in the case of BMI categories of the WHO-WPRO method, underweight (odds ratio [OR], 1.16; 95% confidence interval [CI], 1.15-1.17), overweight (OR, 1.36; 95% CI, 1.35-1.36), and obese (OR, 1.34; 95% CI, 1.33-1.34) groups were more likely to have a normal cognitive function than a normal-weight group. Next, in the case of BMI categories of the WHO method, compared to a normal-weight group, underweight (OR, 1.15; 95% CI, 1.14-1.16) and overweight (OR, 1.06; 95% CI, 1.06-1.07) groups were more likely to have a normal cognitive function; however, obese (OR, 0.62; 95% CI, 0.61-0.63) group was less likely to have it. Lastly, in the case of the 10-group method, as BMI increased, the likelihood to have a normal cognitive function changed like a wave, reaching a global top at group-7 (26.5 kg/㎡ ≤ BMI <28.0 kg/㎡). Conclusion: The associations between BMI and cognitive function differed according to how BMI was categorized among people aged 45 and older in Korea, which suggests that cognitive function may be positively associated with BMI in some categories of BMI but negatively in its other categories. Health policies to reduce cognitive impairment need to consider this association between BMI and cognitive function.

Effects of Antiretroviral Therapy on the Survival of Human Immunodeficiency Virus-positive Adult Patients in Andhra Pradesh, India: A Retrospective Cohort Study, 2007-2013

  • Objectives: The survival outcomes of antiretroviral treatment (ART) programs have not been systematically evaluated at the state level in India. This retrospective study assessed the survival rates and factors associated with survival among adult human immunodeficiency virus (HIV)-infected patients in Andhra Pradesh, India. Methods: The present study used data from 139 679 HIV patients aged ${\geq}15$ years on ART who were registered from 2007 to 2011 and were followed up through December 2013. The primary end point was death of the patient. Mortality densities (per 1000 person-years) were calculated. Kaplan-Meier and Cox-regression models were used to estimate survival and explore the factors associated with survival. Results: The overall median follow-up time was 16.0 months (2.0 months for the deceased and 14.0 months for those lost to follow-up). Approximately 13.2% of those newly initiated on ART died during follow-up. Of those deaths, 56% occurred in the first three months. The crude mortality rate was 80.9 per 1000 person-years at risk. The CD4 count (adjusted hazard ratio [aHR],4.88; 95% confidence interval [CI], 4.36 to 5.46 for < $100cells/mm^3$ vs. > $350cells/mm^3$), functional status (aHR, 3.05; 95% CI, 2.82 to 3.30 for bedridden vs. normal), and body weight (aHR, 3.69; 95% CI, 3.42 to 3.97 for <45 kg vs. >60 kg) were strongly associated with the survival of HIV patients. Conclusions: The study findings revealed that high mortality was observed within the first three months of ART initiation. Patients with poor baseline clinical characteristics had a higher risk of mortality. Expanded testing and counseling should be encouraged, with the goal of ensuring early enrollment into the program followed by the initiation of ART in HIV-infected patients.

Solitary Plasmacytoma of the Sternum

  • Plasmacytoma is a plasma cell neoplasm that locally infiltrates a bone or spreads to extramedullary areas. A new World Health Organization criterion defines solitary plasmacytoma of bone as a localized bone tumor consisting of plasma cells identical to those seen in plasma cell myeloma, which is manifested as a solitary osteolytic lesion in a radiological evaluation. Primary tumors of the sternum are generally malignant, and solitary plasmacytomas of the sternum are very rare tumors. We present herein the case of a patient who had a primary sternal tumor with solitary plasmacytoma and no evidence of multiple myeloma.

  • Paper-based analytical devices (${\mu}PAD$) are highly advantageous for portable diagnostic systems owing to their low costs and ease of use. ${\mu}PADs$ are considered as the best candidates for realizing the World Health Organization (WHO) ASSURED criteria: affordable, sensitive, specific, user-friendly, rapid and robust, equipment-free, and deliverable to end users. However, they have several limitations such as low sensitivity and accuracy. This article reports a mini review for a micro-fluidic paper-based analytical devices (${\mu}PAD$), especially for addressing low sensitivity and accuracy issues.

Chordoid Glioma : A Case Report of Unusual Location and Neuroradiological Characteristics

  • Since the World Health Organization (WHO) classification for central nervous system neoplasms was declared in 2000, chordoid glioma of the third ventricle has been noted as a newly recognized tumor for central nervous system neoplasms. Although there is not enough universal experience to know the nature of this tumor due to its rarity, the origin of chordoid glioma was guardedly proposed to be the ependymal cells of the third ventricle. Such an idea has been primarily based on the specific location of the tumor, that is, third ventricle, suprasellae, and hypothalamus. However, we report a rare case of histologically confirmed chordoid glioma located in the left thalamus, not attached to any of the midline structures having unusual neuroradiological characteristics.

A case of nodal marginal zone B-cell lymphoma of the lower eyelid

  • A marginal zone B-cell lymphoma is a type of B-cell lymphoma which is normally located at the margins of the secondary lymph node follicles. According to 2008 World Health Organization (WHO) classification, there are three main types classified according to the location of invasion: nodal lymphoma in the lymph nodes, splenic lymphoma in the spleen, and extranodal lymphoma in other locations. Recently, we have experienced a rare case of primary nodal marginal zone lymphoma that arose in the lower eyelid. Therefore, we report this case with a review of literature.

수면방해에 대한 복합소음의 종류와 S/N비의 영향 (Effect of Type and S/N Ratio of Combined Noise Sources on Sleep Disturbance)

  • The World Health Organization(WHO) has reported the sleep disturbance as one of the adverse effects of noise. And the effect of combined noise sources on human have been investigated because community noises are seldom heard and are concurrent. Therefore, the effects of combined noise source and its S/N ratio on sleep disturbance were investigated; as a single noise source, road traffic noise was selected, and construction and movie noise were combined with road traffic noise. Tests for sleep disturbance were conducted in bedrooms and the effect on each subjects' sleep was evaluated by questionnaire. The results showed that the rate of awakening increased slightly only when the SPL of construction noise was 10 dB higher than that of road traffic noise. It was also found that the sleep disturbance was affected by the type of combined noise sources and its S/N ratio.

Evaluatinon of the Surface Water in Southwest Area of Vietnam

  • This study surveys the surface water environment in Southwest sea areas of Vietnam in order to evaluate the current environment. It collects and analyzes experimental results of programs, scientific subjects, and international projects relevant to ecological areas and coastlines of Kien Giang province. The methods analyzed and discussed by specialists are evaluated according to pollution coefficients established by the World Health Organization (WHO), sampling methods, and case study analyses. Comparison between the scale of pollution levels and the surface water quality in experimental areas of Long Xuyen Quadrangle shows a light-moderate pollution level. In Ca Mau Peninsula, the surface water source is only slightly polluted. Sea areas and islands are also light-moderately polluted. Computing results from water quality indices (WQI) show the level of pollution in experimental sites. Activities such as constructing infrastructures for tourism, exploiting minerals, and increasing population accelerate environmental pollution by deteriorating inherently sensitive ecosystems like corals, sea grass, mangrove swamps, and Melaleuca forests. Surveying and evaluating the present situation of surface water are fundamental solutions for environmental protection in the Southwest sea areas of Vietnam.

Metaplastic Meningioma Overspreading the Cerebral Convexity

  • Meningioma is relatively common, benign, and extra-axial tumor accounting for about 20% of primary brain and spinal cord tumors. The World Health Organization (WHO) classified these tumors into Grade I (benign), Grade II (atypical), and Grade III (anaplastic) meningioma. Grade I meningioma which is slowly growing tumor and have some rare subtypes. Among them, metaplastic subtype is defined as a tumor containing focal or widespread mesenchymal components including osseous, cartilaginous, lipomatous, myxoid or xanthomatous tissue, singly or in combinations. We report a rare metaplastic meningioma overspreading nearly whole cerebral convexity from main extra-axial tumor bulk in the parietal lobe.

  • In this paper, a study was conducted to find main factorsto Pima Indians Diabetes based on machine learning. Diabetes is a type of metabolic disease such as insufficient secretion of insulin or inability to function normally and is characterized by a high blood glucose concentration. According to a situation report from WHO(World Health Organization), Diabetes is a chronic, metabolic disease characterized by elevated levels of blood glucose (or blood sugar), which leads over time to serious damage to the heart, blood vessels, eyes, kidneys and nerves. And also about 422 million people worldwide have diabetes, the majority living in low-and middle-income countries, and 1.6 million deaths are directly attributed to diabetes each year. Both the number of cases and the prevalence of diabetes have been steadily increasing over the past few decades. Therefore, in this study, we used Support Vector Machine (SVM), Decision Tree, and correlation analysisto discover three important factorsthat predict Pima Indians diabetes with 70% accuracy. Applying the results suggested in this paper, doctors can quickly diagnose potential Pima Indians diabetics and prevent Pima Indians diabetes.
