빠른 교대근무가 피로도, 요중 17-KS, $Na^+,\;Cl^-$ 배설에 미치는 영향 (The Effect of Rapidly Rotating Shift work on the Fatigue Level, Urinary 17-KS, $Na^+$ and $Cl^-$ Excretion)

  • This study was done to investigate the effects of rapidly-rotating shift work of two-day interval on fatigue level and the concentration of urinary 17-KS, $Na^+,\;Cl^-$. The subjects were 20 nursing college students(control group) and 15 nurses in a university hospital and the study was done from Apr. 21 to May 4th, 1999. In the test group, each 5 nurses were allocated to day shift(8 AM-4 PM), evening shift(4 PM-12 MN) and night shift(12 MN-8 AM) respectively. The fatigue level were measured 30 minutes after work start on the 2nd day of work shift. Urine specimens were collected at 8 AM, 4 PM and 12 MN on the 2nd day of work shift in the control group and 30 minutes before and after work on the 2nd day of work shift in the test group. The data were analyzed with SPSS(for Window, ver 7.5). Statistical analysis was performed by using t-test, paired t-test and ANOVA. The results were as follows. 1. The perceived fatigue level in shift work 1) The physical and mental fatigue level were significantly higher in night shift than that in day or evening shift(p<0.05). In the neuro-sensory fatigue level, night shift showed higher tendency than that in day or evening shift, but there were no significant differences between each shifts. 2) Comparison between the control group and the test group: Physical fatigue level was significantly higher in night shift than that in day or evening shift of the control group(P<.001). Mental fatigue level was significantly higher in day or night shift than that in evening shift of the control group(P<.05). In the neuro-sensory fatigue level, test group showed higher tendency than that in the control group, but there were no significant differences between two groups. 3) The total fatigue level was higher in night shift than that in day shift or evening shift(P<.05). In comparing with the control group, night shift and day shift showed higher total fatigue level than that in the control group(p<0.05). 2. The concentration of urinary 17-KS, $Na^+$ and $Cl^-$ In the control group, urinary 17-KS, $Na^+$ and $Cl^-$ showed higher level in afternoon that in morning and night. In the test group, cr in day and evening shift and $Na^+$ in evening shift showed higher level at the end of work. The 17-KS concentration at the begining and the end of work in three shift groups were lower than those in control group(p<0.05), however, $Cl^-$ concentration at the begining of work in day shift, and the end of work in day and evening shift were higher than those in control group(p<0.05). $Cl^-$ concentration at the begining and end of work in night shift were considerably higher than those in control group repectively(p<0.1, p<001). $Na^+$ concentration showed a higher tendency in three shift groups except at the begining of work in night shift, but there were no statistical difference. In comparing concentration of the 17-KS, $Na^+$ and $Cl^-$ among the shift groups, 17-KS concentration showed a lower tendency and $Na^+,\;Cl^-$ showed a higher tendency in night shift: The result of this study showes that biorhythm of shift work nurse was irregular. Fatigue level as the subjective index for evaluating the health problem concerning shift work was higher in night shift and proved to be in accordance with the concentration of urinary 17-KS, $Na^+$ and $Cl^-$ used as objective indices. Disturbation of biorhythm and work stress due to night shift seems to cause the health problem of nurses and decrease of work efficiency. It is considered that work regualtion is necessary for the rational management of the nursing administration.

Night shift preparation, performance, and perception: are there differences between emergency medicine nurses, residents, and faculty?

  • Objective Determine differences between faculty, residents, and nurses regarding night shift preparation, performance, recovery, and perception of emotional and physical health effects. Methods Survey study performed at an urban university medical center emergency department with an accredited residency program in emergency medicine. Results Forty-seven faculty, 37 residents, and 90 nurses completed the survey. There was no difference in use of physical sleep aids between groups, except nurses utilized blackout curtains more (69%) than residents (60%) and faculty (45%). Bedroom temperature preference was similar. The routine use of pharmacologic sleep aids differed: nurses and residents (both 38%) compared to faculty (13%). Residents routinely used melatonin more (79%) than did faculty (33%) and nurses (38%). Faculty preferred not to eat (45%), whereas residents (24%) preferred a full meal. The majority (>72%) in all groups drank coffee before their night shift and reported feeling tired despite their routine, with 4:00 a.m. as median nadir. Faculty reported a higher rate (41%) of falling asleep while driving compared to residents (14%) and nurses (32%), but the accident rate (3% to 6%) did not differ significantly. All had similar opinions regarding night shift-associated health effects. However, faculty reported lower level of satisfaction working night shifts, whereas nurses agreed less than the other groups regarding increased risk of drug and alcohol dependence. Conclusion Faculty, residents, and nurses shared many characteristics. Faculty tended to not use pharmacologic sleep aids, not eat before their shift, fall asleep at a higher rate while driving home, and enjoy night shift work less.

순환교대근무 간호사의 일주기유형에 따른 근무조별 수면시간, 사회적 시차, 주관적 수면장애 비교 (Comparison of Sleep Duration, Social Jetlag, and Subjective Sleep Disturbance in Rotating Shift Nurses According to the Chronotype)

  • Purpose: The purpose of this study was to investigate the sleep duration, social jetlag (SJL), and subjective sleep disturbance according to the individual chronotype in rotating shift nurses. Methods: A total of 344 rotating 3-shift nurses (mean age 28.67 years) were recruited at one university affiliated hospital. They completed the following questionnaires, which were used to assess their chronotype and sleep: the morningness-eveningness questionnaire (MEQ), self-reported sleep duration of work days (SDWshift) and free days (SDFshift) in each shift (day [D], evening [E], night [N]), and sleep disturbance (Insomnia severity index, ISI). SJLshift was calculated as the difference in midsleep (MS=sleep onset+1/2 sleep duration) between work days (MSW) and free days (MSF). Results: Subjects were divided into 3 chronotype groups according to the MEQ; morning (MG, 4.4%) intermediate (IG, 57.8%), and evening groups (EG, 37.8%). SDWD was shortest (4.68 hr) and SDFE was longest (8.93 hr) in the EG. SJLD was longest in the EG (3.77 hr), and SJLN was longest in the MG (7.37 hr). The prevalence of sleep disturbance was 33.3% (MG), 29.6% (IG), and 40.0% (EG), respectively, without any statistical significance. Conclusion: In order to improve the sleep of shift workers, it is recommended that the evening chronotypes should reduce the day shifts and the morning chronotypes should reduce the night shifts. We also propose a study to determine whether these shift assignments can improve the sleep in shift nurses.

종합병원 간호사의 밤번고정근무제도와 이직의도에 관한 인식 (Perceptions on Fixed Night Shift System and Turnover Intention of General Hospital Nurses)

  • Purpose: This study was performed to identify perceptions on fixed night shift system and turnover intention of general hospital nurses and to explore a desirable application plan for a fixed night shift system. Methods: This study was a cross-sectional survey. Participants were 300 nurses working at one general hospital in GyeongGi-Do, Korea. Data were collected from November 3 to 7, 2014, with a structured questionnaires and analyzed using SPSS/PC ver 20.0 programs. Results: Of the participants 73.5% had thought about turnover because of night shifts and the preconditions for desirable fixed night shift system were a payment plan differentiated according to work conditions and an adequate number of nursing staff. Economic and administrative supports like 'raising extra-pay for night shift' and 'increasing the number of paid holidays' were very important for a desirable fixed night shift system. Conclusion: The results indicate that the most important factor for a desirable fixed night shift system to decrease nurses' turnover intention is economic and administrative support according to the needs of the nurses. So nursing managers need to find a desirable fixed night shift system considering nurses' demographic characteristics and organizational characteristics.

Circadian Rhythms Characteristics of Nurses Providing Direct Patient Care: An Observational Study

  • Background: In today's modern world, longer working hours, shift work, and working at night have become major causes of the disruption of our natural circadian rhythms. This study aimed to investigate the effects of the type of shift work (rotating vs. fixed day), duty period (on-duty vs. off-duty), and working period within each shift (nighttime vs. daytime) on the circadian rhythm characteristics of nurses who provide direct patient care. Methods: This cross-sectional study used a purposive sampling method. Cosinor analysis was applied to analyze the actigraphy data of nurses providing direct patient care for seven consecutive days. The linear mixed effects model was then used to determine any variances between shift type, duty period, and working period within each shift for the nurses. Results: The mesor value did not differ according to nurses' shift type, duty period, and working period within each shift. The amplitude was statistically higher in on-duty nurses and in daytime working hours. The acrophase was significantly delayed in nighttime working hours. As well as nurses in rotating shift had experience. Conclusion: Our findings revealed that the peak activity of nurses occurs significantly later at night while working and nurses working during nighttime hours may have a weaker or less distinct circadian rhythm. Thus, this study suggests that limits be placed on the number of rotating nighttime shifts for nurses.

교대근무자에서 각 교대근무간의 수면양상 및 자율신경계 활성도 비교 (Comparison of Sleep Patterns and Autonomic Nervous System Activity among Three Shifts in Shiftworkers)

  • 목 적 : 본 연구는, 교대근무자에 있어 세 교대근무간의 수면양상과 자율신경계활동성을 비교함으로써, 근무자들은 어떤 근무시에 가장 적합하다고 느끼는지 그리고 교대근무자의 심리적, 신체적 문제점의 특성은 무엇인지를 알아보고자 하였다. 또한 본 연구는 근무자들이 교대근무에 좀 더 효과적으로 적응하는데 도움을 주고자 기획되었다. 방 법 : 3일 정도 연속적으로 한 교대근무를 한 후 1일 휴무, 그 다음에 다른 교대로 넘어가는 빠른 순환주기의 교대근무자 51명이 연구에 참여하였다. 각 대상은 수면일기를 작성하였으며 수면잠복기, 수면기간시간, 수면 중 깨어난 횟수를 세 교대근무간에 비교하였다. Epworth 졸음척도와 시각아날로그척도를 사용하여 졸음을 평가하였으며 기분상태는 기분상태척도로 평가하였다. 심박수변동성과, 스트레스 호르몬(아드레날린, 노르아드레날린) 및 그 대사산물을 측정하여 자율신경계 활동성을 살펴보았다. 심박수변동성에는 저주파영역의 스펙트럼 파워, 고주파영역의 스펙트럼 파워, 저주파영역의 파워와 고주파영역의 파워간의 비가 포함되었다. 결 과 : 수면기간시간은 오후근무시 가장 길었고 수면잠복기는 밤근무시 가장 짧았다. 깨어났을 때의 각성도는 오전근무시 가장 저하되었고 작업 중의 각성도는 밤근무 중 가장 떨어지는 것으로 나타났다. 밤근무시 근무자들은 신체적인 피곤감과 인지기능의 저하를 호소하였다. 심박수변동성의 비교를 통해 오후근무시 부교감신경계가 가장 활성화된다는 것을 알 수 있었으며 아드레날린 및 노르아드레날린 분비도 통계적인 의미는 없었지만 오후근무시 가장 감소하였다. 결 론 : 본 연구결과 오후근무가 교대근무자에게 가장 적합한 것으로 나타났다. 오전근무시에는 빛을 아침에 투여하고 밤근무시에는 근무 중에 빛을 투여하면 교대근무에의 적응이 촉진될 것으로 생각된다.

근로환경이 주관적 건강상태에 미치는 영향 (Effects of the Working Environment on Subjective Health Status)

  • Objectives: This study was conducted to prepare fundamental data and prevention measures on health promotion and occupational disease, and to assess the effects of the working environment on subjective health status and absenteeism among workers using data from the third working environment survey in Korea. Methods: This study's subjects were composed of 29,711 wage workers from the 3rd working environment survey data. The dependent variables were several diseases, subjective health status and absences, and the independent variable was the working environment. The collected data were analyzed by One-Way ANOVA, Pearson's correlation and stepwise multiple analysis using the IBM SPSS(ver. 20.0) statistical package program. Results: The effecting factors for cardiovascular disease were age, working shift and emotional state. The effecting factors for anxiety and depression were years of education, working condition, duties, and emotional state. The effecting factors of insomnia were duty and emotional state. The positive effecting factors for absent days were work standing, working shift, number of night shifts, autonomy, and duties. The positive effecting factors of subjective health status were age, work standing, working years, working shift, appropriateness of working hours, leadership of superiors, duties and emotional state. Conclusions: Based on the above results, the author considers that it is necessary to improve the working environment to reduce absent days, such as by reducing of number of night shifts and giving autonomy regarding duties, and to improve the working environment for subjective health status such as by controlling the appropriateness of working hours and stability of the emotional state. In addition, this study provides fundamental data on health promotion and occupational disease among workers.

주야 교대 여성 근로자의 피로도와 영양섭취 실태에 관한 조사연구 (Study on Degree of Fatigue and Nutritional Status of Industrial female shift workers)

  • This study was undertaken to investigate whether the stress caused by day/night shifts on industrial workers can be affected through nutritional status. A sample of 573 female industrial workers, aged 17 through 23 years, from 4 different industries (2 in normal, 2 in 3 shift work pattern) were surveyed by questionaire examining their nutritional status, food intake, dietary habit, sleep complaint, performance of digestive organs, and degree of fatigue. Shift workers were surveyed during night work. The results are summarized as follows: 1. Sleep complaints and degree of fatigue in shift workers were higher than normal workers, whereas the performance of digestive organs were as lower than normal workers. These results showed that shift workers were more stressed than normal workers. 2. The workers who were surveyed were all poor in nutritional status. The intakes of calorie, protein, Ca, riboflavin, and ascorbic acid were lower than Korean Recommanded Dietary Allowences (RDA). Dietary habit of the shift workers was irregular. 3. There was a negative correlation between nutritional status and degrss of fatigue. The intakes of energy nutrients, ascorbic acid, and niain were significantly related to the degree of fatigue in which protein intake was found to be most influential. Among workers with the same level of protein intake, shift workers showed higher degree of fatigue. When the protein intake of shift workers was 100-125% of R.D.A., they showed the same degree of fatigue to the average normal workers.

Effects of Autonomous Community Gardening Activities in the Workplace on Job Stress, Organizational Effectiveness and Self-Esteem of Public Officials Working in Shifts

  • This study was conducted to examine effects of community gardening on job stress and job satisfaction experienced by public officials in general administration working in a 24-hour shift. The subjects were public officials at the Center for Missing Children in the Department of Women and Adolescents at the National Police Agency. The center receives initial reports of disappearance of missing children, the elderly with dementia, and people with intellectual disabilities, sends out the report to each police station, and clears the report after the missing individual is found. Twelve public officials working at the center in a double shift (full day off after a night shift, four-day interval) were grouped into four teams and total 11 of them participated in the experiment. They engaged in autonomous community gardening activities such as sowing seeds, cultivating plants, harvesting, or cooking in the workplace for five weeks from August 19 to September 22, 2018. The findings were as follows. After five weeks of autonomous gardening activities, job stress of the subjects decreased significantly. Their mean scores of job satisfaction and organizational commitment under organizational effectiveness showed a significant increase after the activities. Finally, there were significant differences in their self-esteem after participating in the gardening activities in the workplace. These results suggest that gardening activities in the workplace can help to manage job stress and quality of work life for shift workers.

일부 종합병원 간호사들의 교대근무와 수면문제 및 피로자각증상과의 관련성 (Relationship between Shift Work, and Sleep Problems and Fatigue Symptoms of Nurses for General Hospitals)

  • Purpose: To find out the relationship between the shift work, and disrupted sleep and consequent fatigue symptoms. Methods: The questionnaires were given to 345 nurses of 4 hospitals with over 400 beds in Daejeon. Results: The shift workers showed worse subjective quality of sleep than the daytime workers, and had significantly higher frequency of "cannot get to sleep within 30 minutes", "wake up in the middle of the night" and "wake up in the early morning". The shift workers had significantly higher scores of subjective symptoms of fatigue than the daytime workers, and the fatigue was found to be the mental type. The subjective symptoms of fatigue scores were significantly higher in the group who had "cannot get to sleep within 30 minutes", "wake up in the middle of the night" and "wake up in the early morning". Conclusion: The study results indicated that the shift workers had the higher level of subjective symptoms of physical fatigue as well as the worse quality of sleep than the daytime workers. It is speculated that the factors attributable to the poor quality of sleep or sleep induction disorders of nurses of hospitals could be due in major part to adaptational difficulties resulting from frequent night shifts.
